Patterns of recurrence in idiopathic orbital myositis.

PURPOSE: To characterise patterns of disease recurrence in idiopathic orbital myositis (IOM). METHODS: Multi-centre retrospective longitudinal study of IOM patients. Serial imaging was also analysed. Patients with incomplete clinical records and specific orbital myositis (e.g. thyroid-associated ophthalmopathy) were excluded. RESULTS: Thirty-three patients (Female: 18, mean age: 40.8 ± 16.8-years-old) presenting between 2001 and 2023. Twelve (36.4%) patients had disease recurrence. There was no difference between age or sex predilection of patients with and without recurrence (P = 0.107 and

Posterior localized nodular scleritis mimicking malignancy, a case report and clinical approach.

Aim: To report a patient with choroidal bulging, sub-retinal fluid, and optic nerve head (ONH) swelling who was finally diagnosed with focal nodular posterior scleritis. Case report: A 51-year-old male patient presented to us with acute painful visual loss of his left eye (LE) from 3 days ago. The best-corrected distance visual acuity (BCDVA) was 20/20 and hand motion (HM) for the right eye (RE) and LE, respectively. Fundus examination of the LE showed ONH swelling, choroidal bulging, multiple patches of subretinal fluid accumulation, and retinal pigment epithelial (RPE) corrugations. Orbital

Dog Bites With Resultant Periorbital Cellulitis.

This case study examines the delayed presentation of a facial dog bite, an uncommon occurrence in the periorbital area. Initially perceived as a minor injury, the wound was disregarded in its early stages. However, as the infection progressed, prompting urgent evaluation in the emergency department (ED) as the severity became evident. The patient was diagnosed with an acute facial infection carrying the potential for periorbital cellulitis and abscess formation, necessitating immediate imaging. Prompt evaluation of penetrating animal bites, particularly dog bites, is crucial due to their high

Long-term Outcomes of a Minimally Invasive Surgical Approach in Paediatric Patients with Congenital Dacryocystoceles.

INTRODUCTION: Congenital dacryocystoceles are a rare condition caused by nasolacrimal duct obstruction. Symptoms include epiphora, nasal obstruction, and swelling in the medial canthus. Treatment usually entails probing Hasner's valve open and, if necessary, intubating the nasolacrimal duct. We present a minimally invasive, endoscopic procedure with marsupialisation of the endonasal portion of the cele. The operation avoids additional manipulation of the lacrimal duct to prevent iatrogenic injury. METHODS: This retrospective analysis included a total of nineteen infants or young children (21

Value of Platelet-to-Lymphocyte and Monocyte-to-Lymphocyte Ratios in Differentiating Idiopathic Orbital Inflammation From Orbital Infectious Disease.

PURPOSE: Monocyte-lymphocyte ratio (MLR) and platelet-lymphocyte ratio (PLR) are emerging biomarkers; they have not been widely utilized in orbital disease. This study investigated the role of these ratios in distinguishing orbital inflammation from infection. METHODS: A retrospective review of medical records was conducted to identify adult patients who presented acutely to a single emergency department at an academic medical center and were diagnosed with serologically and biopsy-proven idiopathic orbital inflammation (IOI, n = 9), orbital cellulitis (OC, n = 14), or necrotizing fasciitis (

Complications of acute rhinossinusitis. A clinical radiological review.

Acute rhinossinusitis is defined as a symptomatic inflammation of the nasal fossa and paranasal sinuses. The diagnosis of this disease is clinical and usually does not require imaging evaluation. However, when there is a suspicion of a complication or even for surgical planning, imaging is of primordial importance. The aim of this paper is to provide a concise pictorial review with clinical and imagiological correlation of the most common complications in acute rhinossinusitis to provide relevant clinical data and highlight the most important imaging findings.

Guidelines for the standardized diagnosis and treatment of non-specific orbital inflammation (2024).

Non-specific orbital inflammation (NSOI) is a non-infectious orbital inflammation. Although it is often considered the most common diagnosis in orbital biopsies, it is an exclusionary diagnosis that requires ruling out systemic disease or other possible causes. Its characteristics include acute orbital signs and symptoms, including pain, proptosis, periorbital edema, chemosis, diplopia, and visual impairment. The clinical manifestations and histological findings of NSOI are heterogeneous, without specific diagnostic criteria or treatment guidelines, which poses significant challenges for diagn

Probable IgG4-related Orbital Disease Masked by Exuberant Ocular Surface Pseudoepitheliomatous Hyperplasia.

A 40-year-old woman presented with a mass in her OS for 2 years. Examination revealed a large conjunctival lesion on the nasal bulbar conjunctiva OS and a small upper tarsal conjunctival lesion in the OD. Biopsy OD revealed inflammatory granulation tissue, and OS revealed pseudoepitheliomatous hyperplasia with granulation tissue. The lesion responded well to steroids. She was lost to follow-up and returned with an enlarged lesion with orbital extension and corneal perforation. Orbital exenteration was performed. The entire conjunctiva showed extensive epithelial downgrowth and mononuclear roun

[Differential diagnosis of acute maxillary ethmoiditis complicated by orbital abscess with dacryocystitis and phlegmon of the lacrimal sac in children].

INTRODUCTION: The differential diagnosis of orbital complications of rhinosinusitis with diseases of the lacrimal sac in childhood remains unresolved both due to the similarity of the symptoms of the diseases and due to certain diagnostic difficulties requiring computed tomography. OBJECTIVE: To develop an algorithm for routing a patient to the emergency department of a multidisciplinary emergency hospital based on clinical and diagnostic distinctive features of sinusitis with orbital complications and diseases of the lacrimal sac with reactive edema of the eyelids. Association of Suicidal Status, Inflammation Markers, and Resting-State Functional Activity and Connectivity in Patients With Major Depressive Disorder.

Abstract. Background: This study aimed to identify (1) neural markers of suicide attempt using resting-state functional magnetic resonance imaging (rs-fMRI) and (2) associations between rs-fMRI metrics and resting-state functional connectivity (rs-FC), suicidal phenotype, and peripheral blood inflammation markers. Methods: Inflammation markers (C-reactive protein [CRP], interleukin [IL]-1β, IL-2, and IL-6, and tumor necrosis factor-α TNF-α) and rs-fMRI metrics were measured in 20 healthy controls (HCs) and 42 patients with unipolar depression according to the DSM-5 criteria (n = 21 suicide att

An Evidence-Based Approach to Diagnosing Patients Presenting With Orbital Inflammatory Syndromes (An American Ophthalmological Society Thesis).

PURPOSE: The initial management of patients presenting with orbital inflammatory syndromes varies-with some pursuing a diagnostic workup while others treat without any testing. The evidence behind this rationale is limited. A review of patients with orbital inflammation is performed with an analysis of diagnostic testing to develop a workflow for such patients. DESIGN: A retrospective review of patients presenting with acute orbital inflammation at a single institution from 2011 to 2020 was conducted. METHODS: Orbital imaging was required for inclusion. Patients with high suspicion of neopla

Treatment of Idiopathic Inflammatory Orbital Syndrome (IOIS) With Prominent Lacrimal Gland Involvement Using Oral Non-Steroidal Anti-Inflammatory Drugs (NSAIDs).

Idiopathic orbital inflammatory syndrome (IOIS) is a chronic inflammatory process of unknown etiology, which can either be localized or diffuse. In cases where there is isolated inflammation of the lacrimal gland, it is known as dacryoadenitis. This study focuses on the treatment of a patient with IOIS with prominent lacrimal gland involvement. The mainstay of treatment for idiopathic isolated dacryoadenitis is oral corticosteroids, but non-steroidal anti-inflammatory drug (NSAIDs) is known to be effective in treating idiopathic dacryoadenitis as well. There is no formal study yet to evaluate

Rapid progression of orbital abscess requiring lateral canthotomy in a pediatric patient.

Subperiosteal abscess (SPA) is a rare complication of acute sinusitis in children that may develop rapidly. In this case report, we describe an 11 year-old boy who presented with a large SPA 2 days after being diagnosed with conjunctivitis. The patient required emergent lateral canthotomy and cantholysis (LCC), IV antibiotics, and emergent surgery. It is crucial that emergency physicians be able to identify and treat this vision-threatening complication.

Posterior scleritis following a single infusion of zoledronate for osteoporosis: A case report and literature review.

The most frequently reported adverse reaction to zoledronic acid is an acute phase reaction resembling influenza. While rarer adverse events such as osteonecrosis of the jaw and atypical femoral fractures have gained significant recognition, the ocular adverse effects, particularly scleritis, are not yet fully comprehended. Here, we present the case of a 75-year-old female patient with osteoporosis who developed bilateral redness and intense eye pain 48 h after receiving a 5 mg intravenous dose of zoledronic acid. Clinical presentation suggested bilateral conjunctivitis, but treatment with lev

Varicella mimicking complications of acute rhinosinusitis in an infant.

Varicella is the manifestation of primary infection with the varicella-zoster virus, mainly affecting preschool and school-aged children. The children suffer from a generalised, vesicular rash and fever. Despite the infection's typically non-threatening course, a variety of severe complications have been described. The authors present the case of a female infant suffering from varicella and developing preseptal cellulitis with a frontal abscess while being treated with intravenous antibiotics. Otorhinolaryngology consultation was sought since the clinical image was highly suggestive for sinusit

Acute Myelin Oligodendrocyte Glycoprotein-IgG Optic Neuritis without Optic Nerve Enhancement.

INTRODUCTION: Optic neuritis (ON) is a common neuro-ophthalmological condition and remains a significant indicator of inflammatory conditions affecting the central nervous system. Varying etiologies exist for ON including multiple sclerosis, NMOSD, and MOGAD. Differential diagnosis is achieved using both radiological and serological testing. MRI characteristics of MOG-ON include T2 hypersensitive lesions, nerve swelling, and gadolinium enhancement of the affected optic nerve. While acute MOG-ON usually presents with optic nerve enhancement, recognizing atypical presentations is critical in acc

Systemic sarcoidosis presenting as acute rapidly progressive proptosis.

Sarcoidosis is a generalized systemic chronic inflammation that rarely involves the orbit. As a chronic inflammation, sarcoidosis typically manifests with an insidious onset and slowly progressive course. We report a case of acute-onset proptosis resulting from a rapidly growing diffuse orbital mass that simulated malignant growth, which was biopsy proven to be the first manifestation of systemic sarcoidosis. The patient demonstrated complete resolution of proptosis and systemic involvement with long-term corticosteroid treatment.

Osteoradionecrosis of the lamina papyracea leading to recurrent orbital infections-A case study.

Osteoradionecrosis (ORN) is a well-known complication of radiotherapy (RT) to the sino-nasal cavity and nasopharynx. Here, we report a case of recurrent orbital infections secondary to ORN of the lamina papyracea (LP). A 66-year-old female presented to our unit with right periorbital swelling and pain after having undergone chemotherapy and proton beam irradiation to her ethmoid sinuses for sino-nasal undifferentiated carcinoma (SNUC) 5 years prior. She had also undergone endoscopic sinus surgery for chronic rhinosinusitis about a year prior to the current presentation. Her post-operative cour
