Advances in ocular motor and pupil biomarkers for neurological disorders.

Neurological disorders are often difficult to diagnose and monitor, particularly in the early stages when symptoms may be subtle or nonspecific. Because the visual system engages a large portion of the cerebral cortex and relies on well-defined neural pathways, it offers a unique and accessible window into brain function. In this context, the concepts of oculomics and oculometrics have gained increasing attention. Oculomics refers to the study of systemic and neurological diseases through ocular biomarkers, while oculometrics involves the computational quantification of eye and pupil parameter

What if it is the eyes? The effect of eye movement disorders on cognitive testing in MS.

BACKGROUND: Visual cognitive tests that rely on saccades may be affected by internuclear ophthalmoplegia (INO), irrespective of cognitive ability. INO and cognitive decline are common in people with multiple sclerosis (PwMS). We assessed the impact of INO on visual versus non-visual cognitive tests in PwMS. METHODS: PwMS (n=197) completed auditory and visual neuropsychological tests, and demographically corrected Z-scores were obtained. INO was quantified using the DEMoNS infrared oculography protocol, applying the versional dysconjugacy index (VDI) of area under the saccadic trajectory (AUC)

Virtual reality-based oculography detects internuclear ophthalmoplegia in multiple sclerosis and other neurological disorders.

BACKGROUND: Clinical-neurological examination may underestimate internuclear ophthalmoplegia (INO). We investigated immersive virtual reality (VR)-based eye-tracking in patients with multiple sclerosis (MS), other neurological disorders (ND), and healthy controls (HC) to detect INO. METHODS: In this prospective monocentric study, 471 participants (243 MS, 104 ND, 124 HC) underwent horizontal prosaccade testing (± 10°, ± 20°) using VR-based oculography. INO was determined in a blinded manner based on software-generated reports comparing adduction and abduction velocities. VR-based and clinical

MRI-Negative Internuclear Ophthalmoplegia: A Diagnostic Challenge Emphasizing Clinical Localization.

Internuclear ophthalmoplegia (INO) is a distinct ocular motility disorder localizing to the medial longitudinal fasciculus (MLF). Because the MLF is a microscopic, highly myelinated tract, small inflammatory lesions can cause significant functional conduction blocks that fall below the resolution threshold of typical 1.5T or 3T MRI. This leads to clinical radiographic dissociation, resulting in a significant diagnostic challenge when pathognomonic physical findings are present despite negative neuroimaging. A 16-year-old female presented with acute binocular diplopia and bilateral INO. Physica

Successful Treatment with Secukinumab in a Psoriasis Patient on Hemodialysis.

Moderate to severe psoriasis has been reported as an independent risk factor for IgA nephropathy (IgAN). IgAN is characterized by episodic microscopic hematuria, which can progress to end-stage renal disease (ESRD). Managing therapeutic interventions for psoriasis patients requiring dialysis due to ESRD presents significant challenges. We present a case of severe plaque psoriasis in a patient concurrently diagnosed with IgAN who is dependent on hemodialysis. Over the past two months, his condition has worsened without any identifiable triggers. Physical examination revealed generalized scaly p

[Research progress on the manifestations prevention and treatment of immune-related adverse events in the neuro-ophthalmic efferent system related to immune checkpoint inhibitors].

Immune checkpoint inhibitors (ICI), as a new type of targeted therapeutic drugs, have demonstrated durable efficacy in cancer treatment. However, some patients receiving ICI treatment may be affected by immune-related adverse events (irAE). Compared with irAE in skin tissues, gastrointestinal system, etc., irAE in the neuro-ophthalmic efferent system are relatively rare. The reported irAE in the neuro-ophthalmic efferent system mainly include myasthenia gravis, orbital extraocular myositis, thyroid-associated ophthalmopathy-like conditions, internuclear ophthalmoplegia, Miller-Fisher syndrome,

Chronic Lymphocytic Inflammation With Pontine Perivascular Enhancement Responsive to Steroids Presenting With Predominantly Neuro-Ophthalmic Features.

BACKGROUND: Chronic lymphocytic inflammation with pontine perivascular enhancement responsive to steroids (CLIPPERS) is a rare and poorly understood inflammatory disorder of the central nervous system centered on the pons. It has a characteristic imaging appearance with enhancing and T2-hyperintense punctate and curvilinear lesions in the pons. The lesions lack restricted diffusion and have relatively little perilesional edema. Although patients typically present with gait ataxia and other sequelae of brainstem inflammation including diplopia, there is scant literature focusing on patients who

Bilateral adduction palsy with a paramedian midbrain lesion is most likely wall-eyed bilateral oculomotor subnuclear ophthalmoplegia: a case report of a brainstem infarction.

A 73-year-old female was hospitalized in our Department of Neurology for diplopia due to multiple infarctions in the midbrain and pons. A neurological examination revealed bilateral adduction palsy during horizontal gaze with nystagmus of the contralateral abducting eye. Exotropia and impaired convergence were also noticed. The distinctive symptoms indicated wall-eyed bilateral internuclear ophthalmoplegia (WEBINO). We speculate that the cause of WEBINO in our case was bilateral medial rectus palsy via bilateral damage to the medial rectus subnuclei (MRSN) due to the paramedian midbrain infarc

[Ophtalmoplegia secondary to ponto-mesencephalic infarction : clinical-radiological correlation in the WEBINO syndrome].

We report on a case of posterior midbrain stroke revealed by WEBINO ("Wall-Eyed Bilateral InterNuclear Ophthalmoplegia") syndrome, a relatively rare neuro-ophthalmological clinical entity combining bilateral internuclear ophthalmoplegia and exotropia. Encephalic MRI unraveled a bilateral paramedian ischemic lesion located at the posterior area of the ponto-mesencephalic junction, perfectly correlating to the theoretical localization of the medial longitudinal fasciculi. Clinical identification of this syndrome is key to efficiently guide MRI reading and proper management.

Artificial Intelligence Diagnosis of Ocular Motility Disorders From Clinical Videos.

BACKGROUND: Multimodal artificial intelligence (AI) models have recently expanded into video analysis. In ophthalmology, one exploratory application is the automated detection of extraocular movement (EOM) disorders. This proof-of-concept study evaluated the feasibility of using Gemini 2.0 to recognize EOM abnormalities, identify the affected eye, and recognize specific movement limitations from publicly available, real-world clinical videos. METHODS: We retrospectively collected 114 YouTube videos of EOM disorders, including cranial nerve (CN) palsies, internuclear ophthalmoplegia (INO), sup

Medulloblastoma Presenting With Bilateral Internuclear Ophthalmoplegia of Abduction: A Case Report.

Medulloblastoma is the most common paediatric high-grade central nervous system tumour. To our knowledge, this is the first case of medulloblastoma presenting with bilateral internuclear ophthalmoplegia (INO) of abduction. We describe a rare case of a 14-year-old girl presenting with bilateral abduction deficits and optic disc swelling. Neuroimaging revealed a midline posterior fossa tumour consistent with medulloblastoma compressing the dorsal pons and 4th ventricle. Following surgical resection and adjuvant therapy, ocular motility normalized. This case report highlights the features of a ra

Clinicoradiologic Correlation in Bilateral Internuclear Ophthalmoplegia Revealing a Ruptured Pontine Abscess.

Internuclear ophthalmoplegia (INO) is an uncommon but well-recognized clinical sign, most frequently associated with demyelinating disorders or brainstem infarction. Its occurrence in the context of an infectious process, particularly a brainstem abscess, is exceptional. We report the case of a previously healthy 37-year-old man who presented with acute meningeal signs and binocular horizontal diplopia. Magnetic resonance imaging (MRI) revealed a ring-enhancing pontine lesion with diffusion restriction and posterior interruption of enhancement at the interface with the fourth ventricle, consis

Acute Lacunar Infarct in the Right Dorsal Pons Presenting as Internuclear Ophthalmoplegia Following Percutaneous Cardiac Intervention.

A 58-year-old male with a history of type 1 diabetes, hypertension, smoking, and obesity, was seen by the inpatient neurology team for symptoms of dizziness and diplopia after cardiac catheterization. His symptoms caused him to feel nauseated and vomit and were improved with closing his right eye. It was noted that he was unable to adduct his right eye and had nystagmus of his left eye on exam. He had no ptosis, and convergence was preserved. The remaining physical exam was normal. Initial limited magnetic resonance imaging (MRI) was within normal limits; however, repeat limited brain MRI with
