Part II. Measuring skin pain in humans: Quantitative, psychophysical, and patient-reported tools for dermatology.

Quantifying skin pain is essential for dermatologic research and patient care. This Methods and Techniques in Skin Research article (part II) synthesizes human-focused tools spanning psychophysical and patient-reported measures. A standardized workflow for quantitative sensory testing (QST) outlines the measurement of thermal detection and pain thresholds, mechanical detection and pinprick pain, temporal summation, dynamic mechanical allodynia, vibration, and pressure; these techniques enable phenotyping of gain and loss of somatosensory function. Core patient-reported outcome measures (PROMs)

Hidradenitis suppurativa skin microbiome: In-depth analysis and modulation by systemic therapies.

BACKGROUND: Hidradenitis suppurativa (HS) is a chronic inflammatory skin disease with a multifactorial pathogenesis involving genetic, lifestyle and microbial factors. While cutaneous microbiome shifts have been reported for this disease, the impact of systemic immunomodulators and antibiotics on HS microbiome remains poorly defined. OBJECTIVE: To characterize species-level cutaneous dysbiosis in HS and evaluate the effects of three standard-of-care systemic therapies, clindamycin/rifampicin (ATB), adalimumab (ADA) and secukinumab (SEC) on skin microbial composition. METHODS: We performed 16

[Advances in the application of Janus kinase inhibitors for the uncommon pediatric dermatological disorders].

Janus kinase (JAK) inhibitors have emerged as a research hotspot in pediatric inflammation-mediated autoimmune skin diseases, owing to the ability to target and block the JAK-signal transducer and activator of transcription (JAK-STAT) signaling pathway. This article summarizes the mechanism of action and approved indications of JAK inhibitors, and systematically reviews the efficacy and safety data in pediatric uncommon skin diseases such as lichen planus, connective tissue diseases, hidradenitis suppurativa, haploinsufficiency of A20, and chronic atypical neutrophilic dermatosis with lipodyst

Advancing Our Understanding of Hidradenitis Suppurativa: A Comprehensive Guide for Clinicians Designed to Improve Therapeutic Outcomes for Patients-Part 1.

Hidradenitis suppurativa (HS) is a chronic inflammatory cutaneous disorder that usually starts in the second or third decade of life and has a profound negative impact on quality of life of those who are affected. HS presents with both inflammatory and cicatricial lesions that are often markedly symptomatic, including pain and purulent discharge, and commonly affect areas that are intertriginous, resulting in additional frictional and mechanical discomfort. The pathophysiology is complex and incompletely understood and results in changes that are transcutaneous, involving the epidermis, superf

Advancing Our Understanding of Hidradenitis Suppurativa: A Comprehensive Guide for Clinicians Designed to Improve Therapeutic Outcomes for Patients-Part 2.

Hidradenitis suppurativa (HS) is a chronic inflammatory cutaneous disorder that often starts in the second or third decade of life, induces both inflammatory and cicatricial cutaneous lesions, and frequently involves intertriginous and anogenital anatomic sites. Abscesses, nodules, and draining tunnels are reversible inflammatory lesions that cycle over time through unpredictable periods of exacerbation; cicatricial lesions are permanent and persist as fibrotic scars and nondraining tunnels, the latter reflecting the shells of prior draining tunnels. HS has a profound adverse impact on overall

Diagnostic uncertainty in a patient with severe hidradenitis suppurativa revealing Buschke-Löwenstein tumor.

Buschke-Löwenstein tumor (BLT), also known as giant condyloma acuminatum, is a rare human papillomavirus-associated lesion characterized by locally aggressive growth with the potential for malignant transformation. Diagnosis may be particularly challenging when BLT arises in the setting of severe chronic inflammatory disease. We present a 44-year-old man with severe hidradenitis suppurativa, well-controlled HIV infection, recurrent gluteal abscesses, and chronic sacral osteomyelitis who presented with worsening perineal pain and chronic drainage. Although imaging demonstrated extensive infiltr

OBJECTIVE: To summarize surgical objectives, indications, technical principles, limitations and reconstructive considerations for dissecting cellulitis of the scalp (DCS), and explicitly distinguish DCS‑specific evidence from data extrapolated from hidradenitis suppurativa (HS). METHODS: This narrative review searched PubMed/MEDLINE up to July 2026. Twenty‑four publications focusing on DCS surgical management were included for narrative synthesis. Formal systematic‑review procedures such as PRISMA screening, risk‑of‑bias assessment and meta‑analysis were not performed. RESULTS: Simple draina

Dietary and Nutritional Considerations for Metabolic Dysfunction-Associated Steatotic Liver Disease in Psoriasis and Hidradenitis Suppurativa: A Narrative Review.

Metabolic dysfunction-associated steatotic liver disease (MASLD) is increasingly recognised in patients with chronic inflammatory skin disease, particularly psoriasis, for which epidemiological evidence of increased MASLD prevalence is relatively strong; corresponding data in hidradenitis suppurativa (HS) remain more limited. This narrative review examines the epidemiological and biological links between psoriasis, HS and MASLD, and evaluates dietary and nutritional strategies relevant to their multidisciplinary management. Particular emphasis is placed on distinguishing direct evidence in pat

Treatment Sequencing With Biologic Switching, Staged Excision, and Split-Thickness Skin Grafting for Extensive Hidradenitis Suppurativa.

Treatment of extensive multiregional hidradenitis suppurativa requires sequencing biologic therapy and surgery when active inflammation and irreversible structural disease coexist. A 58-year-old Japanese man with Hurley Stage III disease had inadequate overall control with adalimumab. At the assessment before medial-thigh surgery, the International Hidradenitis Suppurativa Severity Score System score was 48. Because he sought prompt relief from painful drainage and daily wound care, bilateral scarred and tunnel-bearing lesions were excised and reconstructed by staged split-thickness skin graft

Immunoglobulin deposition and plasma cell infiltration define distinct immunopathological patterns in skin diseases.

Immunoglobulin deposition in the skin is an important diagnostic and etiological feature in many skin diseases. However, the relationship between deposited immunoglobulins and the local presence of antibody-producing cells, such as B cells and plasma cells, remains poorly understood. In several autoimmune diseases, including systemic lupus erythematosus and bullous pemphigoid, marked immunoglobulin deposition is observed despite limited B-cell infiltration into the skin, suggesting that the deposited antibodies primarily originate as circulating autoantibodies. In contrast, diseases such as di

Military shaving waivers for pseudofolliculitis barbae: Ethical considerations for dermatologists.

Pseudofolliculitis barbae (PFB) is a chronic inflammatory disorder triggered by shaving and disproportionately affects individuals of African descent. For service members, medically appropriate management may conflict with military grooming standards requiring a clean-shaven appearance. Recent policy changes have placed greater restrictions on shaving waivers and may create career consequences for service members who continue to require accommodations despite treatment. These policies raise ethical concerns involving justice, beneficence, nonmaleficence, and autonomy. Military safety requireme

Targeting an IL-17/IL-36 pathogenic hub in hidradenitis suppurativa with a bispecific antibody.

Hidradenitis suppurativa (HS) is a chronic inflammatory skin disorder with a high unmet need, particularly for treatment-resistant tunnels. Integrated multiomics analysis revealed that HS tunnels are spatially organized, self-amplifying inflammatory circuits driven by synergistic IL-17/IL-36γ activation of a unique keratinocyte subset, which activates neutrophils and inflammatory fibroblasts. This mechanistic insight coupled with spatial coenrichment of IL-17A and IL-36R provided the rationale for HB0043, a bispecific antibody that leverages IL-36R binding to locally dual-block both cytokines.

Editor's Highlights-December 2026.

The effects of climate change, unequal access to biologic therapies, and the limited representation of Indigenous communities and diverse skin tones in educational materials underscore an important point: dermatology does not operate independently of social and environmental factors. The five papers in this issue not only identify these gaps but also call for specific measures to strengthen climate-resilient preparedness, expand safety-net services, and promote truly inclusive education. Equitable care is no longer a nice-to-have option; it is a fundamental standard of clinical quality.

The overlooked surface: cutaneous manifestations of inflammatory bowel disease.

Inflammatory bowel disease (IBD) is characterized by a chronic, idiopathic inflammation of the intestinal tract. The primary types include ulcerative colitis (UC), which affects the colon and rectum with continuous mucosal and submucosal lesions solely and Crohn's disease (CD), which may involve any segment of the gastrointestinal tract with discontinuous lesions. Among extraintestinal manifestations (EIMs), the skin is most frequently involved. Erythema nodosum (EN) and pyoderma gangrenosum (PG) are identified as the most prevalent cutaneous presentations. This review explores the underlying
