Identification and prenatal genetic testing of pathogenic variants in a case of myoclonus-dystonia syndrome.

Myoclonus-dystonia syndrome (MDS) is a movement disorder syndrome characterized primarily by myoclonus as the core feature. In this study, we reported a case of MDS with myoclonus as the prominent feature, which was accompanied by learning disability and special face. We then used copy number variation sequencing (CNV-seq), whole exome sequencing (WES) and Sanger sequencing to verify the MDS related genes of the patient and his family members. We found that the patient carried a heterozygous mutation of SGCE gene c.731dup (p.Asn244Lysfs*6) related to MDS, which inherited from his father. It wa

Clinical and Genetic Spectrum of ANO3-Related Dystonia with Treatment Responses in a Chinese Cohort.

Background: Variants in anoctamin 3 (ANO3) are linked to autosomal dominant dystonia, commonly known as DYT-ANO3 (OMIM: #615034). While craniocervical dystonia constitutes the most frequently observed phenotype, its clinical manifestations display significant heterogeneity. Nevertheless, the data concerning the genetic characteristics, clinical features, and therapeutic outcomes of ANO3-related dystonia within Asian populations remain scarce. Methods: Whole-exome sequencing was conducted on 661 Chinese patients, comprising 356 individuals with dystonia and 305 individuals with non-dystonic mov

Pallidal Deep Brain Stimulation in Dystonia: Investigating Differential Response by Dystonia Distribution.

BACKGROUND: Dystonia is characterized by involuntary intermittent or sustained abnormal movements or postures. Deep brain stimulation (DBS) is an effective treatment, and response has been shown to vary across body regions but literature depicting this pattern is conflicting. Our study analyzes dystonia response to DBS by body distribution using the Burke-Fahn-Marsden Rating Scale motor (BFMRS-M) score and the Global Dystonia Rating Scale (GDRS). METHODS: We reviewed standardized videos of patients with isolated non-acquired dystonia obtained at baseline and post-DBS of unilateral or bilatera

Trio analysis in dystonia identifies de novo KLC1 variants in a kinesinopathy with distinct motor and neurodevelopmental features.

BACKGROUND: Although de novo causation in dystonia is widely acknowledged, there have been only a few trio-sequencing analyses in this field. We sought to prioritise de novo variants in dystonia and characterise the clinical and molecular features associated with the top gene candidate identified after genomic matchmaking. METHODS: We (re)assessed exome-sequencing data for de novo variants in genes with strong mutational constraint in a sample of 257 dystonia trios. Via data sharing, we collected information on individuals with variants in KLC1, encoding a subunit of the axonal-transport moto

Central precocious puberty as the initial manifestation of multisystem involvement caused by de novo heterozygous KMT2B mutation and STS hemizygous deletion: a case report.

BACKGROUND: Pathogenic loss-of-function variants in the KMT2B gene cause a rare autosomal dominant disorder with two major phenotypes: KMT2B-related dystonia (DYT-KMT2B) and KMT2B-related neurodevelopmental disorder (KMT2B-NDD). Central precocious puberty (CPP) has been documented as a comorbidity in patients with DYT-KMT2B, but has not been reported as the primary clinical manifestation of the disorder in patients without dystonia. Deletions or pathogenic variants in the STS gene cause X-linked ichthyosis (XLI). Concurrent KMT2B and STS alterations have not been well characterised, and GnRH a

Regional and Compartmental Changes Drive Progressive Patterns of Striatal Degeneration in X-Linked Dystonia Parkinsonism.

OBJECTIVE: Initially described in 1976, X-linked dystonia parkinsonism (XDP) is a neurodegenerative disease that can be characterized by the presentation of dystonia and parkinsonism symptoms. Although this disease bears some resemblance to other neurodegenerative diseases in terms of symptomatology, the pathological signature of XDP is still unclear, in part because of the limited research that has been conducted. This study aims to delineate in detail the neurochemical changes occurring in the post mortem human XDP striatum and to explore their relevance to clinical symptomatology. METHODS:

Botulinum Toxin Treatment in Musician's Dystonia: Long-Term-Effects and Influencing Factors.

Musician's Dystonia (MD) is a task-specific movement disorder that occurs while playing an instrument and severely limits a musician's ability to perform. While currently no causal therapies are available, symptomatic therapies, particularly the injection of botulinum toxin (BoNT), offer a therapeutic option. The present study presents a 30-year longitudinal evaluation of the clinical course of 156 patients with MD treated with BoNT at a specialist outpatient facility. The evaluation took into account known risk factors, such as family history, time to treatment, initial severity of symptoms,

The heterogeneity of thalamic tremor: report of two cases.

BACKGROUND: Thalamic lesions are a well-recognized cause of secondary tremor, most commonly manifesting as a classic Holmes tremor (HT). This phenotype typically combines postural, kinetic, and rest components and may be accompanied by dystonic features, reflecting disruption of thalamo-cortical motor networks. CASE REPORTS: We report two patients with tremor following unilateral thalamic vascular lesions showing different motor phenotypes. The first patient developed a low-frequency, large-amplitude tremor of the left upper limb after a right lateral thalamic ischemic lesion, consistent with

Child Neurology: Early-Onset, Rapidly Progressive, and Refractory DYT-TOR1A Status Dystonicus.

Primary (genetic) pediatric dystonia, characterized by sustained or intermittent muscle contractions that cause abnormal movements or postures, affects 16.4 in 100,000 children worldwide. It can be caused by various factors, including pathogenic variants in genes such as TOR1A, associated with DYT-TOR1A dystonia. In this report, we present the case of a toddler with DYT-TOR1A treated with deep brain stimulation (DBS) because of his early-onset, rapidly progressive, and refractory status dystonicus. Genetic testing confirmed a maternally inherited pathogenic variant in TOR1A, c.907_909del (p.Gl
