Late-onset recurrent lupus nephritis in a kidney transplant recipient.

Long-term kidney allograft survival continues to be a challenge, despite improvements in the short term. Recurrence of glomerular disease is a significant cause of allograft failure. We report a case of graft dysfunction 27 years post transplant attributed to biopsy-proven recurrent class IV lupus nephritis. Following targeted medication adjustments aimed at treating lupus nephritis, the patient achieved complete remission within 6 months, which has been sustained for 4 years. Although recurrent lupus nephritis is reported to occur up to 16 years after transplantation, it is uncommon, and repr

Integrated Hemodynamic Measures of Cardiac Function and Association With Clinical Outcomes.

BACKGROUND: Aortic pulsatility index (API) and cardiac power output (CPO) are strongly associated with mortality in patients with cardiogenic shock, but their association with outcomes across broader cardiopulmonary disease is unknown. OBJECTIVES: The objective of the study was to examine the association of API and CPO with clinical outcomes across a hospital-based cardiovascular cohort. METHODS: We used a hospital-based longitudinal, observational cohort of patients who underwent right heart catheterization. We calculated API = aortic pulse pressure/pulmonary capillary wedge pressure and CP

Comparison of influenza hemagglutination inhibition titers from capillary blood versus venipuncture.

BACKGROUND: Capillary blood self-collection devices offer a minimally invasive alternative to venipuncture, potentially expanding access to serologic studies in outpatient and remote settings. We aimed to evaluate the concordance of influenza HAI titers between capillary blood collected using a minimally invasive device and venous blood collected via standard venipuncture. METHODS: We compared three paired samples collected from participants during a single visit: venipuncture, staff-assisted capillary device collection (clinic-collect), and participant self-collected capillary device with 72

Multimodal Imaging in Indolent Nonprogressive Multifocal Choroidal Lesions: A Case Report.

Indolent nonprogressive multifocal choroidal lesions are rare benign lymphoid infiltrative disorders confined to the choroid. A 53-year-old asymptomatic man was incidentally found to have a hyperdense lesion adjacent to the left optic nerve on head computed tomography. Fundus examination revealed multiple deep yellow-orange choroidal lesions bilaterally. Fluorescein angiography demonstrated late-phase staining in some lesions, whereas indocyanine green angiography showed more extensive hypofluorescent areas than were visible on color fundus imaging. Swept-source optical coherence tomography (O

Potentiation of GPR68 alleviates post-ischemia BBB dysfunction and brain edema in mice.

Ischemia-reperfusion can increase blood-brain barrier (BBB) permeability and extravasation of peripheral molecules. Further damage to BBB results in blood vessel rupture and hemorrhagic transformation (HT). Associated with these processes, brain edema is common. In previous studies, we and others have shown that Ogerin, a small molecule positive modulator of the acid-sensitive GPR68- reduces ischemic brain injury. Here, we asked whether Ogerin attenuates post-ischemia BBB leakage, HT, or edema. To determine HT and edema, we first analyzed the 2,3,5-triphenyltetrazolium chloride (TTC) images fr

A Hidden Shunt Behind Post-COVID-19 Dyspnea: Partial Anomalous Pulmonary Venous Connection.

Persistent exertional dyspnea following SARS-CoV-2 infection is frequently attributed to post-viral pulmonary pathology. When symptoms are accompanied by right ventricular (RV) dilation and pulmonary hypertension without elevated left-sided filling pressures, alternative etiologies including congenital heart disease must be considered. A 52-year-old man with prior COVID-19 pneumonia presented with one year of progressive exertional dyspnea. Transthoracic echocardiography demonstrated preserved left ventricular systolic function with RV dilation. Right heart catheterization revealed mild precap

The glucose monitoring comparison in primary care study (GluCoCare): Study design, methods, recruitment success, and baseline characteristics of study participants.

BACKGROUND: While both fingerstick blood glucose monitoring (BGM) and continuous glucose monitoring (CGM) are currently available for people managing type 2 diabetes (T2D) with insulin, it is not currently known which is more effective in optimizing glycemia and decreasing disease burden in this population in real-world settings. OBJECTIVES: This two-arm cluster-randomized trial will compare use of BGM versus CGM in individuals with T2D using insulin not meeting A1C goals. We will compare change in hemoglobin A1c (A1C) (primary outcome measure) and Diabetes Distress Scale score (DDS) (seconda

Rare Models: Use of a Novel Translational Model for Diffuse Alveolar Haemorrhage.

Diffuse alveolar haemorrhage (DAH) is rare but associated with high morbidity and mortality. It is underpinned by mechanisms of endothelial dysfunction, inflammation, and vascular remodelling, yet its pathogenesis remains incompletely understood. Progress in translational research is currently limited by the absence of robust animal models of this condition. Equine exercise-induced pulmonary haemorrhage (EIPH), a common and progressive disorder in racehorses, may represent a novel and underutilised translational model for studying DAH, which can be a complication of or associated with other ca

Hypoxia Induces Adaptive Lymphangiogenesis via Cd74 and Vegfr3 to Modulate Pulmonary Hypertension.

BACKGROUND: Pulmonary arterial hypertension (PAH) is characterized by excessive remodeling of the distal arterioles and arteries, driven by endothelial cell (EC) apoptosis and uncontrolled mural cell proliferation. Increasing evidence suggests immune dysregulation in PAH, but one crucial part of the immune system, the pulmonary lymphatics, has been largely overlooked. Patients with idiopathic PAH (IPAH) often develop abnormal tertiary lymphoid structures adjacent to remodeled arteries, yet the role of lymphatics in PAH pathogenesis and vascular remodeling remains unclear. METHODS: Mice with l

Stepwise Interpretation of Albumin Fraction Abnormalities on Serum Protein Capillary Electrophoresis in Nephrotic Syndrome: A Case Series.

Serum protein electrophoresis (SPEP) is commonly used in the diagnostic evaluation of nephrotic syndrome (NS) to rule out monoclonal gammopathies. However, NS itself and various other factors may cause transient bisalbuminemia, resulting in albumin fraction irregularities in capillary SPEP and complicating interpretation. This study proposes a stepwise approach for interpreting albumin fraction irregularities, illustrated through three representative cases of NS. Case 1: a 44-year-old woman with isolated proteinuria, Case 2: a 36-year-old woman with diabetic foot infection, and Case 3: a 58-ye

Insight to Neglected Biomarkers in COVID-19: A Comprehensive Narrative Review".

In the context of COVID-19, a range of neglected biomarkers provide critical insights into the mechanisms of the disease and potential therapeutic targets. This review aims to address this gap by systematically analyzing the diagnostic and prognostic potential of these neglected biomarkers, with particular emphasis on their mechanistic connections to COVID-19 pathophysiology. Reduced levels of adiponectin and prostacyclin (PGI2) and elevated level of endothelin are associated with endothelial dysfunction, whereas elevated levels of endocan and endoglin are indicative of elevated vascular infla

Enhanced deep learning model for prediction of diabetic mellitus on optical coherence tomography angiography images.

BACKGROUND: Diabetes mellitus (DM) is a chronic metabolic disease characterized by dysregulated blood glucose. Prolonged DM can lead to diabetic retinopathy (DR), in which retinal capillaries are damaged by sustained hyperglycemia. Optical coherence tomography angiography (OCTA) is a non-invasive imaging modality for visualizing retinal microvasculature and can detect early changes in both DM patients with and without DR. However, it requires expert evaluation, making early detection costly and time-consuming. This study aimed to develop a high‑performance deep learning framework that can clas

Identification of pulmonary arterial hypertension patients with venous or capillary involvement.

RATIONALE: While therapeutic advances have improved survival for patients with Group 1 pulmonary arterial hypertension (PAH), patients with Group 1.5 "PAH with features of venous/capillary involvement" (formerly pulmonary veno-occlusive disease or pulmonary capillary hemangiomatosis, now collectively termed PVOD/PCH) remain underrecognized, develop serious complications from usual PAH therapy titrations, and suffer high mortality awaiting necessary lung transplant. Identifying PVOD/PCH early before therapy initiation could aid management and expedite transplant referral. OBJECTIVES: We aimed

Evaluation and Management of Genetic Respiratory Disorders Presenting as Hypoxemic Respiratory Failure in the Newborn Infant.

Hypoxemic respiratory failure is a common reason for admission to the neonatal intensive care unit for term and late preterm infants. Some infants have rare disorders due to genetic mechanisms including surfactant dysfunction disorders, alveolar capillary dysplasia with misalignment of the pulmonary veins, and other developmental lung disorders that result in more severe, persistent, and even fatal disease. In this article, we summarize clinical characteristics of the most common causes of neonatal respiratory failure and of rare genetic causes, to help clinicians differentiate between common
