Exploring the association between interoceptive awareness and neuropsychiatric symptoms in people living with Parkinson's disease.

Interoceptive awareness (IA) supports emotional processing and may be affected in people with Parkinson's disease (PwP), where depression and anxiety are common. We examined associations between IA, neuropsychiatric symptoms, cognition, and disease severity in PwP. In a case-control study, 50 PwP and 25 controls completed the MAIA, HDRS, HARS, MoCA, and MDS-UPDRS. IA did not differ between groups. In PwP, lower MAIA Not-Worrying and Self-Regulation scores correlated with depression, anxiety, and disease severity. In addition, lower MAIA Self-Regulation scores correlated with higher orthostatic

Orthostatic hypotension as a marker of increased arterial stiffness in diabetes mellitus.

BACKGROUND: Orthostatic hypotension (OH) is associated with adverse cardiovascular outcomes and may reflect underlying autonomic and vascular dysfunction. Arterial stiffness is a key determinant of cardiovascular risk; however, its relationship with OH in patients with diabetes mellitus (DM) remains unclear. OBJECTIVE: To evaluate factors associated with OH and investigate the relationship between arterial stiffness parameters and OH in patients with DM. METHODS: This single-center cross-sectional study included 193 patients with DM. Orthostatic blood pressure was measured in the supine posi

Clinical prognostic indicators in multiple system atrophy.

Multiple system atrophy (MSA) is a neurodegenerative condition causing parkinsonism, cerebellar ataxia and/or dysautonomia. Typical survival is between 6-10 years, but some people die before five or after 15 years. This heterogeneity complicates advanced planning and clinical trial stratification. MSA prognostication studies have shown conflicting results, possibly due to diagnostic accuracy or study size. We report results from a study of survival prognostic factors in a cohort of 555 MSA patients (including the largest post-mortem confirmed cohort to date of 254 people) gathered through the

[A clinical case of a mixed variant (cardiomyopathy and polyneuropathy) of hereditary transthyretin amyloidosis].

Amyloidosis associated with mutations in the transthyretin gene is the most common form of hereditary systemic amyloidosis. The onset and phenotype of the disease depend on the type of mutation, but the peripheral nervous system and heart are usually affected, and the age of onset of the disease varies from the third to the fifth decade. For the first time in Kuzbass, a case of hereditary transthyretin amyloidosis with a mutation in exon 3 of the TTR gene c.218G>A (Gly73Glu) in a heterozygous state was diagnosed in a 52-year-old man, which is characterized by a long course and late diagnosis (

Blood Pressure Assessment Across the Lifespan: Improving Clinical Research and Clinical Practice: A National Heart, Lung, and Blood Institute Workshop Report.

Hypertension is a modifiable risk factor for cardiovascular disease (CVD) and its prevalence is high in the United States and worldwide. Adequate characterization of blood pressure (BP) is essential for the diagnosis and management of hypertension. However, BP assessment can be challenging because of the unique influences across the lifespan, disease conditions, and physical environmental context. Moreover, complex uncertainties in BP assessment may contribute to underdiagnosis, undertreatment, and preventable morbidity and mortality. Recent advances in BP measurement devices have enabled comp

Iatrogenic Pan-Right Atrial Denervation: Biphasic Autonomic Crash After Complex Scar-Related Flutter Ablation.

BACKGROUND: Right atrial (RA) ablation is routine; however, the RA harbors critical hubs of the intrinsic cardiac nervous system. Extensive ablation can cause autonomic dysfunction. CASE SUMMARY: A 78-year-old woman with prior mitral valve repair underwent ablation for scar-related atrial flutter. Ablation involving the cavotricuspid isthmus, posterior wall, and superior vena cava caused a biphasic autonomic crash. Initially, hypotension was refractory to dopamine infusion for 3 days. Stabilization was achieved after switching to norepinephrine, consistent with profound vasoplegia. Norepineph

Risk of falls and syncope across frailty levels in hypertensive older adults: a longitudinal study.

In older hypertensive adults, frailty may increase the risk of adverse events associated with blood pressure lowering, including syncope and falls. However, the degree of frailty at which these risks become clinically significant remains uncertain. The study aimed to compare clinical characteristics and the incidence of falls and syncope across different frailty levels among older hypertensive adults. We analyzed data from the longitudinal HYPER-FRAIL study, involving hypertensive outpatients ≥ 75 years undergoing comprehensive geriatric assessment. Frailty was defined according to the Clinica

Tolerability and efficacy of full-body head-up tilt sleeping in Parkinson's disease and multiple system atrophy.

Orthostatic hypotension (OH) and the co-existence with supine hypertension (SH) are debilitating consequences of autonomic failure in Parkinson's disease (PD) and multiple system atrophy (MSA). Head-up tilt sleeping (HUTS) has the potential to treat both simultaneously. Within the Heads-Up trial, 20 persons with PD or MSA and both SH and OH were assigned randomly to two HUTS schemes with gradually increasing angles: 1° (placebo), 6° and 12°; or 6°, 12° and 18°. The effect of increasing the head of the bed (per cm) was analyzed with linear mixed models. HUTS did not reduce nocturnal SBP but imp

Risk of adverse events associated with co-administration of quetiapine and Bupleuri Radix: A population-based cohort study.

INTRODUCTION: Bupleuri Radix is a widely used herbal medicine in Asia. Experimental studies suggest it may modulate Cytochrome P450 3A4 (CYP3A4), the primary metabolic pathway of quetiapine, which is a first-line medication for psychosis, potentially altering drug exposure and raising concerns about pharmacokinetic interactions. We examined the risk of acute adverse events associated with concomitant use of Bupleuri Radix-containing formulations and quetiapine. METHODS: We conducted a retrospective cohort study using a Japanese claims database. Individuals aged 18-75 years initiating quetiapi

Orthostatic Blood Pressure, Cardiovascular Disease, and Hypotensive Events.

BACKGROUND: Orthostatic hypotension is thought to be associated with coronary heart disease, falls, and syncope due to low blood pressure (BP) upon standing. METHODS: The ARIC (Atherosclerosis Risk in Communities) study measured supine and standing BP among adult participants aged 45 to 64 years once at baseline and followed them for over 35 years. We evaluated higher and lower supine and standing systolic BP, diastolic BP, mean arterial pressure, pulse pressure, absolute and relative orthostatic changes in BP after standing, and mean BP across positions. Orthostatic blood pressure transitions: Association with symptoms and frailty.

OBJECTIVE: Individuals with hypertension may transition between different orthostatic blood pressure (BP) phenotypes within minutes. We studied the temporal patterns of orthostatic BP responses and their relationship to symptom development in older hypertensive individuals with and without frailty. DESIGN AND METHOD: A cross-sectional study was conducted, including patients aged at least 60 years. BP was measured seated and at 1 and 3-min post-standing. Orthostatic hypotension and orthostatic hypertension (OHT) were defined in accordance with current guidelines. Orthostatic intolerance was de

Evaluation and Management of Hypertension in Older Adults: Current Evidence for an Age-Tailored Approach.

Hypertension is the most diagnosed condition among adults aged 60 years and older and is a major contributor to morbidity and mortality. In this article, we review current evidence on blood pressure evaluation and management in this population. We incorporate the geriatrics "5 Ms" framework, which includes 5 domains pertinent to geriatrics care. We compare the most recent society guidelines on hypertension management among older adults and discuss both resistant hypertension and deprescribing. This review is meant to equip geriatricians with evidence and resources to inform their approach to h

Late saphenous vein graft rupture presenting as a compressive mediastinal pseudoaneurysm.

Late saphenous vein graft (SVG) pseudoaneurysm is a rare but life-threatening complication of coronary artery bypass graft surgery. An 80-year-old man presented with dyspnea and chest pain two decades after CABG (LIMA-LAD, SVG-RCA). Coronary angiography revealed SVG occlusion with contrast extravasation and mass formation adjacent to the right heart chambers. CT angiography confirmed a 39 mm mediastinal pseudoaneurysm from proximal SVG rupture, compressing the right heart chambers and lung, with consequent left ventricular dysfunction (EF 45%), elevated pulmonary pressures, and orthostatic hyp

Distinct Sympathoneural and Cardiovagal Domains of Arterial Baroreflex Function in Autonomic Synucleinopathies.

BACKGROUND AND OBJECTIVES: The arterial baroreflex has sympathoneural and cardiovagal efferent limbs. Whether quantitative measures represent interchangeable indicators of a single physiological construct has been unclear. Moreover, the extent to which results agree in the synucleinopathies pure autonomic failure (PAF), Parkinson disease with or without orthostatic hypotension (PD + OH, PD No OH), and multiple system atrophy (MSA) remains incompletely understood. In a retrospective observational study we comprehensively assessed physiological and neurochemical biomarkers of arterial baroreflex

[Post-COVID postural orthostatic tachycardia syndrome: Documentation of a hyperadrenergic phenotype by comprehensive autonomic testing].

Postural orthostatic tachycardia syndrome (POTS) is a form of dysautonomia characterized by an excessive increase in heart rate upon standing, in the absence of orthostatic hypotension. Post-infectious forms, particularly following SARS-CoV-2 infection, have been increasingly reported. We report the case of a 34-year-old woman presenting with persistent palpitations, marked fatigue, and orthostatic intolerance that developed two weeks after a mild COVID-19 infection. Standard cardiological evaluation revealed no abnormalities. Comprehensive autonomic testing demonstrated an increase in heart r

Psychopharmacological Treatment in Patients with Heart Failure: A Narrative Review of Mood Stabilizers and Antipsychotics.

Heart failure (HF) is a leading cause of morbidity and mortality worldwide, and its co-occurrence with psychiatric disorders poses significant therapeutic challenges. This narrative review examines the safe use of psychopharmacological agents in patients with HF, focusing on mood stabilizers (particularly lithium) and antipsychotics. We summarize clinically relevant pharmacokinetic and pharmacodynamic interactions between these agents and guideline-directed HF therapies, including ACEIs, ARBs, ARNIs, beta-blockers, MRAs, SGLT2 inhibitors, and diuretics. Lithium is particularly prone to interac

Acute myocardial infarction in a patient with HAM/TSP evaluated for circulatory dynamics during mobilization before and after antihypertensive drug modification: a case report.

BACKGROUND: Antihypertensive therapy plays a crucial role in managing cardiovascular disease but may sometimes cause significant hypotension during mobilization, hindering the recovery of activities of daily living. In patients struggling with orthostatic hypotension, evaluating circulatory dynamics using a tilt bed provides rich clinical insights for identifying the underlying pathophysiology. We encountered a rare case in which the patient also had HTLV-1-associated myelopathy/tropical spastic paraparesis (HAM/TSP), prompting an assessment of circulatory dynamics in the passive upright postu
