Melanocyte-Fibroblast Crosstalk Under Visible Light and Infrared-A Exposure: A Narrative Review of Implications for Photoaging and Dyschromia.

Solar radiation extends beyond the ultraviolet spectrum to include visible light and infrared-A wavelengths, which have traditionally been considered biologically inert. Emerging evidence indicates that these longer wavelengths act both independently and in combination to influence communication between skin cells, contributing meaningfully to photoaging and pigmentary changes. This narrative review synthesizes existing peer-reviewed research on how visible light and infrared-A radiation alter signaling between dermal and epidermal cells and how these alterations may underlie disorders of pigm

Hypertonic Dextrose in Aesthetic Dermatology: Route-Dependent Evidence, Biological Rationale, and Research Priorities.

OBJECTIVE: To critically evaluate whether local hypertonic dextrose has sufficient biological and clinical support to be considered an independent dermal biostimulant in aesthetic dermatology. METHODS: PubMed/MEDLINE was searched from inception through June, 2026, with backward and forward citation tracking. Direct evidence included human aesthetic or dermatologic studies using local dextrose and translational studies examining dermal delivery or tissue response. Evidence was appraised by design, comparator, blinding, cointerventions, outcome validity, follow-up, and directness to a dextrose-

Fundamentals of Hyperpigmentation Disorders: Causes, Concerns, and Treatment Strategies, Particularly in Skin of Color Patients.

BACKGROUND: Pigmentary disorders, including hyperpigmentation, are among the most common dermatologic diagnoses in individuals with skin of color (SOC), particularly individuals with African, Asian, or Hispanic heritage, and often carry a significant quality-of-life burden. Although the evidence base has grown, gaps persist in epidemiology, in pathophysiology-driven treatment selection, and in the appropriate representation of Fitzpatrick skin types III to VI in clinical studies. METHODS: A comprehensive review of PubMed and Medline-indexed hyperpigmentation literature published between 2000

A giant solitary Peutz-Jeghers-type polyp in the gastric body.

A 24-year-old young female presented to our hospital with gastrointestinal bleeding for four days. The patient had no family history of polyps or cancer. There were no pigment spots on her skin, lips, or oral mucosa. For diagnostic purposes gastroscopy and colonoscopy were all performed. Gastroscopy revealed a pedunculated polyp with a size of approximately 2.5 * 2.5cm in the gastric body. Colonoscopy showed normal. Then the polyp was excised by electrocoagulation with a snare. After the polyp was excised, the bleeding stopped. Histopathological analysis revealed that the polyp was a Peutz-Jeg

In vitro skin models for pigmentary disorders: from reconstructed tissues to skin-on-a-chip platforms.

Cutaneous pigmentary disorders, including vitiligo, melasma, post-inflammatory hyperpigmentation (PIH) and solar/senile lentigines (SL), are prevalent, chronic and recurrent conditions driven by interactions among melanocytes, keratinocytes, fibroblasts, immune cells, the extracellular matrix (ECM) and environmental stimuli. Conventional animal models and two-dimensional cultures incompletely reproduce the cellular heterogeneity, tissue architecture and the dynamic microenvironment that regulates human pigmentation. To address these challenges, this review evaluates human-relevant in vitro mod

SERCA2b loss of function drives pigmentation by inducing adaptive ER stress and enhancing mitochondrial calcium uptake: significance in pathological hyperpigmentation associated with Darier's Disease.

Pigmentation is a critical protective mechanism that safeguards the skin against UV-induced damage, whereas dysregulated pigmentation predisposes to pigmentary disorders and skin malignancies. Although calcium signaling has emerged as an important regulator of melanogenesis, the identity of the calcium-handling proteins and the molecular mechanisms linking calcium dynamics to pigmentation remain poorly understood. Here, we identify the ER calcium pump SERCA2b as a negative regulator of pigmentation through modulation of ER stress and mitochondrial calcium uptake. We demonstrate that SERCA2b ex

Non-ablative dermal remodeling lasers in acquired dermal macular hyperpigmentation: A scoping review.

BACKGROUND: Acquired dermal macular hyperpigmentation (ADMH) encompasses a group of challenging pigmentary disorders characterized by dermal melanophages and variable lichenoid interface inflammation. Non-ablative dermal remodeling lasers (NADRL), which target water rather than melanin, have been proposed as a potential therapeutic option through neocollagenesis and modulation of the dermal microenvironment. However, the evidence base for these devices in ADMH has not been systematically characterized. OBJECTIVE: To map and characterize the available evidence on NADRL for the treatment of ADM

When two rare diseases meet: atypical hemolytic uremic syndrome and alkaptonuria in two siblings.

Atypical hemolytic uremic syndrome (aHUS) is a rare complement-mediated thrombotic microangiopathy (TMA), whereas alkaptonuria (AKU) is an uncommon inherited metabolic disorder. Their coexistence has not previously been described. We report two siblings from a consanguineous family with CD46-associated aHUS and AKU to highlight an unusual diagnostic challenge and the importance of recognizing coexisting inherited diseases. Two brothers experienced recurrent episodes of TMA characterized by microangiopathic hemolytic anemia, thrombocytopenia, and acute kidney injury, while maintaining preserved

MC1R variants in four Chinese children with red hair and hypopigmentation.

Loss-of-function variants in the melanocortin 1 receptor (MC1R) gene are a well-established cause of red hair in European populations. However, the genetic spectrum and phenotypic effects of MC1R variants in East Asians remain poorly defined, as red hair is exceedingly rare in this population. To characterize the clinical and genetic features of MC1R-associated red hair and hypopigmentation in Chinese pediatric patients and to expand the genotypic spectrum of MC1R in East Asian populations, four unrelated Chinese children with red or reddish-brown hair underwent comprehensive dermatologic and

Keratinocyte Autophagy in Inflammatory Dermatoses: Mechanisms, Disease-Specific Programs and Therapeutic Opportunities.

Autophagy is increasingly recognised as a regulator of epithelial stress adaptation in the skin, but its role in inflammatory dermatoses remains difficult to interpret because autophagy intersects with barrier biology, keratinocyte differentiation, immune signalling, intracellular trafficking and lysosomal degradation. Keratinocytes are immune-competent epithelial cells that integrate cytokine exposure, oxidative stress, microbial sensing and tissue injury into inflammatory output. In this review, we synthesize evidence on keratinocyte autophagy as a disease-dependent regulator of epidermal ho

Gut and skin microbiome-metabolome pathways in vitiligo: from dysbiosis to immune activation and melanocyte dysfunction.

Vitiligo is an autoimmune depigmenting disorder driven by genetic susceptibility, oxidative stress, immune dysregulation, and progressive melanocyte loss. Emerging microbiome-metabolome evidence suggests that gut and skin dysbiosis may contribute to disease progression through the gut-skin axis. This review integrates current evidence linking microbial imbalance with immune activation and melanocyte dysfunction in vitiligo. Studies in patients and experimental vitiligo models show altered gut and skin microbial diversity, enrichment of inflammation-associated taxa, depletion of commensal bacte

Setmelanotide in Bardet-Biedl Syndrome: A Case Report.

Setmelanotide is a melanocortin-4-receptor agonist used for the treatment of hyperphagia in the genetic obesity syndrome Bardet-Biedl. Presented is a case of diffuse hyperpigmentation in a patient treated with setmelanotide, which represents the most common side effect of this medication. Herein is a review of the literature for other potential side effects that warrant regular dermatologic screening, including alteration of existing melanocytic lesions. Exact screening guidelines have not been completely defined, though it is recommended that patients receive baseline and annual examinations

Beyond UV: Visible Light and Pigmentation.

BACKGROUND: Melasma and post-inflammatory hyperpigmentation (PIH) are common dermatologic conditions disproportionately affecting individuals with darker skin phototypes, carrying substantial psychosocial burden and reduced quality of life. Emerging evidence increasingly implicates visible light (VL), particularly high energy visible (HEV) light, as a significant but underappreciated driver of disease appearance and progression. AIMS AND METHODS: This narrative review integrates mechanistic and clinical evidence to examine VL's pathogenic role in melasma and PIH, synthesising data on opsin-me

Impact of previous skin hydration on postsclerotherapy hyperpigmentation: a prospective, randomized, comparative study (SKINCARE Study).

BACKGROUND: Postsclerotherapy hyperpigmentation (PSH) is a frequent and distressing complication. It is a cosmetic concern in aesthetic phlebology, with limited prospective efficacy/safety data. Skin hydration with moisturizer strengthens the skin barrier and may potentially prevent postsclerotherapy hyperpigmentation. METHODS: This prospective, randomized, comparative study included 55 adult patients with reticular veins and telangiectasias. Patients were randomized to receive a daily moisturizer for 30 days before sclerotherapy or to no hydration. All participants underwent a single session

TattleTail: A Rule-Based Tool for Identifying Canonical Pyocin-Encoding Gene Clusters in Pseudomonas aeruginosa.

Tailocins are phage tail-like bacteriocins derived from prophages that have lost the ability to package viral genomes while retaining bactericidal activity against related bacterial strains. In Pseudomonas aeruginosa, these elements are referred to as pyocins and contribute to ecological fitness and antimicrobial applications. Despite lacking essential phage components, pyocin-encoding gene clusters share substantial sequence homology with phage tail genes, creating a major challenge for genome annotation. Current prophage prediction tools, which rely on phage gene homology, can frequently mis

Acute pancreatitis associated with periampullary hamartomatous polyp in Peutz-Jeghers syndrome.

We report the case of an 11-year-old girl with Peutz-Jeghers syndrome (PJS) and a history of bilateral oophorectomy for a sex cord-stromal tumor, who presented with acute abdominal pain, nausea, and vomiting. Laboratory findings indicated acute pancreatitis, and imaging revealed pancreatic enlargement with a large lobulated polypoid lesion (6.2 × 3.8 cm) in the periampullary duodenum, causing biliary and pancreatic ductal dilatation without gallstones. Upper endoscopy confirmed the lesion, and surgical duodenotomy with polypectomy and cholecystectomy was performed. Histopathological analysis i
