X-linked hypophosphataemia diagnosed in adulthood with coexisting seronegative rheumatoid arthritis: a diagnostic pitfall.

INTRODUCTION: X-linked hypophosphataemia (XLH) is the most common inherited cause of renal phosphate wasting, typically diagnosed in childhood but occasionally identified in adulthood. CASE REPORT: We report a case of a 45-year-old woman with long-standing lower limb deformities, short stature, and diffuse joint pain. Laboratory evaluation revealed hypophosphataemia due to isolated renal phosphate wasting. Fibroblast growth factor 23 levels were markedly elevated. Imaging showed skeletal deformities and enthesopathic changes consistent with chronic XLH. Genetic testing identified a pathogenic

Pediatric and Adult Sinonasal Phosphaturic Mesenchymal Tumors: CDKN2A Copy Number Alterations and Their Association with Recurrence.

PURPOSE: Phosphaturic mesenchymal tumors (PMTs) are exceptionally rare neoplasms that secrete fibroblast growth factor 23 (FGF23), leading to tumor-induced osteomalacia (TIO). Owing to their nonspecific clinical manifestations and deceptively bland morphology, diagnosis is frequently delayed unless there is a high index of clinical suspicion or recognition of characteristic histopathological features. While PMTs predominantly arise in the appendicular skeleton, involvement of the sinonasal tract is uncommon, and pediatric sinonasal PMTs have not been reported previously. Although most PMTs fol

Clinical characteristics of tumor-induced osteomalacia lesions in the neurocranium and intracranial structures.

BACKGROUND: Tumor-induced osteomalacia (TIO) is a rare paraneoplastic syndrome caused by excessive fibroblast growth factor 23 (FGF23) secretion. However, TIO lesions located in the neurocranium (NC) and intracranial (IC) structures remain poorly characterized. OBJECTIVE: To characterize the clinical features of TIO lesions in NC and IC structures and to compare them with those located in the sinuses. METHODS: This retrospective study included patients diagnosed with TIO who underwent tumor resection at Peking Union Medical College Hospital between January 2016 and December 2023. Patients wi

Phosphaturic Mesenchymal Tumor: From Invisibility to Bedridden Morbidity.

Tumor-induced osteomalacia (TIO) is a rare paraneoplastic syndrome caused by fibroblast growth factor 23 (FGF23) secreting phosphaturic mesenchymal tumors. We report a 53-year-old man with progressive osteomalacia resulting in bedridden status. Renal phosphate wasting despite normal vitamin D, intact parathormone, and renal function raised suspicion of TIO, confirmed by elevated FGF23. However, the occult tumor was localized only by Ga-68 DOTANOC wholebody PET-CT, highlighting the diagnostic challenges in TIO.

The guanylate cyclase stimulator vericiguat: Insights and adverse events from a FAERS-based pharmacovigilance study.

BACKGROUND: Since its approval by the U.S. Food and Drug Administration in January 2021, vericiguat, a novel soluble guanylate cyclase stimulator, has been increasingly utilized in the treatment of heart failure with reduced ejection fraction. However, comprehensive long-term safety data on vericiguat in large populations are lacking. OBJECTIVE: This study analyzed the adverse events reported to the Food and Drug Administration (FDA) Adverse Event Reporting System (FAERS) to assess the safety profile of vericiguat. MATERIALS AND METHODS: We selected four algorithms to evaluate the signals of

Fibroblast growth factor 23-induced hypophosphatemia in a malignant phosphaturic mesenchymal tumor: presentation of a rare case.

INTRODUCTION: Tumor-induced osteomalacia is a rare paraneoplastic disorder caused by excess fibroblast growth factor 23 (FGF23), most often produced by phosphaturic mesenchymal tumors. Delayed diagnosis may result in severe metabolic and skeletal complications. CASE PRESENTATION: We report the case of a 32-year-old woman with a three-year history of progressive weakness and a rapidly enlarging thoracic mass. Imaging revealed an 18 × 13 cm highly vascularized thoracic lesion with multiple lytic bone metastases. Laboratory evaluation showed severe hypophosphatemia (0.9 mg/dL), renal phosphate w

Recurrent Pregnancy Loss: Novel Insights and Future Directions.

In the last 15 years, the US and UK have fought two major wars in the Persian Gulf region. Controversy has arisen over the nature and causes of health problems among military veterans of these two wars. Toxic exposures have been hypothesised to cause the majority of the long-term health problems experienced by veterans of the 1991 Gulf War. The assessment of these toxic exposures and the resolution of controversy about their health effects provide a unique case study for understanding how toxicological disputes are settled in the US. Neither clinical examination of ill war veterans nor scienti

Biochemical osteomalacia during pregnancy or lactation: an observational study in Sweden.

BACKGROUND: Studies of osteomalacia during pregnancy and lactation are scarce. This is likely due to the hazards of performing X-ray in pregnancy, the inconvenience of the gold standard diagnostic procedure of bone marrow biopsy with histomorphometry and the lack of these resources in maternity health care. Another important reason is the lack of international consensus on how to diagnose osteomalacia clinically. Osteomalacia needs attention due to the limited number of modern studies on its prevalence and globalization, especially in some populations where migration to high latitudes with poo

Ferric carboxymaltose increases fracture risk in patients and reduces bone formation in mice with iron deficiency anemia.

Modern IV iron formulations allow treatment of iron deficiency anemia (IDA) with 1 or 2 infusions. Ferric carboxymaltose (FCM) is a widely used IV iron, which causes hypophosphatemia in most of the patients. Osteomalacia and fractures are increasingly recognized after repeated infusions of FCM. It is unknown why ferric derisomaltose (FDI) rarely causes hypophosphatemia. In this study, we compare the effects of FCM and FDI on fracture risk and investigate potential underlying mechanisms explaining the different effects on bone and mineral metabolism. For this aim, fracture rate and osteomalacia

Personalized treatment pathways for adult osteomalacia.

Osteomalacia, characterized by impaired mineralization of the bone matrix in adults, arises from various causes, with vitamin D deficiency being the most prevalent. Additional etiologies include genetic disorders of vitamin D metabolism and excessive renal phosphate wasting. If left untreated, osteomalacia can result in bone pain, hypocalcemia, fractures, and skeletal deformities. This review provides an overview of current therapeutic strategies for osteomalacia, encompassing both pharmacological and non-pharmacological treatments, as well as a step-by-step management algorithm. We conducted

Comparison of tWo hospital quality Improvement interventions on inappropriate measurement and SupplEmentation of vitamin D: the WISE-D study.

BACKGROUND: Despite a lack of evidence, measurement and supplementation of vitamin D remain frequent in the general population, leading to significant and potentially avoidable healthcare costs. The aim of this study was to evaluate the impact of two quality improvement interventions (minimal vs. intensive) on inappropriate vitamin D measurement and supplementation in hospitalized patients and compare the two interventions. METHODS: We conducted a pre-post intervention study on general internal medicine wards in Switzerland between 01 July 2016 and 31 December 2023. We compared a minimal inter

Tumor-Induced Osteomalacia Localized to an Abdominal Wall Phosphaturic Mesenchymal Tumor by 68Ga-DOTATATE PET/CT.

A ⁶⁸Ga-DOTATATE PET/CT scan was performed on a 40-year-old woman with suspected tumor-induced osteomalacia (TIO), who presented with progressive bone pain, multiple fractures, and hypophosphatemia. The scan revealed a solitary focus of intense somatostatin receptor avidity within the right lower abdominal wall musculature. No other suspicious lesions were identified. On the basis of this finding, the tumor was surgically resected, and histopathology confirmed a phosphaturic mesenchymal tumor (PMT). Postoperatively, the patient's serum phosphate levels normalized. This case underscores the para

Adefovir dipivoxil-induced hypophosphatemic osteomalacia and osteoporosis: a case report and literature review.

The nephrotoxicity of adefovir dipivoxil (ADV) can induce Fanconi syndrome, which causes hypophosphatemic osteomalacia. We report a case of a 67-year-old postmenopausal woman who had been receiving long-term adefovir dipivoxil therapy for chronic hepatitis B. She presented with severe bone pain and multiple pseudofractures, and was diagnosed with drug-induced hypophosphatemic osteomalacia. Following discontinuation of adefovir and supplementation with vitamin D and phosphate, her biochemical parameters normalized and bone pain significantly improved, indicating that osteomalacia had been essen

X-linked hypophosphatemia and spinal cord compression: a systematic review and illustrative case.

STUDY DESIGN: Systematic review. OBJECTIVE: X-linked hypophosphatemia (XLH) is a rare genetic disorder characterized by impaired phosphate homeostasis due to renal phosphate wasting. It leads to osteomalacia and skeletal abnormalities and represents the most common inherited cause of vitamin D-resistant rickets. In some patients, heterotopic ossification of the ligamentum flavum and paravertebral ligaments may result in spinal cord compression and myelopathy. Due to its rarity, large case series evaluating patient characteristics and surgical outcomes are lacking. METHODS: We conducted a PRI
