Intention-to-Leave Among New Zealand Podiatrists: A Cross-Sectional Survey of Frequency and Associated Factors.

BACKGROUND: Workforce retention poses ongoing challenges for podiatric care provision in Aotearoa New Zealand (NZ); however, intention-to-leave (ITL) from the workforce and its associated factors have not been explored. This study aimed to determine the frequency of ITL among NZ podiatrists and identify factors associated with leaving intentions. METHODS: A cross-sectional online survey of NZ-registered practising podiatrists was conducted. Demographics, employment, workplace and attitudinal measures were collected as independent variables. ITL was assessed using (1) a self-reported single-it

Causal fracture from biomarker to clinical performance: A critical reappraisal of the 2D:4D ratio in surgical skills and the developmental-stress-training interaction model.

The second-to-fourth digit ratio (2D:4D) is a sexually dimorphic morphological trait widely used as a retrospective biomarker of prenatal androgen exposure. This critical review reappraises its causal validity as a predictor of complex behavioral phenotypes, using surgical technical performance as a test case, and proposes a Developmental-Stress-Training (DST) interaction model. Each inferential link-prenatal androgen exposure to the 2D:4D phenotype, the phenotype to central motor and visuospatial development, and neurodevelopmental differences to surgical technical performance-was examined fo

Divergent K-wires in the sagittal plane have minimal impact on fixation stability and the resulting early clinical outcomes in patients with transverse patellar fractures.

OBJECTIVE: Transverse patellar fractures are typically treated with tension band wiring using Kirschner wires. This technique converts anterior tensile forces into articular compressive forces, providing the stability essential for early fracture healing and weight-bearing. Fixation configuration significantly influences stability; while divergent constructs offer superior angular stability in other fractures, AO principles recommend parallel wires to permit dynamic fragment compression. Although a divergent angle is often observed on lateral radiographs, its impact on early healing and weight

An integrated experimental-computational framework for elasto-plastic characterization and spatial mapping of canine bone properties.

Bone is a hierarchical and heterogeneous bio-composite material, with cortical and trabecular regions exhibiting distinct mechanical behaviors across scales. Patient-specific Finite Element Analysis (FEA) derived from medical images is increasingly used in both human and veterinary medicine to predict bone mechanical behavior under various loading conditions, assess fracture risk, and evaluate treatment outcomes. However, due to the complexity associated with measuring full elasto-plastic bone properties across multiple scales and bridging microscale properties to macroscale models, simplified

Aqueous extract of Ormocarpum cochinchinense promotes fracture healing through coordinated angiogenic, osteogenic, and cytoprotective mechanisms.

Ormocarpum cochinchinense has long been used in traditional medicine for the management of bone fractures. Despite its widespread ethnomedicinal use, scientific evidence supporting its efficacy and underlying mechanisms in fracture healing remains limited. This study aimed to investigate the safety and fracture-healing potential of the aqueous extract of O. cochinchinense (OCE) with emphasis on angiogenesis and osteogenesis, and to elucidate its role in modulating endothelial cell inflammation. The biological activity of OCE was assessed using a combination of in vitro and in vivo approaches.

Factors associated with outcomes after reverse total shoulder arthroplasty at midterm follow-up.

BACKGROUND: Reverse total shoulder arthroplasty has become the preferred treatment for many patients with irreparable cuff tears, osteoarthritis, and complex proximal humeral fractures. While outcomes are generally favorable, the influence of demographic and surgical factors-such as age, sex, previous surgery, and dominance of the operated arm-on post-operative results remains incompletely understood. METHODS: After institutional review board approval, a retrospective review of prospectively collected data was conducted for all primary reverse total shoulder arthroplasties performed by a sing

Medial patellofemoral ligament reconstruction in osteogenesis imperfecta using quadriceps tendon graft: a rare case report.

Osteogenesis imperfecta (OI) is a rare connective tissue disorder associated with bone fragility and ligamentous laxity, predisposing patients to recurrent patellar instability. Surgical treatment of recurrent patellar instability is challenging because standard medial patellofemoral ligament (MPFL) reconstruction techniques involving patellar tunneling may increase the risk of patellar fracture. We present a 15-year-old male patient with OI type V and recurrent patellar dislocation treated with a minimally invasive MPFL reconstruction using a pedicled quadriceps tendon graft. This patella-spa

Biomechanical and finite element analysis of a novel annular multi-axial locking plate for patellar fracture fixation.

BACKGROUND: Traditional tension band structures for patellar fractures have multiple complications such as wires lossening and soft tissue irritation. In response to the above problems, a new type of annular multi-axis locking plate has been designed for the treatment of patellar fractures. This study aims to compare the biomechanical performance of two methods for stabilizing patellar fractures. METHODS: A three-dimensional finite element model was established to evaluate the biomechanical performance of annular multi-axis locking steel plate fixation for patellar fractures at knee flexion a

Early mobilization after reverse total shoulder arthroplasty is not associated with increased risk of acromial and scapular stress fractures: a retrospective cohort study.

BACKGROUND: Acromial and scapular spine stress fractures (ASFs) are recognized complications following reverse total shoulder arthroplasty (rTSA) and may negatively affect clinical outcomes. The influence of post-operative immobilization duration and timing of rehabilitation on ASF risk remains unclear. This study evaluated whether a shorter post-operative sling immobilization protocol was associated with an increased incidence of ASF following primary rTSA. METHODS: A retrospective single-center comparative cohort study was performed including 698 patients who underwent primary rTSA between

GPX4-Mediated Inhibition of Ferroptosis by Sodium Selenite Ameliorates Bone Metabolic Disorders in Ovariectomized Mice.

Postmenopausal osteoporosis (PMOP) and its associated fracture complications have emerged as pressing public health challenges due to rapidly accelerating global population ageing. The functional decline of bone marrow mesenchymal stem cells (BMSCs) under elevated oxidative stress is a recognised pathogenic cornerstone of PMOP, positioning redox imbalance as a critical therapeutic node. Here, we establish that sodium selenite (Na2SeO3) robustly counters oxidative insults in BMSCs, thereby restoring and enhancing their osteogenic lineage commitment. Furthermore, RNA sequencing analysis indicate

Betaine alleviates renal osteodystrophy by stabilizing ABCG1 mRNA to inhibit HMOX1-mediated ferroptosis in BMSCs.

Renal osteodystrophy is a debilitating complication of chronic kidney disease characterized by deteriorated bone microarchitecture and impaired bone formation, leading to increased fracture risk. Despite the pressing need for anabolic therapies, the cellular mechanisms by which uremic stress compromises osteoprogenitor function remain poorly defined. Here, metabolomic profiling of renal osteodystrophy patient cohorts revealed a profound depletion of circulating betaine. We show that betaine supplementation not only rescued the osteogenic differentiation of bone marrow-derived mesenchymal stem

Investigating the Interacting Mechanisms of Precast Intersecting Cracks in Concrete: Insights from the Simulations Based on Discrete Element Method.

Underground mines often use shotcrete and reinforced concrete as support. Excavation disturbance, in situ stress, and blasting cause initial fractures to propagate. Intersecting fractures are common. This paper studies how intersecting fracture geometry affects fracture propagation under uniaxial compression. DEM and Particle Flow Code (PFC2D, Particle Flow Code) are used to build mesoscopic models with single-intersecting fractures (30-150°) and double-intersecting fractures with five offset levels: Zero, Minor, Moderate, Substantial, and Maximum. For single-intersecting fractures, smaller an

Biochemical and clinical changes during enzymatic replacement therapy in hypophosphatasia: three real-world cases in Spain.

Hypophosphatasia (HPP) is a rare recessive or dominant autosomal disease caused by loss-of-function variants in the ALPL gene, which encodes tissue-nonspecific alkaline phosphatase (TNSALP). Molecules involved in the pathogenesis of HPP are alkaline phosphatase (ALP), inorganic pyrophosphate (PPi; an inhibitor of bone mineralization), pyridoxal-5'-phosphate (PLP; responsible for the neurological complications) and phosphoethanolamine. Asfotase alfa is a recombinant, bone-targeted, human TNSALP approved to treat patients with HPP. Its clinical effectiveness has been demonstrated in real-world s

Influence of thermodisinfection on the mechanical properties of individual human trabeculae from femoral heads.

Allogeneic bone grafts are frequently used in reconstructive orthopaedics and trauma surgery and require appropriate processing to ensure biological safety. Thermodisinfection represents an established procedure; however, its influence on the mechanical properties of individual trabeculae has not yet been sufficiently investigated. Single trabeculae were prepared from human femoral heads and assigned within a paired study design to either native or thermodisinfected conditions. Mechanical characterisation was performed using a two-point bending test. The elastic modulus (E) was determined from

Mesenchymal Stromal Cell Spheroids as an Emerging Regenerative Strategy for Osteonecrosis of the Femoral Head.

Osteonecrosis of the femoral head (ONFH) is a progressive ischemic bone disorder that may lead to subchondral fracture, femoral-head collapse, and total hip arthroplasty. Impaired regeneration reflects persistent ischemia, endothelial dysfunction, defective angiogenesis and bone remodeling, inflammation, oxidative stress, and dysfunction of endogenous mesenchymal stromal cells (MSCs). Although MSC therapy, particularly with core decompression in precollapse ONFH, is a promising joint-preserving approach, conventional two-dimensional (2D)-expanded MSCs are limited by poor survival and retention
