From mild behavioral impairment-checklist (MBI-C) to MBI-distress (MBI-D): a paired assessment and clinical correlates of domain-specific caregiver distress in MCI due to AD.

BACKGROUND: Mild behavioral impairment (MBI) captures later-life onset neuropsychiatric symptoms (NPS) that may herald neurodegeneration. The emotional impact of these early behavioral changes on caregivers is under-measured in pre-dementia care. OBJECTIVE: To develop a brief, domain-aligned caregiver distress scale for MBI (MBI-D) and examine clinical correlates of MBI-related caregiver distress in mild cognitive impairment due to AD (MCI-AD). METHODS: One hundred and four participant-informant dyads with MCI-AD at a Greek memory clinic were included. Caregivers completed the Greek MBI-C an

Molecular Complexities of Dementia: PAISA Mutations and Targeting TAF2N as Therapeutic Avenues.

Millions of individuals worldwide are affected by Alzheimer's disease dementia (ADD) and frontotemporal dementia (FTD), with FTD characterized by degeneration of the frontal and temporal lobes leading to cognitive and behavioral impairments. A subset of Alzheimer's cases exhibits familial inheritance, with the PAISA mutation, a glutamic acid to alanine substitution at codon 280 (E280A) in the PSEN1 gene, being a primary cause of early-onset dementia. PSEN1 encodes a key component of the γ-secretase complex, which cleaves amyloid precursor protein (APP) to generate beta-amyloid (Aβ) peptides. T

Improved polygenic risk prediction for alzheimer's disease and related dementias using deep learning: age and APOE-stratified analysis.

BACKGROUND: Alzheimer’s disease and related dementias (ADRD) are complex, polygenic conditions with substantial public health impact. Accurate genetic risk prediction may enable earlier identification and stratification of individuals at elevated risk. OBJECTIVE: To evaluate the predictive performance of polygenic risk scores (PRS) for ADRD using a Bayesian variational autoencoders approach and to assess the modifying effects of age and APOE genotype on model performance. METHODS: We analyzed data from 276,566 unrelated individuals of European ancestry in the UK Biobank, with a median follow-u

Lack of Oxygen and/or Glucose Differentially Potentiates Aβ40e22q- and Aβ42-Induced Cerebral Endothelial Cell Death, Barrier Dysfunction and Angiogenesis Impairment.

Cerebrovascular damage/dysfunction promote cerebral hypoperfusion early within Alzheimer's Disease (AD). Cerebral hypoperfusion is also a common consequence of cardiovascular risk factors/diseases, typically manifesting in midlife when AD pathology initiates, and contributing to AD onset/progression. We demonstrated that AβQ22 (vasculotropic Dutch mutant) and Aβ42 promote cerebral endothelial cell (cEC) apoptosis, barrier permeability, and angiogenic impairments. Prior research indicates hypoperfusion promotes analogous EC dysfunction. Aβ accumulates within a hypoperfused environment in AD, bu

Sympathetic overactivity-induced type H endothelial cell senescence contributes to the impairment of vascularized osteogenesis by PKM2-mediated glycolysis in preclinical stages of Alzheimer's mice.

Alzheimer's disease (AD) is characterized by early-onset bone loss, yet the underlying mechanisms remain elusive. Here, we demonstrated that sympathetic hyperactivity drives vascularized osteogenesis impairment in preclinical AD through a novel PKM2-glycolysis-mediated bone vascular endothelial cell (EC) senescence pathway. Utilizing systematic transcriptome profiling complemented by in vitro functional assays and in vivo validation studies, we found that norepinephrine (NE)-induced PKM2 downregulation disrupts glycolytic flux, triggering mitochondrial dysfunction-induced senescence (MiDAS) in

The Amyloid Plaque Proteomes of Alzheimer's Disease and Mild Cognitive Impairment.

Amyloid plaques contain numerous proteins in addition to amyloid beta in Alzheimer's disease (AD). Previous localized proteomics identified plaque-associated proteins in late-onset AD, early-onset AD, rapidly progressive AD, preclinical AD, and AD in Down syndrome, although most studies had smaller cohorts and focused primarily on severe pathology. The amyloid plaque proteome in mild cognitive impairment (MCI) has not been evaluated. We evaluated plaque proteomes in MCI and AD with comparisons to neighboring non-plaque tissue and control non-plaque tissue from ROSMAP (151 cases (n = 240 sample

Differential effects of PSEN1 mutations (p.P117L with ataxia and p.P264L without ataxia) in early-onset Alzheimer's disease.

Presenilin 1 (PSEN1) plays a pivotal role in early-onset Alzheimer's disease (EOAD). The clinical phenotype of EOAD is typically marked by cognitive decline, with ataxia rarely reported. We identified mutations at different positions of PSEN1 in two Chinese patients with EOAD. Interestingly, one patient carrying the PSEN1 p.P117L mutation manifested symptoms of ataxia, while another patient harboring the PSEN1 p.P264L mutation did not exhibit any signs of this disorder. Computational analyses using PolyPhen-2, SIFT, Provean, and Mutation Taster predicted both mutations as pathogenic, while str

Socio-economic influences from egg to exit: Emerging biology.

Socio-economic status (SES) impacts life-long health from early development to old age. We review SES differences in biological factors that begin before fertilization and extend into later life, 'egg to exit'. SES gradients are shown in onset and prevalence of chronic conditions including cardiovascular disease, diabetes and dementia, leading up to a 15-year difference in life expectancy in some countries. Our analysis is limited to high-income countries and does not include detailed discussion of gender or ethnic differences. We develop a comprehensive multi-disciplinary framework for SES gr

Clinical features of adolescent-onset functional motor disorders in tertiary movement disorders centers.

BACKGROUND: Functional Motor Disorders (FMDs) represent a diagnostic and therapeutic challenge in pediatric neurology, particularly among adolescents. Their clinical presentation is common but often nonspecific, leading to frequent misdiagnoses and diagnostic delays. We aimed to characterize FMDs in adolescents and to examine the frequency of isolated and combined phenotypes and their associations with demographic and clinical variables. METHODS: In this observational study, data were obtained from the Italian Registry of FMDs, including patients with a clinically definite diagnosis of FMD co

AD diagnosis model based on fusion of heterogeneous brain imaging and genomic data.

Alzheimer's disease (AD) is a common neurodegenerative disorder in the elderly population, and early screening can effectively delay the progression of the disease. Mild cognitive impairment (MCI) occurs prior to the onset of AD; however, the accuracy of existing MCI-to-AD prediction methods remains relatively low. Additionally, small sample sizes and high feature dimensions often lead to model overfitting, highlighting the need for effective early screening approaches. To address the aforementioned issues, this study integrated non-paired multi-modal features-including clinical indicators fro

Dual-tracer PET/CT Imaging using 18F-AV-45 and 18F-AV-1451 for the Diagnosis of Alzheimer's Disease.

Alzheimer's disease (AD) is a progressive neurodegenerative disorder characterized by an insidious onset and gradual deterioration. Clinically, it manifests as cognitive decline and behavioral changes rather than only as generalized dementia. Currently, the diagnostic accuracy of AD remains limited due to the nonspecific clinical manifestations and the low specificity of conventional examinations such as neuropsychological assessments, electroencephalography (EEG), computed tomography (CT), and magnetic resonance imaging (MRI). Positron emission tomography/computed tomography (PET/CT) provides

Neurobiological links between Alzheimer's disease and reward system dysfunction.

Alzheimer's disease (AD) is a highly prevalent progressive neurodegenerative disorder with unclear etiology, complex symptoms, and limited treatment options. Early pathological processes in AD emerge long before the onset of overt cognitive and motor symptoms and involve the accumulation of amyloid-β oligomers and neurofibrillary tangles, accompanied by neuroinflammation and neuronal loss. Importantly, the brain reward system comprises cortical and subcortical structures that share neurochemical pathways and reciprocal connectivity with regions affected during AD progression. Consistent with t

Early-onset axonal pathology and β-amyloidosis in human brains with hematological malignances and cardiovascular diseases.

β-Amyloid (Aβ) and tau pathologies are hallmark lesions of Alzheimer's disease (AD) and they develop in human brain following differential spatiotemporal trajectories. Accordingly, young/adult-onset tau-independent β-amyloidosis is rare. We encountered four such cases among 397 banked brains, with the donors died of hematological malignances (blood cancers) or cardiovascular diseases. To explore the pathological implication, we examined 17 brains (10-87 year-old, y) from blood cancer patients and three (52-82 y) with cardiovascular diseases, focusing on vascular injury, axonal pathology and Aβ

Sex differences in diffusion-weighted imaging outcomes in autosomal dominant Alzheimer's disease.

Alzheimer's disease is characterized by amyloid-β and tau protein accumulation. Growing evidence suggests that white matter degeneration contributes to disease progression. Despite Alzheimer's disease being more prevalent in women, understanding of sex differences in white matter microstructure across the Alzheimer's disease continuum remains limited. This study investigated sex-specific patterns of white matter integrity in individuals genetically predisposed to autosomal dominant Alzheimer's disease. We analysed data from 63 individuals (30 presenilin-1 glutamic acid to Alanine at codon 280

Posterior Cortical Atrophy in the Asia-Pacific: A Report From the PCA Asian Workgroup.

OBJECTIVE: Posterior Cortical Atrophy (PCA) is a distinct dementia syndrome primarily affecting spatial abilities and visual processing. It is associated with degeneration in the posterior part of the brain. PCA is subclassified into PCA-pure and PCA-plus syndromes based on consensus criteria. To address this, the PCA Asia Workgroup was established to study PCA in Asian countries. METHODS: The Asia PCA Workgroup collected demographic and clinical data, including symptoms and signs, from PCA patients. Patients were classified into PCA-pure (solely meeting PCA criteria) and PCA-plus (additional

Quantitative pathology and APOE genotype reveal dementia risk and progression in Lewy body disease.

Dementia in Lewy body diseases (LBD) is common and arises through heterogeneous and incompletely understood pathways. Evidence suggests contributions from genetic factors, including APOE ε4 genotype, co-pathology including concomitant Alzheimer's disease pathology and hypoperfusion related to orthostatic hypotension. However, the relative impact of these factors remains unclear. To address this, we analysed 399 post-mortem brains from LBD cases comprising Parkinson's disease, Parkinson's disease dementia and dementia with Lewy bodies, and controls, integrating APOE genotype, clinical data and

Progress in Machine Learning-Assisted Biosensors for Alzheimer's Disease.

Alzheimer's disease (AD) is the most common cause of dementia, affecting 55 million people worldwide. Its characteristics include the accumulation of senile plaques and neurofibrillary tangles. This disease is associated with changes in the concentration of AD biomarkers, such as microRNAs, amyloid peptides, Tau protein, and neurofilament light chains. Due to the fact that neuropathological processes begin decades before the onset of cognitive symptoms, accurate detection of AD biomarkers is crucial for its early diagnosis. The combination of analytical techniques and machine learning methods

A Two-Stage Framework for Early Detection and Subtype Identification of Alzheimer's Disease Through Multimodal Biomarker Extraction and Improved GCN.

BACKGROUND: Imaging-transcriptomic analysis, through the integration of multimodal magnetic resonance imaging (MRI) and transcriptomic data, provides complementary structural, functional, and molecular information that is crucial for the early detection and mechanistic exploration of Alzheimer's disease (AD). However, effectively extracting features from heterogeneous multimodal data and capturing the associations between microscopic molecular variations and macroscopic brain alterations remain key challenges. Recent advances in deep learning and multimodal integration have enhanced the abilit

Microglia-mediated protection against Alzheimer's disease pathology and detrimental effects in white matter revealed by Ptpn6 deletion.

Genetic variants affecting microglia can cause early-onset neurodegeneration or elevate Alzheimer's disease risk. To nominate regulators of relevant signaling pathways, we developed a genome-wide CRISPR screen in primary macrophages focused on survival. We identified Ptpn6, which encodes the inhibitory phosphatase SHP-1, as a crucial regulator for macrophage survival under reduced CSF1R signaling conditions in vitro. Deletion of Ptpn6 from adult microglia in vivo enhanced survival and decreased neuritic dystrophy around amyloid plaques in the TauPS2APP model of Alzheimer's disease. However, de
