Stevens-Johnson syndrome/toxic epidermal necrolysis as the initial presentation of paraneoplastic anti-TIF1-γ dermatomyositis.

We report a rare case of Stevens-Johnson syndrome/toxic epidermal necrolysis as the initial manifestation of paraneoplastic dermatomyositis in a 50-year-old man subsequently diagnosed with diffuse large B cell lymphoma, in the absence of any identifiable drug exposure. The patient presented with periorbital edema, progressive dusky blistering eruption (Nikolsky sign positive), mucositis, and histopathology consistent with Stevens-Johnson syndrome/toxic epidermal necrolysis. Over the following weeks, he developed proximal muscle weakness, elevated creatine kinase, and cutaneous signs of dermato

Cutaneous clues to diagnosis: the vasculopathic form of anti-MDA5dermatomyositis.

Anti- melanocyte differentiation-associated protein 5 (MDA5) associated dermatomyositis is a rare, heterogeneous disease with multiple phenotypes. This case describes a patient with indolent myopathy, skin lesions and normal creatine kinase (CK). After a period of diagnostic uncertainty, the patient was found to have positive anti-MDA5 and anti-Ro52 antibodies, confirming anti-MDA5 dermatomyositis with cutaneous vasculopathy. Recognition of vasculopathy as a manifestation of dermatomyositis could have led to a much earlier diagnosis. The European Alliance of Associations for Rheumatology /Amer

Clinical and Autoantibody Profile of Inflammatory Myopathies in a Mexican Cohort: A Descriptive Study.

BACKGROUND: Inflammatory myopathies (IIMs) are a group of autoimmune diseases characterized by muscle inflammation and systemic features. Although autoantibody profiling has improved disease classification, regional data from Latin America remain limited. OBJECTIVE: To describe the clinical and serological characteristics of a Mexican cohort. METHODS: We conducted a cross-sectional study of adult patients who met the 2017 EULAR/ACR criteria for IIMs at a tertiary rheumatology clinic in Monterrey, Mexico, from 2015 to 2019. We interviewed patients, reviewed medical records, and performed a my

Case Report: Juvenile dermatomyositis complicated by acute myeloid leukemia: achievement of sustained remission through hematopoietic stem cell transplantation.

Juvenile dermatomyositis (JDM), a pediatric subtype of idiopathic inflammatory myopathy, is a relatively rare rheumatic and immune system disease among children. Although paraneoplastic dermatomyositis (DM) has been documented in adult malignancies, its association with acute myeloid leukemia (AML) in pediatric populations remains exceptionally rare. We present a male patient diagnosed with JDM at age 6, characterized by Gottron's papules and muscle weakness. Despite receiving sequential immunosuppressive therapies and biological agents, the disease followed a relapsing-remitting course over 9

Pancreatic Adenocarcinoma Presented as Paraneoplastic Dermatomyositis Complicated with Rhabdomyolysis: Case Report and Review.

BACKGROUND Dermatomyositis is a rare autoimmune condition characterized primarily by proximal muscle weakness and cutaneous rashes, such as Gottron's papules and heliotrope rash, and is classified as a subtype of idiopathic inflammatory myopathies. While the clinical spectrum of dermatomyositis is broad, its initial onset as fulminant rhabdomyolysis (complicated by acute renal failure) is exceedingly rare. Such atypical presentations necessitate proactive clinical vigilance to ensure a prompt and accurate diagnosis. CASE REPORT Our patient was a 53-year-old woman who presented with progressive

Anti-PM/Scl Antibody-Positive Dermatomyositis With Rapidly Progressive Interstitial Lung Disease in a 19-Year-Old Male: Clinical Implications of a Presumptive Diagnosis Based on Line Immunoassay Positivity.

Anti-PM/Scl antibodies are classically linked to systemic sclerosis-polymyositis overlap, but a dermatomyositis phenotype with interstitial lung disease has also been described. However, the clinical course and therapeutic approach for short-term progressive interstitial lung disease in anti-PM/Scl-positive dermatomyositis remain incompletely defined. A 19-year-old Japanese male developed cough and exertional dyspnea after several weeks of progressive myalgia and proximal muscle weakness. Marked creatine kinase elevation, a high-titer nucleolar-pattern antinuclear antibody, Gottron papules, an

Spectrum of clinical phenotypes associated with myositis-specific and myositis-associated antibodies in juvenile idiopathic inflammatory myositis: Our experience from North India.

BACKGROUND: Myositis-specific autoantibodies (MSAs) and myositis-associated autoantibodies (MAAs) are increasingly being identified in juvenile inflammatory myositis (JIIM). The current study's objective was to characterize the incidence and clinical-phenotypic profile of MSA/MAA in a cohort of JIIM from North India. METHODS: We reviewed the medical records of the Pediatric Rheumatology Clinic of a tertiary care referral centre in Chandigarh, North India (1992 to 2024) and analysed the clinical details of children with JIIM. MSA/MAAs were assayed using a 16-antigen kit immunoblot assay (Eurol

Paraneoplastic Dermatomyositis Preceding Gastroesophageal Junction Adenocarcinoma: A Case Emphasizing the Importance of Early Malignancy Screening.

Dermatomyositis (DM) is an idiopathic inflammatory myopathy marked by proximal muscle weakness and pathognomonic rashes such as heliotrope rash and Gottron papules. DM is associated with an increased risk of malignancy compared to the general population, with 15-30% of cases being paraneoplastic. DM-associated malignancies are often diagnosed at late stages, resulting in cancer being a leading cause of death in this condition. Additionally, DM can mask malignancy by presenting with similar symptoms as the inciting cancer, further emphasizing the importance of early, disease-specific cancer scr

Severe Juvenile Dermatomyositis With Peripheral Nervous System Involvement: A Case Report.

Juvenile dermatomyositis is the most common inflammatory myopathy of childhood. While many cases achieve favorable outcomes, severe forms may develop persistent disease activity, complications, and long-term disability. A five-year-old girl was admitted after 20 days of Gottron's papules and heliotrope rash, with intermittent fever during the first 10 days. She then developed mild proximal-predominant muscle weakness in all four limbs, rapidly progressing to severe generalized weakness with signs of axonal neuropathy over the subsequent 15 days. Despite aggressive immunosuppression with IV met

Antinuclear Antibody (ANA)‑Negative, Anti-melanoma Differentiation-Associated Gene 5 (MDA5)‑Positive Dermatomyositis Presenting as Rapidly Progressive Interstitial Lung Disease: A Case Report.

Dermatomyositis (DM) is a rare, autoimmune myopathy characterized by proximal muscle weakness and pathognomonic skin findings, including Gottron's papules and a heliotrope rash. Anti-melanoma differentiation-associated gene 5 (MDA5)-positive DM is a distinct subtype often associated with rapidly progressive interstitial lung disease (RP-ILD) and may lack the classic features of DM. We present the case of a 43-year-old woman who developed persistent hypoxic respiratory failure after multiple vascular surgeries requiring repeated postoperative blood transfusions. Her respiratory failure was init

Unmasking the Myositis: A Diagnostic Clue to Occult Breast Cancer.

Dermatomyositis (DM) is an idiopathic inflammatory myopathy characterised by proximal muscle weakness and pathognomonic cutaneous features. A well-recognised but underappreciated association exists between DM and underlying malignancy, termed cancer-associated myositis (CAM). The prevalence of malignancy in DM patients is high, with breast, ovarian, lung, and gastrointestinal cancers most commonly implicated. However, the detection of occult malignancy in young women presenting with DM remains particularly challenging due to atypical epidemiology and the limitations of standard imaging in dens

Mucocutaneous manifestations as early indicators of systemic disease: a narrative review.

BACKGROUND: Mucocutaneous findings may provide important early clues to systemic disease. Because the skin, oral mucosa, eyes, and other visible mucosal surfaces are readily accessible during clinical examination, their careful assessment can contribute to earlier recognition of autoimmune, infectious, metabolic, endocrine, inflammatory, and malignant conditions. OBJECTIVE: This narrative review aims to summarize clinically relevant mucocutaneous manifestations that may act as early indicators of systemic disease, with emphasis on their diagnostic significance and practical relevance for clin

Anti-synthetase syndrome with anti-PL-7 antibody positive in a child: a case report and literature review.

BACKGROUND: Anti-synthetase syndrome (ASS) is a rare autoimmune myopathy and forms part of the idiopathic inflammatory myopathies (IIMs). A distinctive feature of ASS is the presence of anti-aminoacyl tRNA synthase (ARS) antibodies, which target synthetases, leading to inflammation in muscles, lungs, and other tissues. CASE PRESENTATION: A case of a 12-year-old Chinese girl with recurrent fever, myalgia, and Gottron's papules is reported. Serum creatine kinase was markedly elevated, and chest CT revealed interstitial changes. Magnetic Resonance Imaging (MRI) of the left thigh indicated soft t
