Characteristics and Outcomes Associated with the Attempt of External Cephalic Version in U.S. Birth Certificate Data.

OBJECTIVES: Malpresentation is the indication for approximately 17% of primary cesarean deliveries (CD). External cephalic version (ECV) is a safe and effective procedure to reduce CD. Our primary objective is to identify associations between patient demographics and rate of ECV attempt. Secondary objectives include determining the rate of ECV attempt in the United States, and assessing neonatal and maternal outcomes associated with ECV attempt. STUDY DESIGN: Data were obtained from the National Center for Health Statistics birth certificate data from 2014 to 2021. We analyzed live, singleton

[Comparison of Methods for Cervical Ripening for Labor Induction for Fetal Indication: Mechanical vs. Pharmacological].

INTRODUCTION: The optimal method for cervical ripening during labor remains debated. AIMS: To determine the effective cervical ripening method based on fetal indications. METHODS: A retrospective cohort study was conducted at a single tertiary center (2010-2021). Enrolled nulliparous women had a Bishop score of ≤6 and vertex presentation undergoing labor induction for fetal well-being indications (including non-reassuring fetal heart rate). Participants were categorized based on the ripening method used: prostaglandin E2 (PGE2) or cervical ripening balloon (CRB). RESULTS: The study included

Oral calcium carbonate as an adjunct to oxytocin infusion for labor induction: a randomized controlled pilot study on feasibility and acceptability.

BACKGROUND: Calcium carbonate supplementation may offer a simple, low-cost adjunctive benefit to synthetic oxytocin that could enhance the effectiveness of medical labor induction, reduce associated risks, and improve outcomes for both mother and baby. However, its potential role as an adjunct uterotonic remains understudied. OBJECTIVE: This study aimed to assess the feasibility and acceptability of conducting a large randomized controlled trial evaluating the effect of coadministering oral calcium carbonate with synthetic oxytocin infusion during labor induction on labor and delivery outcome

Trends in risk factors associated with the increasing labor induction rate: A nationwide register study in Finland 2005-2023.

INTRODUCTION: Induction of labor (IOL) rates have been increasing in high-income countries. Although IOL aims to reduce maternal and neonatal morbidity, the benefits for many common indications remain uncertain. Because IOL may also have disadvantages and place a burden on the health-care system, it is important to clarify the factors underlying the rising national IOL rates. MATERIAL AND METHODS: This descriptive study included all singleton births in cephalic presentation delivered at 37+0 gestational weeks or later between January 1, 2005, and December 31, 2023, in Finland. Data from the F

Manual rotation and maternal positioning for managing fetal occiput posterior position during labor: a network meta-analysis.

BACKGROUND: The occiput posterior (OP) foetal position is a global obstetric challenge associated with increased maternal and neonatal complications. Manual rotation and specific maternal positioning (hands-and-knees, lateral position) are potential interventions for managing persistent OP position during labour; however, their comparative effectiveness remains uncertain. This systematic review and network meta-analysis aimed to evaluate the efficacy of these interventions versus standard care. METHODS: A systematic search of multiple databases was conducted from inception to January 3, 2025

Beyond HELLP: An Unusual Cause of Severe Thrombocytopenia in Pregnancy.

Thrombotic thrombocytopenic purpura (TTP) is a rare yet potentially fatal thrombotic microangiopathy that poses significant risks during pregnancy. It is characterized by microangiopathic hemolytic anemia, profound thrombocytopenia, and widespread microvascular thrombosis. We present a case of pregnancy-associated TTP presenting with isolated severe thrombocytopenia, highlighting the diagnostic challenge. A 27-year-old, G3P1 woman at 31+6 weeks of gestation presented with hypogastric pain and a resolved episode of vomiting. Laboratory evaluation revealed severe thrombocytopenia (platelets: 9 ×

Case control study on determinants of emergency peripartum hysterectomy among mothers managed at Hawassa University Comprehensive Specialized Hospital.

BACKGROUND: Peripartum hysterectomy is the removal of the uterus during a cesarean section, after vaginal delivery, or during puerperium. It is usually performed as a life-saving emergency during abnormal placentation, hemorrhage, or uterine rupture. It has a substantial role in the prevention of maternal mortality and morbidity. This study aimed to identify the determinants of emergency peripartum hysterectomy at Hawassa University Comprehensive Specialized Hospital, Ethiopia. METHODS: A hospital-based, unmatched case-control study was conducted among 66 cases and 198 controls with a case-to-

Effect of spinning babies® on labor progression and cesarean delivery rates: protocol for an open-label, two-arm, parallel randomized controlled trial.

BACKGROUND: Cesarean section rates remain high worldwide, especially in Brazil, where they far exceed international recommendations. Arrest of labor progression is one of the main indications for intrapartum cesarean delivery and is frequently related to challenges in fetal descent, rotation, and positioning within the maternal pelvis. Maternal movement and positioning during labor are recognized strategies to support physiological labor progression. The Spinning Babies's® approach is a non-invasive method based on intentional movements and positioning, developed to facilitate fetal rotation a

Development of a machine learning model to identify the predictors of the neonatal intensive care unit admission.

Scientists aim to create a system that can predict the likelihood of newborns being admitted to the neonatal intensive care unit (NICU) by combining various statistical methods. This prediction could potentially reduce the negative health outcomes, deaths, and medical costs associated with NICU stays by detecting potential cases early on. This study utilized a retrospective cohort design. The primary outcome of the research focused on admissions to the NICU. The real-time data of pregnant women with a cephalic presentation who gave birth between January 2020 and December 2022 were extracted fr

Sonography only, few internal examinations (SOFIE): Birth progress in normal deliveries followed with intrapartum ultrasound.

OBJECTIVE: Intrapartum transperineal ultrasound (ITU) is objective and more reproducible than clinical palpation. We developed and tested a protocol to follow normal, low-risk laboring women. The aim was to provide safe and minimally or non-invasive intrapartum care based primarily on ITU and fetal heart rate tracing, with as little vaginal digital palpation as possible. In a pilot study, we evaluated the feasibility of "Sonography Only, Few Internal Exams" (SOFIE) in a university maternity unit. Immediate or early skin-to-skin contact for mothers and their healthy newborn infants.

RATIONALE: Research supports the beneficial effects of immediate maternal-infant skin-to-skin contact (SSC) after all modes of birth on breastfeeding/lactation and neonatal physiology, but little is known about how it might influence maternal physiology, including postpartum blood loss and placental separation time. Despite the findings from the 2016 Cochrane review of skin-to-skin contact, and although the World Health Organization (WHO) and the United Nations Children's Fund (UNICEF) recommend immediate, continuous, uninterrupted SSC after birth, newborn infants are still separated from thei

The Optimal Method for Labor Induction in Macrosomic Fetuses: Mechanical or Medical?

BACKGROUND: Fetal macrosomia is a risk factor for operative vaginal delivery (VD), shoulder dystocia, obstructed labor, and cesarean section (CS). Induction of labor (IOL) may decrease these risks but also leads to longer labor, increasing the risk of CS. No data exist regarding the optimal method of IOL in macrosomic fetuses, and most studies are limited to the efficacy of medical induction. OBJECTIVES: To compare medical and mechanical IOL in macrocosmic fetuses. METHODS: This retrospective case-control study included pregnant women who underwent IOL and delivered macrosomic neonates at a

Newborn Penile Trauma in Delayed Breech Presentation: A Case Study.

Breech presentation occurs in a small number of term pregnancies and can result in increased neonatal morbidity. Genital trauma, although uncommon, is a possible complication of footling breech presentation, especially when maternal presentation for cesarean section is delayed. One such case is that of a suspected 35-week gestational-age male neonate born via emergency cesarean section for footling breech. The mother had a history of polysubstance abuse and received no prenatal care due to her unawareness of the pregnancy until a delayed presentation at labor and delivery. At delivery, the neo

Effects of the WHO Labour Care Guide on cesarean section in India: a pragmatic, stepped-wedge, cluster-randomized pilot trial.

Cesarean section rates worldwide are rising, driven by medically unnecessary cesarean use. The new World Health Organization Labour Care Guide (LCG) aims to improve the quality of care for women during labor and childbirth. Using the LCG might reduce overuse of cesarean; however, its effects have not been evaluated in randomized trials. We conducted a stepped-wedge, cluster-randomized pilot trial in four hospitals in India to evaluate the implementation of an LCG strategy intervention, compared with routine care. We performed this trial to pilot the intervention and obtain preliminary effectiv
