Exploring the optimal follow-up for systemic sclerosis patients: study protocol for a Dutch multicenter randomized controlled trial.

BACKGROUND: Currently, evidence-based guidelines for the frequency and intensity of follow-up of systemic sclerosis (SSc) patients are not available. Based on expert opinion, an annual extensive evaluation is recommended. A multidisciplinary Care Pathway that integrates this annual extensive evaluation at the Leiden University Medical Center has shown increased patient satisfaction, decreased healthcare utilization, and improved outcomes in SSc patients (1, 2). However, for a subgroup of SSc patients with relatively mild disease, this annual extensive evaluation might be redundant. Therefore,

Advancing drug development for systemic sclerosis by prioritizing findings from human genetic association studies.

OBJECTIVES: SSc is a rare rheumatological disease associated with significant morbidity and mortality. Despite significant recent international clinical trial activity, the yield of approved compounds has been disappointingly low. Our aim was to identify and prioritize potential 'druggable' targets with insights from human genetics, by integrating the available evidence with publicly available bioinformatics sources relevant for SSc drug development. METHODS: Genetic variants for SSc were identified through a search of the GWAS Catalog, and the associated-mapped genes were cross-referenced wi

Assessing digital vasculopathy in systemic sclerosis.

Digital vasculopathy (a spectrum of Raynaud's phenomenon, digital ulceration and critical ischaemia) is one of the most characteristic manifestations of systemic sclerosis (SSc). It is an area of unmet need with a major impact on quality of life: current treatments are only poorly effective. SSc-related digital vasculopathy is a result of structural as well as functional change at the level of both the microcirculation and the digital artery, explaining its severity. This review begins with a brief description of digital vasculopathy, followed by its assessment in the clinical setting, relevan

Second to Fourth Digit Ratio (2D:4D) in Female Patients With Systemic Sclerosis: Evidence for Prenatal Androgen Exposure.

OBJECTIVES: Systemic sclerosis (SSc) demonstrates marked female predominance, suggesting hormonal influences in disease pathogenesis. The second-to-fourth digit ratio (2D:4D), a biomarker of prenatal androgen exposure, has been associated with various autoimmune conditions. This study investigated whether 2D:4D ratios differ between female SSc patients and healthy controls. METHODS: This case-control study enrolled 33 women with SSc (2013 ACR/EULAR criteria) and 30 age-matched healthy female controls. Second and fourth digit lengths were measured bilaterally using digital calipers, and 2D:4D

An autopsy-proven case of normotensive scleroderma renal crisis with refractory pleuroperitoneal effusion.

INTRODUCTION: Scleroderma renal crisis (SRC) is a serious complication of systemic sclerosis that typically presents with hypertension and acute kidney injury (AKI). However, about 10% of cases are normotensive, while refractory pleural or ascitic effusion preceding SRC is rare in clinical practice. Here, we present a case of normotensive SRC preceded by refractory pleural and ascitic effusions. CASE PRESENTATION: The patient was a 66-year-old woman with a history of rheumatoid arthritis who was managed as an outpatient. Three months prior to admission, she developed sudden, generalized refra

Microvascular markers in systemic sclerosis: a comparative OCT-angiography and nailfold videocapillaroscopy study.

PURPOSE: To investigate the effect of disease duration on retinal microvasculature in patients with systemic sclerosis (SSc), and to explore its correlation with nailfold videocapillaroscopy (NFC) findings and systemic organ involvement. METHODS: Patients were divided into subgroups according to disease duration (≤5 years, >5 years, <10 years, ≥10 years). Retinal microvasculature was evaluated by OCT-A in the superficial (SCP) and deep (DCP) plexuses, and peripheral microcirculation was assessed by NFC. Correlations between retinal microvascular parameters, disease duration, systemic involveme

A CD57+CD8+ T cell subset links T cell cytotoxicity to fibrotic lung disease in systemic sclerosis.

Interstitial lung disease (ILD) is a major cause of morbidity and mortality in systemic sclerosis (SSc); however, the immunopathologic mechanisms driving lung disease in SSc are unclear. T cells have been implicated as a likely driver of lung injury in SSc. Here, we evaluated T cells in the blood of patients with SSc-ILD and identified a specific population of cytotoxic CD8+ T cells that was expanded in patients with SSc-ILD. Cytotoxic effector memory CD8+ T cells marked by CD57 expression were preferentially expanded in patients with SSc-ILD compared with patients with SSc but no ILD and cont

A potent inhibitor of PAI-1, MDI-2517, mitigates disease severity in a preclinical systemic sclerosis model.

Systemic sclerosis (SSc) is a complex and heterogeneous condition characterized by progressive fibrosis in multiple organs. Recent studies implicate plasminogen activator inhibitor-1 (PAI-1) in the pathogenesis of SSc, and PAI-1 is considered as a potential target for therapy. Here, using single-cell and spatial RNA-seq analysis of skin biopsies from 18 healthy individuals and 22 SSc patients, we found elevated PAI-1 colocalizing to myofibroblasts with enriched extracellular matrix-associated biological processes. Treatment of SSc dermal fibroblasts with the small-molecule PAI-1 inhibitor MDI-

An Under-Recognized Manifestation of Systemic Sclerosis: Macrovascular Peripheral Arterial Disease.

Systemic sclerosis (SSc) is a rare autoimmune connective tissue disease characterized by skin and visceral fibrosis and by a well-known microangiopathy leading to Raynaud's phenomenon, digital ischemia, pulmonary arterial hypertension, and scleroderma renal crisis. By contrast, macrovascular involvement has been less extensively described and may be under-recognized. We report the case of a 66-year-old woman with a 16-year history of SSc. The diagnosis was based on cutaneous sclerosis, Raynaud's phenomenon complicated by digital necrosis, positive antinuclear antibodies, and interstitial lung

Systemic sclerosis-associated pulmonary arterial hypertension and pulmonary fibrosis: exploring biomarker discriminators with advanced omics in a Caucasian cohort.

INTRODUCTION: Systemic sclerosis (Scleroderma; SSc) is associated with high morbidity and mortality, particularly in patients with pulmonary arterial hypertension (SSc-PAH) and pulmonary fibrosis (SSc-PF). Effective risk stratification and treatment of SSc remains a significant challenge. This proof-of-concept study aimed to identify potential biomarkers capable of distinguishing between three SSc patient groups, defined by no pulmonary involvement (SSc-NLD; n=30), SSc-PAH (n=30), SSc-PF (n=30) compared to healthy controls (HC; n=30). METHODS: The study employed Olink-based proteomics using t

Intercellular communication landscape and its working mechanism in systemic sclerosis-associated interstitial lung disease.

OBJECTIVES: SSc-associated interstitial lung disease (SSc-ILD) is the leading cause of death amongst SSc patients. However, profibrotic factors in SSc-ILD have not been systematically assessed and their working mechanisms remain unclear. METHODS: We performed an integrated single-cell RNA sequencing (scRNA-seq) analysis of 17 normal and 17 SSc-ILD lung tissues using CellChat and Monocle3. Single-cell ATAC sequencing (scATAC-seq) profiles were analysed by Signac. We performed ELISA to detect specific ligand serum levels using specimens of 55 SSc patients with (n = 32) or without (n = 23) ILD.

Assessing the structural validity of the SCL-Dep6 depression subscale using rasch analysis.

Systemic Sclerosis (SSc) is a rare and complex autoimmune disease characterized by skin and internal organ fibrosis, vascular abnormalities, and immune dysfunction. In addition to physical symptoms, many patients experience psychological distress, particularly depression, which can negatively impact treatment adherence and quality of life. The SCL-Dep6 is a subscale of the SCL-90-R questionnaire, consisting of six items that assess core aspects of depression, including low mood, anhedonia, guilt, fatigue, and suicidal thoughts. The aim of this study was to evaluate the structural validity of t

Management of pulmonary arterial hypertension in systemic sclerosis: from classical treatments to new horizons.

Pulmonary arterial hypertension (PAH) is a severe disease characterised by a progressive thickening and obliteration of pulmonary vessels, resulting in increased vascular resistance, elevated pulmonary artery pressures, and right heart failure. Among the various conditions associated with PAH, systemic sclerosis (SSc) is the most common in Western countries. Compared to other forms of PAH, SSc-PAH presents with a more aggressive clinical course, poorer response to conventional therapies and a worse prognosis. However, despite these differences, the overall management of SSc-PAH remains close t

Prevalence and clinical relevance of systematically tested antimitochondrial antibodies in systemic sclerosis.

INTRODUCTION: Antimitochondrial antibodies (AMA), the hallmark of primary biliary cholangitis (PBC), have been inconsistently reported and poorly characterized in systemic sclerosis (SSc). Although recently linked to subclinical dysmotility in a cohort enriched for gastrointestinal (GI) involvement, data on the prevalence and clinical relevance of AMA in real-life SSc cohorts remain limited. METHODS: A retrospective cross-sectional study was conducted on consecutive SSc patients followed at a referral center for the disease. Patients were tested for serum AMA according to reference standards,

A systematic review of therapeutic procedures for the treatment of morphea and systemic sclerosis.

Considering the challenges involved in managing morphea and systemic sclerosis—especially the risks of disease reactivation and inflammation following treatment—and the limited evidence on the effectiveness and safety of treatment methods like energy-based devices and intralesional injections, it is essential to conduct a systematic review to compile and critically evaluate the available data comprehensively. This review aims to evaluate the effectiveness and safety of energy-based devices and intralesional injections in managing morphea and systemic sclerosis, to provide critical evidence tha

Drug development in systemic sclerosis: novel therapeutics to watch.

INTRODUCTION: Systemic sclerosis (SSc) is increasingly understood as a triad of immune dysregulation, progressive fibrosis, and vasculopathy that evolve in parallel. The literature search was conducted using PubMed/MEDLINE, ClinicalTrials.gov, and the European Clinical Trials Database (EudraCT), including studies published in English up to October 2025. AREAS COVERED: This review summarizes the most recent therapeutic strategies now in clinical development and links each to its underpinning pre‑clinical and translational evidence, thereby mapping how mechanistic insights are reshaping drug pi

Carbon dioxide vs. warm-water hand baths in systemic sclerosis with secondary Raynaud's syndrome - A capillaroscopy centered randomized controlled trial.

BACKGROUND: Raynaud's phenomenon (RP) affects nearly all patients with systemic sclerosis (SSc) and causes substantial morbidity and functional impairment. We evaluated the immediate, segment-specific microvascular effects of a single carbon dioxide (CO₂) versus warm water hand baths in SSc with secondary RP, including healthy controls as physiological reference. METHODS: In this single-centre, randomized controlled trial, 24 SSc patients were allocated (1:1) to a 15-min CO₂ bath (2 g/L, 35 °C) or warm water bath (40-42 °C). Twelve healthy controls received a CO2 bath. Nailfold capillaroscopy

Evaluating context effects on PHQ-8 somatic item scores among people with a chronic medical condition: a scleroderma patient-centred intervention network randomised experiment.

AIMS: Assessing depression symptoms in people with a chronic illness is challenging due to possible bias from overlapping somatic symptoms associated with both depression and chronic illnesses. Previous studies, however, have found that people with a chronic illness do not report more somatic symptoms on depression measures than people without a chronic illness with similar levels of mood and cognitive symptoms. The reason for this surprising finding is unknown. Our primary objective was to evaluate differences in mean sum scores of Patient Health Questionnaire-8 (PHQ-8) somatic symptom items
