First case of neonatal bloodstream infection caused by Malassezia furfur in mainland China diagnosed via metagenomic next-generation sequencing: A case report.

The present study reports, to the best of our knowledge, the first case of neonatal bloodstream infection caused by Malassezia furfur in mainland China identified using metagenomic next-generation sequencing (mNGS). Traditional microbiological methods have failed to identify causative organisms, highlighting the diagnostic limitations of neonatal sepsis with atypical presentations. Using this case as a clinical entry point, a systematic review was conducted to consolidate the epidemiological, clinical and prognostic characteristics of previously reported neonatal Malassezia infections and crit

Assessment of iron sufficiency in premature infants treated with darbepoetin using conventional and novel erythrocyte metrics.

OBJECTIVE: Randomized trials demonstrate that administering darbepoetin to extremely low birth weight (ELBW) neonates increases hematocrits and reduces or eliminates transfusions. However, further studies are needed regarding maintaining adequate iron during darbepoetin therapy. STUDY DESIGN: We analyzed datasets from the Neonatal Research Network Darbe Trial and Intermountain Health to assess iron sufficiency during darbepoetin treatment. We tracked traditional markers of erythropoiesis (reticulocytes and RET-He), and novel markers of iron-limited erythropoiesis (Micro-R and HYPO-He). RESUL

Pulmonary Hemorrhage in Extremely Low Birth Weight Infants: Risk Factors and Preventive Measures for an Increased Incidence.

BACKGROUND: Pulmonary hemorrhage is a severe respiratory complication with high mortality in preterm infants; no causal treatment exists. This study aimed to identify risk factors for pulmonary hemorrhage in extremely low birth weight infants (birth weight <1,000 g) treated in a German neonatal unit and to relate these factors to a local three-fold increase in pulmonary hemorrhage incidence over 10 years. PATIENTS AND METHODS: In this retrospective cohort study spanning approximately 10 years, 319 extremely low birth weight infants (including 12 with pulmonary hemorrhage) were divided into lo

Outborn Neonates in Nairobi's Public Hospitals: Mortality Burden and Risk Factors.

BACKGROUND: Neonatal mortality remains a significant challenge in Sub-Saharan Africa. Outborn neonates - those born outside the admitting hospital - are particularly vulnerable, yet their outcomes remain under-characterized in many low-resource urban settings. This study examines the burden, characteristics, and mortality risk among outborn neonates admitted to public neonatal units in Nairobi, Kenya. METHODS: We conducted a retrospective cohort study across four high-volume public hospitals in Nairobi County, Kenya. A stratified random sample of 400 outborn neonates admitted between January

Feasibility and procedural outcomes of ultrasound-guided brachiocephalic vein cannulation in neonates weighing less than 1,000 g: a prospective study.

PURPOSE: To evaluate the anatomical feasibility and procedural outcomes of ultrasound-guided right brachiocephalic vein cannulation for placing non-tunnelled centrally inserted central catheters in neonates weighing less than 1,000 g. METHODS: This single-center, prospective observational study assessed right BCV diameter using high-frequency ultrasound (Phase 1), defining anatomical feasibility as a diameter ≥ 2.0 mm (suitable for a 2 French catheter). Subsequently, ultrasound-guided cannulation was evaluated using a standardized "micro-access" bundle (Phase 2), measuring technical success,

Maternal and Fetal Outcomes in Early-Onset vs. Late-Onset Preeclampsia: A Retrospective Study at a Tertiary Care Institute in India.

Background Preeclampsia is a hypertensive disorder of pregnancy associated with significant maternal and perinatal morbidity and mortality. Distinguishing between early- and late-onset preeclampsia is crucial due to differences in pathophysiology, severity, and outcomes. This study aimed to evaluate and contrast clinical characteristics, complications, and outcomes between the two types. Aim & objective The aim of the study is to compare the maternal and fetal outcomes in pregnancies complicated by early-onset preeclampsia (before 34 weeks) versus late-onset preeclampsia (34 weeks or later) an

Telehealth-Based Neurodevelopmental Assessment in Extremely Low Birth Weight Infants During the Coronavirus Disease 2019 Pandemic: A Cross-Sectional Study.

BACKGROUND: Preterm birth remains strongly associated with long-term neurodevelopmental impairments, including cognitive, language, and motor delays. The coronavirus disease 2019 pandemic disrupted routine in-person developmental follow-up for extremely low birth weight (ELBW) infants, necessitating alternative assessment approaches. This study aimed to evaluate the feasibility of telephone-based neurodevelopmental assessments within an existing neonatal follow-up program and to assess neurodevelopmental outcomes in ELBW infants during the coronavirus disease 2019 pandemic in a middle-income c

Current Evidence, Controversies, and the Future of Biotics in the Prevention of Necrotizing Enterocolitis: A Narrative Review.

Necrotizing enterocolitis (NEC) remains one of the most devastating gastrointestinal disorders affecting preterm infants, with substantial mortality and long-term morbidity despite advances in neonatal care. Increasing evidence implicates intestinal dysbiosis, impaired intestinal barrier function, and dysregulated immune responses as central drivers of NEC pathogenesis, making microbiome-targeted interventions a promising preventive strategy. Collectively termed "biotics," these interventions include probiotics, prebiotics, synbiotics, and postbiotics, each aimed at modulating the developing g

Effectiveness of structured lactation support and human donor milk banking in German NICUs: a stepped-wedge cluster-randomized trial.

BACKGROUND: Although exclusive human milk feeding is beneficial for very low birth weight infants, rates remain suboptimal globally. This hybrid type 1 effectiveness-implementation trial evaluated structured lactation support combined with human donor milk banking in neonatal intensive care units (NICUs), primarily assessing effectiveness, with secondary assessment of implementation and economic outcomes. METHODS: We conducted a stepped-wedge cluster-randomized trial in 15 German Level I NICUs (April 2022-March 2024). The Neo-MILK intervention comprised structured lactation counselling, multi

Intravenous lipid emulsions with or without fish oil for the prevention of liver disease in VLBW neonates: A network meta-analysis.

BACKGROUND: Parenteral nutrition-associated liver disease is a severe complication in very low birth weight (≤1500 g) preterm neonates. Although previous pairwise meta-analyses have compared individual intravenous lipid emulsions, their relative efficacy across all available formulations has not been established in this population. METHODS: We conducted a systematic review and frequentist random-effects network meta-analysis, searching PubMed, Embase, and the Cochrane Central Register through April 2026. Eligible studies were randomized trials of any intravenous lipid emulsion in very low bir

Is necrotizing enterocolitis associated with the 896A/G polymorphism in the toll-like receptor 4 (TLR4) gene?

BACKGROUND: The pathophysiology of necrotizing enterocolitis (NEC) involves changes in intestinal development that hinder its functionality, leading to both metabolic and gene and phenotypic changes. Among the genetic factors the 896A/G polymorphism in the Toll-Like Receptor 4 (TLR4) gene can trigger is an inappropriate and persistent inflammatory response, leading to the progression of lesions and necrosis of the intestinal mucosa, and reduced perfusion of the microvasculature, increasing susceptibility to the disease. AIMS: To determine the prevalence of the 896A/G polymorphism in the TLR4

Awareness and Acceptability of Whole Breast Reconstruction After Mastectomy: A Cross-Sectional Survey Study.

BACKGROUND: Parenteral nutrition associated cholestasis (PNAC) is a frequently observed pathology in extremely low birth weight (ELBW) infants. Its pathogenesis is determined by the composition and duration of parenteral nutrition (PN) as well as the tolerance of enteral feeds (EF). "Aggressive" nutrition is increasingly used in ELBW infants to improve postnatal growth. Little is known about the effect of "aggressive" nutrition on the incidence of PNAC. We analyzed the influence of implementing an "aggressive" nutritional regimen on the incidence of PNAC and growth in a cohort of ELBW infants.
