Comparison of the Effectiveness of Vojta Therapy and the NDT Bobath Concept in the Treatment of Congenital Muscular Torticollis in Infants-A Retrospective Cohort Pilot Study.

Background/Objectives: Congenital muscular torticollis (CMT) affects 0.3-3.9% of infants, requiring early physiotherapy to prevent deformities. Vojta and NDT Bobath therapies are widely used, yet comparative evidence remains limited. To compare Vojta versus NDT Bobath efficacy in improving head tilt and cervical rotation in infants with CMT. Methods: Retrospective cohort study (2016-2024) at Polish Mother's Memorial Hospital included 53 infants under 5 months with ultrasound-confirmed CMT. Non-random allocation based on therapist availability introduced selection bias. Participants received Vo

Polysomnographic characteristics in patients with idiopathic dystonia.

BACKGROUND: Several aspects of the occurrence of sleep disturbances in dystonia remain unclear. Here, we assessed sleep architecture with polysomnography (PSG) and investigated the association of specific findings with mood, quality of life and severity of disease. METHODS: Twelve patients with idiopathic dystonia underwent overnight PSG and findings were compared to those of a normative collective. Subjective sleep quality was evaluated with the Pittsburgh-Sleep-Quality-Index (PSQI) and the Epworth-Sleepiness-Scale (ESS). PSG findings, PSQI and ESS were correlated to motor scores of the Burk

Pallidal and subthalamic deep brain stimulation for cranio-cervical dystonia: Optimal stimulation sites and a convergent pathway.

BACKGROUND: Though deep brain stimulation (DBS) has emerged as a promising treatment for idiopathic cranio-cervical dystonia (iCCD), the best location at which to stimulate remains unclear at a granular level. This study aimed to identify optimal sites and related white matter pathways of globus pallidus internus (GPi) and subthalamic nucleus (STN) for DBS therapy. METHODS: We analyzed a total of 70 iCCD patients treated with bilateral DBS, targeting the STN (n = 40) or GPi (n = 30). A retrospective cohort (n = 48) was utilized for training, while a prospective cohort (n = 22) was used for ou

Cognitive impairment in cervical dystonia: core clinical feature or secondary effect related to non-motor symptoms?

AIM OF THE STUDY: To investigate subtle cognitive dysfunction in patients with cervical dystonia (CD) as a potential independent non-motor feature of the disease or as a consequence of interactions between motor and other non-motor symptoms (NMS). CLINICAL RATIONALE FOR THE STUDY: Cognitive impairment represents one of the most common non-motor symptoms in patients with cervical dystonia. However, the interrelations between cognitive dysfunction, motor symptoms, and other non-motor symptoms remain insufficiently explored. MATERIAL AND METHODS: Patients with CD (n = 34) underwent comprehensiv

The impact of psychological distress on quality of life in isolated cervical dystonia: a case-control study.

BACKGROUND: Isolated cervical dystonia (iCD) is the most common form of focal dystonia and is characterized by sustained or intermittent neck muscle contractions that lead to abnormal postures and pain. These motor symptoms, together with the limited and temporary effects of current treatments, may be associated with psychological distress and reduced quality of life (QoL). OBJECTIVE: To examine the associations between psychological symptoms-specifically anxiety and depression-and QoL in individuals with iCD. METHODS: This cross-sectional study included 47 patients with iCD for ≥ 1.5 years

Predictors of clinically significant underlying etiologies in children presenting with torticollis.

BACKGROUND: Torticollis is a common presentation in paediatric emergency departments (PEDs), usually benign but sometimes the first sign of serious pathology. METHODS: We retrospectively reviewed children <18 years presenting with torticollis to the PED at Tel Aviv Sourasky Medical Center (2018 through 2023). The primary outcome was a clinically significant underlying condition: (1) infectious aetiology, (2) head, neck, or cervical spine tumour or (3) atlantoaxial rotatory fixation (AARF) requiring intervention. RESULTS: Among 352 children (median age 4.8 years; 52% male), 59 (17%) had a cli

Management of neonatal atlantoaxial rotatory subluxation (AARS) using a rigid spinal immobiliser.

We present a very rare case of neonatal atlantoaxial rotatory subluxation (AARS) and its management.A term female neonate born via vacuum-assisted delivery presented with distress immediately after birth. On examination, she had right-sided torticollis and an audible cervical spine 'click'. Imaging confirmed AARS. Spinal precautions were initiated, and she was transferred to a surgical tertiary children's hospital. A rigid cervical collar was applied initially with limited success for immobilisation. Hence, after a multidisciplinary team meeting, it was changed to a rigid spinal immobiliser. M

Delayed presentation of muscular torticollis- an observational study.

INTRODUCTION: Torticollis is a common disorder that may occur in newborns to adults. The most common cause of torticollis is the tightness and shortening of one sternomastoid muscle, also known as congenital muscular torticollis (CMT). This paper presents our experience in surgical management of torticollis, where secondary effects were observed. MATERIALS AND METHODS: This 10-year retrospective observational study included all patients with CMT. The demographic and clinical details of the patients were recorded. The treatment included surgical division of both lower heads of the sternocleido

Chronic Irreducible C1-C2 Rotatory Subluxation Presenting as Painful Torticollis in a Child Managed with Posterior Lateral Mass Fixation: A Case Report.

INTRODUCTION: Atlantoaxial rotatory subluxation (AARS) is an uncommon yet important cause of painful torticollis in children. While most acute cases respond to conservative management, delayed diagnosis may result in fixed and irreducible deformity requiring surgical stabilization. Chronic irreducible cases are rare and pose significant management challenges. CASE REPORT: A 9-year-old girl presented with persistent painful torticollis for 6 weeks following minor trauma. Clinical examination revealed a classic cock-robin posture with severely restricted cervical motion. Computed tomography con

Osteopathic Manipulative Treatment for the Breastfeeding Dyad: A Scoping Review.

BACKGROUND: Human milk is widely recognized as the optimal source of infant nutrition. While 83.9% of mothers initiate breastfeeding, only 46.3% exclusively breastfeed to 3 months. Common barriers include latch and suck difficulties, painful feeding, and breast infections or engorgement. Osteopathic manipulation, including osteopathic manipulative treatment (OMT) and therapy (OMTh), has been proposed as supportive intervention for breastfeeding challenges. RESEARCH AIM: This scoping review examines the current literature on OMT/OMTh in the breastfeeding dyad and identifies gaps to inform futu

Closed reduction in the emergency room following traumatic C1-C2 rotary subluxation: a paediatric perspective.

Atlantoaxial rotatory subluxation (AARS) represents a significant cause of acute torticollis in children involving rotational displacement of C1 on C2. Unlike common idiopathic torticollis, AARS represents a true subluxation that frequently necessitates reduction by an experienced provider to restore alignment. Here, we present the case of a child who developed AARS after falling out of bed. A CT scan confirmed acute C1-C2 rotatory subluxation. The patient was successfully treated with closed reduction under sedation in the emergency department by a paediatric spine surgeon, followed by immobi

Patient Perceptions of a Preventive Effect of Long-Term Botulinum Neurotoxin Therapy in Cervical Dystonia.

Patients with cervical dystonia (CD) often believe that disease severity would have progressed beyond the pre-treatment level if botulinum neurotoxin (BoNT) therapy had not been initiated. The aim of the present study was to assess the perceptions of long-term BoNT-treated patients with CD regarding the expected course of disease severity over the next 10 years under the hypothetical assumption that BoNT therapy is discontinued. Fifty patients with idiopathic CD receiving long-term BoNT therapy were screened, and 43 patients were included. Disease severity at the day of recruitment was assesse

Integrating Osteopathic Care Into Treatment of the Breastfeeding Dyad: Anatomic and Physiologic Considerations.

Care of the breastfeeding dyad involves an interprofessional and multidisciplinary team that may include lactation consultants, physicians, bodyworkers, and more. One often underutilized resource in the care of both breastfeeding parents and their infants is osteopathic physicians (Doctors of Osteopathic Medicine or DOs). DOs are full-scope physicians who have additional training in osteopathic manipulative treatment (OMT). Osteopathic manipulative treatment is a form of manual medicine that can be utilized to optimize the breastfeeding relationship. It can be useful to address latch challenge

Craniofacial Asymmetric Patterns in Congenital Muscular Torticollis: A Computed Tomography-Based Visual Classification and Cluster Analysis With Clinical Implications.

Congenital muscular torticollis (CMT) is a common postural disorder in infants, frequently accompanied by craniofacial asymmetry. Despite its prevalence, detailed characterization of the 3-dimensional (3D) craniofacial morphology in CMT remains limited. This study aimed to explore the craniofacial patterns in children with CMT using 3D computed tomography scan reconstruction and assess their relationship with clinical classifications. Forty-four children aged 5 to 92 months were included in this study. Ten craniofacial parameters were visually assessed by multiple raters, including medical and

Endoscopic surgery in lambdoid-related plagiocephaly: detailed technique, what risks, and long-term results.

Endoscopic suturectomy represents an early intervention for craniosynostosis. For unilateral lambdoid craniosynostosis, this study reviews the existing literatures and summarizes a single-center experience with endoscopic management of lambdoid-related plagiocephaly. Our findings indicate that endoscopic surgery combined with orthotic helmet therapy can effectively improve cranial deformities, torticollis, and other associated abnormalities. However, the correction of compensatory deformities and the long-term outcomes related to Chiari malformation and other neurocognitive functions still req
