Time-dependent pattern of liver injury biomarkers in neonates with hypoxic-ischemic encephalopathy undergoing therapeutic hypothermia.

UNLABELLED: Hypoxic-ischemic encephalopathy (HIE) often results in multi-organ damage. Liver injury following HIE is typically characterized by time-dependent altered patterns of alanine aminotransferase (ALT), aspartate aminotransferase (AST) or total bilirubin (TB) concentrations. This study aims to describe age-dependent patterns of liver injury biomarkers during and after therapeutic hypothermia (TH). Liver injury biomarkers (ALT, AST, TB) over the first 10 days of postnatal age (PNA) from six cohorts, including 428 neonates with moderate-to-severe HIE treated with TH were pooled. Statisti

Recurrent atypical ductal hyperplasia in the setting of gynecomastia in a male patient: a unique finding and brief literature review.

Gynecomastia (GM) is a benign condition involving glandular proliferation within male breast tissue. Atypical ductal hyperplasia (ADH) is considered a high-risk lesion due to its association with an increased risk of breast cancer; however, the literature remains sparse regarding the male population with underlying ADH. ADH in the setting of GM. Currently, the reported incidence of ADH with GM ranges from 0.4% to 4.5%. Here, we present a case of a male patient with recurrent ADH after a previous excision for bilateral ADH that was upstaged to ductal carcinoma in situ (DCIS) after definitive su

Targeting the Parkinson's disease interactome: a systems pharmacology and molecular dynamics investigation unveils Berberis aristata-derived phytoconstituents as potential modulators of tyrosine hydroxylase and p53: a systems and molecular dynamics investigation.

BACKGROUND: Various types of UVB radiation source (290-320 nm) are used in treating psoriasis and their therapeutic mechanism has been attributed to immunosuppressive properties. Recently, a new UVB source generated by a 308-nm excimer laser has been introduced for the treatment of psoriasis. OBJECTIVE: In this study we investigated the immunohistochemical evaluation of T cells and the expression of various apoptosis-related molecules in the psoriatic hyperproliferative skin before and after treatment with 308-nm monochromatic excimer light (MEL). METHODS: Ten patients (three women and seven

Identification of human transferrin receptor as an entry co-receptor for parvovirus B19 infection of human erythroid progenitor cells.

Parvovirus B19 (B19V), a member of the genus Erythroparvovirus within the Parvoviridae family, infects human erythroid progenitor cells (EPCs) of bone marrow and fetal liver and causes various hematological disorders. The minor capsid protein VP1 of B19V contains a unique N-terminal region (VP1u) that facilitates virus binding and internalization into EPCs via its receptor-binding domain (RBD). We previously identified tyrosine-protein kinase receptor UFO (AXL) as a proteinaceous receptor for B19V infection of EPCs. In this study, we employed an ascorbate peroxidase 2 (APEX2)-based proximity l

Case Report: Homozygous KISS1R mutation associated with congenital hypogonadotropic hypogonadism in two siblings: pulsatile GnRH therapy restores pituitary architecture and induces pubertal development.

Congenital hypogonadotropic hypogonadism (CHH) is a rare disorder characterized by deficient production, secretion, or action of gonadotropin-releasing hormone (GnRH), the central regulator of the reproductive axis. We report two siblings from a consanguineous family with CHH caused by a homozygous nonsense mutation (c.182C > A; p. Ser 61Ter) in exon 1 of KISS1R gene. The 15-years-old male proband presented with absent puberty, micropenis, and gynecomastia. Endocrine evaluation revealed low gonadotropin levels and brain magnetic resonance imaging (MRI) showed pituitary hypoplasia. His 12-years

The Role of Cannabis in Recurrent Adolescent Gynecomastia.

Gynecomastia is a common condition affecting adolescent males, and recurrence following surgical excision is not uncommon. Cannabis has been proposed as a potential endocrine system disruptor, though its role in gynecomastia or its recurrence remains unclear. We present a case series of 3 adolescent males who underwent initial excision for gynecomastia, reported postoperative cannabis use, and developed recurrence requiring secondary surgery. All patients had normal baseline endocrine evaluations and varied in body mass index, disease severity, and laterality. Recurrence occurred despite stabl

Immune and non-immune hydrops fetalis in a Saudi tertiary center: etiologies, antenatal predictors, perinatal outcomes, and one-year survival in a seven-year cohort.

BACKGROUND: Hydrops fetalis encompasses diverse fetal disorders with high perinatal mortality, and contemporary regional data are limited. METHODS: We conducted a single-center retrospective cohort study at a Saudi tertiary referral hospital (2016-2022). Pregnancies with antenatal hydrops (≥2 fluid-filled compartments) that were delivered at our center were included and classified as immune or non-immune hydrops fetalis (NIHF). For NIHF, multivariable logistic regression identified predictors of (i) live birth vs. intrauterine fetal death (IUFD)/termination in the antenatal cohort and (ii) on

Differential patterns of human parvovirus B19 seropositivity among diabetic and febrile children and adolescents: evidence from the Aseer region of Saudi Arabia.

INTRODUCTION: Human parvovirus B19 (B19V) is a significant causative agent of a diverse range of clinical manifestations in children. It is the primary cause of pediatric "erythema infectiosum," also known as "fifth disease." The virus can also cause hydrops fetalis during in-utero infections. Global serosurveys indicate that approximately 50% of individuals acquire protective immunity against the virus by the age 15; however, seroprevalence data vary depending on the geographic region and population studied. METHODOLOGY: In the present study, we report epidemiological data on B19V infectivit

Prenatal echocardiographic diagnosis and outcomes of fetal sustained supraventricular tachycardia: a ten-year single-center experience.

Fetal echocardiography plays a pivotal role in the diagnosis and management of fetal arrhythmias, allowing evaluation of the electrophysiological mechanism and guiding appropriate medical therapy. Herein, we describe our ten-year experience as a tertiary referral center in Southern Italy in the diagnosis and management of fetal sustained supraventricular tachycardia. Thirty-two consecutive fetuses were included: 16 fetuses with atrioventricular reentrant tachycardia, 7 fetuses with ectopic atrial tachycardia, 8 fetuses with atrial flutter, and 1 fetus with permanent junctional reciprocating ta

Spironolactone-Induced Bilateral Gynecomastia in a Patient With Comorbid Conditions.

Gynecomastia is a benign proliferation of male breast glandular tissue caused by an imbalance between estrogen and androgen activity. Spironolactone, a potassium-sparing diuretic commonly prescribed for edema and ascites in chronic liver disease, is a recognized cause of drug-induced gynecomastia because of its antiandrogenic effects. We report the case of a man in his 50s with chronic liver disease and type 2 diabetes mellitus who presented with bilateral breast swelling and mild tenderness of one month's duration. Clinical examination revealed bilateral soft glandular enlargement without nip

Evaluation of a Zika virus-like particle-based vaccine in a highly penetrant non-human primate model of congenital infection.

Zika virus (ZIKV) is an arthropod-borne Orthoflavivirus that caused a major outbreak in Central and South America in 2015-16. During that outbreak, ZIKV infection during pregnancy was associated with high rates of adverse outcomes, including miscarriage and a spectrum of neurological birth defects collectively termed congenital Zika syndrome (CZS). Ten years later, there are still no licensed ZIKV vaccines, underscoring the need for robust experimental models to evaluate vaccine efficacy against congenital disease. Here, we evaluated a highly pathogenic nonhuman primate pregnancy model for its

Mirror Syndrome (Ballantyne Syndrome): Prenatal Diagnosis, Pathophysiology, and the Role of Fetal Therapy-A Narrative Review.

Mirror syndrome is a rare maternal-fetal condition associated with fetal hydrops and a high risk of adverse maternal and fetal perinatal outcomes. Its diagnosis is challenging due to the lack of standardized diagnostic criteria and its clinical and biochemical overlap with preeclampsia. This narrative review summarizes current evidence on mirror syndrome, with a focus on prenatal diagnosis, placental pathophysiology and the role of targeted fetal therapy. Mirror syndrome is most frequently associated with non-immune causes of fetal hydrops and available evidence supports a central role for the
