Polyamines, cardiorespiratory fitness, body composition, and mitochondrial function in older patients with coronary artery disease.

AIMS: Higher dietary spermidine intake has been associated with lower cardiovascular mortality. Spermidine and spermine support cellular homeostasis and mitochondrial function across species, but their relevance to physical and mitochondrial function in humans remains unclear. We examined associations of dietary spermidine and circulating and skeletal muscle spermidine and spermine with physical performance, body composition, and mitochondrial respiratory capacity in older patients with coronary artery disease. METHODS AND RESULTS: We included 187 patients with coronary artery disease [median

Managing growth and puberty concerns in individuals with Duchenne muscular dystrophy: Updated guidance and future directions.

Long-term glucocorticoid therapy is standard of care for individuals with Duchenne muscular dystrophy (DMD) but often results in impaired growth and delayed or incomplete puberty, impacting their physical and psychosocial health. Although DMD endocrine care has evolved, leading to inclusion in the 2018 Care Considerations, these issues remain prevalent and concerning, and gaps in care persist. The Growth and Puberty Working Group of the OPTIMIZE DMD Consortium, an international multidisciplinary group including endocrinology and neuromuscular specialists with DMD expertise, conducted a literat

Apoptosis Repressor with Caspase Recruitment Domain in Uncommon Epidermal Growth Factor Receptor-Mutant Lung Adenocarcinoma: An Aging Hypothesis.

Uncommon epidermal growth factor receptor (EGFR)-mutant lung adenocarcinoma comprises clinically and biologically heterogeneous subgroups with variable sensitivity to targeted therapy. Incomplete responses and eventual progression suggest that factors beyond receptor inhibition influence disease persistence during therapy. This hypothesis-generating narrative review examines whether apoptosis repressor with caspase recruitment domain (ARC), encoded by nucleolar protein 3 (NOL3), warrants investigation as a downstream node in the regulation of apoptosis within an aging-informed framework. The a

Nuclear blebs are composed of variable chromatin states and display increased transcription initiation relative to elongation.

Nuclear blebs are herniations of the nucleus that occur in many human conditions including aging, heart disease, muscular dystrophy, and many cancers. Nuclear blebbing causes nuclear rupture and cellular dysfunction. However, understanding the formation, stability, and identification of nuclear blebs remains an ongoing challenge. Our previous studies reveal that nuclear blebs are best hallmarked by decreased DNA density. To determine if chromatin decompaction underlies decreased DNA density in nuclear blebs, we investigated the histone composition of nuclear blebs across multiple cell lines. T

Atrial Standstill: From Molecular Genetics to Clinical Management - A Narrative Review.

Atrial standstill (AS) is the complete absence of atrial electrical and mechanical activity caused by diffuse atrial myocardial disease, distinct from sinus node dysfunction. First documented in 1897 and often misdiagnosed as atrial fibrillation, it arises from SCN5A loss-of-function, NPPA, or EMD/LMNA mutations, muscular dystrophies, and infiltrative or idiopathic fibrotic processes. Diagnosis requires absent P waves, absent atrial electrograms, and failure of atrial capture at maximal pacing output, with late gadolinium enhancement MRI and electroanatomic mapping demonstrating diffuse fibros

Case Report: Duchenne muscular dystrophy-associated cardiomyopathy presenting as adolescent nonischemic heart failure-from red-flag clues to genetic confirmation and multisystem management.

BACKGROUND: Duchenne muscular dystrophy (DMD) can involve the myocardium and progress to cardiomyopathy and heart failure; however, diagnosis is frequently delayed because initial clinical manifestations are often atypical. In adolescent males presenting with nonischemic cardiomyopathy of unclear etiology, early integration of a coherent evidence chain spanning neuromuscular clues, multimodality imaging, and genetic testing is pivotal for etiologic clarification and optimization of long-term management. CASE SUMMARY: The patient was a 17-year-old adolescent male who presented repeatedly with

Nasopharyngeal Leiomyosarcoma: Diagnostic Challenges and Management of a Rare Head and Neck Sarcoma.

Leiomyosarcoma (LMS) is a rare, smooth-muscle malignancy and an uncommon sinonasal tumor, often diagnosed at a late stage due to non-specific symptoms and complex anatomy. We report an 82-year-old man with a two-month history of facial pain, epistaxis, and right-sided hearing loss. Imaging and endoscopic evaluation showed a friable mass involving the posterior nasal cavity and nasopharynx. Biopsy confirmed a giant cell-rich spindle cell LMS. He underwent endoscopic resection with positive margins. Postoperative complications and comorbidities delayed the initiation of adjuvant radiation therap

Challenging Genetic Diagnosis in Siblings With Primordial Dwarfism, Muscle Wasting, Osteogenesis Imperfecta, Craniofacial Dysmorphism, and Ectodermal Anomalies.

Three siblings of consanguineous Pakistani parents have primordial dwarfism with microcephaly, failure to thrive, delayed developmental milestones, thin and lean physique, dysplastic limb joints, ectodermal anomalies, and craniofacial anomalies including dolichocephaly, micrognathia, and irregular dentition. Variable findings are muscular dystrophy and osteogenesis imperfecta. We hypothesized a novel syndrome and launched a disease gene search. At the gene locus identified by single-nucleotide polymorphism-based linkage mapping, genome sequencing detected a very rare intergenic variant g.13276

Co-Occurrence of Facioscapulohumeral Muscular Dystrophy and Myasthenia Gravis: A Systematic Review and Meta-Analysis of Individual Patient Data.

BACKGROUND: Co-occurrence of facioscapulohumeral muscular dystrophy (FSHD) and myasthenia gravis (MG) is rare but clinically critical, as treatable MG symptoms may be misattributed to FSHD progression. This systematic review and meta-analysis aimed to comprehensively characterize patients with both conditions and to quantitatively synthesize the available individual patient data. METHODS: Following PRISMA 2020 guidelines, we searched four databases until 3 April 2026 for original reports providing individual-patient data on confirmed FSHD and MG co-occurrence. We performed descriptive and qua

The Role of TXNIP in Skeletal Muscle Physiology and Related Diseases: Current Perspectives.

Thioredoxin-interacting protein (TXNIP) is a multifunctional regulatory protein of the α-arrestin family that acts as a redox sensor, an energy-responsive molecule, and a transcription-associated regulator. TXNIP has emerged as a key regulator of skeletal muscle homeostasis and has been implicated in the pathogenesis of multiple muscle disorders. Skeletal muscle is a principal site of systemic glucose disposal, accounting for more than 70% of insulin-stimulated glucose uptake in the human body. TXNIP has been implicated in sarcopenia, obesity-related insulin resistance, limb ischemia-induced m

Reduced Lumbar Lordosis in Fibromyalgia: Radiographic Evidence of Lumbar Straightening by L1-L5 Cobb Angle.

BACKGROUND: Reduced lumbar lordosis has been observed in patients with low back pain, but lumbar straightening has not been well characterized in fibromyalgia. We hypothesized that fibromyalgia would be associated with reduced lumbar curvature on standing radiographs. OBJECTIVE: To evaluate lumbar lordosis, quantified by the L1-L5 Cobb angle, in patients with fibromyalgia compared with patients with other rheumatic diseases and low back pain. DESIGN: Retrospective, cross-sectional observational study. SETTING: Rheumatology office practice. PARTICIPANTS: A consecutive series of 245 patients

TTN-related limb-girdle muscular dystrophy caused by a homozygous c.107837A>C variant: first report in an African patient.

Tibial muscular dystrophy (TMD) is a late-onset autosomal dominant distal myopathy initially described in the Finnish population and caused by monoallelic, mostly missense, TTN variants. More recently, earlier-onset limb girdle muscular dystrophy phenotypes have been reported in patients carrying biallelic TTN variants. We describe the first African patient homozygous for a previously described TMD related TTN variant presenting with a limb-girdle muscular dystrophy (LGMD) phenotype. The proband, an ambulant 11-year-old boy originally from Libya, presented with progressive lower limb weakness

Combined qualitative and quantitative MRI reveals short-term progression in facioscapulohumeral muscular dystrophy.

Facioscapulohumeral muscular dystrophy (FSHD) is characterized by slowly progressive and heterogeneous muscle involvement; therefore, clinical measures show limited sensitivity to short-term change. Magnetic resonance imaging (MRI) provides useful biomarkers of muscle degeneration and disease activity. To investigate one-year longitudinal changes in lower leg muscles of patients with FSHD using qualitative (T1-weighted [T1w], STIR) and quantitative MRI (fat fraction [FF], water T2), and to assess their relationship with clinical function. Ebselen restores mitochondrial function through metabolic rewiring in Barth syndrome and related mitochondrial diseases.

Despite major advances in the genetic diagnosis of mitochondrial disorders, effective disease-modifying therapies remain scarce. Here, we identify ebselen (EBS) as a promising therapeutic candidate through a phenotypic drug screen in a yeast model of Barth syndrome, a cardiomyopathy caused by defective cardiolipin maturation. EBS improved oxidative phosphorylation-dependent growth across diverse fungal models of mitochondrial diseases, including defects in complex I, complex IV, mitochondrial DNA maintenance, mitochondrial translation, and ATP synthase. Therapeutic efficacy was further validat

Immune effects of faecal microbiota transplantation in amyotrophic lateral sclerosis: a pilot multicentre, randomised, masked, sham-controlled, proof-of concept trial.

BACKGROUND: The gut microbiota modulates peripheral immunity, which is altered in amyotrophic lateral sclerosis (ALS). Faecal microbiota transplantation (FMT) may exert immunological effects, but its impact in ALS remains unclear. METHODS: FETR-ALS was a multicentre, randomised, masked, sham-controlled proof-of-concept trial conducted at four Italian ALS centres. Adults with ALS of 18 months duration or less and forced vital capacity of at least 70% predicted were randomly assigned (2:1) to receive FMT or a sham procedure (SP). The intervention consisted of two nasojejunal administrations of

AAVcc47-μDys Gene Therapy Ameliorates Respiratory Pathology in mdx Mouse Model of Duchenne Muscular Dystrophy.

Duchenne muscular dystrophy (DMD) is a progressive X-linked neuromuscular disorder caused by deficiency of dystrophin - a protein essential for muscle integrity. Loss of dystrophin results in global muscle weakness, including in the muscles of respiration. As a result, respiratory failure is a leading cause of morbidity and mortality in patients with DMD. Gene therapy using adeno-associated virus (AAV) is ideal for monogenetic diseases. However, the dystrophin gene is too large to be packaged into AAV; therefore, microdystrophin (μDys) containing important elements of the dystrophin gene is us

Identifying regulatory muscle processes in infantile and adult Pompe patients using a multi-omic and morphological approach.

Pompe disease is a lysosomal glycogen storage disorder caused by a deficiency of the enzyme acid alpha-glucosidase (GAA). There are two different clinical phenotypes, which are based on the age of onset, GAA variants, and residual enzyme activity. To better understand the regulation of age-dependent pathways in Pompe disease, we performed a multi-modal approach. We included mass spectrometry (MS) (liquid chromatography coupled to tandem mass spectrometry) and bulk-RNA sequencing on skeletal muscle samples from patients with late onset Pompe disease (LOPD) (n = 21) and infantile-onset Pompe dis

Advances in Biomarkers for Muscular Dystrophies: a Comprehensive Review.

BACKGROUND: Muscular dystrophies (MDs) are a group of genetically heterogeneous diseases characterized by progressive muscle degeneration and functional impairment. Current treatment options remain limited, highlighting the urgent need for biomarkers to enhance diagnosis, monitor disease progression, and evaluate treatment efficacy across different subtypes, such as Duchenne muscular dystrophy (DMD), Becker muscular dystrophy (BMD), facioscapulohumeral muscular dystrophy (FSHD), and limb-girdle muscular dystrophy (LGMD). METHODS: A search of the MEDLINE/PubMed databases was conducted for orig

Use of split intein and myotropic AAV vectors enables effective preclinical gene therapy for dysferlinopathy.

Limb-girdle muscular dystrophy type R2 (LGMDR2) is a genetic disorder characterized by progressive skeletal muscle weakness and degeneration. It is caused by mutations in the DYSF gene, which prevent dysferlin expression. Dysferlin protein is essential for muscle cell membrane repair. Gene therapy using adeno-associated viral (AAV) vectors to restore full-length dysferlin expression is a promising therapeutic approach for LGMDR2. However, the large size of the dysferlin cDNA (6.2 kilobases) exceeds the ∼4.7-kilobase packaging capacity of AAV vectors, posing a challenge to effective delivery of
