The etiology, clinical picture, diagnosis and treatment of chronic inflammatory diseases of the conjunctiva are described. It is emphasized that, despite the respective specific etiology, the symptomatology has common factors, treatment is difficult and all too often only symptomatic, and chronic inflammatory diseases of the conjunctiva sooner or later lead to the so-called dry eye syndrome.

A follow-up study of chronic conjunctivitis in arc welders in a foundry in Italy was conducted from 1979 up to 1989. Airborne dust concentration in the foundry often exceeded the TLV-TWA limit. The frequency of chronic conjunctivitis was markedly higher amongst the welders than in the unexposed controls (RR = 4.25). The incidence rate of conjunctivitis was 49.8% in the exposed group; manual welders showed a higher incidence rate (56.9%) than non manual welders (risk ratio = 1.76). The authors used immunofluorescence and immunoperoxidase tests to study a group of 101 patients with acute or chronic conjunctivitis, etiologically unrelated to conventional bacterial pathogens, and a control group of 30 healthy adults. Positive titers of IgG in serum and of IgA in lacrimal secretions against Chlamydia, detected by IPA, correlated with the identification of microorganisms by direct immunofluorescence. The use of both tests allows a precise evaluation of the stage of the infection and of its evolutive pattern.

This retrospective review of 111 patients with ocular cicatricial pemphigoid (CP) identifies 29 patients (26%) with glaucoma. Twenty-seven of these patients had a history of glaucoma for a mean of 11.3 years before the diagnosis of CP was made. Most had advanced glaucoma, with a long history of medication use, optic nerve damage, and visual field loss. Patients with CP and glaucoma were more likely to manifest continued high-grade conjunctival inflammation than those with CP alone despite treatment for CP (P less than 0.05). The studies involved 254 workers of a Precise Devices Plant. The impact of working conditions (chemical pollutants and lighting) on the sight organ was examined. In the test group a considerable percentage (29.5%) of chronic conjunctivitis was found. The disease results from a long-lasting exposure to dusts of various origin, as well as from improper lighting. The posttraumatic and post-inflammatory impairments of the sight organ, diagnosed by ophthalmological examinations, were not of occupational origin.

The epidemiologic features of feline immunodeficiency virus (FIV) infection were evaluated in 2,765 cats from the United States and Canada. Of these cats, 2,254 were considered by veterinarians to be at high risk for the infection, and 511 were healthy cats considered to be at low or unknown risk. This study is aimed at presenting the profile of ocular lesions observed in the inmates of a leprosy rehabilitation centre. Corneal involvement was found to be the highest (52.7%), followed by lesions of the eyebrows (47.3%) and eyelids (30.9%), uveal tract involvement (18.1%) and chronic conjunctivitis (1.8%).

Chlamydia has been found to be the cause of a high percentage of cases of neonatal and chronic conjunctivitis in an inner-city population. Setleis syndrome, an autosomal recessive disorder characterized by "coarse" face, temporal cutis aplasia, double upper eyelashes, absent lower eyelashes, chronic conjunctivitis, and prominent thick lips, was reported previously in 8 Puerto Rican children. We report on 3 unrelated children (one mentally retarded) with Setleis syndrome who are not of Puerto Rican descent. Two of our patients had imperforate anus, which has not previously been reported. The evolution of the phenotype over time is illustrated.

The diagnostic roles of enzyme immunoassay and indirect immunofluorescence tests were studied in 99 patients with various grades of active trachoma and in 72 patients with inactive trachoma. Normal individuals and patients with non-specific chronic conjunctivitis were used as a control group. Chlamydial antigens were detected in the conjunctival swabs from a large proportion of patients with severe or moderate trachoma. Cases with 'antigen-negative' but clinically severe trachoma showed presence of specific IgG in the blood, thus indicating that serological tests may complement the ELISA test

The present study analyses the precorneal teat film using schirmer's test and tear film breakup time (BKUT) in 400 eyes of patients with various types of leprosy. An abnormal tear film BKUT (47.2%) was observed to be much more informative in the present series than the study of schirmer's test alone (25.4%). In patients with lagophthalmos an abnormal tear film BKUT (70%) was higher as compared to an abnormal schirmer's test (40%); while in cases of lagophthalmos with ectropion an abnormal schirmer's test was seen less frequently (26.8%) than in cases of lagophthalmos without ectropion (58.3%).

A comprehensive eye checkup programme was carried out in 1140 cases of affected community to evaluate the exposure response and exposure effect with Methyl Isocynate on human eyes. The final evaluation of these cases was made with standard control group of same socioeconomic status in a non-gas affected slum area of Bhopal. The main chronic lesions noticed were chronic conjunctivitis, refractive changes, deficiency of tear secretion and persistent corneal opacities of various forms.

Tear film flow and stability studies were carried out in patients suffering from acute and chronic conjunctivitis by performing Schirmer test-I and break-up-time (BUT) measurements. In this clinical study, patients with chronic conjunctival inflammation demonstrated a premature break-up of tear film despite normal amounts of aqueous tears. In acute catarrhal conjunctivitis the values of BUT were within normal limits, whereas the values of Schirmer test-I were significantly higher (t-test, P less than 0.001) and are considered to be due to reflex hypersecretion of tears.
