Cytosolic Phosphoenoylpyruvate Carboxykinase Deficiency: Clinical, Biochemical, and Genetic Features of Five Non-Finnish Patients.

Cytosolic phosphoenoylpyruvate carboxykinase (PEPCK-C) is an essential, rate-limiting enzyme in the gluconeogenesis pathway. PEPCK-C deficiency presents with hypoglycaemia, hyperlactataemia and hepatopathy, and was first reported in association with bi-allelic PCK1 variants in 2014. A Finnish cohort with a common homozygous variant (c.925G>A, p.(Gly309Arg)) is well-described, but few other genotypes are reported. Five non-Finnish probands with PEPCK-C deficiency with novel genotypes are presented. All five presented with hypoglycaemia (hypoketotic in three), lactic acidosis, and elevated trans

Continuous glucose monitoring versus self-monitoring of blood glucose in gestational diabetes: an updated systematic review and meta-analysis of randomized controlled trials.

AIMS: Continuous glucose monitoring (CGM) benefits pregnant women with type 1 or type 2 diabetes, but its role in gestational diabetes (GDM) remains uncertain. We aimed to compare the effects of CGM with self-monitoring of blood glucose (SMBG) on glycemic, maternal, and neonatal outcomes in women with GDM. METHODS: We compared CGM with SMBG in women with GDM through a systematic search across randomized controlled trials (RCTs) in PubMed, Cochrane Library, Embase, and Scopus. We evaluated glycemic, maternal and neonatal outcomes using a random-effects model. RESULTS: Eleven RCTs (n = 1225) m

Congenital double lower lip in a newborn with feeding difficulties.

We describe the case of a full-term male newborn admitted on the first day of life due to feeding difficulties and hypoglycemia, who presented with a protruding bifid mass along the mucosa of the lower lip that interfered with breastfeeding. Double lip is a rare congenital condition caused by hyperplasia of the lip mucosa, most commonly affecting the upper lip. Surgical removal is generally recommended if the condition interferes with speech, sucking, or, more commonly, for cosmetic reasons. In this case treatment was conservative, resolving feeding difficulties without the need for surgery, w

Clinical Presentation and Molecular Characteristics of Kabuki Syndrome With Congenital Hyperinsulinism: A Retrospective Study.

INTRODUCTION: Kabuki syndrome (KS) is a rare disease predisposing to congenital hyperinsulinism (CHI). The incidence of CHI in KS may be higher than considered in practice, and appropriate management of hypoglycemia would reduce long-term neurologic morbidity in these patients. Thus, the aims of our study were to estimate the occurrence rate of CHI-related hypoglycaemia in KS, to describe its evolution, and to identify potential genotype-phenotype correlations. METHODS: We conducted a single-centre retrospective study in patients with KS from 1999 to 2024. RESULTS: Among the 18 patients iden

Congenital Hyperinsulinism in Neonates: Diagnostic Challenges and Management in Two Cases With KCNJ11 and ABCC8 Mutation.

We report two cases of persistent neonatal hypoglycemia associated with mutations in the KCNJ11 and ABCC8 genes, encoding the Kir6.2 and SUR1 subunits of the adenosine triphosphate-sensitive potassium channel. In both cases, symptoms were non-specific and initially attributed to other conditions, including suspected infection. Standard treatment with enteral and parenteral glucose infusions failed to restore euglycemia. Diazoxide was administered without a clinical response. Both patients were then treated with octreotide, resulting in stabilization of glycemic levels. Genetic testing confirme

Maternal Hypoglycemia and Reduced Foetal Growth in Pregnancies Following Roux-en-Y Gastric Bypass: A Prospective Cohort Study.

INTRODUCTION: The risk of delivering a small-for-gestational-age (SGA) neonate is increased after Roux-en-Y gastric bypass (RYGB). Because glucose is the foetus's primary energy source and postprandial hypoglycemia is common after RYGB, the Bariatric surgery And consequences for Mother and Baby In pregnancy (BAMBI) study examined associations between trimester specific maternal hypoglycemia and offspring birthweight as well as other perinatal outcomes. MATERIALS AND METHODS: Twenty-three pregnant women with RYGB and 23 BMI- and parity-matched controls were followed prospectively with continuo

Neonatal Hyperinsulinemia and Maternal Diabetes Associated With a Hepatocyte Nuclear Factor 4 Alpha (HNF4A) Variant.

Maturity-onset diabetes of the young type 1 (MODY1) is caused by autosomal dominant variants in the hepatocyte nuclear factor 4 alpha (HNF4A) gene. This genetic defect results in a characteristic biphasic phenotype, typically manifesting as neonatal hyperinsulinism (HI) followed by the development of diabetes mellitus (DM) in young adulthood. HNF4A acts as a master transcriptional regulator for insulin gene expression and hepatic metabolic pathways, providing a molecular basis for understanding the link between neonatal hyperinsulinemic hypoglycemia and DM in young adulthood. Therefore, accura

Outbreak of Gastrointestinal Sepsis Associated with Bacillus cereus-Contaminated Human Milk Fortifier in Preterm Neonates.

We report a cluster of acute gastrointestinal illness in seven preterm neonates temporally associated with a single batch of powdered human milk fortifier (HMF). Symptoms developed within 3-4 h of ingestion and included profuse diarrhea, dehydration, hypoglycemia, and shock. Blood and stool cultures were sterile. Microbiological analysis of unopened HMF sachets revealed contamination with Bacillus cereus and other spore-forming organisms. Withdrawal of the implicated batch and reinforcement of infection-control measures resulted in no further cases. This outbreak highlights the risk of pathoge

Microstructural white matter changes in infants with transient neonatal hypoglycemia revealed by diffusion tensor imaging: a preliminary cross-sectional study.

BACKGROUND: Neonatal hypoglycemia (NH) is a common metabolic disorder that has been closely linked to abnormal neurodevelopment and brain injury. However, microstructural alterations in white matter among infants with transient NH remain poorly characterized. This study employed diffusion tensor imaging (DTI) to investigate whether transient NH induces microstructural white matter injury and to examine its correlation with blood glucose fluctuations. METHODS: In this retrospective cohort study, we enrolled 20 infants with transient NH and 22 sex- and gestational age (GA)-matched normoglycemic

Association of diagnostic oral glucose tolerance test values with neonatal hypoglycemia in diet-controlled gestational diabetes mellitus: a retrospective cohort study.

BACKGROUND: Whether diagnostic oral glucose tolerance test (OGTT) values are associated with neonatal hypoglycemia (NH) in women with diet-controlled gestational diabetes mellitus (GDM) remains uncertain. We aimed to evaluate the associations of diagnostic OGTT values with neonatal glycemic outcomes and to identify other determinants of NH in this population. METHODS: In this retrospective cohort study, 2,051 singleton pregnancies with diet-controlled GDM were included from a maternal and child health center in Liaocheng, China, between January 2019 and August 2023. Dysglycemia and Adverse Pregnancy Outcomes Among Individuals with Polycystic Ovary Syndrome.

BACKGROUND: Polycystic ovary syndrome (PCOS) has been associated with adverse pregnancy outcomes, although reported risks vary across studies. Metabolic heterogeneity within PCOS may contribute to this variability. Dysglycemia during pregnancy may identify a subgroup at increased obstetric risk. The study aims to evaluate whether dysglycemia is associated with preeclampsia and other adverse pregnancy outcomes among individuals with PCOS. METHODS: We performed a secondary analysis of the prospective Fatty Liver in Pregnancy (FLIP) cohort (n = 1,321). PCOS was defined by chart review of the ele

Continuous Glucose Monitoring Metrics and Perinatal Outcomes in Pregnant Women with Type 2 Diabetes.

BACKGROUND: The rising incidence of early-onset type 2 diabetes (T2D) has made it the leading cause of pregestational diabetes worldwide. However, real-world data on glycemic control during pregnancy and its association with perinatal outcomes remain limited. OBJECTIVE: To examine associations between continuous glucose monitoring (CGM)-derived metrics and perinatal outcomes throughout pregnancy and across gestational windows in women with T2D. METHODS: We conducted a single-center retrospective cohort study including all pregnant women with T2D who delivered between January 2020 and August

A regional quality-improvement collaborative to reduce early neonatal hypothermia in critically ill newborns.

BACKGROUND: Early postnatal hypothermia (EPH) remains a significant clinical concern in critically ill transported neonates. A previous regional study reported an alarming EPH incidence rate of 82.1%, with wide variation across delivery facilities, underscoring the urgent need for quality improvement (QI) in early body temperature management. This study aimed to reduce the percentage of EPH in critically ill transported newborns by 20% to 25% within 1 year. METHODS: A standardized, evidence-based thermal care bundle and comprehensive staff training were implemented using multiple Plan-Do-Stud

Early postnatal hypothermia in critically transported newborns at a Chinese tertiary care neonatal centre: a retrospective study.

BACKGROUND: Neonatal hypothermia, especially occurring in the early postnatal period, is a critical condition associated with increased morbidity and mortality. Effective thermal management is essential, yet regional data on early postnatal hypothermia (EPH) incidence among critically ill neonates requiring inter-hospital transport are limited. METHODS: This retrospective, multicentre, observational cohort study involved critically ill newborns transported from six delivery hospitals, including two women's hospitals and four general hospitals, to a tertiary care neonatal centre between Januar

Comparison of Diluted Insulin (U10) Versus Standard (U100) Insulin in Automated Insulin Delivery Systems: Reducing Time Below Range in Young Children.

BACKGROUND: Treating young children with Type 1 diabetes (CwT1D) is challenging due to their very small need for insulin. We investigated the safety of diluted insulin (U10) in the AID system and how its use affected glycaemic outcomes compared to standard insulin. METHODS: Patients aged 0-4 years who initiated treatment with diluted insulin in an AID system during the years 2020-2025 were identified from the diabetes registry (N = 19). They had a maximum daily insulin dose of ≤ 6 units. An age and gender-matched control group (N = 27) using standard insulin (U100) in an AID system was determ
