Chd8 Deficiency in Zebrafish Causes Autism-Like Behavioral Deficits.

BACKGROUND: Autism spectrum disorder (ASD) is a neurodevelopmental disorder with strong genetic and environmental components. Despite progress made over the past decades, no effective therapies targeting the core symptoms of ASD are currently available. More research is required to explore the underlying mechanisms of ASD and discover potential therapeutic targets. Chromodomain helicase DNA-binding protein 8 (CHD8) is one of the most significant high-confidence ASD risk genes identified to date. However, the precise roles and mechanisms of CHD8 in neurodevelopment and behaviors remain incomple

Detection of the Heterozygous Recurrent MAX p.(Arg60Gln) Variant in Two Females Confirms and Expands the Phenotypic Spectrum of Polydactyly-Macrocephaly Syndrome.

A recurrent de novo germline variant in the MAX gene, p.(Arg60Gln), has recently been associated with polydactyly-macrocephaly syndrome in six unrelated individuals. Affected individuals presented with progressive macrocephaly, post-axial polydactyly, developmental delay, autistic features and a series of craniofacial, brain, cardiac, ocular, and renal anomalies. Here, we describe two unrelated female probands with the known recurrent MAX variant, c.179G>A p.(Arg60Gln), who presented with the emerging phenotypes of the MAX-associated syndrome. We also propose that genitourinary abnormalities,

Syndrome of the Month: An Update on Smith-Kingsmore Syndrome: Characterization of Developmental Milestones and a Review of the Literature.

Smith-Kingsmore syndrome (SKS) is a rare autosomal dominant condition characterized by neurodevelopmental differences, macrocephaly/megalencephaly, describable facial features, sleep-wake abnormalities, hyperphagia, and overgrowth. SKS is caused by pathogenic gain-of-function variants in MTOR which lead to hyperactivation of the mTOR pathway. In this review, we discuss the history, epidemiology, molecular basis, clinical features, and management considerations for Smith-Kingsmore syndrome. In addition, we provide insight on early developmental milestones through a report on 14 individuals with

Agrawal Disease (Megalencephalic Leukoencephalopathy with Subcortical Cysts): A Rare Neurological Case Report.

Megalencephalic leukoencephalopathy with subcortical cysts (MLC) is a rare leukodystrophy marked by early-onset macrocephaly, delayed milestones and progressive motor decline. It shows characteristic magnetic resonance imaging (MRI) findings, including diffuse white matter changes and subcortical cysts, especially in the anterior temporal lobes. We report a case of a 10-month-old female from a nonconsanguineous Agarwal family in North India, presenting with macrocephaly and developmental delay. MRI revealed classic features of MLC. The case highlights the importance of early diagnosis even in

Fetal Macrocephaly: Prenatal Findings and Follow-Up in Cases With High Risk for Abnormal Outcome.

OBJECTIVE: Fetal macrocephaly (HC Z-score ≥ +2) is a common reason for referral for neurosonography. While most cases are benign and asymptomatic, syndromic macrocephaly poses a significant risk of abnormal neurodevelopment. This study aimed to describe the sonographic and genetic features of fetuses at the highest risk of syndromic macrocephaly. METHODS: We retrospectively analyzed 19 fetuses with either HC Z-scores ≥ +2.5 or those with ≥ +2 and associated anomalies, evaluated between January 2015 and October 2024. Assessments included fetal neurosonography, anomaly scans, MRI, and genetic w

Prior Head Circumference Measurements in Infants With Older Subdural Hematomas: Did We Miss It?

OBJECTIVE: To establish whether infants with macrocephaly and non-acute subdural hematomas (SDHs) had missed opportunities for measuring fronto-occipital circumference (FOC) and obtaining neuroimaging prior to diagnosis. Macrocephaly due to SDH in infancy is initially detected by FOC measurements and/or neuroimaging, as neurologic symptoms may be subtle or missed. METHODS: Retrospective chart review of infants with macrocephaly and non-acute SDHs referred to child abuse pediatricians for suspected abusive head trauma (AHT) over an 8-year period. A study sample of 35 infants met inclusion crit

Factors Affecting Macro-Structural Development in the Cerebral Cortex: The Potential Role of Tissue Removal Through Pruning and Apoptosis.

Several theories have been developed to explain mechanisms of macro-structural development in the cerebral cortex, including external skull constraints, axonal tension, differential proliferation of neural progenitors, differential cortical expansion, and axonal pushing. These theories are not necessarily mutually exclusive, and some combination thereof may be required in order to fully explain and characterize complex folding, sulcal development and thinning in the cortex. This manuscript provides an overview of the leading theories of contributing factors to macro-structural cortical develop

Head Growth Trajectories During the First Year of Life and Risk of Autism Spectrum Disorder.

Atypical infant head circumference (HC)-including increased rates of macrocephaly and microcephaly-has been linked to autism spectrum disorder (ASD). However, specific head growth trajectories associated with ASD remain poorly defined. This retrospective case-control study aimed to delineate these trajectories and examine their relationship to height. The study sample included 262 children diagnosed with ASD and 560 matched controls. Growth measures at 1, 2, 4, 6, 9, and 12 months of age were obtained from health clinics in southern Israel. The sample was classified into seven clusters based o

Prenatal Phenotype of a Heterozygous Missense CHD4 Variant in a Fetus With Widened Cerebral Subarachnoid Space, Increased Head Circumference and Polyhydramnios.

CHD4-associated Sifrim-Hitz-Weiss syndrome (SIHIWES) is an autosomal dominant intellectual developmental disorder. The postnatally clinical manifestations primarily include heart defects, macrocephaly, and hypotonia. We report a well-documented prenatal case of SIHIWES presenting with increased head circumference, polyhydramnios, and widened cerebral subarachnoid spaces. Trio-based whole exome sequencing identified a de novo likely pathogenic variant in CHD4, confirming the diagnosis of SIHIWES in the fetus. Our report expands the prenatal phenotypic spectrum of SIHIWES and highlights the impo

An atypical case of macrocephaly and severe intellectual disability associated with a missense variant in the guanine nucleotide exchange factor-1 domain of TRIO.

BACKGROUND: Trio rho guanine nucleotide exchange factor (TRIO), encodes a guanine nucleotide exchange factor (GEF) that is critical for neurodevelopment and regulates Rho guanosine triphosphatase. It has been shown that missense variants in the GEF1 or GEF2 domains or loss-of-function variants in TRIO are associated with microcephaly while gain-of-function variants in the spectrin repeat domain result in macrocephaly. Rare cases of macrocephaly linked to missense variants in the GEF1 domain have been reported; however, clinical details remain limited. CASE PRESENTATION: We describe the case o

The Head Circumference Height Index (HCH-I) to Quantify Relative Macrocephaly and Aid Identification of Hypochondroplasia in Children.

Hypochondroplasia (HCH) is a rare skeletal dysplasia caused by pathogenic variants in the FGFR3 gene. We hypothesized that the relative disproportion between head circumference and height in HCH might be diagnostically informative and generated a simple index of head-stature disproportion to help pediatricians diagnose HCH. The Head Circumference Height Index (HCH-I), based on formal statistical principles, is defined as height Z-score - 1/2 head circumference Z-score, with the Z-scores based here on the UK90 growth reference. An HCH-I below the cut-off of -2 indicates substantial head-height

MAX-Related Disorder: Expanding the Phenotype of the Recurrent p.Arg60Gln Variant.

We report on two patients with four-limb postaxial polydactyly and a recurrent de novo missense variant (c.179G>A) in the MAX gene. This variant has been previously reported in four other cases of polydactyly, progressive macrocephaly, and ophthalmologic abnormalities. Our two cases represent the fifth and sixth known occurrences of this apparently rare genetic disorder. In addition to the previously reported findings, our patients exhibit novel clinical features, including orofacial clefting, congenital heart defects (coarctation of the aorta, secundum atrial septal defect), natal teeth, and

NF1-Specific Growth Charts for Head Circumference Over the First 3 Years of Life.

BACKGROUND AND OBJECTIVES: Macrocephaly is among the most common findings in neurofibromatosis type 1 (NF1) and may be associated with other clinical manifestations of the genetic syndrome. NF1-specific growth charts that account for expected macrocephaly may increase sensitivity for detecting atypical growth. We aimed to produce NF1-specific growth charts of head circumference for the age range of 0-3 years and to assess their potential clinical impact. METHODS: Using electronic health records from the Children's Hospital of Philadelphia, we collected head circumference measurements from chi

Macrocephaly and Digital Anomalies Expand the Phenotypic Spectrum of PGAP2 Variants in Hyperphosphatasia with Impaired Intellectual Development Syndrome 3 (HPMRS3).

Glycosylphosphatidylinositols (GPIs) anchor over 150 proteins as GPI-anchored proteins (GPI-APs) with crucial roles in diverse biological processes. The highly conserved biosynthesis of GPI-APs involves precise steps with at least 21 genes, categorized as PIG and PGAP genes. Pathogenic variants in these genes are linked to human diseases, highlighting the importance of each biosynthesis step. PGAP2 stands out among these genes due to its association with an expanded clinical spectrum of neurodevelopmental disorder (NDD) phenotypes with biallelic pathogenic variants. We present four patients fr
