Metagenomic next-generation sequencing in tuberculous periprosthetic joint infection: pathogen-detection yield and clinical characteristics compared with common bacterial PJI.

OBJECTIVE: To compare the pathogen-detection yield of metagenomic next-generation sequencing (mNGS) and conventional culture in tuberculous periprosthetic joint infection (TB-PJI) and common bacterial PJI (CB-PJI), and to characterize clinical differences and exploratory associations with TB-PJI. METHODS: A retrospective cohort included 197 patients with periprosthetic joint infection treated between January 2016 and June 2024 who underwent both conventional culture and mNGS during the same diagnostic or surgical episode. Patients were classified as CB-PJI (n=178) or TB-PJI (n=19). Paired pat

Age-dependent immune heterogeneity and the limited diagnostic utility of peripheral blood biomarkers in pediatric tuberculosis: a real-world cohort study with multidimensional model evaluation.

BACKGROUND: Pediatric tuberculosis (TB) remains a diagnostic challenge due to atypical presentations and low pathogen detection rates. Overlapping clinical features with Mycoplasma pneumoniae (MP) infection further complicate differential diagnosis. Immune-inflammatory parameters are age-dependent, yet how age modifies these profiles in children with TB and MP co-infection remains poorly understood. OBJECTIVE: To investigate age-dependent heterogeneity in immune-inflammatory profiles among children with tuberculosis (TB) and to evaluate the additive diagnostic value of peripheral blood biomar

Primary gastric tuberculosis mimicking gastric lymphoma: a case report and multimodality imaging findings.

Gastric tuberculosis represents a rare form of extrapulmonary tuberculosis. Owing to its non-specific clinical presentation, the condition is highly prone to misdiagnosis. It can mimic peptic ulcer disease or malignancy, typically manifesting as ulcerative lesions or gastric outlet obstruction, whereas less frequent presentations include massive gastrointestinal haemorrhage and gastric perforation. Consequently, establishing a definitive diagnosis remains challenging. This case report aims to augment the current understanding of this entity. Herein, we describe a 50-year-old female who present

Case Report: Distal ureteral obstruction decades after presumed intrauterine device removal in a woman with uterus didelphys.

BACKGROUND: An ectopic intrauterine device (IUD) can rarely cause ureteral obstruction and hydroureteronephrosis. Diagnosis may be delayed when a prior removal procedure is believed to have been complete. CASE PRESENTATION: A 48-year-old woman presented with a 1-year history of right flank pain and urinary frequency after repeated treatment for presumed urinary tract infection and possible genitourinary tuberculosis without sustained improvement. More than 20 years earlier, an IUD had been inserted before uterus didelphys was recognized. Two to three years later, an unintended pregnancy led t

Pitfalls in the Diagnosis and Classification of Crohn's Disease: Misdiagnosis, Overdiagnosis, and Misclassification.

Crohn's disease (CD) remains challenging to diagnose and classify accurately despite advances in endoscopy, cross-sectional imaging, intestinal ultrasound, and inflammatory biomarkers. Earlier diagnosis and treat-to-target strategies have improved outcomes but have also increased the risk of misdiagnosis, overdiagnosis, and misclassification of disease activity or severity. Misdiagnosis may expose patients without CD to unnecessary immunosuppressive therapy while delaying recognition and treatment of the true underlying condition, including drug-induced enteropathy, bile acid malabsorption, ir

Azole chroman-4-one, tetralone and chromen-4-one derivatives: synthesis, Mycobacterium tuberculosis CYP121A1 enzyme inhibition and antimycobacterial activity.

Tuberculosis (TB) remains a major global health challenge, driven by the emergence of drug-resistant Mycobacterium tuberculosis (Mtb) strains and the limitations of existing treatment regimens. Twenty CYP enzymes unique to Mtb represent an underexploited class of targets, offering opportunities for selective inhibition. Among these, CYP121A1 enzyme has been identified as a promising target. Three series of azole derivatives employing either a chroman-4-one, tetralone or chromen-4-one pharmacophore were synthesised and evaluated against Mtb CYP121A1 (binding affinity, KD) and against wild type

Sequential Pleural Fluid and Serum Biomarker Measurements in Patients Undergoing Medical Thoracoscopy for Persistently Undiagnosed Pleural Effusion: A Pilot Study from a TB-Endemic Tertiary Centre.

INTRODUCTION: Pleural effusion remains diagnostically challenging when standard thoracentesis does not establish an aetiology. This pilot analysis described sequential biomarker measurements in patients proceeding to medical thoracoscopy and explored their associations and discrimination across final diagnostic classifications. METHODS: Prospectively collected data from a single-centre tertiary referral cohort were analysed. Pleural-fluid adenosine deaminase (ADA) and serum complement components 3 and 4 (C3 and C4) were obtained on day 1, whereas pleural-fluid cancer antigen 125 (CA125) and l

Beware the Muscle: Silent Exercise-Induced Aminotransferase Elevation as a Diagnostic Pitfall in Hepatology and Pretreatment Screening-A Report of Two Cases.

BACKGROUND: In clinical hepatology practice, pretreatment liver enzyme screening plays a key role in making management decisions, such as initiation of antituberculosis therapy or surveillance of chronic hepatitis B. Unrecognized extrahepatic sources of aminotransferase elevations can lead to misdiagnosis, inappropriate treatment alteration, and unnecessary invasive or costly investigations. CASE PRESENTATION: We present two cases of active young males who were undergoing liver biochemical testing for therapeutic decision-making for other underlying illnesses. Patient one was undergoing basel

Two Case Reports of Drug-Resistant Tuberculous Pleurisy in Children: Thoracoscopic Pleural Biopsy and PCR-Based Drug-Resistance Testing.

BACKGROUND: Drug-resistant tuberculous pleurisy is difficult to diagnose in children because pleural fluid is paucibacillary, culture is slow and frequently negative, and Xpert detects only rifampicin resistance. CASE PRESENTATION: We report two children in whom thoracoscopic pleural biopsy combined with PCR-high-resolution melting (HRM) drug-resistance testing identified drug-resistant tuberculosis. Case 1, an 8-year-old girl with tuberculous pleurisy, showed persistent pleural thickening and a new mass after two months of standard first-line therapy; pleural tissue PCR detected rpoB and inh

Physician-modified fenestrated thoracic endovascular aortic repair for a rapidly developing infectious distal aortic arch aneurysm after intravesical bacillus Calmette-Guérin therapy.

An 84-year-old man developed hemoptysis 3 weeks after receiving intravesical bacillus Calmette-Guérin therapy for bladder cancer. Computed tomography revealed a new saccular distal aortic arch aneurysm involving the origin of the left subclavian artery, with periaortic inflammation. Computed tomography performed 3 months earlier had shown no aneurysm. Urine polymerase chain reaction testing was positive for the Mycobacterium tuberculosis complex, supporting the diagnosis of a bacillus Calmette-Guérin-related infectious aortic aneurysm. Given the prohibitive surgical risk, emergency endovascula

Development and preliminary validation of a CD161 immunoscore-based screening method for latent tuberculosis infection in a Chinese training cohort.

Current latent tuberculosis infection (LTBI) screening is limited by tuberculin skin test (TST) cross-reactivity, return-visit requirements, and interferon-gamma release assay (IGRA) costs. We developed a flow cytometry and deep learning CD161 immunoscore screening method, evaluating it across two phases (N = 269) in Jiangxi, China. Phase 1 (n = 110; 92 males, 18 females) used healthy controls (n = 88) and clinically cured, post-treatment tuberculosis (TB) patients (n = 22) for model development. This post-treatment cohort served as a proxy and was not equivalent to untreated LTBI. Phase 2 (n

CT-Based Clinical-Radiomics for Differentiating Talaromyces marneffei Spondylitis From Tuberculous Spondylitis Before Pathogen Confirmation: A Multicentre Diagnostic Study in an Endemic Region.

BACKGROUND: Talaromyces marneffei spondylitis (TMS) is an uncommon but clinically important form of talaromycosis in endemic regions. Where talaromycosis and tuberculosis overlap, TMS may be worked up as tuberculous spondylitis (TBS), delaying fungal investigations and antifungal treatment. OBJECTIVES: To assess whether pretreatment CT, routine clinical variables and radiomics features can help differentiate TMS from TBS at the initial assessment stage. PATIENTS/METHODS: This retrospective multicentre study included 263 patients meeting the prespecified composite reference standard (83 TMS a

Modulation of anti-tuberculosis drug-induced liver injury by gut microbiota metabolites: Insights from network pharmacology.

BACKGROUND: Effective treatment of tuberculosis is often hampered by anti-tuberculosis drug-induced liver injury (ATB-DILI). This study aimed to identify the key targets and mechanisms by which gut microbiota metabolites regulate ATB-DILI. METHODS: Integrative bioinformatics was used to screen for common targets shared by gut microbiota metabolites and ATB-DILI. The target list was refined through protein-protein interaction network and hub gene analyses. Functional enrichment analysis was performed on the core targets, followed by construction of a microbe-metabolite-target regulatory networ

UShER-TB: scalable, comprehensive, accessible phylogenomic analysis of Mycobacterium tuberculosis.

Background. Mycobacterium tuberculosis, the bacterium responsible for tuberculosis, remains a leading cause of global infectious disease deaths. While genomic epidemiology is essential for understanding transmission dynamics and the rise of antibiotic resistance, significant computational limitations currently prevent comprehensive phylogenetic analysis of the vast number of publicly available M. tuberculosis genomes.Methods. We developed UShER-TB, a comprehensive collection of community resources, including a computational analysis pipeline designed for scalable phylogenomic analysis of M. tu

Voices from the front line: Community health workers' perspectives on effectiveness and needs in Madagascar's tuberculosis response.

Community health workers (CHWs) are central to primary health care delivery in Madagascar, yet their perspectives on what enables effective work are rarely centered in program design and implementation. This study aims to examine the lived experiences of Malagasy CHWs engaged in tuberculosis (TB) program activities and to develop context-specific recommendations to strengthen their effectiveness in Madagascar's TB response. We conducted a qualitative study nested within the formative phase of a participatory action research initiative aimed at strengthening Madagascar's community health workfo

Alignment-Free Tool for Assemblies and Reads in Genomic Surveillance of Tuberculosis.

Whole-genome sequencing strengthens tuberculosis surveillance, but incomplete lineage schemes can misclassify isolates. We developed Pathotypr, an alignment-free tool for assemblies and reads, using 2,145 markers derived from 26,813 genomes. Across 88,071 samples, Pathotypr improved concordance, expanded uncommon lineage coverage, and detected resistance markers, supporting molecular epidemiology and public health surveillance.
