Clinical and Transcriptomic Response to Secukinumab in Korean Patients with Moderate-to-Severe Hidradenitis Suppurativa.

INTRODUCTION: Hidradenitis suppurativa (HS) is a chronic inflammatory skin disease characterized by recurrent painful nodules, abscesses, and tunnels, primarily affecting intertriginous areas. While IL-17-targeting biologics, including bimekizumab and secukinumab, have shown efficacy in moderate-to-severe HS, data in Asian populations remain limited. The aim of our study was to assess the clinical efficacy of secukinumab in Korean patients with moderate-to-severe HS and to perform an exploratory analysis of treatment-associated transcriptomic changes. METHODS: Twenty-five patients with modera

The role of surgery in hidradenitis suppurativa: expert-based holistic approaches.

Hidradenitis suppurativa (HS) is a chronic, inflammatory skin disease characterized by painful nodules and tunnels, which are predominantly found in intertriginous skin areas such as the axillary, inguinal and anogenital regions. The condition significantly impacts patients' quality of life owing to its chronicity and debilitating nature. While a combined medical and surgical approach is recommended for severe cases, misconceptions exist regarding the role and timing of surgery for HS, often influenced by patient fear and hesitation. A comprehensive, multidisciplinary approach involving a mult

A Genome-Wide Association Study of Hidradenitis Suppurativa from the VA's Million Veteran Program.

UNLABELLED: <p>Introduction: Data from family and twin studies as well as prior genome-wide association meta-analyses suggest that hidradenitis suppurativa (HS) has a hereditary component. This GWAS identifies genetic variants associated with HS. METHODS: Individuals with a diagnosis of HS (defined as at least one instance of ICD9 705.83 or ICD10 L73.2) were identified within the Million Veteran Program. Multi-population and population-specific (African, European, and Hispanic ancestries) case-control genome-wide association studies (GWAS) were performed. Lead single-nucleotide polymorphisms

Risk of irritable bowel syndrome in patients with hidradenitis suppurativa: a global-federated, multicenter cohort study.

Hidradenitis suppurativa (HS) is a chronic inflammatory skin disease increasingly viewed as systemic. Irritable bowel syndrome (IBS) has been proposed as a comorbidity, yet longitudinal data remain limited. A retrospective cohort study was conducted using the U.S. Collaborative Network of the TriNetX platform, including adults diagnosed with HS from 2005-2023 and matched controls without HS. Patients with prior IBS or cancer were excluded. Propensity score matching (1:1) accounted for demographic, socioeconomic, and clinical variables. Incident IBS, defined by ICD-10-CM codes, was assessed ove

Mucinous Syringometaplasia: Practical Insights from a New Case.

Mucinous syringometaplasia is a rare adnexal process that may mimic mucinous carcinoma or metastatic mucinous adenocarcinoma, creating diagnostic challenges. After reviewing the recent report by Furtado et al, we present a brief additional case involving a longstanding ulcerated plaque on the dorsal hand of a 61-year-old man. Histology demonstrated an epidermal invagination connected to eccrine ducts lined by nonkeratinizing squamous epithelium with abundant goblet cells, strongly positive with Alcian blue, and negative for CDX2, supporting a primary cutaneous eccrine-derived mucinous metaplas

[Hidradenitis suppurativa and psoriasis. Therapeutic response to secukinumab].

Hidradenitis suppurativa and psoriasis are chronic inflammatory systemic dermatoses. The immune system plays a fundamental role in their pathogenesis, as each involves an immunological imbalance and the participation of numerous cytokines, including interleukin-17. The coexistence of these conditions can significantly impair quality of life, thus requiring integrated and targeted therapeutic approaches. We report the case of a 47-year-old woman diagnosed with hidradenitis suppurativa who developed psoriasis lesions over time. After receiving multiple therapeutic regimens without response, she

Insights into the regulation and signaling landscape of cardiotrophin-like cytokine factor 1 (CLCF1).

Cardiotrophin-like cytokine factor 1 (CLCF1) is an essential gene that shows exceptional sequence conservation among vertebrates. CLCF1 loss-of-function mutations cause Crisponi/cold-induced sweating syndrome-2, a disorder with severe and complex phenotypes, underscoring the non-redundant roles of CLCF1. Initially identified as a member of the interleukin-6 (IL-6) cytokine family with activity in neurons and immune cells, CLCF1 is now recognized for broader roles in thermoregulation, glomerular function, myelopoiesis, oncogenesis, and muscle fitness. Despite growing interest in the multifacete

Assessing Disease Progression in Hurley I Hidradenitis Suppurativa Patients: A Nested Case-Control Study.

INTRODUCTION: Hidradenitis suppurativa (HS) is a chronic, progressive, inflammatory skin disease that leads to structural skin alterations, usually assessed using Hurley stages. In Hurley stage I, the earliest phase, structural changes have not yet occurred. However, disease progression varies among patients; some remain indefinitely in this stage, while others advance to more severe forms. The incidence and factors driving progression remain unclear. The aims of this study were to determine the incidence of Hurley II and III cases in a cohort of Hurley I patients and to explore potential fact

PSMA4 as a Druggable Target in Hidradenitis Suppurativa: Evidence From Mendelian Randomization and Single-Cell Transcriptomics.

Hidradenitis suppurativa (HS) is a chronic inflammatory skin disease with limited treatment options and frequent drug resistance. Novel therapeutic targets are urgently needed. We performed a druggable genome-wide Mendelian randomization (MR) analysis using blood cis-expression quantitative trait locus (eQTL) and HS genome-wide association study (GWAS) data. Colocalization, transcriptomic validation, single-cell RNA sequencing, and cell-cell communication analyses were integrated to explore gene function and cell-type specificity. We identified eight genes that showed significant associations

New Hidradenitis Suppurativa in Children: Influence of Neighborhood Socioeconomic Status.

Hidradenitis suppurativa (HS) is a chronic inflammatory skin condition with early onset and disproportionate impact on females and African American youth. In our cross-sectional study of 9695 pediatric HS patients in a single US health system, pediatric patients residing in neighborhoods with lower SES had greater odds of having a new HS diagnosis in unadjusted models and after sequential adjustment for age, sex, and race and ethnicity. Fully adjusted odds ratios (95% confidence intervals) were neighborhood SES quintile, Q1 (lowest): 4.05 (1.76-9.32), Q2: 2.86 (1.13-7.22), Q3: 2.94 (1.22-7.09)

YAP1::MAML2 Fusion in Adnexal Tumors Combining Poroma and Sebaceous Components: Report of Two Cases.

Poroma is a benign sweat gland tumor harboring YAP1::MAML2 and YAP1::NUTM1 fusion transcripts in most of the cases. Recently, YAP1 fusions have also been detected in trichoblastomas and in 1 sebaceoma. To expand the spectrum of YAP1 -fused adnexal tumors, we report herein 2 cases of adnexal tumors with YAP1::MAML2 fusion transcript combining a poroma and a sebaceous component. These 10- and 7-mm diameter neoplasms arose in a 55-year-old man and in a 90-year-old woman and were located on the back and the right breast, respectively. Microscopically, both lesions harbored a biphasic appearance re

The Effects of Walk-In Dermatology Appointments for Atopic Dermatitis and Hidradenitis Suppurativa Management.

Atopic dermatitis (AD) and hidradenitis suppurativa (HS) are two inflammatory skin conditions characterized by intermittent flares that may lead some patients to seek treatment in urgent care or emergency departments (ED). At our institution, we implemented a walk-in appointment process for patients seen in the ED for urgent dermatologic concerns. After implementation of this program, patients received treatment by a pediatric dermatologist faster: 75.7 days versus 19.1 days for AD pre- and post-implementation, respectively, and 89.9 days versus 9.33 days for HS pre- and post-implementation, r

Soft tissue augmentation options for volume replacement following parotidectomy: A review of the literature.

BACKGROUND: Salivary gland tumors primarily arise in the parotid gland with surgery representing the cornerstone of treatment. Parotidectomies often lead to a soft tissue deficit that may benefit from reconstruction to reduce incidence of Frey's syndrome, improve aesthetic outcomes, and allow for appropriate postoperative therapies. Several options exist including avascular tissue transfer, locoregional tissue flaps, and microvascular free flaps. METHODS: This paper is a literature review exploring the efficacy of the various reconstructive options, and includes randomized control trials, pro
