Relevance of animal models to human uveitis.

Uveitis represents a wide spectrum of intraocular inflammatory conditions and includes various autoimmune and infectious etiologies. The relevance of animal models of uveitis to human diseases remains a key issue with major implications for the translational research and development of therapeutic strategies. Histopathological findings in patients with Vogt-Koyanagi-Harada disease, birdshot retinochoroidopathy and anterior uveitis are correlated to those observed in different animal models. Even though evidence based on histopathology is usually irrefutable, similar features may be due to diff

Photoreceptor outer segment abnormalities as a cause of blind spot enlargement in acute zonal occult outer retinopathy-complex diseases.

PURPOSE: To investigate the correlation between visual field (VF) defects in diseases of the acute zonal occult outer retinopathy (AZOOR)-complex and their spectral-domain optical coherence tomographic (OCT) findings. DESIGN: Observational case series. METHODS: Patients with AZOOR, multiple evanescent white dot syndrome (MEWDS), and multifocal choroiditis and panuveitis (MCP) examined in a private practice retinal referral center had threshold VF testing and spectral-domain OCT examination performed using a device capable of obtaining a block of 128 B-scans in a 6 x 6-mm region centered on t

[Multifocal choroiditis associated with serpiginous choroiditis and choroidal neovascularization: a case report].

BACKGROUND: We treated a patient with multifocal choroiditis (MFC) associated with serpiginous choroiditis and choroidal neovascularization (CNV). CASE: In a 27-year-old woman whose left eye showed serpiginous choroiditis near the optic disc and multiple irregular chorioretinal scars and exudates in peripheral and near-peripheral zones, fluorescein angiography demonstrated diffusely scattered hyperfluorescent dots and late leakage from retinal vessels. The lesion near the optic disc showed hypofluorescence in the early venous phase, and hyperfluorescence in the late phase. Sub-Tenon injection

Idiopathic choroidal neovascularization as an early manifestation of inflammatory chorioretinal diseases.

PURPOSE: We describe four patients with idiopathic choroidal neovascularization (ICNV) who developed inflammatory chorioretinal diseases in the ipsilateral or contralateral eye. METHODS: The medical records of 58 eyes of 58 patients with an initial diagnosis of ICNV (age range, 19-49 years; mean, 34.9 years) were reviewed. RESULTS: Of the 58 patients, 4 women (7.0%) with moderate to high myopia (age range, 17-39 years) developed inflammatory chorioretinal diseases. In Cases 1 and 2, white dots were transiently seen in the deep retina of the contralateral eye, consistent with the clinical fea

Inadvertent scleral perforation after strabismus surgery: incidence and association with refractive error.

BACKGROUND: Inadvertent scleral perforation is a rare but serious complication of extraocular muscle surgery for strabismus. We evaluated the incidence of unwanted scleral penetration after strabismus surgery and its association with refractive error. METHODS: This cross-sectional noncomparative case series comprised 453 eyes from 236 patients who underwent strabismus surgery. After dilation, each patient had fundus examination using indirect binocular ophthalmoscope. Chorioretinal scars around the suture area, which presumed the previous scleral perforation, were detected. We assessed whethe

Eye manifestations of intrauterine infections and their impact on childhood blindness.

Intrauterine infections are important causes of childhood blindness in both developed and developing countries. Chorioretinal scars are the most characteristic eye manifestation of a congenital or prenatal infection. The various ocular manifestations of congenital infections, summarized by the mnemonic TORCH, and recent additions to the "other" category (lymphocytic choriomeningitis virus and West Nile virus) are discussed.

Preventing reactivation of ocular histoplasmosis: guidance for patients at risk.

BACKGROUND: Ocular histoplasmosis syndrome (OHS), a significant cause of vision loss in young and middle-aged adults, is associated with the fungus Histoplasma capsulatum (Hc). There is considerable evidence that recurrent reactivation of perimacular ocular histoplasmosis lesions is an important cause of disease progression and that vision loss is at least, in part, a consequence of host sensitivity to fungal antigen. METHODS: The etiology and pathogenesis of OHS is reviewed and specific recommendations are made for patients with OHS that may decrease the risk of reactivation of ocular histop

Evolution of retinal laser therapy: minimum intensity photocoagulation (MIP). Can the laser heal the retina without harming it?

Laser photocoagulation is a photo-thermal therapy validated by landmark studies and commonly accepted as the standard of care for various retinal diseases. Although its mechanism of action is still not completely understood, it is normally administered with visible endpoints, true intra-retinal burns that cause chorioretinal scars, which, with time, evolve into expanding areas of atrophy. New hypotheses on the mechanism of action of laser photocoagulation suggest that its therapeutic benefits derive from biologic activities that cannot be inducted within the "burned" area of photocoagulation n
