Association of Gallstone Disease and Cholecystectomy With Duodenal Polyps and Barrett's Oesophagus.

BACKGROUND AND AIM: We hypothesized that duodenal polyps (DP) and Barrett's oesophagus (BE) coincide in the same patients and that biliary disease, such as cholelithiasis (GS) or cholecystectomy (CCx), increases the risk for both DP and BE. We used a case-control study design to test our hypothesis. METHODS: Using the healthcare records of the University of Pennsylvania Health System (UPHS), we compared the frequency of GS or CCx in patients with DP or BE with their corresponding frequencies in a control population of patients without DP or BE. Odds ratios (OR) and their 95% confidence interv

Are there age-related differences in the frequency of post-reflux peristaltic waves during impedance-pH monitoring?

OBJECTIVES: The post-reflux swallow-induced peristaltic wave (PSPW) reflects esophageal chemical clearance. Pediatric data on this parameter remain limited. The aim of this study was to evaluate the behavior of the PSPW in children of different ages. METHODS: We conducted a retrospective analysis of pediatric multichannel intraluminal pH/impedance, studies were performed for suspected gastroesophageal reflux (GER). We included those with a normal reflux index, symptom index <50%, symptom association probability <95% and a number of reflux episodes within normal parameters for age. Patients we

One PPI may not fit all-prevalence of CYP2C19 phenotypes in patients with Barrett's esophagus.

BACKGROUND: Barrett's esophagus (BE) is associated with longstanding gastroesophageal reflux disease and can progress to esophageal adenocarcinoma. BE is primarily managed with proton pump inhibitors (PPIs), which are metabolized by cytochrome P450 2C19 (CYP2C19). CYP2C19 polymorphisms affect PPI plasma levels, which are categorized as poor (PM), intermediate (IM), normal (NM), rapid (RM), and ultra-rapid (UM) metabolizers. This study assessed the prevalence of CYP2C19 phenotypes in patients with BE and concomitant hiatal hernias (HH). METHODS: This was a single-institution retrospective revi

Circadian syndrome and its components in relation to incident gastroesophageal reflux disease: a prospective cohort study from the UK Biobank.

BACKGROUND: Circadian Syndrome (CircS), a cluster encompassing metabolic dysregulation, short sleep duration, and depression, has been implicated in various chronic diseases. Gastroesophageal reflux disease (GERD) is a highly prevalent digestive disorder; however, its prospective association with CircS remains unclear. METHODS: This prospective cohort study utilized data from the UK Biobank, including 330,925 participants free of GERD at baseline. CircS was defined as meeting at least four of seven components. The primary outcome was incident GERD, identified through the first recorded diagno

Riddle syndrome with RNF168 mutation in first Asian family cluster exhibiting pulmonary fibrosis and achalasia.

Riddle syndrome is a rare autosomal recessive disorder caused by RNF168 mutations, characterized by radiosensitivity, immunodeficiency, and dysmorphic features. We reported the first Asian family cluster involving a homozygous RNF168 c.91T>C (p.Cys31Arg) mutation in three siblings. The proband, a 37-year-old female, presented with severe restrictive lung disease, achalasia, and progressive respiratory failure requiring mechanical ventilation. Two sisters exhibited milder phenotypes with dysmorphic features and elevated alpha-fetoprotein. Whole-genome sequencing confirmed the mutation in all af

Current status of minimally invasive surgery for thoracic esophageal diverticula: systematic review and meta-analysis.

INTRODUCTION: Thoracic esophageal diverticula, including epiphrenic and parabronchial types, are rare clinical entities representing about 30% of all esophageal diverticula. Thoracic diverticula of the pulsion type have traditionally been associated with motility disorders and treated by stapled diverticulectomy with or without myotomy/fundoplication in patients experiencing disabling symptoms of dysphagia, regurgitation or pulmonary aspiration. Purpose of this study was to review the existing literature and estimate outcomes of minimally invasive surgery. METHODS: Systematic review and meta-

Biomechanical Assessment During High-Resolution Impedance Manometry Enhances Characterization of Inconclusive Esophagogastric Junction Outflow Obstruction.

BACKGROUND: Esophagogastric junction outflow obstruction (EGJOO) identified on high-resolution manometry (HRM) remains a clinically heterogeneous and inconclusive finding. Additional complementary testing with functional lumen imaging probe (FLIP) or timed barium esophagram (TBE) is recommended to confirm clinically relevant obstruction. We aimed to assess the association of novel Four-Dimensional (4D) HRM metrics against FLIP and TBE outcomes among symptomatic patients with inconclusive EGJOO on HRM. METHODS: Adult patients with inconclusive HRM-EGJOO who completed corresponding FLIP and TBE

Optimal cannabinoid-terpene combination ratios suppress mutagenicity of gastric reflux in normal and metaplastic esophageal cells.

BACKGROUND: Esophageal adenocarcinoma (EAC) frequently arises from chronic exposure to acid and bile reflux, with secondary bile acids, such as deoxycholic acid (DCA), contributing to its pathogenesis through mechanisms involving reactive oxygen species (ROS), oxidative DNA damage, and resistance to apoptosis. The human endocannabinoid system (ECS) regulates diverse anti-inflammatory, antioxidant, and analgesic pathways implicated in disease modulation and may counteract the harmful effects of gastroesophageal reflux. Despite its therapeutic promise, effective pharmacological activation of the

Predictive model for estimating total acid exposure time in patients with esophageal hypomotility disorders.

The latest Chicago Classification of esophageal motility disorders categorizes esophageal hypomotility disorders (EHD) into absent contractility (AC) and ineffective esophageal motility (IEM). While these disorders are commonly associated with gastroesophageal reflux disease (GERD), they are not pathognomonic. To improve diagnostic accuracy, provocative maneuvers during high-resolution esophageal manometry (HRM) -such as multiple rapid swallows (MRS) and single solid swallows (SSS) -have been introduced.

Reflux Seen on a Barium Swallow is Not a Substitute For Ambulatory Reflux Monitoring in Symptomatic Patients.

OBJECTIVE: Gastroesophageal reflux (GER) is often reported in barium studies (GERB). We aimed to evaluate whether GERB associates with parameters defining conclusive reflux using Lyon criteria on esophageal physiological testing (GERpH). METHODS: Adults who underwent both ambulatory reflux monitoring off antisecretory therapy and barium esophagogram between 2018 and 2023 were eligible for inclusion in this retrospective study. All reflux monitoring studies were independently reviewed to extract acid exposure time (AET) and number of reflux episodes. GERpH was diagnosed based on Lyon consensus
