Cortical gradient compression links cognition and transcriptomic signatures in primary angle-closure glaucoma.

The cerebral cortex is organized along hierarchical gradients that integrate sensory and cognitive functions, yet whether this organization is disrupted in primary angle-closure glaucoma and contributes to cognitive impairment remains unclear. Here we examine cortical functional gradients in individuals with primary angle-closure glaucoma using resting-state functional magnetic resonance imaging and transcriptomic analyses. We show that patients exhibit compression and reorganization of cortical gradients, particularly within visual and default mode networks, and that these alterations are ass

Visual field progression in varying severities of treated patients with primary angle closure glaucoma.

To evaluate the rate and determinants of visual field (VF) progression across different severities of primary angle-closure glaucoma (PACG). Chinese patients with PACG who had ≥ 5 reliable VFs and ≥ 5 years of follow-up were included. Disease severity was classified by baseline mean deviation (MD) as mild (≥-6.0dB), moderate (-6.01 to -12.0dB) or severe (<-12.0dB). VF progression was categorized as slow (>-0.25dB/year), intermediate (-0.25 to -1.0dB/year), or fast (≤-1.0dB/year). Among 783 patients evaluated, 477 met the inclusion criteria: 140 (29.4%) mild, 161 (33.8%) moderate, and 176 (36.9

Minimally invasive glaucoma surgery in the surgical landscape of primary angle closure glaucoma: Current innovations and future trends.

Primary angle-closure glaucoma (PACG) is a major cause of irreversible blindness worldwide, with the highest burden in Asia. Its pathogenesis involves the interplay of multiple mechanisms, including pupillary block, plateau iris configuration, lens-related crowding, and dynamic choroidal expansion. Traditional surgical management has relied on trabeculectomy, lens extraction, or transscleral cyclophotocoagulation, which often achieve substantial intraocular pressure (IOP) control but carry significant risks and require prolonged recovery. Minimally invasive glaucoma surgery (MIGS) expands trea

Glymphatic system impairment in primary angle-closure glaucoma: A cross-sectional study of disease severity.

The glymphatic system plays a critical role in brain waste clearance and has been implicated in neurodegenerative diseases. Primary angle-closure glaucoma (PACG) is increasingly recognized as a neurodegenerative disorder extending beyond the eye. This study aimed to investigate glymphatic system function in patients with PACG using diffusion tensor imaging analysis along the perivascular space (DTI-ALPS). This cross-sectional study included patients with PACG and age- and sex-matched healthy controls. All participants underwent magnetic resonance imaging, including diffusion tensor imaging. Th

Dynamic brain states and molecular signatures in primary angle-closure glaucoma.

BACKGROUND: Primary angle-closure glaucoma (PACG) has traditionally been regarded as an ocular disorder, but accumulating evidence suggests broader central nervous system involvement. Although previous neuroimaging studies have identified static functional abnormalities, the dynamic properties of large-scale brain networks and their associated molecular signatures in PACG remain insufficiently understood. METHODS: We applied Leading Eigenvector Dynamics Analysis to resting-state functional MRI data from 44 patients with PACG and 57 healthy controls to characterize recurrent whole-brain dynami

Genetic Risk Factors and Clinical Implications of Glaucoma in the Saudi Population: A Review.

Most glaucoma genetic data derive from European and East Asian cohorts, leaving high-consanguinity Middle Eastern populations under-characterized. This review synthesizes 33 Saudi-specific genetic studies (2014-2024, >9000 participants) to define a population-level glaucoma genetic architecture that diverges substantially from global models and carries direct precision medicine implications. Three findings distinguish the Saudi landscape. First, CYP1B1 functions as the dominant causal gene across both primary congenital glaucoma (PCG) and juvenile-onset open-angle glaucoma (JOAG), accounting f
