Isolated pulmonary Cryptococcus neoformans infection presenting with severe pulmonary involvement: a case report.

BACKGROUND: Pulmonary cryptococcosis is an uncommon fungal infection typically seen in immunocompromised hosts. In individuals with only mild immunosuppression, the disease is rare, often mimicking bacterial pneumonia, leading to diagnostic delays and inappropriate treatment. CASE PRESENTATION: We report a 50-year-old Chinese man with poorly controlled diabetes mellitus and myasthenia gravis on low-dose corticosteroids who presented with fever, cough, and hypoxemic respiratory failure. Initial imaging suggested severe community-acquired pneumonia, and broad-spectrum antibiotics were administe

Based on a radiomics-clinical nomogram to predict the therapeutic effect of pulmonary cryptococcosis pneumonia.

We aimed to develop a radiomics-clinical nomogram to predict the therapeutic effect of PC pneumonia. A total of 255 PC pneumonia patients (165 cases with good therapeutic effect and 90 cases with poor therapeutic effect) were retrospectively enrolled from two centers. 190 PC pneumonia patients from Center 1 were randomly divided into a training group (133 cases) and an internal validation group (57 cases) at a ratio of 7:3. The data from Center 2 were used as the external validation group (65 cases). The radiomics features selected from unenhanced CT were input into multiple machine learning m

Pulmonary cryptococcosis with cavitation in an immunocompetent child: a case report and literature review.

BACKGROUND: In recent years, the incidence of pulmonary cryptococcosis (PC) has risen among patients without human immunodeficiency virus (HIV) infection, including individuals with preserved immune function. The clinical manifestations of PC are largely non-specific, frequently presenting with fever, cough, chest tightness, or chest pain. In some cases, PC remains asymptomatic, which increases the likelihood of misdiagnosis or delayed recognition. The present study reports the diagnostic evaluation and therapeutic course of a child with PC and normal immune function, accompanied by a literatu

Host immunosuppression and antifungal pressure cooperatively shape fungal adaptation and pulmonary microenvironment in cryptococcosis.

Cryptococcus neoformans remains a major opportunistic pathogen causing life-threatening pulmonary and systemic infections in immunocompromised individuals. Effective disease control is challenged by limited therapeutic options and by the pathogen's remarkable ability to adapt under immune and pharmacological pressures. Corticosteroids, while indispensable for managing inflammatory and autoimmune conditions, can disrupt immune homeostasis and worsen fungal infections. To investigate how corticosteroid-induced immunosuppression interacts with antifungal therapy, we employed a murine model of pul

Pulmonary cryptococcosis showing a solitary nodule in patients with previous malignancy.

Pulmonary cryptococcosis is one of the pulmonary fungal infections caused by the inhalation of Cryptococcus neoformans. Pulmonary cryptococcosis shows various radiological findings and sometimes a pulmonary nodule on CT. We experienced three cases of pulmonary cryptococcosis showing a solitary nodule in patients with previous malignancy. All three cases show a newly emerging or growing nodule. Two cases whom FDG-PET was tested showed accumulation of FDG. All three cases underwent pulmonary resection and were pathologically diagnosed with pulmonary cryptococcosis. It is difficult to distinguish

Pediatric Cryptococcosis Presenting as a Mediastinal Mass: A Case Report And Endobronchial Ultrasound-Guided Transbronchial Needle Aspiration Protocol.

Endobronchial ultrasound-guided transbronchial needle aspiration (EBUS-TBNA) is a minimally invasive diagnostic technique. While widely used in adults, its application for diagnosing mediastinal masses in pediatric patients remains underreported. This gap poses challenges when addressing atypical presentations, such as pulmonary cryptococcosis in immunocompetent children, which is frequently misdiagnosed due to nonspecific symptoms. We present the case of a healthy nine-year-old male with an eight-day history of fever and cough. Computed tomography revealed a mediastinal mass in the left hilum

Severe Pulmonary Cryptococcosis in an Immunocompetent Patient Presenting With Acute Respiratory Failure.

Pulmonary cryptococcosis may present with nonspecific respiratory symptoms and, in patients with underlying reactive airway disease, can act as a trigger for refractory asthma exacerbation leading to acute respiratory failure, even in immunocompetent individuals. A 60-year-old immunocompetent male, with a negative HIV test, no chronic immunosuppressive therapy, and a remote brief corticosteroid course well below the threshold for clinically significant immunosuppression, presented with approximately two months of progressive dyspnea and cough after multiple treatment courses for presumed asthm

An Organoselenium Compound Induces Death in Cryptococcus gattii Yeast by Apoptosis and Necrosis and Shows Antifungal Efficacy in Galleria mellonella.

Cryptococcus gattii is an etiological agent of invasive pulmonary cryptococcosis in both immunocompetent and immunocompromised individuals. Current antifungal therapy is limited by toxicity, high cost, and emerging tolerance, supporting the need for new therapeutic alternatives. Although the organoselenium compound LQA_78 previously demonstrated antifungal activity against Cryptococcus neoformans, its effect on C. gattii remains unclear. Then, the aim of this work is to evaluate the antifungal activity of LQA_78 against C. gattii strains, its effects on virulence factors, and the underlying me

Diagnostic accuracy of bronchoalveolar lavage fluid-based testing for pulmonary cryptococcosis: A systematic review and meta-analysis.

BACKGROUND: Pulmonary cryptococcosis(PC) presents diagnostic challenges because of its non-specific clinical and radiological manifestations. Bronchoalveolar lavage fluid (BALF)-based testing, which includes latex agglutination (LA) and lateral flow assay (LFA), offers a minimally invasive diagnostic method, yet its pooled diagnostic accuracy remains unclear. METHODS: We systematically searched PubMed, Embase, Cochrane Library, and Scopus from inception to May 2026. Studies evaluating BALF-based testing for PC with extractable 2 × 2 data were included. The methodological quality of relevant s

Pulmonary Cryptococcosis in Apparently Immunocompetent Hosts: Host Susceptibility, Occult Immunodeficiency and Clinical Implications.

Pulmonary cryptococcosis (PC) in apparently immunocompetent hosts-now the majority of non-HIV cases in Asia-cannot be explained by the traditional opportunistic infection paradigm. Three epidemiological observations point toward occult immune susceptibility: familial clustering documented in multiple case reports and small family studies despite shared environmental exposure, normal routine immunological screening, and species-specific infection patterns between Cryptococcus neoformans and Cryptococcus gattii. We propose a three-layer framework: upstream genetic variants in pattern recognition

The Diagnostic Challenge in Pulmonary Cryptococcosis Among Immunocompetent Patients: The 'False-Negative' Serum Cryptococcal Antigen.

OBJECTIVE: To delineate the demographic, clinical, and radiological features of immunocompetent patients with pulmonary cryptococcosis (PC) who test negative for serum cryptococcal antigen (CrAg), a subgroup that poses a diagnostic challenge. METHODS: Participants were stratified into CrAg-positive (n = 128) and CrAg-negative (n = 71) cohorts. Comparative analyses were performed on clinical presentation, chest computed tomography (CT) imaging features, and laboratory parameters. RESULTS: Patients with false-negative CrAg results were more likely to be asymptomatic (46.5% vs. 20.3% in the CrA

Pulmonary cryptococcosis with an endobronchial cryptococcoma in a Warmblood mare.

This report describes pulmonary cryptococcosis with a large endobronchial cryptococcoma in an 8-year-old Warmblood mare. The mare was presented initially with fever and lethargy, and clinical signs typical of bacterial pneumonia. Markedly elevated serum amyloid A (SAA) concentrations indicated a severe systemic inflammatory response. Over a period of 6 wk, the condition progressively worsened and was refractory to multiple courses of broad-spectrum antimicrobial therapy as administered in a field setting. Diagnostic imaging following referral revealed extensive consolidation of the left lung a

Pulmonary cryptococcosis with or without immunocompromise: a retrospective comparative study of clinical and CT features and influential factors for lesion absorption.

BACKGROUND: Pulmonary cryptococcosis (PC) is an invasive fungal infection with rising incidence in immunocompetent populations. The differential clinical and imaging characteristics between immunocompromised (ICP) and non-immunocompromised (NICP) patients remain incompletely characterized, limiting precision in diagnosis and management. METHOD: A retrospective study was conducted on 117 patients with confirmed PC from a single tertiary center. Patients were stratified into ICP and NICP groups based on refined criteria. Clinical symptoms, laboratory findings, and chest CT imaging features were

Machine learning approaches to identify rare fungal and tuberculous infections in hospitalized patients.

OBJECTIVE: Diagnosing atypical pathogen-induced infectious diseases is a complex and challenging problem, given their unique characteristics. A-priori identification of features using modern AI approaches can facilitate the diagnostic process, potentially leading to improved outcomes, early diagnosis, and effective resource utilization. We aimed to develop an AI-driven clinical decision support system for identifying rare and atypical infections - a composite of six microbiologically and clinically distinct diseases (blastomycosis, cryptococcosis, histoplasmosis, mucormycosis, pneumocystosis,

MoS2-supported multiepitope protein electrochemical biosensors: A simple and highly selective approach for the diagnosis of systemic mycoses.

Cryptococcosis is a life-threatening fungal infection with global public health impact, affecting both immunocompromised individuals and the general population. Accurate clinical management requires differentiation of Cryptococcus species and detection of subclinical infections. However, current diagnostic methods suffer from limited sensitivity and, notably, selectivity. Here, we report a highly sensitive and selective electrochemical immunosensor for rapid and simple serological diagnosis of cryptococcosis. The sensor employs novel multi-epitope chimeric proteins (A, B, C, and D) directly an

Ileocecal Paracoccidioidomycosis Presenting as a Pseudotumoral Lesion in a Pediatric Patient: A Histopathological Diagnostic Challenge.

Ileocecal pseudotumoral lesions in pediatric patients pose a significant diagnostic challenge, particularly in endemic regions where infectious and neoplastic conditions overlap. Gastrointestinal paracoccidioidomycosis (PCM) is rare and may closely mimic intestinal tuberculosis or malignancy, leading to potential misdiagnosis. We report a 14-year-old girl presenting with constitutional symptoms and imaging findings of ileocecal wall thickening with mesenteric lymphadenopathy. Endoscopy revealed ulceroinfiltrative lesions suggestive of neoplasia. Histopathological evaluation demonstrated granul

Fungal and Atypical Pneumonia.

Fungal and atypical bacterial pneumonias are emerging infectious threats in companion animal medicine, fueled by ecological disruption, globalized pet travel, and immunomodulatory therapies being increasingly used. These infections challenge clinicians in ways that conventional bacterial pneumonia does not-presenting insidiously, extending beyond the lung, and not responding to empirical treatment. Delayed or missed diagnosis remains a costly, defining hallmark. Recognizing these conditions demands a shift in clinical thinking: one that weighs host susceptibility, exposure history, and geograp

Evaluating the Use of Lower Dose Flucytosine for the Treatment of Cryptococcal Meningitis: A Clinical Trial.

BACKGROUND: Cryptococcosis is a leading cause of death among people with human immunodeficeincy virus (HIV). Guidelines recommend combination therapy with amphotericin B and flucytosine, but standard flucytosine dosing of 100 mg/kg/day is associated with hematologic toxicity. The FLOOR trial investigated whether lower flucytosine dosing maintained therapeutic efficacy and reduced toxicity. METHODS: This phase 2, single-arm, open-label trial enrolled 48 adults with HIV and confirmed cryptococcal meningitis at 2 hospitals in Uganda. Participants received 10 days of flucytosine at 60 mg/kg/day i
