Optimizing Device Selection in Percutaneous Paravalvular Leak Closure: A Comparative Study of Different Transthoracic and Transesophageal Echocardiographic Techniques.

BACKGROUND: Clinically significant paravalvular leak (PVL) may complicate both surgical and transcatheter valve implantation. Percutaneous PVL closure (PPVLC) is becoming an increasingly attractive alternative to redo surgery, with demonstrated lower mortality and morbidity rates. Echocardiographic techniques are crucial for accurate diagnosis, defect sizing, and determining the appropriate size of the sealing devices. AIMS: There is no consensus on the optimal imaging modality for PVLs. We aimed to compare transthoracic and transesophageal echocardiographic measurements to accurately determi

The Prognostic Value of Platelet-Albumin-Bilirubin Score in Patients Undergoing Transcatheter Aortic Valve Replacement.

BACKGROUND: Transcatheter aortic valve replacement (TAVR) has emerged as a well-established treatment option for patients with aortic valve stenosis and/or regurgitation. However, risk stratification in patients indicated for TAVR remains challenging. This study aimed to evaluate the predictive value of the platelet-albumin-bilirubin (PALBI) score on post-TAVR mortality. METHODS: A total number of 812 patients with aortic stenosis and/or aortic regurgitation who underwent TAVR were consecutively enrolled in this study. PALBI score was calculated based on preoperative baseline bilirubin levels

Transcatheter aortic valve-in-valve implantation in failed stentless valves: a single-center experience.

OBJECTIVES: Valve-in-valve transcatheter aortic valve implantation (ViV-TAVI) offers an alternative to repeat cardiac surgery in selected patients. However, ViV-TAVI in previously implanted stentless valves is more challenging than in a stented valve. This single-center study aimed to analyze the outcomes of ViV-TAVI in stentless valves. METHODS: Data were retrospectively collected from 1476 patients undergoing TAVI between January 2018 and December 2023. The primary study endpoint was 30-day and follow-up mortality. Secondary outcomes included Valve Academic Research Consortium (VARC)-3 tech

Isolated Atrial Amyloidosis in a Patient with Aortic Regurgitation and Paroxysmal Atrial Fibrillation.

A 65-year-old woman was admitted to our hospital with heart failure secondary to aortic regurgitation and paroxysmal atrial fibrillation (AF). During surgery for valve-sparing aortic root replacement, a biopsy was obtained from the right atrium because of paroxysmal AF and mild left ventricular hypokinesis. A histopathological examination revealed atrial natriuretic peptide amyloidosis, and the patient was clinically diagnosed with isolated atrial amyloidosis (IAA). Although IAA is rarely diagnosed clinically, it should be considered in patients with AF and heart failure.

Age-Related Outcomes of Transcatheter Aortic Valve Replacement in Patients with Pure Severe Aortic Regurgitation.

INTRODUCTION: Transcatheter aortic valve replacement (TAVR) is increasingly used, especially in older and high-risk surgical patients with pure severe aortic regurgitation (PSAR). However, the efficacy of TAVR in younger patients with aortic regurgitation (AR) remains unclear. This study explored the therapeutic efficacy and safety of TAVR in these two groups and compared their outcomes. METHODS: Patients with AR who underwent TAVR at Zhongshan Hospital between June 2021 and September 2023 were included and categorized into younger and older age groups based on whether they were less than 75

Paediatric aortic regurgitation secondary to Behçet's disease: aortic cusp replacement with autologous pericardium.

Behçet's disease is a multisystem inflammatory disorder, occasionally complicated by the need for aortic surgery due to vasculitis in children in whom growth potential is crucial. This report details the case of a 4-year-old girl with severe aortic regurgitation secondary to Behçet's disease. An initial echocardiographic scan revealed a thickened, prolapsed aortic valve with severe regurgitation, leading to a confirmed diagnosis of Behçet's disease with aortic regurgitation. Following stabilization with steroids and immunosuppressants, aortic cusp replacement surgery using autologous pericardi

An Infective Endocarditis Case Report Involving Both Native Aortic and Mitral Valves Due to Streptococcus Vestibularis.

Infective endocarditis (IE) is a life-threatening disease, with its mortality rate varying depending on the infectious agent. Streptococci are among the most common causes of infective endocarditis. However, Streptococcus vestibularis has rarely been associated with human infections, typically affecting patients with underlying conditions such as immunosuppressive diseases, valve replacement, rheumatic heart disease, and hemodialysis. We present the case of a 26-year-old man who presented with fever, unanticipated weight loss, and fatigue. Although no typical risk factors for infective endocar

Impact of annulus-cusp mismatch on mid-term outcomes of aortic valve repair with valve-sparing aortic root replacement.

OBJECTIVES: This study aimed to investigate mid-term outcomes of aortic valve repair with valve-sparing aortic root replacement based on different grades of annulus-cusp mismatch and identify optimal aortic root geometries for this procedure. METHODS: A retrospective analysis was conducted between October 2011 and July 2022. Patients were stratified into three groups based on predicted coaptation length calculated using an annulus-cusp mismatch formula: no-mismatch (coaptation length > 4 mm, n = 52), mild-mismatch (2 mm ≤ coaptation length ≤ 4 mm, n = 28) and severe-mismatch (coaptation lengt

Mitral Regurgitation in the Setting of Acute Aortic Regurgitation: It's All About Timing.

Presystolic mitral regurgitation is a rare, echocardiographic finding that is often ignored and misdiagnosed given its occurrence in the diastolic phase of the cardiac cycle. However, its presence indicates underlying structural or functional abnormalities that warrant further investigation, such as severe aortic regurgitation, atrioventricular dyssynchrony, and cardiomyopathies with elevated left ventricular end-diastolic pressure. The management of presystolic mitral regurgitation focuses on repairing the underlying pathology. In this E-challenge, the authors review a case of presystolic mit

Multimodal imaging in the assessment of quadricuspid aortic valve.

Quadricuspid aortic valve (QAV) is a rare congenital anomaly of the aortic valve, with an incidence of 0.05-0.1%, often associated with aortic regurgitation. The condition typically presents between the ages of 46 and 50, with a slight male predominance. While diagnosis is generally made via transthoracic echocardiography (TTE), this method can occasionally fail to identify QAV, necessitating the use of transoesophageal echocardiography and cardiac computed tomography for more accurate assessment of valve morphology. We present the case of a 57-year-old male who experienced chest pain for thre

Totally 3-dimensional endoscopic ventricular septal defect closure and aortic valve replacement via a periareolar incision.

There are no prior reports of totally endoscopic minimally invasive cardiac surgery for combined ventricular septal defect closure and aortic valve replacement. We utilized a periareolar incision as the main access, inserting a cardioplegia line, vent tube and the aortic cross-clamp entirely through this incision. Only three ports (main incision, camera port and left-hand port) were used. A three-dimensional endoscope facilitated precise techniques. The type I ventricular septal defect was approached and closed from the aortic side using a Gore-Tex patch, with mattress sutures for the lower ed

A case of constrictive pericarditis with aortic insufficiency: the role of cardiac magnetic resonance imaging.

Transthoracic echocardiography is recommended as a diagnostic test of choice for constrictive pericarditis. Nevertheless, several limitations exist. Hereby, we present the role of cardiac magnetic resonance imaging in a case of constrictive pericarditis with concurrent aortic insufficiency that masked the echocardiographic features of ventricular interdependence.

Association of Aortic Root and Valve Morphology With De Novo Aortic Valve Regurgitation After Implantation of Left Ventricular Assist Device.

BACKGROUND: The development of aortic valve regurgitation (AR) negatively affects the survival of patients with continuous-flow left ventricular assist device (LVAD) support. Although several risk factors have been identified, little is known about the effect of preoperative aortic root and valve morphology on the development of de novo AR after LVAD implantation. METHODS: Between April 2018 and September 2023, 87 patients underwent durable LVAD implantation at our department. Of these, the 15 eligible patients who underwent preoperative electrocardiography-synchronized cardiac contrast-enhan

Improved quantification of aortic regurgitation with direct regurgitant jet measurement by four-dimensional flow cardiovascular magnetic resonance in complex congenital heart disease.

BACKGROUND: Due to the presence of complex flow states and significant jet eccentricity in patients with congenital heart disease (CHD), accurate quantification of aortic regurgitation (AR) using standard echocardiographic or conventional cardiovascular magnetic resonance (CMR) imaging measures remains challenging. Four-dimensional flow (4DF) CMR permits transvalvular flow quantification under non-laminar flow states, although it has not been well validated for AR quantification in CHD. METHODS: In 186 patients with moderate or complex CHD, we evaluated the agreement between different methods

A new guiding strategy for treating the free margin of the leaflet in an aortic root remodelling procedure.

BACKGROUD: In valve-sparing aortic root replacement procedure, leaflet prolapse can be corrected by shortening the length of the free margin. But precisely determining the extent of the reduction remains a problem. This study wants to explore the effectiveness of a guiding strategy in treating the free margin of the leaflet in a modified aortic root remodelling procedure with external sub-valvular ring. METHODS: Between January 2021 and May 2024, 12 patients with aortic root aneurysms underwent modified aortic root remodelling with an external sub-valvular ring (10 males and two females). The
