BACKGROUND: Gallbladder cholesterolosis and adenomyomatosis (GA) share overlapping cross-sectional imaging features on CT enterography and MRCP, creating a significant diagnostic pitfall. Histopathology remains the only definitive means of distinction.
CASE PRESENTATION: A 44-year-old asymptomatic female was incidentally found to have gallbladder wall thickening and cholelithiasis on CT enterography. MRI/MRCP demonstrated focal fundal wall thickening with intramural high-signal foci, which was interpreted as fundal-type GA with a coexisting gallstone. Elective laparoscopic cholecystectomy was

Gallbladder cancer (GBC) commonly arises in gallbladders affected by cholelithiasis and chronic cholecystitis, although malignancy develops in only a small minority of patients with stones. This disparity has directed attention to the local conditions created by concentrated bile, stone surfaces, persistent microbes and altered bile acid and lipid metabolism. Evidence in this field is difficult to compare because bile, stones, mucosa and tumor tissue measure different biological processes, while human associations, animal experiments and cross-sectional omics studies are often discussed togeth

Cholecystocolonic fistula (CCF) is a rare type of cholecystoenteric fistula, which are sometimes diagnosed incidentally in patients undergoing cholecystectomy. They are usually a complication of chronic cholecystitis, often secondary to gallstones, and they are difficult to diagnose due to their rarity and vague clinical presentation. If untreated, they can lead to complications such as acute cholangitis, biliary peritonitis, and biliary cirrhosis. This highlights the importance of early recognition of CCF and timely treatment. While the generally accepted management is surgical, there are no

BACKGROUND Thyrotoxicosis is associated with well-recognized and clinically significant complications, including congestive heart failure and cardiac arrhythmias. The relationship between hyperthyroidism and hepatic dysfunction is complex, with several proposed pathophysiological mechanisms; however, direct cholestatic injury is frequently overlooked. Although mild liver enzyme elevation is common in hyperthyroidism, clinically overt jaundice is rare. Nevertheless, increasing evidence from case reports and case series suggests that thyrotoxicosis, especially Graves disease, can result in chole

Xanthogranulomatous cholecystitis (XGC) is a rare, benign inflammatory condition that closely mimics gallbladder carcinoma clinically, radiologically, and intraoperatively. We present the case of an elderly Indian man in his 80s with multiple comorbidities, with vomiting and anorexia, whose radiological and positron emission tomography-computed tomography (PET-CT) findings showed a fluorodeoxyglucose (FDG)-avid gallbladder mass with extension into the liver, cystic duct, common hepatic duct, and common bile duct, highly suggestive of gallbladder carcinoma. However, histopathological examinatio

BACKGROUND: Patients undergoing bariatric surgery have a substantially elevated risk of gallstone disease, both at baseline and as a consequence of rapid postoperative weight loss. The optimal management of biliary pathology identified at the time of bariatric surgery remains unsettled. We aimed to describe the clinical, radiological, surgical, and histopathological characteristics, and the short-term safety, of simultaneous laparoscopic cholecystectomy performed during bariatric surgery in patients with documented preoperative biliary pathology.
METHODS: A single-center retrospective cohort

INTRODUCTION: There is no distinct classification system to evaluate the bone defect in previously managed acetabular fractures. We propose a new classification system for bone defect evaluation in a previously managed acetabular fracture that will be helpful for total hip arthroplasty (THA).
MATERIALS AND METHODS: The preoperative pelvis radiographs of 99 THA patients with previous acetabular fractures with at least 2 years of follow-up were evaluated by 10 experienced surgeons (Paprosky and new classification systems). As per the new classification system, the five types of bone defects are

BACKGROUND: While the inflammation-to-carcinoma transition in gallbladder carcinogenesis is well recognized, the molecular pathogenesis of gallbladder adenomyomatosis (GBA), which is clinically associated with both chronic cholecystitis (CCS) and gallbladder carcinoma (GBC), remains poorly understood. Hence, we aimed to evaluate the premalignant potential of GBA and to identify previously unrecognized molecular pathways involved in gallbladder carcinogenesis.
METHODS: We first conducted single-cell RNA sequencing and downstream bioinformatics analyses on samples from nine patients with CCS, G

BACKGROUND: Anatomical variations of the biliary system constitute one of the principal risk factors for bile duct injury (BDI) during laparoscopic cholecystectomy (LC). Preoperative imaging modalities, such as abdominal ultrasonography and magnetic resonance cholangiopancreatography (MRCP), play a pivotal role in identifying potential biliary anatomical variations. In addition, meticulous adherence to standardized surgical techniques during LC serves as a critical safeguard against BDI. Therefore, heightened awareness and comprehensive understanding of biliary anatomical variations are essent

Isolated gallbladder tuberculosis (GBTB) is a rare disease, even in endemic areas, and is often misdiagnosed due to its nonspecific clinical and imaging findings. Histopathological evaluation, demonstrating granulomatous inflammation with caseous necrosis, remains the only definitive method of diagnosis. Thus, maintaining a high index of suspicion and routine histological assessment of resected specimens are vital for its timely management, especially in patients with a history of tuberculosis. We report a similar case in a 50-year-old male patient with imaging suggestive of gallbladder perfor

OBJECTIVES: Adenomyomatosis (ADM) is generally considered a benign condition. However, it can be associated with chronic cholecystitis - a known risk factor for gallbladder cancer. Therefore, studies have proposed follow-up with ultrasound for asymptomatic patients with focal ADM. Currently, there are no formal recommendations regarding the frequency and length of follow-up. The aims of this study were to assess the growth of ADM lesions during follow-up and to examine the differences between larger and smaller ADM lesions.
METHODS: 144 patients who underwent MRI-MRCP at our institution betwe

BACKGROUND: Cholecystoduodenal fistula (CDF) is a rare complication of recurrent gallstone disease, characterized by chronic inflammation, localized ischemia, and tissue necrosis. In this study, the clinical outcomes associated with laparoscopic manual suturing and stapler-assisted closure were compared in the context of CDF repair with accompanying video presentations. METHODS: Overall, 21 patients diagnosed with CDF who underwent treatment at Fujian Provincial Hospital between January 2010 and December 2024 were retrospectively analyzed. Participants were categorized into two groups: those w

BACKGROUND: Chronic calculous cholecystitis (CCC), caused by persistent gallstone-induced inflammation, can lead to development of small cell neuroendocrine carcinoma (SCNEC) of the gallbladder through chronic irritation, cellular damage, and repair. This report describes a rare case of advanced SCNEC of the gallbladder potentially induced by CCC and highlights its clinical features and treatment challenges.
CASE SUMMARY: A 64-year-old woman with a history of CCC presented with persistent right upper quadrant pain. Imaging revealed a mass in the gallbladder and hepatic hilum. The patient unde

Neuroendocrine tumors (NETs) of the gallbladder are uncommon and are often difficult to diagnose preoperatively due to their nonspecific clinical and imaging findings. A 39-year-old man presented with right upper quadrant abdominal pain without fever, jaundice, or signs of peritoneal irritation. Abdominal ultrasonography revealed a 9 mm polypoid lesion in the neck of the gallbladder, and contrast-enhanced computed tomography demonstrated an 8 mm slightly enhancing lesion with gallbladder wall thickening consistent with cholecystitis. Blood tests showed no elevation of inflammatory markers. Alt

BACKGROUND: Xanthogranulomatous cholecystitis (XGC) is an uncommon histologic variant of chronic cholecystitis characterized by marked fibrosis with infiltration of macrophages and foamy histiocytes. Lymphoplasmacytic cholecystitis (LPC), another variant, has historically been considered specific to primary sclerosing cholangitis (PSC) and is associated with autoimmune pancreatitis. IgG4-related cholecystitis (IgG4-RC) represents gallbladder involvement in IgG4-related disease (IgG4-RD), in which autoimmune pancreatitis is the most common manifestation. Approximately 25-32% of patients with au

A woman in her 60s with a known history of JAK2-positive essential thrombocythemia presented with a one-week history of jaundice. Laboratory testing revealed elevated bilirubin and alkaline phosphatase levels. Abdominal ultrasonography identified a 1-cm gallstone, and laparoscopic cholecystectomy was performed. Histopathological evaluation revealed chronic cholecystitis along with a 5-mm lymph node in the gallbladder neck harboring atypical megakaryocytes with multinucleation and nuclear lobulation. Immunohistochemistry confirmed megakaryocytic origin with strong positivity for CD61 and suppor

Left-sided gall bladder (LSGB) is a rare anatomical variant that increases the risk of bile duct injury, particularly when combined with other gall bladder pathology. A 57-year-old male with a several-year history of intermittent post-prandial epigastric pain had hepatic steatosis and gall bladder polyps (largest 9 mm) on ultrasound, and hepatobiliary scintigraphy showed a reduced gall bladder ejection fraction consistent with biliary dyskinesia. Elective laparoscopic cholecystectomy was performed; at laparoscopy, the gall bladder was unexpectedly located to the left of the falciform ligament,

This study evaluated the initial feasibility and safety of reduced-port robotic cholecystectomy using the Revo-i robotic system (meerecompany Inc.) in nine patients diagnosed with symptomatic gallstones and chronic cholecystitis. All surgeries were successfully completed without complications or conversion to conventional methods. Postoperative outcomes were excellent, with minimal pain immediately after surgery, significantly reduced by the next day. Most patients were discharged on postoperative day one. Cosmetic outcomes were outstanding, with barely noticeable scars at the umbilical and fl

Cholecystocolonic fistula (CCF) is a rare complication of chronic cholecystitis, characterised by an abnormal connection between the gallbladder and colon. It predominantly affects elderly patients with a history of gallstone disease and chronic inflammation. It is often difficult to diagnose preoperatively due to nonspecific symptoms, low sensitivity on current available imaging, and is often discovered intraoperatively. A high index of suspicion for a CCF should be maintained in patients with longstanding chronic cholecystitis, even when preoperative imaging is unrevealing. Surgeons should b

Focal gallbladder wall thickening may raise suspicion for inflammatory, hyperplastic, or malignant processes. We report a rare case of chronic cholecystitis associated with adenomyosis and osseous metaplasia in a 43-year-old woman with multiple comorbidities. The patient presented with right upper quadrant abdominal pain. Ultrasonography revealed focal gallbladder wall thickening without calculi. She underwent uneventful laparoscopic cholecystectomy. Gross and microscopic pathological examination demonstrated chronic cholecystitis with mucosal hyperplasia, intramural diverticula consistent wit
