Prenatal Ultrasonographic Detection of Intracardiac Thrombi in Canine Fetuses With Congenital Heart Defects.

A 5-year-old pregnant Newfoundland bitch was evaluated by monitoring fetal viability. Transabdominal ultrasonography of the reproductive tract revealed a homogeneous and hyperechoic mass within the right ventricle associated with abnormal tricuspid valve in a fetus. A similar mass was also visualized in the right atrium of another fetus. The bitch delivered 11 puppies, four of which died after parturition. Necropsy showed tricuspid stenosis in all puppies, with concomitant pulmonary stenosis in one of them. Moreover, an intraventricular thrombus and an interatrial thrombus were each revealed i

Randomized controlled trials in valvular heart disease: the evolving role of multimodality imaging.

Valvular heart disease represents a significant global health burden, with an estimated prevalence of 2.5% in high-income countries and projected increases due to population ageing. Randomized controlled trials in valvular heart disease have undergone substantial evolution, shifting from mortality-focused endpoints toward comprehensive assessments integrating imaging parameters and patient-centered outcomes. Cardiovascular imaging modalities, including echocardiography, cardiac computed tomography, and cardiac magnetic resonance, have become pivotal in trial design, patient selection, and endp

Rare regulatory mutations disrupt mesenchymal molecular programs driving endocardial cushion formation in bicuspid aortic valve.

Bicuspid aortic valve, a prevalent congenital malformation, predisposes individuals to severe complications. Although the condition exhibits substantial heritability, known protein-coding and common regulatory mutations explain a minority of cases. To assess the contribution of rare regulatory variants, here we integrate high-resolution three-dimensional genome organization profiling with matched whole-genome sequencing from eight individuals with bicuspid aortic valves and eight with standard tricuspid aortic valves. In bicuspid aortic valve patients, mutation-driven chromatin rewiring affect

Cryoextraction for Airway Bleeding After Valve-in-Valve Procedure.

BACKGROUND: Hemoptysis after percutaneous valve implantation is a rare but life-threatening complication. CASE SUMMARY: An 84-year-old woman with prior mechanical aortic and biological tricuspid valve replacements presented with tricuspid stenosis and regurgitation. A transcatheter valve-in-valve procedure using a 24.5-mm Myval was performed successfully. However, she developed severe hemoptysis, hypoxia, and ventilation difficulties shortly after the procedure. Flexible bronchoscopy with cryoprobe-assisted clot extraction was performed, removing large obstructive clots from the bronchi. A se

Phloroglucinol Exposure During Early Pregnancy and Risk of Major Congenital Malformations: A Nationwide Cohort Study in France.

BACKGROUND AND AIMS: Phloroglucinol is a spasmolytic drug widely prescribed in France for gastrointestinal or pelvic pain, and frequently used during pregnancy despite limited evidence regarding its safety. We aimed to evaluate the association between first-trimester phloroglucinol exposure and the risk of major congenital malformations (MCMs) in a nationwide cohort. METHODS: Using data from the French EPI-MERES cohort based on the national health data system (SNDS), we identified all pregnancies ending after 22 weeks between 2010 and 2022, excluding women with phloroglucinol use in the year

Multivalve Involvement in Aortic Stenosis: Insights From a Narrative Review.

Many patients with severe aortic stenosis present with concomitant multivalvular heart disease. The management of this condition remains challenging and requires a multidisciplinary approach that integrates clinical, hemodynamic, and multimodality imaging data to define the most effective and durable treatment strategy. Although randomized evidence to guide treatment decisions in this setting is limited, percutaneous interventions to address additional valvular lesions are being increasingly adopted. This review provides an overview of the pathophysiology of valvular diseases commonly associat

Brucella anthropi Endocarditis: An Unusual Pathogen.

BACKGROUND: The genus Brucella has expanded considerably in the 21st century. With the advent of advanced phylogenetic analyses, a close genetic relationship between Brucella and Ochrobactrum has been identified, leading to reclassification of Ochrobactrum species within the genus Brucella. Among these, Brucella anthropi (formerly Ochrobactrum anthropi) is increasingly recognized as a rare cause of invasive human infection. We report a clinically significant case of B. anthropi infective endocarditis and review the available literature. METHODS: We report a case of B. anthropi infective endoc

Heart Failure Revealing Occult Rheumatoid Aortitis Presenting as a Giant Ascending Aortic Aneurysm With Functional Aortic Regurgitation: A Case Report.

Rheumatoid arthritis (RA) is a systemic inflammatory disease that can rarely involve the aorta and remain clinically silent until severe complications occur. We report the case of an 80-year-old woman with no cardiovascular risk factors who presented with acute left-sided heart failure that led to the diagnosis of previously unrecognized rheumatoid arthritis. Transthoracic echocardiography demonstrated global left ventricular hypokinesia with a left ventricular ejection fraction of 34%, a structurally preserved tricuspid aortic valve, and moderate-to-severe functional aortic regurgitation rela

Clinical and imaging phenotypes.

Functional/Secondary tricuspid regurgitation (STR) accounts for over 85% of clinically significant tricuspid regurgitation (TR) and is associated with adverse prognosis and impaired quality of life. Advances in percutaneous tricuspid valve (TV) interventions underscore the need to differentiate TR aetiologies, mechanisms, and phenotypes. STR is subdivided into atrial (A-STR), caused by right atrial dilation and tricuspid annular enlargement without significant leaflet tethering, and ventricular (V-STR), resulting from right ventricular dilation/dysfunction with leaflet tethering. A-STR, increa

Extracorporeal life support in lung transplantation with dual-lumen cannulas: A systematic review.

BackgroundRefractory end-stage lung failure may benefit from extracorporeal life support (ECLS) as a bridge to lung transplantation. Venovenous (VV) extracorporeal membrane oxygenation (ECMO) is indicated in patients who have failed conventional medical treatment and mechanical ventilation. The Avalon Elite Bi-Caval Dual-Lumen Catheter (Getinge) and the Crescent Jugular Dual-Lumen Catheter (MC3, Inc., Medtronic) have been used for this configuration. Venoarterial (VA) ECMO may be used in patients with acute right ventricular (RV) failure, haemodynamic instability or refractory respiratory fail

Robotic degenerative mitral valve repair: an evolution.

Robotic mitral valve repair (RMVr) has advanced considerably since its introduction at the turn of the millennium. Surgical techniques and technological innovations have addressed many of the limitations of early RMVr procedures. Today, RMVr is recognized as a safe, effective, and adaptable approach to degenerative mitral valve disease, with applications extending to commonly associated conditions such as atrial fibrillation and tricuspid valve disease. Patient selection criteria are now more rigorously defined, and robotic surgical teams are expanding the scope of practice to include increasi

Sex Differences in Left Ventricular Size and Function before and after Mitral Valve Repair.

BACKGROUND: Among healthy subjects, women have smaller cardiac size than men even after indexing for body surface area (BSA), as well as higher left ventricular ejection fraction (LVEF) and left ventricular (LV) global longitudinal strain (GLS). Despite this, little is known about sex differences in LV remodeling in patients with mitral valve prolapse (MVP) undergoing mitral valve repair (MVr). The aim of this study was to assess pre- and post-MVr sex differences in LV size and function in patients with MVP. METHODS: Patients with MVP who underwent surgery between 2007 and 2024 at two heart v

Baseline B-Type Natriuretic Peptide and Clinical Outcomes Following Transcatheter Tricuspid Valve Replacement.

Transcatheter tricuspid valve replacement (TTVR) has emerged as a nonsurgical approach for patients with severe tricuspid regurgitation. However, clinical variables which may impact the overall patient response to TTVR remain elusive. B-type natriuretic peptide (BNP) may reflect disease severity, which in turn may help predict response to TTVR and identify patients at increased risk of adverse outcomes. We evaluated the association between baseline BNP and clinical outcomes (all-cause hospitalization or death) and Kansas City Cardiomyopathy Questionnaire at 30-days and 1-year among 148 patient

Leadless pacemaker implantation via right jugular approach in a patient with a bicaval valve system: a case report.

BACKGROUND: Tricuspid regurgitation in elderly patients with multiple comorbidities presents unique challenges, especially after prior valve interventions and when advanced heart failure symptoms persist. New device-based therapies such as bicaval valve stents (TricValve®) can offer alternatives for patients not suitable for surgery but may complicate pacemaker implantation. CASE SUMMARY: We report an 85-year-old woman with a history of coronary artery disease, prior valve-in-valve transcatheter aortic valve implantation (TAVI) for a degenerated aortic prosthesis, and permanent atrial fibrill
