The 'early CSP' is not a CSP: 3D transvaginal neurosonographic study of fetal cavum septi pellucidi formation and septal development.

OBJECTIVES: To describe the in-vivo development of the fetal cavum septi pellucidi (CSP) using transvaginal ultrasonography (TVS) and to propose a hypothesis underlying non-visualization of the septum pellucidum (NVSP). METHODS: In this retrospective study, three-dimensional ultrasound volumes of the fetal brain acquired using TVS between 17 + 0 and 24 + 6 weeks' gestation in structurally normal fetuses between January 2022 and November 2025 were analyzed. Multiplanar reconstruction was performed to allow assessment of midline structures. Corpus callosal (CC) length and CSP width were measure

Quantitative MRI Assessment of Olfactory System in Septo-Optic Dysplasia Spectrum and Related Midline Brain Malformations.

BACKGROUND AND PURPOSE: Although olfactory system abnormalities have been described in septo-optic dysplasia (SOD), prior studies have relied on qualitative or semiquantitative assessment, and the reproducibility of continuous MRI measurements remains unknown. We assessed the reproducibility of quantitative MRI measurements of the olfactory sulcus (OS) and olfactory bulb (OB) in patients with the SOD spectrum and related midline brain malformations, and examined their associations with consensus olfactory system abnormality scores. MATERIALS AND METHODS: In this IRB-approved, single-center, re

Predictors of Seizure Development in Septo-optic Dysplasia: A Retrospective Study.

ObjectiveTo elucidate factors associated with epilepsy in children with septo-optic dysplasia (SOD).MethodPatients (<21 years) diagnosed with SOD (2013-2023) were identified. Multivariate binomial regression predicted seizures in patients with SOD.ResultsWe identified 107 children (M:F = 46:61) with SOD. Among those, 103 had seizure data. Fifty-two (52/103; 50.5%) experienced seizures. Median age of seizure onset was 9 months (IQR: 4 months -2 years 5 months). Abnormal neurologic examinations were seen in 82.6% with seizures (P = .002). Global developmental delay was noted in 46 (43.0%) and as

Genetic and environmental drivers of craniofacial, brain, and pituitary disorders.

Congenital pituitary hormone deficiency (CH) is a rare disorder that usually involves deficient production of growth hormone (GH) and other pituitary hormones. Many cases are syndromic, affecting other aspects of craniofacial development and sometimes other organs. There are many genes implicated in congenital pituitary hormone deficiency, and some of them also cause more severe disorders like septo-optic dysplasia (SOD) and holoprosencephaly (HPE). It is not clear why mutations in the same gene can cause such variable clinical features. In addition, the molecular etiology of many cases is not

Expanding the clinical and molecular spectrum of TUBB2B through distinct variants identified across multiple families.

TUBB2B encodes a β-tubulin isotype essential for neuronal proliferation, migration, and organization during brain development. Pathogenic heterozygous variants in TUBB2B are associated with neurodevelopmental disorders including polymicrogyria and corpus callosum abnormalities. However, the phenotypic spectrum remains heterogeneous, most likely reflecting variant-specific effects on microtubule formation and stability. Homozygous TUBB2B variants are exceedingly rare, with one family reported to date. We describe five individuals in four families with rare TUBB2B variants. Four variants are des

Prenatal multimodality imaging of the fetal septum pellucidum-verum complex: A retrospective imaging study of suspected septopreoptic holoprosencephaly.

BACKGROUND: From the radiographic perspective, the septum pellucidum (SP) and septum verum (SV) Complex (SPVC) has been tacitly understood. Microdissection and diffusion tensor imaging (DTI) have now well established that they are not mere membranes but contain septal nuclei and nerve fibers; the Superior (SF) and Inferior fascicles (IF) forming the SP, and precommissural fibers of the fornix (PrCFx) in SV. OBJECTIVE: We aimed to delineate the topography of normal and abnormal SPVC using ultrasound (US), T2-weighted magnetic resonance imaging (MRI), and DTI in fetuses and provide an algorithm

Absent Septum Pellucidum in Fetal Development: Diagnostic Challenges, Associated Anomalies, and Prognostic Uncertainty-A Structured Narrative Review.

Background/Objectives: Absent septum pellucidum (ASP) is a rare fetal midline brain finding that may occur in isolation or alongside broader central nervous system (CNS) malformations, genetic disorders, or septo-optic dysplasia (SOD). Accurate prenatal diagnosis and counseling remain challenging because apparently isolated ASP may be reclassified following fetal magnetic resonance imaging (MRI), postnatal neuroimaging, or specialist assessment. This structured narrative review aimed to synthesize current evidence on prenatal imaging findings, associated anomalies, genetic evaluation, and post

Clinical and genetic basis of congenital gonadotropin deficiency.

STUDY QUESTION: What is the clinical and genetic overlap across subtypes of congenital gonadotropin (Gn) deficiency? SUMMARY ANSWER: This study reveals substantial clinical and genetic overlap among Gn deficiency disorders, with shared genetic and developmental features across congenital hypogonadotropic hypogonadism (CHH), combined pituitary hormone deficiency (CPHD), and syndromic forms of Gn deficiency. WHAT IS KNOWN ALREADY: Congenital Gn deficiency includes a subset of hypogonadotropic hypogonadism (HH) and can result from defects at the level of the hypothalamus or the pituitary. It in

MRI findings of an incidental ectopic posterior pituitary gland in an adult: Case report and review of T1 hyperintense lesions in the literature.

The ectopic posterior pituitary represents a rare congenital malformation attributable to aberrant embryogenesis of the neurohypophysis. Although it may occur as an isolated anomaly, it is frequently concomitant with other congenital malformations of the central nervous system. Despite extensive investigations, the precise etiology remains elusive; however, accumulating evidence suggests that its pathogenesis may parallel that of septo-optic dysplasia, thereby implicating a genetic predisposition. Earlier accounts predominantly attributed this anomaly to traumatic mechanisms. In contrast, cont

A Case of Emanuel Syndrome Diagnosed with Congenital Diaphragmatic Hernia at 15 Weeks and 1 Day of Gestation Followed by Increased Nuchal Translucency: A Case Report and Literature Review.

Congenital diaphragmatic hernia (CDH) diagnosed in the first or early second trimester is exceedingly rare. We herein present a unique case of Emanuel syndrome identified through early sequential findings of increased nuchal translucency (NT) and CDH. A 34-year-old Japanese woman was referred at 14 weeks of gestation because of an elevated NT of 3.9 mm detected at 12 weeks, with no other markers of aneuploidy. At 15+1/7 weeks, an ultrasound revealed right-sided heart displacement, a tubular structure near the heart, and a cystic structure on the left, presumed to be the stomach and intestines,

Novel markers on microvascular flow imaging for identifying holoprosencephaly.

OBJECTIVES: To establish gestational age-specific reference ranges for two new indices, the forehead-anterior cerebral artery angle (FACAA) and forehead-anterior cerebral artery distance (FACAD), and to objectively evaluate their diagnostic effectiveness, measured using Doppler technology with microvascular flow imaging, for identifying holoprosencephaly, even the semilobar and lobar types, in early pregnancy. METHODS: We evaluated FACAA and FACAD in 462 normal fetuses between 12 and 34 gestational weeks (GW) to generate normative reference ranges. Additionally, we evaluated these two indices

Tutorial: Exploring the Speech-Language Pathologist's Role in Childhood Blindness and Low Vision.

PURPOSE: Children with vision differences face unique challenges in accessing classroom communication, which can affect language development, academic achievement, and social participation. To ensure effective communication, professionals must understand each child's diverse access needs and individual preferences. Professionals must also be aware of the nature of the child's lived visual experience, including the strategies that allow the child with low vision to access sustained and meaningful vision, compared to those strategies preferred by the child who is blind. METHOD: This tutorial di

Recurrent Hypoglycaemia Leading to Early Diagnosis of Septo-Optic Dysplasia in a Small-for-Gestational-Age Infant-A Case Report.

Septo-optic dysplasia (SOD) is a rare condition with highly heterogenous clinical manifestations and can be a diagnostic challenge. It can present with pituitary hormone deficiencies, growth failure, visual impairment, and neurological symptoms. SOD can be diagnosed at different time points-from the prenatal period to childhood. Our team cared for a baby girl who presented with recurrent hypoglycaemia, conjugated hyperbilirubinemia, and hormonal deficiencies, prompting investigations that resulted in an early diagnosis of SOD. This case report highlights the importance of considering neuroimag

Understanding septo-optic dysplasia: Endocrine implications and ophthalmic consequences.

Septo-optic dysplasia (SOD) is a heterogeneous neurodevelopmental disorder classically defined by optic nerve hypoplasia, hypothalamo-pituitary dysfunction, and midline brain abnormalities, although the full triad is not consistently present. This review synthesises current evidence on the developmental, endocrine, ophthalmic, and neuroradiological dimensions of the SOD/optic nerve hypoplasia spectrum. Shared embryological origins of the optic pathways, hypothalamus, and pituitary, together with disruption of inductive signalling pathways and pathogenic variants in developmental regulators inc

Towards credible digital twins for basic and preclinical research.

Atomoxetine (Strattera(R)) is a selective norepinephrine (noradrenaline) reuptake inhibitor that is not classified as a stimulant, and is indicated for use in patients with attention-deficit hyperactivity disorder (ADHD). Atomoxetine is effective and generally well tolerated. It is significantly more effective than placebo and standard current therapy and does not differ significantly from or is noninferior to immediate-release methylphenidate; however, it is significantly less effective than the extended-release methylphenidate formulation OROS(R) methylphenidate (hereafter referred to as osm
