Inter-rater reliability and clinical utility of the modified ferriman-gallwey scale in routine practice.

OBJECTIVES: To evaluate the reliability and clinical utility of the modified Ferriman-Gallwey (mFG) score in routine practice when applied by clinicians with differing experience and by patients for self-assessment. METHODS: In this cross-sectional reliability study, 43 reproductive-aged women were assessed for hirsutism. Three clinicians (reproductive endocrinologist, general gynecologist, and resident) independently scored participants using the mFG scale, and patients self-graded their hirsutism. Inter-rater reliability was examined using intraclass correlation coefficients (ICC) and weigh

Not quite naked: the bare necessities of human body hair evolution.

A striking characteristic that sets modern humans apart from their ancestors is the lack of a thick fur coat. Despite being popularly dubbed 'the naked ape', humans are covered in 2-3 million vellus hair follicles producing tiny hair fibres. This review explores why humans evolved thinner body hair, with a focus on the need for an effective cooling mechanism. We address anthropological evidence and traits that developed in parallel with humans losing their fur. We then discuss the differences between thick terminal hairs and thin vellus hairs, and the role of the dermal papilla in the regulati

Redefining testosterone reference ranges for adult females.

PURPOSE: To define an age-continuous reference range for testosterone (T) in females. METHODS: A large-scale retrospective analysis of anonymised serum T collected via capillary sampling from a private healthcare service. 5,323 female individuals (19-59 years) with no history of reproductive conditions, normal BMI and regular cycles were analysed to establish a robust, age-continuous reference range for T, including probability density distributions and specific quantiles. Competing models of log T values as a function of age were statistically assessed, and the best model selected. Additiona

Bilateral ovarian steroid cell tumor in a postmenopausal woman with progressive hyperandrogenism.

Severe hyperandrogenism with new-onset virilization in postmenopausal women is rare and often associated with androgen-secreting tumors. Nevertheless, a comprehensive differential diagnosis must be considered, including ovarian hyperthecosis and Cushing syndrome. We report a 71-year-old woman with progressive hirsutism over 1-2 years, presenting with severe hyperandrogenism and insulin resistance. Physical examination revealed moderate hirsutism with a score of 15 according to the modified Ferriman-Gallwey scale, androgenic alopecia, dental diastema, macroglossia, and acanthosis. Hormonal anal

Cyclosporine related adverse events in aplastic anemia patients treated with immunosuppressive therapy.

BACKGROUND: Few studies have focused on cyclosporine-related adverse events (AEs) in aplastic anemia (AA) patients undergoing immunosuppressive therapy and receive cyclosporine maintenance. MATERIALS AND METHODS: We conducted a retrospective study to analyze the relationship between cyclosporine concentrations at different time points and AEs in AA patients who received cyclosporine maintenance. Cutoff values of cyclosporine concentrations for predicting every AE were defined by receiver operating characteristic (ROC) curves respectively. RESULTS: A total of 382 patients were included. Steroid Cell Tumor of the Ovary Presenting With Postmenopausal Bleeding and Hyperandrogenism: A Case Report.

Steroid cell tumors of the ovary are rare, often hormonally active neoplasms that may present with features of hyperandrogenism. We report the case of a 60-year-old woman who initially presented with postmenopausal bleeding and a reassuringly thin endometrium, with symptoms attributed to hormone replacement therapy. Over time, the development of progressive hirsutism and persistently elevated serum testosterone levels prompted further investigation. Multidisciplinary team involvement, including endocrinology input, was pivotal in identifying a suspected androgen-secreting ovarian tumor and gui

The Voice of Cantú: Lower Voice Pitch Is a New Phenotypic Feature of Cantú Syndrome.

Cantú syndrome (CS) is a rare genetic condition caused by pathogenic variants in either ABCC9 or KCNJ8, leading to gain-of-function of KATP-channels. The main clinical features are hypertrichosis and cardiovascular abnormalities. This study investigates the voice characteristics in individuals with CS, an aspect that has received little attention in previous research. A total of 18 participants with molecular genetically confirmed CS were recruited, 11 females and 7 males, aged 9-63 years. Voice pitch analysis was conducted using the Voice Tools application, comparing these findings with norma

Clinical characterization of Japanese children with Cushing's disease.

Cushing's disease (CD) is very rare in children. Nineteen children (≤18 years) with CD (median age at diagnosis: 13 [6-17] years; 12 females) were retrospectively analyzed using medical records (1994-2025) from Toranomon Hospital, Tokyo, Japan. Facial changes (88.9%), weight gain with decreased growth rate (88.9%), central obesity (88.9%) and hirsutism (94.4%) were frequently observed at diagnosis. Median time to diagnosis was 2.7 (0.25-5.8) years. Screening for endogenous hypercortisolism was assessed by 24-hour urinary free cortisol levels, serum cortisol levels at 23:00, and serum cortisol

Chromosomal Rearrangements Identified in Three Additional Patients With Generalized Congenital Hypertrichosis With Gingival Hyperplasia Involving the 17q24.2-q24.3 Locus.

We describe three unrelated individuals with congenital generalized hypertrichosis with gingival hyperplasia (CGHGH), each carrying a distinct structural rearrangement (duplication, deletion, inversion) at 17q24.2-q24.3 identified by CMA and WGS. Despite differences in the type of rearrangement, all three patients seem to exhibit alterations affecting the genomic architecture of a cluster of genes, particularly involving the ABCA family (notably ABCA5, ABCA6, ABCA9, ABCA10), MAP2K6, and potassium channels (KCNJ16, KCNJ2). These findings suggest that disruption of the local chromatin organizati

Prevalence and Clinical Features of Polyendocrine Metabolic Ovarian Syndrome in the Gulf Cooperation Council Countries: A Systematic Review and Meta-Analysis.

Background: Polyendocrine metabolic ovarian syndrome (PMOS/PCOS) is the most common hormonal disorder in women of reproductive age and is linked to infertility as well as long-term metabolic and psychological problems. In the Gulf Cooperation Council (GCC) region, rising obesity, dietary changes, and sedentary lifestyles may be increasing its burden. However, prevalence estimates remain highly inconsistent due to differences in diagnostic criteria and measurement methods rather than true variation in disease rates. Objective: This study aimed to describe the situation by systematically pooling

Expanding the therapeutic landscape of minoxidil for androgenetic alopecia: topical, oral and sublingual formulations.

Androgenetic alopecia (AGA) is the most common form of non-scarring alopecia, affecting up to 80% of men and 50% of women by the age of 70. Minoxidil, initially developed as an oral antihypertensive, has become one of the most widely used therapies for AGA due to its safety and efficacy. This review presents an overview of current evidence on topical, oral and sublingual minoxidil. Topical minoxidil, the only FDA-approved treatment for AGA in both men and women, promotes hair growth through several proposed mechanisms described in the article. Randomised controlled trials show that 5% formulat

Anti-androgen therapy in polyendocrine metabolic ovary syndrome (PMOS, formerly PCOS): efficacy, safety, and phenotype-oriented clinical decision-making.

Polyendocrine metabolic ovarian syndrome (PMOS, formerly polycystic ovary syndrome [PCOS]), hereinafter referred to as PMOS/PCOS, is the most prevalent endocrine metabolic disorder among women of reproductive age. It is characterized by a self-perpetuating cycle in which hyperandrogenemia (central in several, but not all, phenotypes) acts as the primary driver, exacerbating reproductive dysfunction, metabolic disturbances, and psychological distress. Anti-androgen therapy remains an important treatment for alleviating hyperandrogenism-related clinical signs, including hirsutism, acne, and fema
