Atypical hemolytic uremic syndrome (aHUS) is a rare, chronic, and life-threatening thrombotic microangiopathy (TMA). The disease is most frequently linked to genetic or acquired dysregulation of the alternative complement pathway and affects both children and adults. The paucity of a gold standard for diagnostic testing and the variety of clinical manifestations are the primary reasons for the challenge in accurately diagnosing aHUS, which can have an impact on patient care. This is due to the fact that the more prevalent autoinflammatory diseases are multifactorial in nature, with both geneti

The complement cascade is a vital component of the innate immune system. It consists of around 50 proteins, including circulating complement proteins and membrane-bound receptors. It operates through three main activation pathways: classical, lectin, and alternative. Dysregulation of the complement system is implicated in several kidney diseases, such as complement-mediated thrombotic microangiopathy, lupus nephritis (LN), C3 glomerulopathy and kidney transplantation. The complement system also extends beyond immune responses, influencing cellular processes through the "complosome" - an intrac

BACKGROUND: ABO-incompatible kidney transplantation (ABOi-KT) expands donor availability, but postoperative anti-ABO antibody rebound can lead to rare complications such as thrombotic microangiopathy (TMA), often resulting in graft loss.
CASE PRESENTATION: A 61-year-old underwent ABOi-KT (A → B) after standard desensitization. Despite achieving pre-transplant anti-A IgG and IgM titers of 1:1, a marked postoperative rebound occurred, with both titers rising to 1:1024 by postoperative day 6, accompanied by TMA and acute rejection manifesting as elevated serum creatinine (sCre) (maximum 8.63 mg/

Narsoplimab (narsoplimab-wuug; YARTEMLEA®) is a recombinant human immunoglobulin G4 monoclonal antibody targeted against mannan-binding lectin-associated serine protease 2 (MASP-2) being developed by Omeros Corporation initially for the treatment of haematopoietic stem cell transplantation (HCT)-associated thrombotic microangiopathy (TA-TMA). MASP-2 inhibition by narsoplimab is thought to provide benefit in patients with TA-TMA by decreasing or preventing lectin complement pathway-mediated cellular injury. In December 2025, narsoplimab received its first approval, in the USA, for the treatment

Thrombotic thrombocytopenic purpura (TTP), an insidious and often devastating thrombotic microangiopathy, represents one of the few true hematologic emergencies. Since the identification of the von Willebrand factor (VWF) cleaving protease ADAMTS13 in the early 21st century, tremendous advances have been made in the diagnosis and treatment of the disease. However, as with many areas of science, new knowledge begets new questions about the pathophysiology of both congenital and immune TTP, which identify gaps in our understanding of the mechanistic effects of ADAMTS13 and its inhibitors. Improv

Patients who undergo hematopoietic stem cell transplantation (HSCT) face a high risk of acute kidney injury (AKI), which significantly increases the likelihood of developing chronic kidney disease (CKD). Proteinuria is also frequently observed after HSCT and is associated with an increased risk of CKD progression. Thrombotic microangiopathy (TMA), stemming from endothelial injury caused by factors such as total body irradiation (TBI), calcineurin inhibitors, tyrosine kinase inhibitors, and graft-versus-host disease, is a major contributor to both AKI and CKD. We report a case of a woman in her

INTRODUCTION: Thrombotic microangiopathy (TMA) is a common pathological phenotype of immunoglobulin A nephropathy (IgAN). Low-density granulocytes (LDGs) exhibit potent pro-inflammatory properties and promote neutrophil extracellular trap (NET) formation, playing a pivotal role in thrombus development. This study aims to investigate the long-term prognosis of IgAN-TMA and explore the role of LDGs in its pathogenesis.
METHODS: Sixty patients with IgAN-TMA and 249 high-risk IgAN controls were included. LDGs (CD14lowCD15high) were quantified via flow cytometry among 15 IgAN-TMA patients, 20 IgAN

INTRODUCTION: Discovery of biomarkers predictive of endotheliopathy after hematopoietic stem cell transplant (HSCT) has the potential to more rapidly identify vulnerable patients, guide treatment decisions, track responses to therapy, and inform targets for novel treatment. This study aimed to evaluate the levels and predictive capabilities of glycocalyx biomarkers for endotheliopathy in the first 100 days following HSCT in pediatric and young adult patients.
METHODS: Parallel prospective observational pilot studies were performed to examine markers related to endothelial glycocalyx structure

BACKGROUND: Complement-mediated thrombotic microangiopathy (CM-TMA) results from dysregulated alternative pathway activation. Although extra-renal manifestations are well-recognized, presentation as a pancreatic mass is exceedingly rare and risks misdiagnosis as malignancy, delaying critical intervention.
CASE PRESENTATION: A 28-year-old male presented with malignant hypertension and rapidly progressive renal impairment. Abdominal CT revealed a pancreatic tail mass suspicious for neoplasia. However, histopathology of both renal and pancreatic tissues demonstrated microvascular thrombosis and

Thrombotic microangiopathy (TMA) is a rare but life-threatening complication in idiopathic inflammatory myopathies. Although TMA has been increasingly recognised in anti-melanoma differentiation-associated gene 5 antibody-positive dermatomyositis, it remains exceptionally uncommon in anti-signal recognition particle (SRP) antibody-positive immune-mediated necrotising myopathy (IMNM). We describe a 62-year-old woman with anti-SRP antibody-positive IMNM who developed rapidly progressive TMA shortly after the initiation of immunosuppressive therapy, including tacrolimus. At presentation, she exhi

Atypical hemolytic uremic syndrome (aHUS) is one of the thrombotic microangiopathy (TMA) syndromes characterized by thrombocytopenia, Coombs-negative hemolytic anemia, and renal dysfunction without evidence of Shiga toxin-producing Escherichia coli (E. coli) (STEC) or thrombotic thrombocytopenic purpura (TTP). aHUS is a rare, potentially fatal entity. We present a case of a six-year-old male with vomiting and abdominal pain who developed hematuria and jaundice, along with acute kidney injury (AKI). The patient was diagnosed with aHUS and managed with plasma infusions and exchange, immunosuppre

Snakebite remains a cause of morbidity in tropical regions, with viperid venoms typically causing local tissue injury (edema, bleeding, ecchymoses), vasculotoxicity, coagulopathy, and acute kidney injury. Delayed immune-mediated encephalopathies are uncommon. We report a 49-year-old woman from Birbhum district, West Bengal, India, who sustained a bite from a Russell's viper in June 2025 (rainy season). She developed vasculotoxic envenoming and acute kidney injury requiring hemodialysis for 5 days, followed by apparent systemic recovery and discharge. Seven days later, she developed abrupt scan

BACKGROUND: Whether IgA nephropathy (IgAN) in patients with systemic lupus erythematosus (SLE) represents a coincidental comorbidity or a distinct clinico-pathological entity remains unclear. This study aimed to characterise the demographic, clinical, pathological, and prognostic features of this rare association.
METHODS: We conducted a systematic review of the PubMed and Embase databases to 31 May 2025 were conducted to identify all reported cases of biopsy-proven IgAN in patients with SLE, excluding cases with concomitant thrombotic microangiopathy, negative Gd-IgA1 immunostaining, prior I

Thrombotic microangiopathies (TMAs) that occur during pregnancy or the postpartum period-including preeclampsia/HELLP syndrome (hemolysis, elevated liver enzymes, low platelets), thrombotic thrombocytopenic purpura, and atypical hemolytic uremic syndrome (aHUS)-present a diagnostic challenge owing to their overlapping clinical features. We report a case of a 21-year-old primigravida of African origin who developed HELLP syndrome followed by postpartum aHUS. The clinical course was marked by thrombocytopenia, hemolytic anemia, acute kidney injury, and massive proteinuria. Kidney biopsy revealed

INTRODUCTION: Post-transplant-associated complications arising from endothelial injury, such as sinusoidal obstruction syndrome (SOS) and transplant-associated thrombotic microangiopathy (TA-TMA), are associated with high mortality rates following hematopoietic stem cell transplantation (HSCT). Syndecan-1 (SDC1), a cell-surface proteoglycan released into the circulation upon endothelial damage, is a potential biomarker for vascular integrity. We previously demonstrated that elevated serum SDC1 levels were associated with disease severity and poor prognosis in patients with COVID-19.
METHODS:
