Magnetic resonance features in advanced retinoblastoma - what a paediatric radiologist needs to know.

Retinoblastoma is the most common intra-ocular malignancy in children worldwide. Current treatment modalities include focal therapy such as laser or cryotherapy, intravenous chemotherapy, intra-arterial chemotherapy and surgical enucleation. According to the current retinoblastoma staging systems, there are a few common features indicating advanced disease stage, which can subsequently predict the prognosis and the necessity of surgical enucleation and adjuvant chemotherapy. This review uses annotated magnetic resonance (MR) images to illustrate these features, which include (1) retinal detach

Anti-VEGF drugs compared with laser photocoagulation for the treatment of diabetic retinopathy: a systematic review and economic analysis.

BACKGROUND: Diabetic retinopathy is a major cause of sight loss in people with diabetes, with a high risk of macular oedema, vitreous haemorrhage or other complications. Panretinal photocoagulation is the primary treatment for proliferative retinopathy. Anti-vascular endothelial growth factor drugs are used to treat various eye conditions and may be beneficial for people with proliferative or non-proliferative retinopathy. METHODS: The Anti-VEGF In Diabetes project sought to investigate the clinical and cost-effectiveness of using anti-vascular endothelial growth factor to prevent retinopathy

Lower incidence of diabetic retinopathy and worsening events after phentermine assisted weight loss across a large U.S. cohort.

BACKGROUND/OBJECTIVES: Medication assisted weight loss for treatment of obesity has rapidly increased. The effect of this treatment with phentermine on diabetic retinopathy (DR) is underexplored. SUBJECTS/METHODS: Retrospective cohort study. The U.S. Collaborative Network of the TriNetX platform was queried for patients diagnosed with overweight and obesity and prescription of phentermine. Patients were propensity score matched across baseline demographics and systemic risk factors at the time of medication initiation and compared to those diagnosed with overweight and obesity without usage o

[The effects of intraoperative use of angiogenesis inhibitors on outcomes and complication rates in the surgical treatment of proliferative diabetic retinopathy].

UNLABELLED: Treatment of proliferative diabetic retinopathy (PDR) complications, such as vitreous haemorrhage and tractional retinal detachment, as well as macular involvement, remains a complex multifactorial challenge. The use of angiogenesis inhibitors (AIs) at different stages of patient management is being investigated. In particular, intraoperative use of AIs appears to be pathogenetically justified. PURPOSE: This study evaluates the effect of intraoperative administration of AIs on the outcomes and complication rates in the surgical treatment of PDR. MATERIAL AND METHODS: A comparativ

OCTA as a reliable prognostic tool for active NVD treated with Panretinal Photocoagulation and/or ranibizumab.

PURPOSE: To compare the prognosis of neovascularization of the disc (NVD) after panretinal photocoagulation (PRP) and/or ranibizumab treatment, based on OCT angiography (OCTA) patterns. METHODS: In this prospective study, treatment-naive patients with stage IV diabetic retinopathy (DR) and NVD were imaged with 6x6 mm2 OCTA scans. NVD was classified according to OCTA morphological features: different sources (retinal arteries and veins), different activities (exuberant vascular proliferation (EVP)+ and EVP-) and different configurations (type I&II, III and IV). All patients were treated with P

Indications and Outcomes of Patients Undergoing Immediate Sequential Bilateral Vitreoretinal Surgery: A Case Series.

UNLABELLED: <p>Introduction: Bilateral vision threatening vitreoretinal disease requiring urgent surgery such as proliferative diabetic retinopathy, vitreous haemorrhage and retinal detachment are not uncommon presentations. Accepted practice has been to address each eye in separate, staged procedures. However, there are specific circumstances in which operating on both eyes in the same anaesthetic sitting may be preferable. Immediate sequential bilateral vitreoretinal surgery (ISBVS) has rarely been described in the literature. We present a series of cases and discuss the indications, risks a

Optical coherence tomography angiography as a tool for diagnosis and monitoring of sickle cell related eye disease: a systematic review and meta-analysis.

Sickle cell retinopathy (SCR) is an ocular manifestation of sickle cell disease (SCD). In SCR abnormal sickling of erythrocytes is associated with sight-threatening complications such as neovascularisation, vitreous haemorrhage, maculopathy and retinal detachment. Optical coherence tomography angiography (OCTA) is a novel imaging modality enabling non-invasive assessment of retinal vasculature. This systematic review provides an up-to-date evaluation of the role of OCTA in SCR diagnosis and management. We searched MEDLINE/PubMed, Cumulated Index to Nursing and Allied Health Literature (CINAHL)

Penetrating orbital injury by a nine-centimetre lead sinker.

A male in his early 40s presented with a penetrating orbital injury after a nine-centimetre lead sinker was propelled into his right orbit while fishing. He reported pain, ptosis and visual acuity reduced to light perception. X-rays, preferred over CT due to metallic artefacts, revealed the sinker in the inferior orbital fissure with an intact eyeball. Initial surgical extraction attempts triggered a severe vagal response, necessitating endoscopic navigation for safe removal. Postoperatively, despite an intact globe, he developed vitreous haemorrhage and retinal detachment, requiring vitrectom

Associations between obstructive sleep apnoea and the development and severity of retinal vein occlusion.

BACKGROUND/OBJECTIVES: Emerging research suggests obstructive sleep apnoea (OSA) as a potential risk factor for retinal vein occlusion (RVO), but the impact of sex, race, and ethnicity, and the role of OSA in RVO progression, remains unclear. This study explored demographic differences in the association between OSA and RVO and compared the severity of RVO in patients with and without OSA. SUBJECTS/METHODS: This retrospective cohort study analysed aggregated, de-identified electronic health record data of US patients. Adults who received ophthalmological services were grouped by baseline RVO

Macular ischaemia after inadvertent perforation of the eyeball and intravitreal injection of anesthetic agents during retrobulbar anaesthesia.

Perforation of the eyeball during retrobulbar anesthesia is rarely reported. A case of inadvertent ocular perforation during retrobulbar anesthesia in cataract surgery is presented. Retinal complications and follow up after six months are described. A 58-year-old man presented with vision loss following cataract surgery on his right eye by phacoemulsification and intraocular lens implantation under retrobulbar anaesthesia performed one week earlier. Visual acuity was 0.05. Fundus examination revealed a vitreous haemorrhage in the lower area. A rounded haemorrhage was present next to the lower

Combined hamartoma of the retina and retinal pigment epithelium: unusual rapid progression and early surgical intervention.

We report a case of a young woman who presented with a vitreous haemorrhage associated with a combined hamartoma of the retina and retinal pigment epithelium (CHRRPE). During her clinical course, while the vitreous haemorrhage resolved, the lesion enlarged with a worsening epiretinal membrane. The patient's visual acuity reduced from 0.06 to 0.4 logMAR (Snellen equivalent of 6/7.5 to 6/30) with worsening metamorphopsia over a 13-week period. She shortly underwent pars plana vitrectomy with a membrane peel with an improved anatomical and visual outcome. The case presented shows an uncommon prese

Sensitivity and utility of ultra-wide field imaging for the detection of peripheral retinal breaks in a large Irish tertiary referral centre.

BACKGROUND: Ultra-wide field imaging (UWFI) is an invaluable tool for screening, documentation, and follow-up of retinal pathology. However, despite widespread use, the optimal image acquisition strategy and accuracy for detecting potentially-treatable peripheral retinal breaks is not fully known. We report our data from Dublin, Ireland. METHODS: A retrospective analysis was carried out on all consecutive patients undergoing treatment of peripheral retinal breaks in the Royal Victoria Eye and Ear Hospital from March 2022 to August 2024. Patients with pre-operative UWFI using Optos California

Sudden Loss of Vision Following Cosmetic Autologous Fat Transfer.

Cosmetic tourism and facial aesthetic procedures are increasing in popularity worldwide, with patients often seeking low-cost interventions abroad. Autologous fat transfer (AFT) is commonly performed for periorbital rejuvenation and carries a risk of rare but severe ophthalmic complications. We describe a case of unilateral visual loss following periorbital AFT, in which the underlying mechanism was scleral penetration leading to vitreous haemorrhage, retinal incarceration, and extraocular muscle entrapment. Surgical management with pars plana vitrectomy restored partial vision, although motil
