CD8+ T cells retain protective functions despite sustained inhibitory receptor expression during Epstein-Barr virus infection in vivo.

Epstein Barr virus (EBV) is one of the most ubiquitous human pathogens in the world, persistently infecting more than 90% of the adult human population. It drives some of the strongest human CD8+ T cell responses, which can be observed during symptomatic primary infection known as infectious mononucleosis (IM). Despite high viral loads and prolonged CD8+ T cell stimulation during IM, EBV enters latency and is under lifelong immune control in most individuals that experience this disease. We investigated whether changes in T cell function, as frequently characterized by PD-1 up-regulation, occu

Chronic Active Epstein-Barr Virus Infection With Systemic Vasculitis and Pulmonary Arterial Hypertension in a Child.

Introduction: A chronic active Epstein-Barr virus (EBV) infection (CAEBV), which is characterized by persistent "infectious mononucleosis-like" symptoms, can lead to cardiovascular complications, including coronary artery aneurysms. No published studies have reported an occurrence of chronic EB virus infection in conjunction with systemic vasculitis and pulmonary hypertension. Case Presentation: Herein, we present a case of a 9-year-old boy with CAEBV, associated with pulmonary arterial hypertension (PAH) and systemic vasculitis. Recurrent skin ulcers were a major early clinical manifestation

Viral DNA Loads in Various Blood Components of Patients With Epstein-Barr Virus-Positive T-Cell/Natural Killer Cell Lymphoproliferative Diseases.

To evaluate diagnostic values for Epstein-Barr virus (EBV) DNA loads in different blood components of patients with EBV-positive T-cell/natural killer cell lymphoproliferative diseases, EBV DNA loads were compared among disease categories in each blood component from 59 patients. Plasma viral loads were significantly higher in "active" disease in chronic active EBV infection. EBV DNA was not detected in the plasma from 7 patients in whom EBV DNA was detected in peripheral blood mononuclear cells and whole blood. Diagnostic cutoff values for whole blood EBV DNA loads of patients with chronic ac

Modelling the dynamics of EBV transmission to inform a vaccine target product profile and future vaccination strategy.

Epstein-Barr virus (EBV) is one of the most common human viruses and the cause of pathologies such as infectious mononucleosis (IM) and certain cancers. No vaccine against EBV infection currently exists, but such vaccines are in development. Knowledge of how EBV is transmitted at the population level is critical to the development of target product profiles (TPPs) for such vaccines and future vaccination strategies. We present the first mathematical model of EBV transmission, parameterised using data from England, and use it to compare hypothetical prophylactic vaccines with different characte

Epstein-Barr virus infection with acute acalculous cholecystitis in previously healthy children.

BACKGROUND: Acute acalculous cholecystitis (AAC), an inflammatory process of the gallbladder (GB) in the absence of gallstones, typically occurs in seriously ill patients. AAC can complicate primary Epstein-Barr virus (EBV) infection, but it is an atypical clinical presentation. AIM: The aim of our study was to analyse AAC occurrence in children with primary symptomatic EBV infection who had been admitted to the hospital. METHODS: We retrospectively evaluated the medical documentation of 181 children with EBV infection who were diagnosed based on the presence of viral capsid antigen IgM anti

Primary EBV Infection Induces an Acute Wave of Activated Antigen-Specific Cytotoxic CD4+ T Cells.

CD4+ T cells are essential for immune protection against viruses, yet their multiple roles remain ill-defined at the single-cell level in humans. Using HLA class II tetramers, we studied the functional properties and clonotypic architecture of EBV-specific CD4+ T cells in patients with infectious mononucleosis, a symptomatic manifestation of primary EBV infection, and in long-term healthy carriers of EBV. We found that primary infection elicited oligoclonal expansions of TH1-like EBV-specific CD4+ T cells armed with cytotoxic proteins that responded immediately ex vivo to challenge with EBV-in

[Spontaneous rupture of the spleen in a child].

Non traumatic or spontaneous splenic ruptures are rare but potentially fatal. Mortality is mainly due to delayed and therapeutic diagnosis as well as to the risks associated with a predisposed condition and with the severity of underlying pathologies. Splenectomy is necessary in the majority of cases. They can occur either in subject with macroscopically healthy spleen but, for instance, with infectious mononucleosis (IMN) or malaria or in subjects with pathologic spleen due to tumor, for example, but even in patients with some coagulopathies. We here report the case of a 6 year old child foll

Methylation analysis of plasma DNA informs etiologies of Epstein-Barr virus-associated diseases.

Epstein-Barr virus (EBV) is associated with a number of diseases, including malignancies. Currently, it is not known whether patients with different EBV-associated diseases have different methylation profiles of circulating EBV DNA. Through whole-genome methylation analysis of plasma samples from patients with nasopharyngeal carcinoma (NPC), EBV-associated lymphoma and infectious mononucleosis, we demonstrate that EBV DNA methylation profiles exhibit a disease-associated pattern. This observation implies a significant potential for the development of methylation analysis of plasma EBV DNA for

Infectious mononucleosis, active component, U.S. Armed Forces, 2002-2018.

Infectious mononucleosis (IM) is an acute infectious illness characterized by swollen lymph nodes, fever, pharyngitis, fatigue, and head and body aches. This report describes the incidence rates, trends, and demographic correlates of IM among active component service members during 2002-2018. During the surveillance period, there were 23,780 incident cases of IM, resulting in an overall incidence rate of 104.2 cases per 100,000 person-years (p-yrs). The incidence of IM diagnoses was highest among the youngest age groups and decreased with increasing age. The rate of incident IM diagnoses was m

EBV-Associated Lymphoproliferative Disorders: Update in Classification.

Although about 90% of the world's population is infected by EBV only a small subset of the related infections result in neoplastic transformation. EBV is a versatile oncogenic agent involved in a multitude of hematopoietic, epithelial, and mesenchymal neoplasms, but the precise role of EBV in the pathogenesis of many of the associated lymphoid/histiocytic proliferations remains hypothetical or not completely understood. Additional studies and use of evolving technologies such as high-throughput next-generation sequencing may help address this knowledge gap and may lead to enhanced diagnostic a

[A boy with a spontaneous rupture of the spleen; a complication of infectious mononucleosis].

BACKGROUND: Spontaneous rupture of the spleen occurs in 0.1-0.2% of patients with infectious mononucleosis and is associated with a high mortality (9-13%). Rupture is not normally preceded by a trauma. CASE DESCRIPTION: A previously healthy 17- year-old boy presented at the Emergency Department with acute onset of pain in his left upper abdomen and left shoulder. Initially he showed no signs of shock, but he did have signs of an acute abdomen. In addition, spots of pus were seen on both pharyngeal arches. Urgent ultrasound showed free fluid in his abdomen, and CT scan revealed active bleeding

[Acute HIV infection pathologically mimicking T-cell lymphoma].

A 49-year-old man was hospitalized with infectious mononucleosis-like symptoms, hepatosplenomegaly, and circulating abnormal lymphocytes. Hepatosplenic T-cell lymphoma was suspected, and bone marrow aspiration and liver biopsy were performed. The pathological diagnosis was T-cell lymphoma. When explaining malignant lymphoma to the patient, he disclosed his Men Who Have Sex with Men (MSM) status. HIV RNA test showed a markedly high viral load, and TCR gene rearrangement was negative. In view of these results, the patient was diagnosed with acute HIV infection. The previous lack of information c

Higher fish consumption and lower risk of central nervous system demyelination.

BACKGROUND/OBJECTIVES: The evidence for diet as a risk factor for multiple sclerosis (MS) is inconclusive. We examined the associations between fish consumption and risk of a first clinical diagnosis of central nervous system demyelination (FCD), a common precursor to MS. METHODS: The 2003-2006 Ausimmune Study was a case-control study examining environmental risk factors for FCD, with participants recruited from four regions of Australia and matched on age, sex, and study region. Dietary intake data were collected using a food frequency questionnaire. We used conditional logistic regression m

Atomic structure of the Epstein-Barr virus portal.

Herpesviridae is a vast family of enveloped DNA viruses that includes eight distinct human pathogens, responsible for diseases that range from almost asymptomatic to severe and life-threatening. Epstein-Barr virus infects B-cells and epithelial cells, causing infectious mononucleosis, as well as a number of cancers. Epstein-Barr infection cannot be cured since neither vaccine nor antiviral drug treatments are available. All herpesviruses contain a linear double-stranded DNA genome, enclosed within an icosahedral capsid. Viral portal protein plays a key role in the procapsid assembly and DNA pa

Infectious Mononucleosis Management in Athletes.

Infectious mononucleosis is a common condition occurring in athletic training rooms. Most cases are due to Epstein-Barr virus infections (upward of 90%). Although treatment generally consists of symptomatic care, there is clinical variation in laboratory workup leading to diagnosis and in the method of return to play decision making. The authors suggest a systematic approach to laboratory evaluation and return to play decisions to minimize clinical variation. The most feared complication of infectious mononucleosis is potential splenic rupture. There have been several examples of the successfu
