[Olfactory Nerve: The Vulnerability Inherent in its Unique System and Neurological Diseases].

The olfactory nerve possesses unique anatomical features, including direct central nervous system (CNS) projection and continuous regeneration. Scientific advances have elucidated mechanisms such as combinatorial receptor coding and signal amplification. This review summarizes these foundations and examines olfactory dysfunction in COVID-19 and Parkinson's disease (PD). In COVID-19, evidence suggests that SARS-CoV-2 targets sustentacular cells rather than olfactory neurons, causing gene downregulation and parosmia attributed to incomplete peripheral filtering, while direct CNS invasion remains

A 3-N nose-to-brain urolithin a nanomotor targeting microglial mitophagy in neuroinflammation.

Cognitive impairment is the primary manifestation of neuroinflammation-related central nervous system diseases. Intranasal administration is an effective method, bypassing the blood-brain barrier and delivering drugs to the brain. Herein, we designed a biomimetic self-propelled nanomotor with an inflammation-targeting capacity. This nanomotor comprised a hollow mesoporous manganese dioxide (HMnO2) core and a polydopamine (PDA) shell. HMnO2 effectively catalyzed the conversion of endogenous H2O2 into H2O and O2, enabling the movement of the nanomotor into a wider area to reduce neuroinflammatio

Systematic review of literature regarding the isolation of mesenchymal adult stem cells from the olfactory epithelium.

BACKGROUND: The olfactory mucosa has emerged as a promising source of mesenchymal stem cells with neurogenic potential. These cells exhibit neural, glial, and mesenchymal properties, making them attractive candidates for regenerative medicine, particularly in treating neurodegenerative and immunemediated disorders. METHODS: This systematic review analyzed existing literature on the isolation, characterization, and therapeutic applications of olfactory mucosa mesenchymal stem cells. The review assessed variations in isolation techniques, culture conditions, and differentiation potential, as we

Chitosan-Based Nanoparticles for Nose-to-Brain Drug Delivery: A Real Path toward Effective CNS Therapy?

Treating central nervous system (CNS) disorders remains a major clinical challenge. The blood-brain barrier (BBB), systemic toxicity, and first-pass metabolism are key obstacles. These factors limit the effective drug delivery to the brain. Intranasal administration has emerged as a noninvasive strategy to bypass the BBB. This approach enables direct drug delivery to the brain through the olfactory and trigeminal nerve pathways, commonly referred to as nose-to-brain (N2B) delivery. In this context, chitosan (CS), a biocompatible and mucoadhesive polysaccharide with permeation-enhancing propert

Alpha-band fronto-limbic hyperconnectivity in adolescents with major depression and suicidal ideation.

BACKGROUND: Adolescents with major depressive disorder (MDD) are at high risk for suicide, but the neurophysiological correlates of suicidal ideation remain unclear. Resting-state EEG functional connectivity offers a promising approach to investigating brain network alterations associated with suicide risk. METHODS: We recruited first-episode, treatment-naïve adolescents with MDD with suicidal ideation (SI-MDD, n = 30), without suicidal ideation (NSI-MDD, n = 38), and healthy controls (HC, n = 33). Resting-state EEG was used to construct source-level functional connectivity networks based on t

Advanced Sensing and Delivery Technologies for Nose-to-Brain Administration: From Nanocarriers to Sensor-Integrated Organ-on-Chips.

Central nervous system (CNS) disorders represent a growing healthcare burden, and various drugs are developed for their treatment. However, the blood-brain barrier (BBB) prevents over 98% of therapeutics from reaching brain tissue. Intranasal delivery provides a promising alternative by exploiting olfactory and trigeminal nerve pathways to circumvent the BBB. This review surveys recent advances in nose-to-brain delivery technologies, from carrier design to evaluation methods. Polymeric and lipid-based nanocarriers show enhanced mucosal penetration and prolonged residence time, and microneedle

SHANK3 mutations disrupt olfactory valence coding across species, with cortical amygdala mechanisms identified in mice.

Mutations in SHANK3 are a leading monogenic cause of autism spectrum disorder (ASD), often associated with profound sensory abnormalities. However, the impact of SHANK3 deficiency on olfactory processing and the underlying neural mechanisms remains unclear. Here, we identify a cross-species disruption of olfactory valence perception in individuals with SHANK3 mutations and in Shank3 mutant mice. Patients carrying SHANK3 mutations exhibited impaired valence-oriented sniffing and electroencephalography (EEG) responses, whereas Shank3B-/- mice displayed blunted behavioral responses to both attrac

Anosmia in a patient with Silver-Russell syndrome.

The human sense of smell is important for the awareness of danger like gas, fire or spoiled food, perception of the flavours of food and drink, and social interaction. The impact of limited olfactory capacity or total loss of the sense of smell may be associated with decline in the quality of life and depressive symptoms. Various conditions can be the cause of olfactory disorders, including congenital absence of the olfactory nerve. We present a boy with Silver-Russell syndrome suffering from anosmia.

Connectivity of serotonin neurons reveals a constrained inhibitory subnetwork within the olfactory system.

Neuromodulators flexibly adjust how sensory information is processed by acting upon a diverse set of receptors expressed by different neuronal types within a given network. The morphological and physiological traits of individual inhibitory local interneuron (LN) types, as well as their connectivity within sensory networks, enable each LN type to support different computations and are therefore ideal targets for modulatory neurons to have widespread impacts on network activity. In this study, we combined detailed connectivity analyses, serotonin receptor expression, neurophysiology, and comput

Heritability of the olfactory bulb and its associated brain network.

The olfactory bulb (OB), the first central relay of the olfactory pathway, plays a critical role in odor perception and exhibits remarkable structural plasticity shaped by environmental influences. This raises a fundamental question about the extent to which OB volume and its integration within the wider olfactory network is genetically determined. While both OB volume and function have been implicated in a range of neurodegenerative and neuropsychiatric disorders, such as Parkinson's disease, schizophrenia, and depression, all of which involve known genetic risk factors, the heritability of t

Rethinking early Alzheimer's disease: An impaired internal model updating disorder?

Although sporadic Alzheimer's disease (AD) has long been conceptualized primarily within a molecular framework centered on amyloid-β and hyperphosphorylated tau pathology, this perspective does not fully explain the early vulnerability of specific brain networks or the emergence of non-cognitive symptoms, even decades before memory decline. The present review directly addresses this question by proposing an integrative, systems-level, translational framework that links age-related sensory dysfunction-most notably olfactory impairment-to the failure of predictive sensorimotor internal models an

Targeting Sensory Neuropathies Through Neurotrophin-Based Approaches.

Neurotrophins, including NGF, BDNF, and NT-3, are crucial regulators of neuronal survival, synaptic plasticity, and regeneration in sensory systems. Emerging evidence suggests that neurotrophin signaling may represent a common biological axis underlying both sensory neuron vulnerability and repair across auditory, visual, and olfactory systems. This review examines the roles of neurotrophins in maintaining neuronal integrity, promoting synaptic repair, and facilitating functional recovery in sensory pathways. We delved into the molecular mechanisms by which neurotrophins influence neuronal sur

Localized amyloid-β oligomers in the lateral entorhinal cortex drives olfactory dysfunction through aberrant neuronal and network activity.

Olfactory dysfunction is one of the most common early features of Alzheimer's disease (AD), yet its underlying neural mechanisms remains unclear. The lateral entorhinal cortex (LEC), a central node in the olfactory network, is among the earliest regions affected by AD pathology. However, the direct evidence supporting a causal link between LEC dysfunction and olfactory disorders in early AD is limited. In the present study, we explored whether and how LEC dysfunction contributes to early AD olfactory dysfunction by injecting amyloid-β1-42 (Aβ1-42) oligomers into the LEC to induce localized Aβ

The Cribriform Plate: A Multifaceted Neuroimmune Hub in CNS Health and Disease.

The cribriform plate (CP) functions as a dynamic neuroimmune interface through which olfactory nerve bundles exit the brain within a specialized perineural microenvironment (cpPME). While traditionally viewed as a passive structural barrier, emerging evidence positions the CP as a central hub for cerebrospinal fluid (CSF) drainage, glymphatic-lymphatic clearance, and antigen presentation. This review provides a comprehensive understanding of recent advances in cpPME research, highlighting the adaptive remodeling of the immune landscape in response to neuroinflammation and aging.
