Q fever, caused by the gram negative obligate intracellular bacterium Coxiella burnetii, remains a globally relevant but underappreciated zoonosis in the 21st century. Despite decades of research, the disease continues to pose diagnostic, epidemiological, and clinical challenges that hinder timely recognition and effective management. This review aims to synthesize current advances in the epidemiology, pathogenesis, and diagnosis of C. burnetii, while critically examining the One Health gaps that sustain its persistence as a re-emerging zoonosis. We evaluate the strengths and limitations of ex

For over 80 years, the genus Coxiella was considered monotypic, comprising only Coxiella burnetii, the aetiological agent of Q fever. Recent studies, however, have revealed several candidate species and genetic lineages associated with ticks, many of which act as endosymbionts that are essential for tick development. This association is particularly notable in the members of the Rhipicephalus sanguineus complex, where Candidatus Coxiella massiliensis (zoonotic) and Candidatus Coxiella mudrowiae (unknown pathogenicity) have been described. Given the close relationship between these ticks, domes

Coxiella burnetii is a zoonotic pathogen that causes Q fever in humans. There are many known reservoirs of C. burnetii, including cattle, sheep, and goats with an expanding list of potential reservoirs including birds, reptiles, ticks and additional mammalian species, such as swine. Feral swine are a highly invasive species in the United States with significant populations and a broad geographic distribution. The role of feral swine in the transmission and spread of C. burnetii is poorly understood, although a recent report identified overlap between feral swine seroprevalence and human Q feve

Pathogens such as Toxoplasma gondii, lentiviruses (e.g., CAE), Hypoderma spp., Neospora caninum, Mycoplasma spp., and pestiviruses are important for goat farming in Lithuania; however, data on their prevalence remain limited. To address this gap, a multi-pathogen study was conducted between 2021 and 2024 using selected ELISA kits (ID.vet, Innovative Diagnostics, France). A total of 380 blood samples were collected from 30 goat herds across different regions of Lithuania; the sample size varied depending on the pathogen. Serum samples were tested for antibodies, and seroprevalence was calculate

Coxiella burnetii, the causative agent of Q fever in humans, is an important worldwide pathogen. The main reservoirs for the bacterium are ruminants, specifically sheep and goats, which act as major shedders of the bacterium in the environment. In addition, the bacterium causes significant economic losses on small ruminant farms due to abortion and other reproductive disorders, such as weak neonates or stillborn animals. Transmission between animals and humans can occur mainly through the inhalation of aerosols, direct contact with products of labour or fomites, or through the vectorial route

Q fever is a globally distributed zoonosis caused by Coxiella burnetii, which is mainly harbored by farm ruminants. Although some studies have reported the presence of C. burnetii in cats, cat-associated Q fever is very rare. In this study, we report the case of a 30-year-old man who was diagnosed with Q fever in May 2024. The diagnosis was based on the results of next-generation sequencing and confirmed by nested PCR. Epidemiological investigations suggest that he had not come into contact with any infection sources other than two cats, one of which was a parturient cat. The blood samples of

OBJECTIVES: Peritoneal dialysis-related peritonitis (PDAP) is a common and severe complication of peritoneal dialysis (PD), and it constitutes a major cause of PD treatment failure and patient mortality. The most common organisms causing PDAP are Staphylococcus aureus, Enterococcus spp, Escherichia. coli, and Pseudomonas aeruginosa. Although conventional anti-infective therapy can lead to the recovery of most cases, 10-20% of patients still progress to refractory peritonitis (RP). Query fever (Q fever), a zoonotic disease, caused by Coxiella burnetii, is an infectious disease that has long bee

Q fever, caused by Coxiella burnetii, is a zoonotic infection with highly variable clinical manifestations, often leading to diagnostic delays. Although typically associated with livestock exposure and presenting with nonspecific febrile illness or hepatitis, its clinical spectrum is broad. We report an unusual case of a 53-year-old previously healthy male who presented with high-grade fever, epigastric pain radiating to both upper quadrants, marked jaundice, severe acute kidney injury, and dark urine. Initial imaging revealed bilateral perinephric and periureteral stranding, gallbladder wall

BACKGROUND: Q fever is a zoonotic disease caused by Coxiella burnetii and is endemic worldwide. Q fever endocarditis is commonly found in adults and is rarely seen in children. Infectious endocarditis can also lead to renal damage. Here, we present a case of Q fever endocarditis-associated glomerulonephritis in a Chinese boy with positive Antiproteinase 3 antibody (PR3)-antineutrophil cytoplasmic antibody (ANCA).
CASE PRESENTATION: A 12-year-old Chinese boy presented with intermittent fever and hematuria for 2 months. He was diagnosed with Tetralogy of Fallot at birth and underwent multiple c

BACKGROUND: Q fever is a zoonotic infection caused by the bacterium Coxiella burnetii. French Guiana, largely covered by the Amazon rainforest, is considered a hyper-endemic region. While ruminants are the primary reservoirs worldwide, the reservoir in French Guiana remains debated, likely relying on wild fauna. This study aimed to identify spatiotemporal clusters of human Q fever in the Cayenne area and investigate their relationships with environmental factors using remote sensing data.
METHODS: A retrospective study was conducted on acute Q fever human cases from January 2007 to December 2

In April-June 2017, a cluster of 4 third-trimester abortions caused by Coxiella burnetii was diagnosed on a dairy farm with 356 Holstein cows in Uruguay. We investigated 1) the seroprevalence of C. burnetii in cows delivering normal calves in this herd, 2) whether non-aborting cows gave birth to seropositive and/or PCR-positive newborn calves in the year of the abortion cluster, and 3) whether seronegative newborn calves seroconverted after ingesting colostrum replacer. In March-September 2017, serum samples were collected from 133 calving cows and their calves immediately after birth and befo

INTRODUCTION: Fever or inflammation of unknown origin (FUO/IUO), bacteremia, and sepsis represent challenging diagnostic scenarios in clinical practice. Over the past decade, multiple systematic reviews and meta-analyses have reported quantitative data regarding the diagnostic accuracy, clinical impact, and cost-effectiveness of fluorine-18 fluorodeoxyglucose positron emission tomography combined with computed tomography ([18F]FDG PET/CT) in these complex infectious and inflammatory conditions. We performed an umbrella review of published systematic reviews and meta-analyses to provide a compr

BACKGROUND/OBJECTIVES: Q fever, caused by Coxiella burnetii, is typically treated with doxycycline, but its efficacy is limited in chronic cases and may be poorly tolerated. Systemic ciprofloxacin shows limited activity for acute Q fever. However, inhaled liposomal formulations may provide therapeutic benefit.
METHODS: Two inhaled ciprofloxacin formulations (Lipoquin® and Apulmiq®) were evaluated in an A/J mouse model of Q fever and compared with intraperitoneal ciprofloxacin and oral doxycycline. Initially, pharmacokinetic studies were performed to determine an appropriate dosing regimen for

BACKGROUND: Q fever endocarditis (QFE) is a rare, life-threatening infection caused by Coxiella burnetii, accounts for 5-6% of culture-negative endocarditis cases and predominantly affects prosthetic or structurally abnormal valves. Diagnosis is frequently delayed by negative blood cultures and subtle or atypical echocardiographic findings.
CASE PRESENTATION: We describe a 45-year-old man with dual mechanical valves and ischemic cardiomyopathy who presented in cardiogenic shock with acute decompensated heart failure and cardiogenic shock. Transesophageal echocardiography revealed severe prost

BACKGROUND AND AIM: Q fever, caused by Coxiella burnetii, is a globally distributed zoonosis with major public health and livestock production implications. Dromedary camels (Camelus dromedarius) are increasingly recognized as important reservoirs, particularly in arid and semi-arid regions. In Jordan, however, comprehensive national-level data integrating serology, molecular detection, and spatial epidemiology are lacking. This study aimed to estimate the serological and molecular prevalence of Q fever in camels, identify associated risk factors, and describe the spatial distribution of infec
