Enteric fever caused by Salmonella enterica serovar Paratyphi remains a critical public health challenge due to rising incidence and increasing antimicrobial resistance. The development of effective polysaccharide conjugate vaccines targeting Salmonella Paratyphi requires high-purity O-antigen polysaccharide (PS) conjugated to immunogenic carrier proteins. This study optimized a robust and scalable purification-conjugation methodology, specifically tailored for O-antigen from Salmonella Paratyphi. Initial purification utilized ultrafiltration via tangential flow filtration (TFF), followed by s

BACKGROUND: Enteric infections impose a substantial global health burden annually, especially in countries with inadequate sanitation. This study aims to assess the trends in the burden of enteric infections in China.
METHODS: Prevalence, incidence, deaths, and disability-adjusted life years (DALYs) attributed to enteric infections by etiology in China were leveraged from the Global Burden of Disease Study 2021. Temporal trends in the burden of enteric infections from 1990 to 2021 were determined by percent changes and average annual percent change (AAPC). Decomposition analysis for percent c

Salmonellosis remains a major cause of morbidity and mortality in low- and middle-income countries, despite the availability of effective vaccines against Salmonella enterica serovar Typhi (S. Typhi). In response, substantial efforts have been underway to develop vaccines against the key serovars responsible for invasive non-typhoidal Salmonella (iNTS) disease, such as S. Typhimurium and S. Enteritidis, as well as against S. Paratyphi A, which, together with S. Typhi, is responsible for enteric fever. The O-antigens (OAg) are considered potential protective antigens; therefore, the most advanc

Intestinal infestation with ascaris worms is very common worldwide and usually causes minimal or no abdominal symptoms. Severe abdominal complications such as bowel obstruction may occur, but bowel perforation from ascariasis is rare. We present a case of a seven-year-old boy who initially presented with typhoid fever and then developed ascaris peritonitis requiring emergent surgery. We discuss preoperative diagnosis and perioperative management, as well as the purported mechanisms of coinfection with typhoid fever and ascariasis that can lead to small bowel perforation.

BACKGROUND: Typhoid and paratyphoid fevers represent a global health burden, especially in Southern Asia, exacerbated by the increase in antimicrobial resistance. While vaccines against Salmonella Typhi have been successfully introduced, a vaccine against S. Paratyphi A is not available, yet. Efforts to develop an effective vaccine targeting both Salmonella serovars are currently ongoing. GVGH is developing a bivalent vaccine constituted by the Vi-CRM197 typhoid conjugate vaccine (TCV), and the Salmonella Paratyphi A O-antigen (O:2), also conjugated to the CRM197 carrier protein (O:2-CRM197).

Enteric fever is a serious systemic infection that collectively encompasses typhoid and paratyphoid fevers, caused by Salmonella enterica serovars Typhi and Paratyphi, respectively. It is a major public health concern worldwide and may present a complex clinical challenge to physicians in endemic regions. We present a report of three cases highlighting atypical presentations and complications of enteric fever. The first case reports on a 55-year-old man who presented with septic shock complicated by sequential tachyarrhythmias, despite a structurally normal heart and normal serum electrolyte l

Background: Typhoid fever is a multisystemic illness caused by Salmonella typhi and Salmonella paratyphi, transmitted fecal orally through contaminated water and food. It is a rare diagnosis in the US, with most cases reported in returning travelers. Hepatitis and cholestasis are rare sequelae of salmonella infection. However, acute liver failure (ALF) is exceptionally uncommon. We report a case of typhoid fever in a returning traveler to the US progressing to ALF. Case Presentation: A 48-year-old man presented with high-grade fever, abdominal pain, vomiting, acholic stools, dark urine, and ye

BACKGROUND: Hepatitis A is an acute viral infection of the liver caused by hepatitis A virus (HAV) that is acquired through the feco-oral route. It has the highest incidence among the four major acute viral hepatitis types (A, B, C, and E) and usually occurs in early childhood. However, the prevalence of acute hepatitis A has recently increased among teenagers and young adults, and it is usually misdiagnosed. This study emphasizes the significance of awareness among healthcare workers about the increasing incidence of acute hepatitis A among this group to ensure accurate diagnosis and appropri

BACKGROUND: We assessed persistence of typhoid immunity conferred by Vi polysaccharide-tetanus toxoid (Vi-TT) conjugate vaccine (TCV) four years post-vaccination and immunogenicity of a booster dose of Vi-TT given at age five.
METHODS: In 2018, a phase 3 trial of Vi-TT in Malawi randomised children 1:1 to receive Vi-TT or meningococcal capsular group A conjugate vaccine (control). Subsequently, TCV was licensed and recommended in the region. In 2023, children vaccinated at 9-11 months in the original trial received a second (Booster- TCV) or first (1st-TCV) Vi-TT dose, at age five. Serum coll

BACKGROUND: Reporting of notifiable infectious diseases was overall impacted by the COVID-19 pandemic. This could affect disease surveillance and thus, outbreak detection, as it is based on historical data. For effective outbreak detection, it is crucial that communicable disease control professionals can rely on accurate and valuable alarm thresholds to ensure timely and adequate response to potential outbreaks.
OBJECTIVE: In this study, we take the first steps in the development of a methodology that adjusts for the impact of the COVID-19 pandemic on the number of notifications of notifiabl

BACKGROUND: The relationship between ambient temperature and infectious disease incidence lacks comprehensive documentation. Our study, therefore, sought to systematically determine the national association between temperature and the incidence of infectious diseases, categorized into respiratory, gastrointestinal and enterovirus, and vector-borne categories. We aimed to study the association between extreme cold and heat extreme temperature on infectious disease occurrence among children and teenagers, and to evaluate the secular trends in these diseases in relation to temperature extremes.

BACKGROUND OBJECTIVES: India is both a tropical and subtropical region, where common causes of acute undifferentiated febrile illness (AUFI) include malaria, dengue fever, chikungunya fever, enteric fever, leptospirosis, scrub typhus, and Japanese encephalitis. These illnesses contribute significantly to patient morbidity and mortality, although the exact burden of each infection varies regionally. Given the high prevalence of these tropical diseases, prioritizing differential diagnoses in cases of AUFI is crucial. In recent years, dengue, chikungunya, malaria, typhoid fever, scrub typhus, and

Leptospirosis is a zoonosis of global distribution. The U.S. Centers for Disease Control and Prevention has designated leptospirosis a nationally notifiable disease. There is need to raise awareness of the burden of leptospirosis among health care givers and policy makers in Africa. The aim of this review was to highlight the current situation of leptospirosis in Africa and suggest a One Health approach of addressing its status as a leading zoonosis. In tropical regions, the nonspecific symptoms of fever, myalgia and arthralgia result in misdiagnosis of leptospirosis with malaria, yellow fever

BACKGROUND: The Athens Plague (430-427 BCE) was a devastating outbreak during the Peloponnesian War, described in detail by Thucydides. Although there is ongoing discussion, the specific cause of the epidemic remains undetermined.
OBJECTIVES: This review re-examines the Athenian Plague through historical and microbiological lenses, reassessing diagnostic hypotheses and proposing the plausibility of an ancient pneumonic strain of Yersinia pestis.
SOURCES: We analysed Thucydides' original account alongside interdisciplinary literature from classical studies, paleopathology, epidemiology, and m

BACKGROUND: SNP based association studies have revolutionized the field of biomedicines. Enteric fever is a systemic disease with etiologic agent Salmonella enterica serovar typhi and paratyphi. It is a serious health issue worldwide and presents wide variations in incidence, rates, and severity. Previous investigations revealed that genetic variations may lead to susceptibility to typhoid fever. The current study was performed to investigate the potential association of PARK2_e01(-697) polymorphism with the susceptibility to typhoid in the Punjabi population.
METHODS: For this case-control s
