Longitudinal Left Atrial Remodeling With Pharmacological Rhythm Versus Rate Control in Paroxysmal and Persistent Atrial Fibrillation.

PURPOSE: Evidence regarding left atrial (LA) remodeling with pharmacological rhythm control remains limited, particularly in patients with atrial fibrillation (AF) outside heart-failure-specific populations. METHODS: This single-center retrospective cohort included 156 adults with paroxysmal or persistent AF receiving mutually exclusive pharmacological rate-control (n = 92) or rhythm-control (n = 64) therapy who underwent two transthoracic echocardiograms. LA remodeling was assessed using serial echocardiography, and clinical outcomes were evaluated after the second examination. RESULTS: Amo

An Explainable Machine Learning Model Integrating Clinical Parameters for Identifying Left Atrial Appendage Thrombus in Non-valvular Atrial Fibrillation.

OBJECTIVE: This study aimed to develop a predictive model for left atrial appendage thrombus (LAAT) in patients with non-valvular atrial fibrillation by integrating clinical and echocardiographic features using machine learning algorithms. The SHapley Additive exPlanations (SHAP) framework was applied to interpret the model and quantify the contribution of each predictor, thereby enhancing the accuracy and clinical interpretability of LAAT risk prediction. METHODS: In this retrospective study, 325 patients with non-valvular atrial fibrillation who underwent transesophageal echocardiography at

Pulsed field ablation with Renal denervation for persistent Atrial fibrIllation: Rationale and design of the PRAISE-AF randomized trial.

BACKGROUND: Atrial fibrillation (AF) is the most common sustained cardiac arrhythmia worldwide and is associated with increased risks of stroke, heart failure, hospitalization, disability, and mortality. Catheter ablation has become an established rhythm-control strategy for symptomatic AF; however, the recurrence rate after ablation remains substantial, especially in patients with persistent AF. Pulsed field ablation (PFA) is an emerging nonthermal ablation technology with myocardial tissue selectivity and a potentially improved safety profile compared with conventional thermal ablation. The Role of High-Normal Potassium Levels in the Maintenance of Sinus Rhythm Following Cardiac Surgery in Patients with Persistent Atrial Fibrillation.

HIGHLIGHTS: Potassium Optimization: Maintaining high-normal potassium levels (>4.4 mmol/L) during and after bypass significantly improves the restoration and maintenance of sinus rhythm.High Success Rates: 87% of patients achieved sinus rhythm post-surgery, with 71% remaining stable at 48 hours.Predictors of Failure: A larger left atrial diameter was identified as a primary reason for failed cardioversion and AF recurrence.Improved Recovery: Restoring sinus rhythm led to better hemodynamic stability and significantly reduced the duration of ICU stays. UNLABELLED: Atrial fibrillation (AF) is h

Adjunctive Low-Voltage Area Ablation Reduces Atrial Fibrillation Recurrence in Persistent Atrial Fibrillation: A Systematic Review, Meta-Analysis, and Trial Sequential Analysis.

BACKGROUND: Adjunctive low-voltage area ablation (LVAA) has been proposed as a substrate modification strategy to improve outcomes of catheter ablation in persistent atrial fibrillation (AF); however, randomized controlled trials (RCTs) evaluating LVAA have yielded inconsistent findings. OBJECTIVES: This meta-analysis sought to evaluate the efficacy of LVAA in addition to pulmonary vein isolation (PVI) compared with PVI alone in patients with persistent AF. METHODS: Electronic databases were searched from inception to September 2026 for eligible RCTs. The primary endpoint was recurrent AF/at

Unraveling the Enigma of Postablation Rhabdomyolysis: Diagnostic Dilemmas and Therapeutic Strategies.

Rhabdomyolysis is characterized by the breakdown of skeletal muscle tissue and the release of intracellular contents into the circulation, and it poses diagnostic challenges due to its heterogeneous presentation and potential for serious complications. We present a case of a 44-year-old male with a history of hypertension, anxiety, and persistent atrial fibrillation (AF) who developed rhabdomyolysis the morning after a scheduled catheter ablation. He presented with myalgias, muscle cramps, weakness, and brown-colored urine; laboratory studies revealed a creatine phosphokinase (CPK) of 23,757 U

Transient changes in P-wave terminal force after electrical cardioversion of persistent atrial fibrillation.

BACKGROUND: P-wave terminal force in lead V1 (PWTFV1) reflects left atrial remodeling and is associated with atrial fibrillation (AF) outcomes, but its time course after electrical cardioversion of persistent AF is unclear. METHODS: We retrospectively analyzed 108 patients with persistent AF after successful electrical cardioversion, with 12‑lead electrocardiograms at 30 min, 1 month, and 3 months. Terminal P-wave duration (PNTV1) and amplitude (PNAV1) in lead V1 were measured and PWTFV1 calculated as their product; changes are expressed in absolute magnitude. Correlations with left atrial di

One-year outcomes in patients with persistent atrial fibrillation using CARTOFINDER mapping with OCTARAY: a Shinshu-AB registry study.

INTRODUCTION: Persistent atrial fibrillation (PerAF) has no established method of catheter ablation. Although catheter ablation using CARTOFINDER has shown some therapeutic effects, the associated prolonged procedure time remains a challenge. The identification of focal or rotational activation sites by CARTOFINDER using OCTARAY™ Mapping Catheter (OCTARAY) suggests that it can more efficiently detect the activation sites owing to its greater coverage of the left atrium (LA). This study aimed to compare the outcomes of catheter ablation guided by CARTOFINDER versus BOX isolation (BOXI) in patie

Enhancing Persistent Atrial Fibrillation Ablation: The Added Value of Vein of Marshall Ethanol Infusion in a Tailored Strategy.

BACKGROUND: Outcomes following catheter ablation for persistent atrial fibrillation (AF) remain suboptimal. Low-voltage areas (LVA), as a surrogate for atrial fibrosis, have been targeted due to their association with arrhythmia recurrence. Ethanol infusion in the vein of Marshall (VoM) has emerged as an adjunctive strategy that may enhance ablation efficacy. We evaluated the effectiveness of a tailored ablation strategy guided by the presence of LVA and incorporating VoM ethanol infusion in patients with persistent AF. METHODS: This prospective, single-center study included patients referred

Building a surgical atrial fibrillation program: Programmatic evolution, safety, and three-year rhythm outcomes of hybrid ablation.

OBJECTIVE: Surgical ablation is a guideline-supported treatment for symptomatic persistent and long standing persistent atrial fibrillation (AF) refractory to catheter ablation, but how a program is built, refined, and sustained has not been described. We sought to describe the development of a de novo surgical ablation program as well as its procedural evolution, safety profile, and long-term rhythm outcomes. METHODS: We reviewed the programmatic evolution of our program and growth of procedural volume and complexity. We also analyzed the long-term electrophysiologic and safety outcomes of a

Rationale and design of a multicenter randomized trial of adjunctive non-pulmonary vein trigger ablation in persistent atrial fibrillation: The EXPORT trial.

BACKGROUND: Pulmonary vein (PV) isolation in paroxysmal atrial fibrillation (AF) is strongly endorsed with a class IA recommendation for those with symptomatic AF who are refractory to antiarrhythmic drug (AAD) therapy to reduce symptoms, recurrence, and disease progression. However, in persistent AF, the efficacy of PV isolation alone is less pronounced than in paroxysmal AF, underscoring the necessity of adjunctive ablation strategies. Whether ablation of non-PV triggers after PV isolation improves the long-term procedure outcome is unknown. The EXPORT trial aims to evaluate the effects of s

Hybrid Ablation and Edge-to-Edge Repair for Atrial Fibrillation Induced Cardiomyopathy and Functional Mitral Regurgitation.

BACKGROUND: New minimally invasive techniques have been developed for cardiac abnormalities. CASE SUMMARY: We report a successful case of combining transcatheter edge-to-edge repair and convergent procedures to treat a 63-year-old woman who presented with functional mitral regurgitation, persistent atrial fibrillation, and tachycardia-induced cardiomyopathy with an ejection fraction of 35%. DISCUSSION: This case showcases the combined use of 2 relatively new, minimally invasive procedures. As experience with these procedures grows, they can be used for more unusual or extreme presentations.

Machine learning for enhancing the ablation outcomes using recurrence quantification analysis in human persistent atrial fibrillation.

BACKGROUND: Several ablation strategies, including dominant frequency (DF), complex fractionated atrial electrograms (CFAEs), and rotors, have been used to target the drivers of atrial fibrillation (AF). The success rate of these strategies remains suboptimal. OBJECTIVE: This work aims to develop a model using recurrence quantification analysis (RQA) variables and machine learning (ML) algorithms to predict the responses of ablating intracardiac electrograms (EGMs) and their impact on terminating AF and cycle length changes. METHODS: 3206 non-contact EGMs were collected from 10 persistent AF

Learning Curve of Pentaspline Pulsed-Field Ablation With Posterior Wall Isolation for Persistent Atrial Fibrillation.

BACKGROUND: Pentaspline pulsed-field ablation (PFA) has emerged as a novel modality for pulmonary vein isolation (PVI) and posterior wall isolation (PWI) in persistent atrial fibrillation (AF). However, the learning curve associated with combined PVI + PWI and the contribution of anatomical factors to acute lesion completeness remain incompletely defined. METHODS: We analyzed 182 consecutive patients with persistent AF undergoing fluoroscopy- and intracardiac echocardiography-guided PFA. Acute incomplete lesions were defined as residual conduction requiring additional PFA applications after s

Pre-ablation rate control therapy and direct-to-catheter ablation in patients with persistent atrial fibrillation: A DECAAF II subanalysis.

BACKGROUND: For patients with persistent atrial fibrillation (PersAF), initial treatment strategies often involve rate or rhythm control before offering ablation. This study compared two approaches: (1) catheter ablation without prior medication (referred to as direct-to-catheter ablation, DTCA) and (2) catheter ablation after initial rate control with either diltiazem or metoprolol. OBJECTIVE: This study included two independentanalyses of patients with persistent atrial fibrillation (PersAF) undergoing catheter ablation. Aimed at evaluating the potential impact of pre-ablation rate control

Risk factors of failed electrical cardioversion in patients with persistent or long-standing persistent atrial fibrillation.

BACKGROUND/AIMS: Identifying clinical risk factors of failed electrical cardioversion (ECV) for persistent or long-standing persistent atrial fibrillation (AF) can guide selection of rhythm control. METHODS: A total of 1058 patients who underwent ECV for persistent or long-standing persistent AF at multiple centres were retrospectively reviewed. Patients were divided into three groups: group 1 maintained sinus rhythm (SR) for >1 year, group 2 maintained SR ≤1 year after ECV, and group 3 had failed ECV. SR maintenance was assessed via regular electrocardiography follow-ups or Holter. RESULTS:

From bench to bedside: The clinical relevance of atrial electrical remodeling in atrial fibrillation therapy.

Atrial electrical remodeling spans molecular, electrical, and structural alterations that shorten refractoriness, facilitate reentry, and ultimately create the atrial substrate underlying atrial fibrillation (AF). These changes include ion channel dysfunction, calcium-handling abnormalities, oxidative injury, and deranged proteostasis that are tightly intertwined with atrial fibrosis and cardiomyopathy. High-density mapping and advanced imaging now allow in vivo staging of this substrate: left atrial low-voltage areas and functional conduction phenomena identify regions of fibrosis and conduct
