BACKGROUND: Persistent left superior vena cava (PLSVC) is associated with coronary sinus (CS) thrombosis.
CASE SUMMARY: An 85-year-old woman with PLSVC presented with fever due to acute pyelonephritis. Transthoracic echocardiography revealed a mural thrombus within a dilated CS, with a contiguous free-floating thrombus extending into the right atrium. Anticoagulation with warfarin resulted in resolution of the free-floating component; however, the mural CS thrombus persisted.
DISCUSSION: This case illustrates the interplay between congenital venous anomalies and transient hypercoagulability

BACKGROUND: Inferior vena cava filters are used in patients with recurrent pulmonary embolism and/or deep vein thrombosis who have contraindications to anticoagulation or in whom it has failed. Common complications are related to vascular access and deployment; less frequent complications include mechanical failure such as filter strut fracture.
CASE SUMMARY: We present the case of a 58-year-old woman who had severe cough followed by expectoration of a metallic fragment identified as a fractured strut of a previously implanted inferior vena cava filter (in 2014) (Celect, Cook Medical). A seco

BACKGROUND: Severe pectus excavatum (PE) can distort cardiac anatomy and alter intracardiac flow, yet its contribution to device-associated thrombosis is rarely reported.
CASE SUMMARY: A 41-year-old woman with severe PE and a patent foramen ovale closed using an occluder developed recurrent pulmonary and systemic embolic events despite anticoagulation. Imaging revealed biatrial compression, resulting in biatrial injury and device-associated thrombosis. With ongoing left atrial thrombus growth and cerebral embolic events while on anticoagulation therapy, she underwent a single-stage procedure:

Acute myocardial infarction (AMI) remains a leading cause of morbidity and mortality worldwide. Despite advances in early reperfusion and percutaneous coronary intervention (PCI), a subset of patients develops severe arrhythmic, thrombotic, and mechanical complications that markedly worsen prognosis and require rapid recognition and multidisciplinary management. We describe a 66-year-old man with multiple cardiovascular risk factors who presented with an anteroseptal ST-elevation myocardial infarction and underwent urgent PCI with stent implantation in the left anterior descending artery. Eigh

OBJECTIVE: Transthoracic echocardiography (TTE) is widely used for cardiac evaluation in acute ischemic stroke (AIS), but it may miss intracardiac thrombi that can be detected by transesophageal echocardiography (TEE). The stress hyperglycemia ratio (SHR), which adjusts admission glucose for baseline glycemia, has emerged as a novel biomarker in cardiovascular disease. However, its association with intracardiac thrombus undetected by TTE in AIS remains unclear.
METHOD: This retrospective study included 486 AIS patients who underwent TEE despite negative baseline TTE findings. Demographic, lab

Behçet's disease (BD) is a rare vasculitis characterised by mucocutaneous lesions and ocular, vascular and neurological manifestations. We report a man in his early 20s who presented with dyspnoea, haemoptysis, pulmonary arterial thromboses and a large right ventricular thrombus. Initial investigations for infectious and autoimmune causes were negative, complicating diagnosis. A revisited history and new examination revealed oral and genital aphthosis, fulfilling the international criteria for BD. The patient was treated with anticoagulation and high-dose immunosuppression, resulting in rapid

Intracardiac thrombus in the right heart has a great risk of embolizing to the pulmonary circulation leading to great morbidity and mortality. Removal of large thrombi incurs an increased risk of embolization, especially if the clot is more acute and not fully adherent. We present a case of transcatheter removal of a thrombus at the right atrial and superior vena cava (RA-SVC) junction using a novel technique in a patient with transfusion-dependent Fanconi anemia status, bone marrow transplant, and steroid-resistant graft-versus-host disease using a coaxial system of a Penumbra Thrombectomy ca

BACKGROUND: Children with congenital and acquired heart disease (CAHD) are at high risk for venous thromboembolism (VTE). Traditional anticoagulants such as vitamin K antagonists (VKAs) and low-molecular-weight heparin (LMWH) present challenges in pediatrics due to burdensome administration, frequent monitoring, and adherence issues. Direct oral anticoagulants (DOACs) have transformed adult anticoagulation, but their role in pediatric CAHD remains uncertain. OBJECTIVE: To evaluate the efficacy and safety of DOACs compared with standard-of-care (SOC) anticoagulation in pediatric patients with C

Pediatric abdominal aortic aneurysms are exceedingly rare, particularly in infants, and pose unique diagnostic and surgical challenges. We present a case of an infant diagnosed antenatally with intracardiac rhabdomyomas who later presented with infantile spasms. Neuroimaging and genetic testing confirmed tuberous sclerosis complex with a pathogenic de novo TSC2 mutation. Tuberous sclerosis complex is associated with dysregulation of the mammalian target of rapamycin (mTOR) signaling pathway, leading to abnormal vascular smooth muscle proliferation, structural vessel wall weakness, and predispo

BACKGROUND: An inferior vena cava thrombus (IVCT) occurs in approximately 4-10% of Wilms tumor (WT) cases. The presence of an IVCT complicates surgical management and potentially affects patient outcomes. We reviewed all patients with a WT with and without an IVCT treated at our center, comparing their characteristics and outcome.
PATIENTS AND METHODS: We retrospectively reviewed pediatric patients with a WT treated between November 2014 and July 2023. Patients were categorized according to the presence or absence of an IVCT. Data on demographics, clinical presentation, imaging findings, trea

Endovascular foreign body fragmentation, migration, and embolization to the right heart is a rare and late complication of the central venous port systems. The MediPort (Bard Medsystems, Reading, Massachusetts, USA) device consists of a port chamber attached to a central catheter implanted into the central venous system and used in patients with cancer for the administration of chemotherapy, parenteral nutrition, or blood transfusions. Interval radiologic survey, both intraoperatively and post-procedure, is crucial for investigating the possibility of both early and late complications, such as

Multisystem inflammatory syndrome in children (MIS-C), a postinfectious complication of severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2), is characterized by hyperinflammation and a prothrombotic state. Thrombosis involving intracardiac devices is rare but potentially life-threatening. We report a 16-year-old girl who had transcatheter closure of a fossa ovalis atrial septal defect using an Amplatzer septal occluder at 4 years of age. During the ongoing coronavirus disease 2019 pandemic, she presented with fever, rash, elevated inflammatory markers, and positive SARS-CoV-2 antibody

Background: Left ventricular (LV) thrombosis after blunt trauma is uncommon and is most often attributed to traumatic coronary artery injury; however, it can also arise from severe myocardial contusions. Here, we report a case of LV thrombosis due to severe myocardial contusion without coronary artery injury. Case Presentation: A 36-year-old man struck by industrial fan fragments presented with hemorrhagic shock. Focused Assessment with Sonography for Trauma revealed cardiac tamponade. An emergent sternotomy was performed under cardiopulmonary bypass via the femoral vessels, which exposed seve

BACKGROUND: Intracardiac echocardiography (ICE) is increasingly used to guide left atrial appendage occlusion as an alternative to transesophageal echocardiography (TEE), particularly in elderly patients for whom general anesthesia may pose additional risks. Real-world comparative safety data in older adults remain limited, with prior studies including only small ICE cohorts. We aimed to compare short- and long-term outcomes of ICE- versus TEE-guided left atrial appendage occlusion in adults aged ≥80 years.
METHODS: We queried the TriNetX US Collaborative Network to identify patients aged ≥80

BACKGROUND: Behçet disease (BD) is a multisystem vasculitis with variable cardiovascular involvement. Hughes-Stovin syndrome (HSS), featuring pulmonary artery aneurysms and venous thrombosis, is considered a rare variant within the BD spectrum. Intracardiac mass formation in either vasculitis is exceptionally rare.
CASE SUMMARY: A 32-year-old man with prior mucocutaneous ulcers presented with hemoptysis and chest pain. Multimodality imaging revealed pulmonary arterial thrombosis and lobulated intracardiac mass attached to the native tricuspid valve (TV). Histopathology showed inflammatory org
