National trends in lumbar facet cyst surgical management: rising fusion utilization without improved 5-year outcomes over decompression alone.

BACKGROUND CONTEXT: Lumbar facet cysts can contribute to lumbar radiculopathy and stenosis. Certain patients with lumbar facet cysts may be considered for surgical intervention, which can include decompression (D) alone or decompression with fusion (D+F). However, the use of one modality versus the other remains a topic of discussion. PURPOSE: The current study utilized a large, national dataset to characterize yearly rates of D versus D+F utilization, identify factors independently associated with D+F relative to D, compare 90-day overall costs (measured by insurer-payments), and analyze 5-y

Morphological Analysis of the Intervertebral Foramen and Feasibility of Transforaminal Full-Endoscopic Spine Surgery.

BACKGROUND: Transforaminal full-endoscopic spine surgery (TF-FESS) is a minimally invasive surgical procedure for lumbar spinal disease; however, it is technically demanding in patients with foraminal stenosis because of anatomical problems. This procedure has unique surgery-related complications, such as exiting nerve root injury, causing postoperative dysesthesia or muscle weakness, resulting in a poor clinical outcome. Therefore, we have to avoid this complication. The objective of this study was to analyze the morphology of the intervertebral foramen and assess the feasibility of TF-FESS.

Predictive Role of MRI-Detected Modic Changes in the Chronicity and Severity of Low Back Pain: A Systematic Review and Meta-Analysis.

The main focus of this systematic review and meta-analysis was to establish a correlation between vertebral subchondral endplate changes (Modic changes, or MCs) and low back pain (LBP). This review also wanted to determine whether MCs serve as valid predictors of progressive degenerative spinal diseases and surgical outcomes. In accordance with PRISMA guidelines, an extensive literature search was conducted across major databases for peer-reviewed studies published up to 31 May 2025. The Joanna Briggs Institute (JBI) instrument and the Newcastle-Ottawa Scale (NOS) were used in assessing the qu

Sacral Insufficiency Fracture Following L4-S1 Circumferential Fusion in Corticosteroid-associated Bone Fragility: A Case Report and Biomechanical Considerations.

INTRODUCTION: Sacral insufficiency fracture (SIF) is an uncommon but increasingly recognized complication following lumbosacral fusion, particularly in patients with compromised bone quality. The biomechanical vulnerability of the L5-S1 junction, combined with altered load transfer after anterior and posterior reconstruction, may predispose to early sacral failure. CASE REPORT: A 55-year-old male with lytic spondylolisthesis at L5-S1, repeated corticosteroid exposure, and neurogenic claudication underwent staged circumferential L4-S1 fusion (L5-S1 anterior lumbar interbody fusion, L4-L5 obliq

Multifidus fatty infiltration and vertebral translation are key factors associated with movement-related low back pain in L5 isthmic spondylolisthesis.

INTRODUCTION: In isthmic lumbar spondylolisthesis (ILS), persistent pain is a major indication for surgical intervention, yet it correlates poorly with vertebral slippage. Therefore, this study analyzes the influence of translation, segment degeneration and paraspinal muscle morphology on pain in patients with isolated ILS at the L5 level. METHODS: This cross-sectional study retrospectively enrolled 78 patients (46 females, 32 males). Fatty infiltration (FI) of the multifidus (MF) and erector spinae (ES) muscles, as well as the psoas index, were assessed. Segmental degeneration was graded for

Intradiscal Injection of Adinizer-Processed Minimally Microfragmented Adipose Tissue for Degenerative Lumbar Spondylolisthesis: A Case Report With a Two-Year Follow-Up.

Degenerative lumbar spondylolisthesis is a common cause of chronic low back pain and functional limitation in older adults. Although conservative treatment is generally considered first-line management, some patients continue to experience persistent symptoms, and surgery may not always be feasible or acceptable. We report the two-year clinical course of a 72-year-old woman with symptomatic degenerative lumbar spondylolisthesis who underwent intradiscal injection of Adinizer-processed minimally microfragmented adipose tissue (MFAT). She had longstanding chronic low back pain for approximately

Subacute post-traumatic ascending myelopathy after cervical spinal cord injury: a rare and fatal complication.

Subacute post-traumatic ascending myelopathy (SPAM) is a rare but devastating complication of spinal cord injury (SCI). It is character-ized by progressive neurological deterioration extending several segments above the primary lesion within days to weeks after trauma. The underlying pathophysiology remains uncertain, and treatment strategies are not standardized. A 38-year-old man sustained trau-matic C6-7 spondylolisthesis with bilateral facet dislocation following a motorcycle accident. Initial magnetic resonance imaging (MRI) demonstrated cord contusion and edema extending from C5 to C7. A

Paraspinal musculature and adjacent segment disease after lumbar fusion: a systematic review and meta-analysis of fusion with and without posterior instrumentation.

BACKGROUND CONTEXT: Adjacent segment disease (ASD) is the principal long-term complication following lumbar fusion, contributing to pain, disability, and the need for revision surgery. While biomechanical and patient-related factors have been proposed, iatrogenic injury to the paraspinal musculature during posterior exposure may also promote ASD. Minimally invasive anterior, oblique, and lateral approaches, which avoid posterior dissection, may mitigate this risk. PURPOSE: To compare rates of radiographic (rASD) and symptomatic ASD, ASD-related reoperation, and secondary outcomes including fu
