INTRODUCTION: Progressive multifocal leukoencephalopathy (PML) is a progressive demyelinating infection of the central nervous system. PML arises in immunosuppressive conditions or circumstances necessitating immunosuppressive treatment, such as malignancies. Recently, data have been published regarding the use of different immune checkpoint inhibitors (ICIs) for the treatment of PML. However, the extent to which ICIs impact and improve PML in individual patients remains uncertain.
CASE: A 68-year-old female patient, diagnosed with multiple myeloma in 2012 and with a history of using various

Progressive multifocal leukoencephalopathy (PML) is a rare, often fatal demyelinating disease of the central nervous system caused by reactivation of latent JC virus in immunocompromised individuals. Despite advances in understanding its pathogenesis, PML remains a diagnostic challenge due to its diverse clinical manifestations, radiographic overlap with other disorders, low incidence and subsequent lack of clinical familiarity. Diagnostic delays, often extending months after symptom onset, are common and contribute to poor outcomes. This review outlines the key clinical features, risk factors

INTRODUCTION: The majority of progressive multifocal leukoencephalopathy (PML) cases is still represented by patients affected by acquired immunodeficiency syndrome (AIDS). Diagnosis of PML relies on histopathological findings or by the combination of clinical signs, radiological evidence, and molecular positivity of the JC virus in cerebrospinal fluid. However, AIDS status predisposes to various diseases involving the brain, testing the diagnostic ability of the clinician.
CASE DESCRIPTION: We describe a PML case in a patient with AIDS, in whom lumbar puncture was initially impossible for se

BACKGROUND: Long-term brain atrophy and cognitive function have rarely been reported in patients without pre-existing brain disorders who survived progressive multifocal leukoencephalopathy (PML).
CASE PRESENTATION: Patient 1 was a 38-year-old woman with late-onset combined immunodeficiency, who presented with progressive cognitive decline and right hemiparesis. Brain MRI revealed a hyperintense lesion in the left frontal lobe. Patient 2 was a 53-year-old man in remission from follicular lymphoma, who experienced cognitive deterioration and displayed hyperintense signals in the left frontal a

This article reports the case of a 58-year-old male patient who presented with a generalized rash, intermittent fever, and a concomitant perianal abscess, as well as mycoplasma pneumoniae psittacosis. The initial symptom was episodic slurred speech, which progressively developed into symptoms such as delayed responses and left-sided limb weakness. Cranial magnetic resonance imaging revealed gradually progressive intracranial lesions characterized primarily by asymmetric white matter damage, with no enhancement upon contrast administration. Serum HIV antibody testing was positive. Next-generati

Polyomaviruses are non-enveloped viruses with double-stranded circular DNA genome. Currently, 13 members of the Polyomaviridae family have been classified as human polyomavirus (HPyVs). Despite high seroprevalence values have been reported for the majority of the HPyVs worldwide, the main mode of transmission remains to be elucidated, and simple and common routes, such as faecal-oral and respiratory have been suggested. In general, HPyVs are responsible for asymptomatic primary infection, followed by asymptomatic lifelong persistent infection. In situations of severe immunosuppression, viral r

INTRODUCTION: Neurological involvement in Wiskott-Aldrich syndrome (WAS) - an inborn error of immunity caused by mutations in the WAS gene - is primarily documented through isolated case reports, limiting systematic synthesis and clear characterization of clinical patterns. We conducted a systematic review to characterize central and peripheral nervous system involvement in patients with confirmed WAS.
METHODS: A systematic search was conducted in PubMed, Embase, Scopus, and Web of Science following the PRISMA 2020 guidelines. Studies reporting neurological manifestations in WAS were eligible

BK and JC Polyomaviruses (BKPyV and JCPyV) are ubiquitous human pathogens capable of establishing lifelong, asymptomatic persistence in the majority of the global population. While decades of research have focused on their lytic replication cycles and the development of severe diseases, such as polyomavirus-associated nephropathy (PVAN) caused by BKPyV and progressive multifocal leukoencephalopathy (PML) caused by JCPyV, their primary evolutionary strategy is one of persistence rather than pathogenesis. This review shifts the perspective from a replication-centric framework towards an evolutio

We report the intraoperative ultrasound findings in a male patient with history of stage IVb mantle cell lymphoma who developed new-onset personality changes and gait instability. Brain imaging revealed an extensive right frontal white matter lesion with subtle multifocal spreading. Given the absence of contrast enhancement on MRI and considering the high-dose steroid therapy administered, an open biopsy was performed due to suspicion of cerebral lymphoma. During procedure, intraoperative ultrasound with and without microbubble contrast was carried out, demonstrating distinct echotexture featu

Progressive multifocal leukoencephalopathy (PML) is a demyelinating central nervous system disease caused by John Cunningham virus (JCV) reactivation in immunosuppressed patients. Although classically associated with advanced HIV infection, PML also occurs in patients with hematological malignancies, organ transplants, or on immunomodulatory therapies. The mortality rate can be high, ranging from 20 to 90%, depending on the underlying condition. Clinical presentation is variable, depending on the site of brain involvement, and diagnosis requires integration of clinical, radiological, pathologi

Progressive multifocal leukoencephalopathy (PML) is a rare brain infection caused by JC polyomavirus (JCPyV) in immunocompromised individuals, for which no effective antiviral therapy exists. Diverse cellular therapy approaches using polyomavirus-specific T cells (PyVST) has emerged as a therapeutic option. We aimed to evaluate the real-world effectiveness of PyVST in patients with PML. We retrospectively analyzed 64 patients from 13 centers who received PyVST alongside standard care. Conditions predisposing to PML included mainly hematologic malignancies (33/64, 51.6%). One-year survival was

Bispecific antibodies (BsAb) are a class of T-cell-redirecting therapies used in a variety of advanced malignancies. BsAb can be associated with clinically significant toxicities, including cytokine release syndrome (CRS), immune effector cell-associated neurotoxicity syndrome (ICANS), and infections. CRS occurs in up to two thirds of patients and can be associated with ICANS in rare cases. CRS is best treated with tocilizumab, while glucocorticoids are effective in treating both. BsAb-associated CRS and ICANS are mostly low-grade and are confined to the step-up and first full doses. However,

Despite improved survival in recent years, long-term outcomes in patients surviving progressive multifocal leukoencephalopathy beyond the first year remain poorly defined. This international multicenter retrospective study aimed to characterize the three-year prognosis of patients with progressive multifocal leukoencephalopathy who had survived at least one year, identify factors associated with favorable outcomes and late mortality, and determine recurrence rate. Data were collected through standardized questionnaires across forty-one centers in twelve countries. Patients were eligible if the

An 81-year-old woman with follicular lymphoma treated with obinutuzumab and bendamustine developed cognitive impairment and dysarthria. Three months before death, neurological exams showed dysarthria, right hemiparesis, and gait disturbance. Blood tests showed lymphocytopenia (lymphocyte 10.4%). Cerebrospinal fluid (CSF) findings were unremarkable, including with respect to cytology. Brain MRI demonstrated a mass-like hyperintense lesion in the left parietal lobe and band-like abnormalities in the left fronto-temporal white matter that lacked contrast enhancement. Symptoms progressed to hemipl

PURPOSE: To report the clinical characteristics of JC virus (JCV) retinitis.
METHODS: A retrospective case report.
RESULTS: A 29-year-old male patient, diagnosed with HIV at AIDS stage, had brain lesions compatible with progressive multifocal leukoencephalopathy. Fundus examination showed areas of retinal atrophy and changes in the pigment epithelium. A similar worsening of the retinal and brain lesions was observed, despite several treatments. The vitreous sample analyzed using Next Generation Sequencing analysis was positive for JCV. No other pathogens were identified. The retinal lesions

BACKGROUND: Progressive multifocal leukoencephalopathy (PML) is a fatal demyelinating disease caused by JC polyomavirus (JCV) that affects immunosuppressed individuals and has no approved therapy. Here we evaluate the safety and efficacy of third-party, off-the-shelf, partially HLA-matched BK virus-specific T cells (BK-VST) in patients with PML.
METHODS: We conducted a single-center phase 2 study where 37 patients received most closely HLA-matched BK-VST at 2.0 x 105 T cells/kg, with additional infusions permitted for subjects who did not achieve a complete response. Twenty-three patients had
