Concomitant PLACK Syndrome, MYBPC3-Related Cardiomyopathy, and Nutritional Deficiency.

PLACK syndrome is a rare genodermatosis that features peeling skin, leukonychia, acral punctal keratoses, cheilitis, and knuckle pads. We report a case of a 6-year-old female with PLACK syndrome, attributed to a homozygous variant in CAST, and concurrent dilated cardiomyopathy, explained by a heterozygous variant in MYBPC3. She also had severe protein calorie malnutrition. Although the patient's cardiomyopathy is attributable to her MYBPC3 variant, this case adds to the growing list of reports describing PLACK syndrome with coinciding cardiomyopathy and highlights a unique presentation featuri

Pachydermodactily - the great imitator of arthritis: a case series.

Pachydermodactily is a rare digital fibromatosis of unknown origin, mainly affecting adolescent boys. It presents as symmetrical, painless thickening of the soft tissues, usually around the proximal interphalangeal joints (PIP). Patients often experience delayed diagnoses, receive unnecessary treatments, or are misdiagnosed with chronic inflammatory arthritis. Although the exact cause remains unclear, pachydermodactyly may be associated with repetitive mechanical trauma, such as rubbing or interlacing the fingers, which can lead to secondary skin thickening. Treatment is often not required giv

Severe Dilated Cardiomyopathy with PLACK Syndrome Caused by a Novel Truncating Variant in the CAST Gene.

Background: PLACK syndrome is an ultra-rare autosomal recessive disorder caused by biallelic loss-of-function variants in CAST, which encodes calpastatin, an endogenous inhibitor of calpains. The syndrome is classically defined by peeling skin, leukonychia, acral punctate keratoses, cheilitis, and knuckle pads. Although the phenotype has been largely restricted to dermatological manifestations, emerging reports suggest dilated cardiomyopathy (DCM) as a systemic complication. Methods: We investigated five affected children from three sibships of an extended consanguineous family. Clinical evalu

Peeling Skin, Leukonychia, Acral Punctate Keratoses, Cheilitis and Knuckle Pads (PLACK) Syndrome: An Updated Review of Cases and Identification of a Recurrent CAST Variant in Two Patients.

Peeling skin, leukonychia, acral punctate keratoses, cheilitis, and knuckle pads (PLACK) syndrome (OMIM616295) is an exceptionally rare autosomal recessive genodermatosis caused by loss-of-function pathogenic variants in the CAST gene, encoding calpastatin. A total of 19 cases have been described in 12 articles, among families with unique de novo genetic variants of CAST. We describe two pediatric PLACK cases with a homozygous loss-of-function CAST variant (c.571G>T, p.Gly191Ter) which represents the only recurrent genetic variant of PLACK syndrome reported in the literature.

A new syndromic case of hearing loss and ectodermal anomalies associated with a recurrent missense variation in GJB6 gene.

UNLABELLED: GJB2 and GJB6 variants, encoding Cx26 and Cx30 respectively, are the most frequently involved genes commonly contributing to hereditary hearing loss either isolated or in combination with skin abnormalities. GJB6 variations are classically associated with two distinct conditions: non-syndromic hearing loss and hidrotic ectodermal dysplasia, type Clouston, the latter typically not involving deafness. METHOD: Whole genome sequencing (WGS) was used to find genetic variants after clinical features of a 13-year-old female patient were recorded. RESULTS: In this report, we describe the

CASTing the net wider: A case report of PLACK syndrome associated with dilated cardiomyopathy.

PLACK syndrome (OMIM 616295) is a rare genodermatosis associated with peeling skin, leukonychia, acral punctate keratosis, cheilitis, and knuckle pads and is caused by loss-of-function mutations in the CAST gene, which encodes calpastatin, a calcium-dependent protease. This case report highlights a case of PLACK syndrome presenting with the unique findings of striate hyperkeratosis on the palms as well as life-threatening cardiomyopathy. We review why CAST mutations might impact cardiac function and raise awareness of the potential association between PLACK syndrome and cardiac manifestations.

Dual role of pulmonary surfactant corona in modulating carbon nanotube toxicity and benzo[a]pyrene bioaccessibility.

Inhaled carbon nanotubes (CNTs) can deposit in the deep lung, where they interact with pulmonary surfactant (PS) to form coronas, potentially altering the fate and toxicity profile of CNTs. However, the presence of other contaminants in combination with CNTs may affect these interactions. Here, we used passive dosing and fluorescence-based techniques confirm the partial solubilization of BaPs adsorbed on CNTs by PS in simulated alveolar fluid. MD simulations were performed to elucidate the competition of interactions between BaPs, CNTs, and PS. We found that PS play two opposing roles in alter

Heterogeneous aggregation of carbon and silicon nanoparticles with benzo[a]pyrene modulates their impacts on the pulmonary surfactant film.

Inhaled nanoparticles (NPs) can deposit in alveoli where they interact with the pulmonary surfactant (PS) and potentially induce toxicity. Although nano-bio interactions are influenced by the physicochemical properties of NPs, isolated NPs used in previous studies cannot accurately represent those found in atmosphere. Here we used molecular dynamics simulations to investigate the interplay between two types of NPs associated with benzo[a]pyrene (BaP) at the PS film. Silicon NPs (SiNPs), regardless of aggregation and adsorption, directly penetrated through the PS film with minimal disturbance.

Long-term clinical outcome of collagenase clostridium histolyticum treatment is independent of Dupuytren Diathesis Score.

Collagenase clostridium histolyticum (CCH) is a pharmaceutical, non-surgical treatment option for Dupuytren Disease. However, recurrence is common, and predictors of treatment outcome of CCH treatment are largely unknown. In this retrospective study, we analysed the possible correlation between Abe's Dupuytren Diathesis Score (DDS) and recurrence after treatment with CCH. In a total of 74 patients, with an average follow-up of 5 years, we found an overall recurrence rate of 67% after 5y but no correlation with DDS. Sub-scale analysis indicated that the presence of knuckle pads was associated w

A Rare Case on Capecitabine Induced Acquired Palmoplantar Keratoderma.

Acquired palmoplantar keratoderma (PPK) is a non-hereditary hyperkeratosis of the palms and soles that is caused by various factors, including chemotherapeutic agents. The purpose of this case report is to present a rare case of acquired PPK caused by the chemotherapeutic agent capecitabine. A 54-year-old female complained of painful erythematous plaques on her palms and soles with history of consuming capecitabine. Physical examination revealed scaly erythematous plaques on the palmoplantar surface and knuckle pads on both hands. Histopathological features showed hyperkeratosis, acanthosis, v

Palmoplantar keratoderma, pseudo-ainhum and knuckle pads in an African patient: A case report.

Hereditary palmoplantar keratoderma is a rare heterogenous group of genodermatoses characterised by hyperkeratosis of the palms and soles. Genetic alterations affecting proteins of the keratin cytoskeleton, cornified cell envelope, desmosomes and gap junction proteins have been implicated in the pathogenesis of inherited palmoplantar keratoderma. Reports of palmoplantar keratoderma in the African population are scarce. Herein, we report a case of a 29-year-old HIV-infected African female, who presented to a tertiary hospital with complaints of a painful left fourth toe, secondary to a constric

Nonsense mutations in KRT1 caused recessive epidermolytic palmoplantar keratoderma with knuckle pads.

BACKGROUND: Epidermolytic palmoplantar keratoderma (EPPK) is characterized by diffuse hyperkeratosis affecting palms and soles with suprabasal epidermolysis or vacuolar degeneration histopathologically. The disorder is caused by heterozygous mutations in KRT9 or KRT1. Dominant-negative mutations in KRT1 could also result in epidermolytic ichthyosis with EPPK, a more severe entity affecting the entire body. OBJECTIVE: To investigate the genetic basis and pathogenesis of two unrelated patients with EPPK and knuckle pads, both of whom were born to consanguineous parents of Chinese origin. METHO
