Tremor or myoclonus? A practical clinical approach to distinguishing two common hyperkinetic movement disorders.

Tremor and myoclonus are among the most common hyperkinetic movement disorders encountered in neurological practice, with tremor being considerably more common than myoclonus in everyday clinical practice, yet distinguishing between them remains a frequent diagnostic challenge. Although their definitions are conceptually distinct, several disorders - including cortical tremor, dystonic tremor, and generalized polymyoclonus - can mimic one another, leading to diagnostic uncertainty and inappropriate management. This practical review provides clinicians with a structured approach to recognizing

Clinical Reasoning: A 13-Year-Old Boy With Acute-Onset Generalized Dystonia, Rostrocaudal Gradient, and Severe Bulbar Dysfunction.

A 13-year-old adolescent boy developed progressive generalized slowness over 1 month, followed by 10 days of rapidly worsening speech loss, dysphagia, rigidity, and dystonic posturing. Acquired evaluations, including CSF, infectious studies, autoimmune encephalitis panels, and brain MRI, were unrevealing, and empirical corticosteroid/antiviral therapy produced no meaningful improvement. Examination showed severe generalized dystonia with a rostrocaudal severity distribution, prominent bulbar and upper limb involvement, hyperreflexia, ankle clonus, and bilateral Babinski signs. Symptomatic medi

Insights into X-linked Dystonia-Parkinsonism and Spinocerebellar Ataxia type 36 Through the Lens of Amyotrophic Lateral Sclerosis/Frontotemporal Dementia.

The abnormal expansion of repetitive DNA sequences falls within the spectrum of neurological conditions driven by dual pathogenic pathways of RNA toxicity and protein toxicity. Within this spectrum, hexanucleotide repeat disorders (HRDs) represent a rare but important subgroup: C9orf72-amyotrophic lateral sclerosis/frontotemporal dementia (C9orf72-ALS/FTD), X-linked dystonia-parkinsonism (XDP) and spinocerebellar ataxia type 36 (SCA36), unified by the presence of expanded intronic repeats capable of forming non-canonical nucleic acid structures. Despite this shared repeat architecture, they di

Transcriptomic disruption and hypoactivity in DYT-SGCE medial ganglionic eminence-patterned inhibitory neurons.

Myoclonus dystonia is a Mendelian inherited, childhood-onset dystonic disorder, caused by mutations in the autosomal dominantly inherited gene SGCE, and in which both motor and psychiatric phenotypes are observed. Results from murine and in vivo human studies suggest that dystonia is caused by disruption to neuronal networks, in particular the basal ganglia-cerebello-thalamo-cortical circuit. Work focused on the cortical component implicates disruption to neuronal excitatory-inhibitory balance as being a key contributor to the observed phenotypes. Our previous work, focused on cortical excitat

Dystonia-deafness syndrome 1 caused by ACTB p.(Arg183Trp) de novo variant: one novel case extending the phenotypic spectrum.

Dystonia-deafness syndrome 1 (DDS1) is a rare disorder caused by the p.(Arg183Trp) heterozygous variant in ACTB. Patients present with congenital/early-onset sensorineural deafness, then childhood/adult-onset generalized dystonia. We describe a 38 y.o patient with additional features, including brain malformations: corpus callosum and vermis hypodysplasia. These novel features expand the clinical spectrum of DDS1, and suggests an overlap with ACTB-related Baraitser-Winter syndrome. Furthermore, we report a limited improvement after Globus Pallidus internus Deep Brain Stimulation (GPi-DBS) cont

Subthalamic Deep Brain Stimulation for Chorea-Acanthocytosis: A Single-Center Case Series.

INTRODUCTION: Chorea-acanthocytosis (ChAc) is a rare autosomal recessive neurodegenerative disorder characterized by progressive movement disorders. Deep brain stimulation (DBS) targeting the globus pallidus internus has shown efficacy in managing ChAc. However, evidence regarding subthalamic nucleus (STN) DBS remains limited, with only two cases previously reported from our center. OBJECTIVES AND METHODS: We analyzed seven consecutive patients with ChAc who underwent STN DBS at Ruijin Hospital (2010-2024). Motor symptoms were assessed using the Burke-Fahn-Marsden Dystonia Rating Scale (BFMDR

Convergence Deficits in Myoclonus-Dystonia Point to Cerebellar Impairment.

BACKGROUND: Myoclonus-dystonia (M-D) is a monogenic movement disorder, with proposed cerebellar dysfunction. Vergence eye movement deficits, characteristics of degenerative cerebellar disease, have not been studied in M-D. Cerebellar transcranial alternating current stimulation (tACS) is considered a potential therapeutic approach. OBJECTIVES: To assess vergence and prosaccade performance as markers of cerebellar dysfunction in M-D and to evaluate the effects of cerebellar 50 Hz tACS on these eye movements. METHODS: Vergence and prosaccade performance were examined in 14 M-D patients carryin

Probabilistic Lesion Mapping to Optimize Thalamotomy Targets for Focal Hand Dystonia.

OBJECTIVE: Focal hand dystonia (FHD) severely impairs task-specific motor control, yet the optimal surgical target for stereotactic intervention remains uncertain. This study aimed to identify the precise thalamic lesion site associated with symptomatic improvement and to clarify its network connectivity. METHODS: We retrospectively analyzed 164 patients with FHD (mean age = 42.0 years, 26.2% women) who underwent stereotactic thalamotomy of the ventral lateral thalamus. Voxel-wise probabilistic lesion mapping was applied to relate lesion locations to clinical outcomes. Structural connectivity

Efficacy of Deep Brain Stimulation for the Treatment of Monogenic Dystonia Symptoms: A Systematic Review.

BACKGROUND: Deep brain stimulation (DBS) is an essential treatment option for disabling segmental or generalized dystonia. An underlying monogenic etiology is increasingly recognized as an important predictor of DBS outcomes. Moreover, the genetic background of dystonia is continuously expanding, posing new challenges in the tailored counseling of patients regarding advanced therapies. METHODS: To improve the quality of available evidence on the efficacy of DBS for treating monogenic dystonia, we conducted a systematic review in accordance with PRISMA guidelines. We applied a rigorous methodo

Divergent Cerebello-Cortical Network Responses to Emotional Stress in Myoclonus Dystonia.

BACKGROUND: In myoclonus dystonia (DYT-SGCE) psychiatric features such as anxiety and depression are frequent comorbidities. The neural mechanisms underlying these negative emotional states in DYT-SGCE remain poorly understood. Task-based functional magnetic resonance imaging (fMRI), by measuring task-evoked functional connectivity during emotion processing, offers mechanistic insight into the pathophysiology of nonmotor features in DYT-SGCE. OBJECTIVE: The aim was to investigate effective connectivity alterations in the neural network associated with the experience of negative emotions in DY

Nonmotor symptoms of craniocervical dystonia: a multicentre international study-using the Dystonia nonmotor symptoms questionnaire.

Non-motor symptoms (NMS) are increasingly recognized as significant contributors to the overall burden of craniocervical dystonia (CCD), yet they remain underdiagnosed. This international, multi-centre observational study aimed to evaluate the prevalence and burden of NMS in 167 patients with adult-onset idiopathic CCD across Poland, the UK, and Germany, using the validated Dystonia Non-Motor Symptoms Questionnaire (DNMSQUEST). Participants were assessed at the end of their botulinum neurotoxin treatment cycle. The majority of patients (94.6%) reported at least one NMS, and over half experienc

"Tapping" into neural inhibition in focal hand dystonia: an evaluation of a finger-tapping task using TMS and fMRI.

BACKGROUND: Focal hand dystonia (FHD) is a task-specific movement disorder characterized by involuntary muscle contractions during skilled hand use. Impaired intracortical inhibition is a core feature of FHD, yet its relationship to functional brain activity during motor tasks remains poorly understood. This study combined transcranial magnetic stimulation (TMS) and functional MRI (fMRI) to investigate the relationship between intracortical inhibition, as measured by the cortical silent period (cSP), and task-based brain activation in individuals with FHD. METHODS: Sixteen individuals with FH

Long-term benefit of GPi-DBS in YY1-related dystonia: a case report.

Gabriele-de Vries syndrome (GADEVS) is a rare genetic disorder caused by mutations in the YY1 gene. It often leads to developmental delay, cognitive difficulties, and congenital abnormalities. About half of affected individuals develop movement disorders, including dystonia. Evidence for effective treatment is still limited. We describe a 35-year-old male patient with juvenile-onset generalized dystonia. His symptoms did not improve with medication. Genetic testing showed a previously unreported de-novo YY1 nonsense variant. Because of the progression of symptoms, he underwent bilateral deep b

Phenotypic and genotypic heterogeneity in a series of cases associated with dopa-responsive dystonia.

INTRODUCTION: Dopa-responsive dystonia (DRD) includes a group of neurological disorders that manifest with lower limb dystonia and an excellent response to levodopa. Parkinsonism is commonly associated with DRD, and many cases of GCH1 gene mutations without dystonia but with evidence of nigrostriatal pathway degeneration, indistinguishable to idiopathic Parkinson's disease (PD), have been recently described. This phenotypic variability has been observed even among members of the same family. METHOD: Clinical and genetic description of 14 patients from 4 different families with DRD associated

Divergent Recovery Paths in Musicians' Dystonia.

BACKGROUND: Musician's dystonia, characterized by a loss of voluntary motor control during performance, predominantly manifests as two forms: Musician's Hand Dystonia (MHD) and embouchure dystonia (ED). ED involves more muscle groups compared to MHD, complicating diagnosis and treatment. Moreover, treatment options for MHD are either not viable for ED, such as botulinumtoxin injections, or less effective in ED, such as anticholinergic drugs. METHODS: This study aimed to compare long-term subjective playing abilities at the onset of dystonia and after treatment between ED and MHD patients. We

Attenuated melanopsin-mediated activity in focal dystonia.

BACKGROUND AND OBJECTIVES: Focal dystonia (FD) is an extrapyramidal movement disorder, but various disease aspects challenge the view of a condition only involving neuronal motor systems. Here, we were interested in potential changes of retinal functions, accessible by chromatic pupillometry. METHODS: Quantitative chromatic pupillometry was performed in 26 persons with FD and 20 age-/sex-matched healthy controls. Upon dark adaptation, standardized red (632 nm) and blue (462 nm) light pulses (100 cd/m2) were delivered to the pupils. Constrictions were recorded one and 6 s after the stimuli. Th

Luteolin use in Integrated Stress Response: insight from a case of EIF2AK2-related dystonia.

This study presents the first reported case of a 3-year-old child with EIF2AK2-related dystonia treated with adjunctive luteolin supplementation. EIF2AK2-related dystonia, characterized by exacerbations during infections, is associated with disruptions in the integrated stress response (ISR). The ISR, a cellular signaling pathway activated in response to stress, culminates in the phosphorylation of eIF2α, which modulates protein synthesis and can induce cell death. Pathogenic variants in EIF2AK2 disrupt this pathway, contributing to the development of dystonia. Luteolin, a flavonoid possessing
