Comparison of EEG features between patients with and without reduced 123I-MIBG myocardial scintigraphy uptake: A 6-year retrospective study of 230 patients with suspected dementia.

OBJECTIVE: Dementia with Lewy bodies (DLB) remains a diagnostic challenge due to its clinical overlap with other dementia-related diseases. Although 123I-metaiodobenzylguanidine (MIBG) myocardial scintigraphy provides high specificity for DLB and is one of the indicative biomarkers, its application is constrained by substantial cost and limited accessibility. Electroencephalography (EEG), by contrast, is inexpensive and widely available, but currently serves only as a supportive biomarker. The aim of this study is to investigate the association between these biomarkers. METHODS: This retrospe

Mediastinal lymphadenectomy strategy for Siewert II adenocarcinoma of the esophagogastric junction: a retrospective cohort study.

BACKGROUND: Siewert type II adenocarcinoma of the esophagogastric junction (AEG) is characterized by complex, bidirectional lymph node metastasis. This retrospective study evaluates recurrence and mortality risks associated with varying extents of mediastinal lymph node dissection. METHODS: A retrospective analysis was conducted on 221 Siewert type II AEG patients who underwent radical surgery (McKeown or Left Thoracotomy [LT] approach) between June 2016 and October 2023. Patients were grouped according to the extent of lymphadenectomy: complete mediastinal and abdominal lymph node dissection

Surgical Approaches and Perioperative Outcomes in Mediastinal Paragangliomas: A 20-Year Comprehensive Systematic Review.

BACKGROUND: Mediastinal paragangliomas (MPs) are rare, highly vascular neuroendocrine tumors whose surgical resection is the gold standard but carries a high risk of perioperative complications due to the MPs' proximity to major cardiovascular structures with potential life-threatening hemorrhage. Due to their rarity, the literature consists primarily of case reports. Our systematic review aims to synthesize the evidence from the last two decades to propose a standardized, multidisciplinary approach to the diagnosis and surgical management of MPs. METHODS: A systematic literature review was c

Long-Term Response Without Immune-Related Adverse Events to Atezolizumab Treatment in TMB-High Thymoma: A Case Report from the KOSMOS-II Study.

Background: Thymic epithelial tumors (TETs), including thymic carcinomas and thymomas, are rare malignancies originating in the mediastinum. Therapeutic options remain limited for patients experiencing disease progression following platinum-based chemotherapy. High tumor mutational burden (TMB) is uncommon in thymic malignancies but may predict response to immunotherapy. We report a patient with TMB-high TET who participated in the KOSMOS-II study in South Korea and achieved a durable response to atezolizumab without developing immune-related adverse events (irAEs). Case presentation: A 73-yea

Advancing mediastinal sampling in non-metastatic lymphadenopathy.

Despite advances in PET/CT, tissue confirmation remains essential for mediastinal lymphadenopathy. EBUS-TBNA is minimally invasive but limited by cytology, particularly in benign disease and lymphoma. Emerging EBUS-guided techniques, notably transbronchial mediastinal cryobiopsy, improve diagnostic yield where tissue architecture is required. The multicenter randomized study1 strengthens evidence for its added value, while underscoring the need for selective application and standardized, indication-specific trials.

Unicentric Castleman Disease of the Mesentery Mimicking a Gastrointestinal Stromal Tumor: A Case Report.

Unicentric Castleman disease (UCD) is a rare benign lymphoproliferative disorder commonly found in the mediastinum and retroperitoneum, with mesenteric involvement being exceptionally uncommon. We present a case of a 21-year-old male with a history of constipation, fatigue, and recurrent right iliac fossa pain for six months. Initial imaging suggested a gastrointestinal stromal tumor (GIST); however, the final diagnosis of UCD hyaline vascular (HV) variant was made based on immunohistochemical (IHC) analysis and histopathological reports. Surgical excision led to complete resolution of symptom

A Case Report of Hydatid Cyst Mimicking Thyroid Mass With Mediastinal Extension and Bony Erosion: A Successful Non-surgical Approach to Treatment.

We report an unusual manifestation of hydatid disease, presenting as a progressively enlarging, painless swelling in the right infraclavicular region. Imaging studies revealed a large necrotic mass extending into the superior mediastinum with the destruction of the adjacent right sternoclavicular joint; this was initially suspected to be thyroidal in origin. However, histopathological examination was consistent with a ruptured hydatid cyst, and Echinococcus serology was positive. Due to anatomical considerations, surgical intervention was not pursued, and the patient was managed with oral anth

Solitary Fibrous Tumor With a 12-Year Recurrence Interval in a Female Patient: A Case Report.

Solitary fibrous tumor (SFT) is a rare mesenchymal neoplasm characterized by unpredictable biological behavior, with the potential for late recurrence even in tumors initially classified as low risk. We report the case of an 83-year-old woman with a history of complete resection of a right thoracic SFT 12 years earlier, who was found on surveillance imaging to have a new anterior left mediastinal mass. Positron emission tomography-computed tomography (PET-CT) demonstrated low metabolic activity (maximum standardized uptake value (SUVmax) 2.32). Surgical exploration revealed a 6.5 × 5 cm lesion

Mesenteric Castleman disease: Two case reports and review of literature.

BACKGROUND: Castleman disease (CD) is a rare lymphoproliferative disorder most commonly occurring in the mediastinum and hilar regions. Mesenteric involvement is rare, and diagnosis primarily relies on histopathological examination. CASE SUMMARY: We report two cases of mesenteric CD with gastrointestinal symptoms accompanied by weight loss. Case 1 underwent complete surgical resection of the lesion. Postoperative pathology confirmed the hyaline vascular type, and the prognosis was favorable. Case 2 also underwent complete surgical resection initially, with postoperative pathology confirming t

Comparison of 1.1-mm and 1.7-mm Probes in Transbronchial Mediastinal Cryobiopsy: A Randomized Trial.

INTRODUCTION: Recent studies have shown that combined use of endobronchial ultrasound-guided transbronchial needle aspiration (EBUS-TBNA) and cryobiopsy provides a higher diagnostic yield for mediastinal lesions as compared with EBUS-TBNA alone. Currently, cryoprobes with diameters of 1.1mm and 1.7mm have been reported to be used for mediastinal cryobiopsy. METHODS: We conducted a randomized controlled trial to evaluate and compare the diagnostic safety and efficacy profile the two cryoprobes in mediastinal disease. Consecutive patients with mediastinal lesions (≥1cm in the short axis) were p

Best practices in shape-sensing robotic bronchoscopy with mobile cone beam computed tomography guidance: how I do it.

Recent studies have demonstrated that shape-sensing robotic bronchoscopy with mobile cone-beam computed tomography (CBCT) guidance results in superb diagnostic outcomes for the diagnosis of peripheral lung lesions. Given the more favorable safety profile and the ability to also stage the mediastinum with endobronchial ultrasound when indicated, the combination of these technologies is likely to replace percutaneous computed tomography (CT)-guided biopsies of peripheral lung nodules in the years to come. Hands-on training in simulators, mannequins or cadaveric models is paramount before adoptin

Safety of positive gastrointestinal contrast media. Updated guidelines by the ESUR Contrast Media Safety Committee.

Many reports on adverse effects related to positive gastrointestinal (GI) contrast media (CM) predate 2000; therefore, a literature review on their current safety profile was warranted. This article reviews the literature and updates the Contrast Media Safety Committee guidelines of the European Society of Urogenital Radiology on the safety of positive GI iodine-based and barium sulphate-based CM. A systematic literature search (2000-2025) identified 2 randomised controlled trials, 2 comparative studies, 17 reviews, and 29 case reports on the adverse effects of positive GI CM. Case Report: Complete remission of thymic carcinoma using dose-dense chemotherapy.

Thymic epithelial tumors, including thymoma and thymic carcinoma, are rare malignancies of the mediastinum, often associated with poor long-term outcomes in advanced stages. Surgical resection and platinum-based chemotherapy have been the cornerstone of management for resectable and advanced, unresectable disease, respectively, with the use of neoadjuvant chemotherapy alone or chemotherapy combined with radiation in specific cases. We report the case of a patient diagnosed with advanced hormone receptor-positive/HER2-positive breast cancer and a synchronous, advanced thymic carcinoma. After co

Diagnostic Factors Associated with Sarcoidosis in Patients Referred for EBUS-TBNA Due to Mediastinal Lymphadenopathy.

Sarcoidosis is a multisystem granulomatous disease of unknown aetiology that frequently presents with mediastinal lymphadenopathy and often requires invasive diagnostic procedures. Endobronchial ultrasound-guided transbronchial needle aspiration (EBUS-TBNA) is widely used in this setting; however, a definitive diagnosis cannot always be established at first attempt. This study aimed to identify clinical, laboratory, and radiological factors associated with a definitive diagnosis of sarcoidosis in patients referred for EBUS-TBNA. A retrospective analysis was performed including patients undergo
