UNLABELLED: A 50-year-old man with long-standing persistent atrial fibrillation received radiofrequency catheter ablation after electrical cardioversion. The second radiofrequency application on anterior antrum of the right superior pulmonary vein induced sinus arrest following a sinus cycle length prolongation. Sinus arrest persisted. Right ventricular pacing was required for 3 min until a sinus rhythm was restored. Preprocedural contrast-enhanced computed tomography revealed that the ablation site was near the right sinus node artery (SNA). Retrospective analysis during catheter ablation con

BACKGROUND: Pulsed field ablation (PFA) is increasingly applied in persistent atrial fibrillation (PerAF) and long-standing persistent atrial fibrillation (LSPAF). Previous reviews of mixed AF populations suggested potential advantages in procedural efficiency and esophageal safety, but PerAF/LSPAF-specific evidence on long-term rhythm outcomes and on the value of posterior wall or extended substrate strategies remains limited, with substantial heterogeneity in lesion sets, endpoint definitions, and comparator types.
METHODS: We searched PubMed, Web of Science, Embase, Cochrane Library, and S

BACKGROUND: Radiofrequency ablation (RFA) combined with left atrial appendage closure (LAAC) has long been used in the treatment of atrial fibrillation (AF). Pulsed field ablation (PFA) is now increasingly adopted due to its great precision and improved safety profile. This report presents a rare case of PFA performed for persistent AF and atrioventricular reentrant tachycardia (AVRT), along with LAAC, under non-intubated general anesthesia.
CASE DESCRIPTION: A 36-year-old male patient presented with a 10-year history of recurrent chest tightness and palpitations, which had worsened during th

OBJECTIVE: This study aimed to develop and validate a predictive nomogram for heart failure in patients with hypertension and persistent atrial fibrillation (HT-PeAF).
METHODS: We retrospectively analyzed clinical data from patients with HT-PeAF hospitalized at Chinese People's Liberation Army General Hospital between 2010 and 2019. Predictive variables were selected based on multivariable logistic regression analysis and clinical relevance, which were then incorporated into a clinical nomogram. Model performance was evaluated using the area under the receiver operating characteristic curve (

BACKGROUND: Pulsed field ablation (PFA) and percutaneous closure of the left atrial appendage are increasingly used in patients with atrial fibrillation. An interaction between this form of energy and metallic structures in the heart is an emerging problem. We first report a left atrial appendage closure (LAAC)-limiting PFA using a novel dual energy ablation catheter, solved by switching to the classic radiofrequency ablation.
CASE SUMMARY: An 85-year-old woman with heart failure with preserved ejection fraction and a history of persistent atrial fibrillation (status post pulmonary vein isola

BACKGROUND: Incomplete left atrial appendage clip closure is a recognized complication and may increase thromboembolic risk.
CASE SUMMARY: A 72-year-old woman with persistent atrial fibrillation presented with a left middle cerebral artery stroke while on apixaban. History included mitral valve repair with epicardial left atrial appendage clip and AV-junction ablation with a leadless pacemaker. Local transesophageal echocardiogram (TEE) reported complete ligation without thrombus; cardiac computed tomography at our institution demonstrated incomplete ligation with a narrow residual ostium and

PURPOSE: Evidence regarding left atrial (LA) remodeling with pharmacological rhythm control remains limited, particularly in patients with atrial fibrillation (AF) outside heart-failure-specific populations.
METHODS: This single-center retrospective cohort included 156 adults with paroxysmal or persistent AF receiving mutually exclusive pharmacological rate-control (n = 92) or rhythm-control (n = 64) therapy who underwent two transthoracic echocardiograms. LA remodeling was assessed using serial echocardiography, and clinical outcomes were evaluated after the second examination.
RESULTS: Amo

OBJECTIVE: This study aimed to develop a predictive model for left atrial appendage thrombus (LAAT) in patients with non-valvular atrial fibrillation by integrating clinical and echocardiographic features using machine learning algorithms. The SHapley Additive exPlanations (SHAP) framework was applied to interpret the model and quantify the contribution of each predictor, thereby enhancing the accuracy and clinical interpretability of LAAT risk prediction.
METHODS: In this retrospective study, 325 patients with non-valvular atrial fibrillation who underwent transesophageal echocardiography at

BACKGROUND: Atrial fibrillation (AF) is the most common sustained cardiac arrhythmia worldwide and is associated with increased risks of stroke, heart failure, hospitalization, disability, and mortality. Catheter ablation has become an established rhythm-control strategy for symptomatic AF; however, the recurrence rate after ablation remains substantial, especially in patients with persistent AF. Pulsed field ablation (PFA) is an emerging nonthermal ablation technology with myocardial tissue selectivity and a potentially improved safety profile compared with conventional thermal ablation. BACKGROUND: Adjunctive low-voltage area ablation (LVAA) has been proposed as a substrate modification strategy to improve outcomes of catheter ablation in persistent atrial fibrillation (AF); however, randomized controlled trials (RCTs) evaluating LVAA have yielded inconsistent findings.
OBJECTIVES: This meta-analysis sought to evaluate the efficacy of LVAA in addition to pulmonary vein isolation (PVI) compared with PVI alone in patients with persistent AF.
METHODS: Electronic databases were searched from inception to September 2026 for eligible RCTs. The primary endpoint was recurrent AF/at

Rhabdomyolysis is characterized by the breakdown of skeletal muscle tissue and the release of intracellular contents into the circulation, and it poses diagnostic challenges due to its heterogeneous presentation and potential for serious complications. We present a case of a 44-year-old male with a history of hypertension, anxiety, and persistent atrial fibrillation (AF) who developed rhabdomyolysis the morning after a scheduled catheter ablation. He presented with myalgias, muscle cramps, weakness, and brown-colored urine; laboratory studies revealed a creatine phosphokinase (CPK) of 23,757 U

BACKGROUND: P-wave terminal force in lead V1 (PWTFV1) reflects left atrial remodeling and is associated with atrial fibrillation (AF) outcomes, but its time course after electrical cardioversion of persistent AF is unclear.
METHODS: We retrospectively analyzed 108 patients with persistent AF after successful electrical cardioversion, with 12‑lead electrocardiograms at 30 min, 1 month, and 3 months. Terminal P-wave duration (PNTV1) and amplitude (PNAV1) in lead V1 were measured and PWTFV1 calculated as their product; changes are expressed in absolute magnitude. Correlations with left atrial di

INTRODUCTION: Persistent atrial fibrillation (PerAF) has no established method of catheter ablation. Although catheter ablation using CARTOFINDER has shown some therapeutic effects, the associated prolonged procedure time remains a challenge. The identification of focal or rotational activation sites by CARTOFINDER using OCTARAY™ Mapping Catheter (OCTARAY) suggests that it can more efficiently detect the activation sites owing to its greater coverage of the left atrium (LA). This study aimed to compare the outcomes of catheter ablation guided by CARTOFINDER versus BOX isolation (BOXI) in patie

BACKGROUND: Outcomes following catheter ablation for persistent atrial fibrillation (AF) remain suboptimal. Low-voltage areas (LVA), as a surrogate for atrial fibrosis, have been targeted due to their association with arrhythmia recurrence. Ethanol infusion in the vein of Marshall (VoM) has emerged as an adjunctive strategy that may enhance ablation efficacy. We evaluated the effectiveness of a tailored ablation strategy guided by the presence of LVA and incorporating VoM ethanol infusion in patients with persistent AF.
METHODS: This prospective, single-center study included patients referred

Atrial fibrillation (AF) is an abnormal heart rhythm related to an increased risk of heart failure, dementia, and stroke. The distinction between valvular and non-valvular AF remains a debate. In this study, proteomics and metabolomics were integrated to describe the dysregulated metabolites and proteins of AF patients relative to sinus rhythm (SR) patients. Totally 47 up-regulated and 41 down-regulated proteins in valvular AF, and 59 up-regulated and 149 down-regulated proteins in non-valvular AF were recognized in comparison to SR patients. Moreover, 58 up-regulated and 49 significantly down

OBJECTIVE: Surgical ablation is a guideline-supported treatment for symptomatic persistent and long standing persistent atrial fibrillation (AF) refractory to catheter ablation, but how a program is built, refined, and sustained has not been described. We sought to describe the development of a de novo surgical ablation program as well as its procedural evolution, safety profile, and long-term rhythm outcomes.
METHODS: We reviewed the programmatic evolution of our program and growth of procedural volume and complexity. We also analyzed the long-term electrophysiologic and safety outcomes of a

BACKGROUND: Pulmonary vein (PV) isolation in paroxysmal atrial fibrillation (AF) is strongly endorsed with a class IA recommendation for those with symptomatic AF who are refractory to antiarrhythmic drug (AAD) therapy to reduce symptoms, recurrence, and disease progression. However, in persistent AF, the efficacy of PV isolation alone is less pronounced than in paroxysmal AF, underscoring the necessity of adjunctive ablation strategies. Whether ablation of non-PV triggers after PV isolation improves the long-term procedure outcome is unknown. The EXPORT trial aims to evaluate the effects of s

BACKGROUND: New minimally invasive techniques have been developed for cardiac abnormalities.
CASE SUMMARY: We report a successful case of combining transcatheter edge-to-edge repair and convergent procedures to treat a 63-year-old woman who presented with functional mitral regurgitation, persistent atrial fibrillation, and tachycardia-induced cardiomyopathy with an ejection fraction of 35%.
DISCUSSION: This case showcases the combined use of 2 relatively new, minimally invasive procedures. As experience with these procedures grows, they can be used for more unusual or extreme presentations.
