Endorsed but Undefined: Multidisciplinary Counselling for Closed Spinal Dysraphism - A National Multispecialty Survey from India.

Objective To characterize current antenatal management practices for closed spinal dysraphism (CSD) among Indian specialists and determine whether practice variation is specialty-dependent or distributed within specialties. Methods A cross-sectional expert-reviewed and pilot-tested web-based survey (15 substantive items) was distributed to obstetricians, perinatologists, and radiologists across India (January-September 2023). Descriptive statistics and chi-square tests were used to evaluate the associations between respondents' characteristics and clinical practices. Response dispersion was qu

Imaging of Congenital Anomalies of the Spinal Cord: Part 1.

Spinal dysraphism (SD) encompasses a spectrum of congenital malformations that develop during spinal embryogenesis (between the second and sixth gestational weeks). Broadly, SDs are classified into "open" or "closed" based on the presence (or lack of) of an overlying cutaneous defect. In general, open SDs are associated with significant neurologic impairments warranting prompt diagnosis and surgical correction-including in utero interventions. Radiology plays a key role in identification and distinguishing SDs thus providing information for surgical triage, prognostication, and outcome assessm

Imaging of Congenital Anomalies of the Spinal Cord: Part 2.

Closed spinal dysraphisms without a subcutaneous mass comprise diverse congenital anomalies arising from disrupted primary or secondary neurulation, often presenting with subtle clinical features yet significant long-term neurologic, orthopedic, and urologic morbidity. Pathophysiology frequently involves intrinsic cord maldevelopment or tethered cord syndrome. Imaging, particularly MR imaging, is central to diagnosis, enabling categorization into simple (eg, intradural lipoma, filum terminale lipoma, and persistent terminal ventricle) and complex lesions (eg, dermal sinus tract, neurenteric cy

Long-term neurological stability after decompression with duraplasty without resection of a non-dysraphic thoracic spinal lipoma: a 30-year follow-up case report.

Intradural spinal lipomas without associated dysraphism are rare, and the optimal surgical strategy remains controversial, particularly for thoracic lesions strongly adhered to neural structures. A 17-year-old female presented with progressive spastic gait and sensory disturbances in the lower extremities that developed during pregnancy. Magnetic resonance imaging showed an intradural lesion at the T5-T8 level consistent with a spinal lipoma. Because of the absence of a clear dissection plane and strong adhesion to neural elements, tumor resection was not attempted. Instead, decompression and

Caudal duplication syndrome in a young girl: Gastrointestinal and genitourinary duplication with spinal dysraphism.

Caudal Duplication Syndrome is an extremely rare congenital malformation in which structures derived from notochord and embryonic cloaca are duplicated to a variable extent. We report a case of 6-year-old female who presented with complete lower gastrointestinal and genitourinary duplication, large omphalocele, and spinal dysraphism. Bladder unification was performed due to left sided urethral hypoplasia and excision of left sided less developed duplicated gut. Patient is now on medical management due to voiding dysfunction. This case highlights complexity of diagnosis and management challenge

Latex sensitization and allergy in children with spinal dysraphism: A meta-analysis.

CONTEXT: Children with Spinal Dysraphism have historically demonstrated a higher prevalence of latex allergy which resulted in efforts to establish latex-free environments for these children. Whether this has reduced latex sensitization and allergy prevalence is uncertain. OBJECTIVE: This systematic review evaluates and summarizes the current evidence of the prevalence of latex allergy in children with spinal dysraphism. METHODS: MEDLINE, Embase, and PubMed databases were searched for studies investigating the prevalence of latex sensitisation and allergy in patients with spina dysraphism fr

Clinical characteristics and surgical outcomes of split cord malformations: A retrospective analysis.

BACKGROUND: Split cord malformation (SCM) is a rare congenital anomaly characterized by a longitudinal division of the spinal cord into two hemicords. Clinical manifestations are diverse, often including neurological, orthopedic, and cutaneous abnormalities. Surgical management remains the mainstay of treatment, though outcomes and complication profiles require further study. This study aimed to analyze the clinical spectrum, surgical management, and outcomes of pediatric and young adult patients with SCM. METHODS: A retrospective review was conducted of patients with SCM who underwent surger

Intraoperative Neurophysiological Monitoring in Spina Bifida and Tethered Cord Surgeries: A Systematic Review of Clinical Outcomes.

Spina bifida and tethered cord syndrome (TCS) are forms of spinal dysraphism requiring surgical intervention to prevent progressive neurological decline. Intraoperative neurophysiological monitoring (IONM) provides real-time feedback, allowing surgeons to minimize intraoperative injury during these procedures. This systematic review analyzed 19 primary studies published between 2015 and 2025 examining the role of IONM techniques, including somatosensory evoked potentials (SSEP), motor evoked potentials (MEP), electromyography (EMG), and bulbocavernosus reflex (BCR), in spina bifida and tethere

A rare case of split cord malformation type II with concomitant myelocystocele: illustrative case.

BACKGROUND: Split cord malformation (SCM) and myelocystocele (MC) are rare forms of spinal dysraphism arising from aberrant embryogenesis at distinct developmental stages. Although both lesions share embryological origins, their concomitant occurrence is exceedingly uncommon. OBSERVATIONS: The authors report the first case of a type II SCM coexisting with an MC in an infant. The patient underwent microsurgical detethering and repair of the MC. Postnatal postoperative MRI revealed a type II SCM of the thoracolumbar spine. At 17 months, the patient demonstrated neurological symptoms consistent

Maternal health in women with spina bifida: A scoping review.

BACKGROUND: An increasing number of women with spina bifida are reaching reproductive age and may consider pregnancy. However, limited evidence on how spina bifida affects maternal health can hinder tailored counseling and guidance, contributing to uncertainty during pregnancy-related decision-making. OBJECTIVE: This scoping review aimed to synthesize existing literature on maternal health among women with spina bifida, focusing on pregnancy and childbirth outcomes, women's health-related outcomes, care needs, and health-related decision-making across pregnancy, childbirth, and parenting cont

Emerging biomedical imaging applications of tumor-derived extracellular vesicles for premetastatic niche detection: Biological rationale, engineering strategies, and translational challenges.

BACKGROUND: Accurate fetal imaging is essential to the practice of maternal-fetal medicine. While ultrasonography has been the traditional mainstay of fetal imaging, its ability to resolve critical features of central nervous system (CNS) anatomy remains limited. As interest in intrauterine therapy for myelomeningocele has increased, so has the need for more accurate, noninvasive imaging of the CNS. Fetal magnetic resonance imaging (MRI) promises to fill the gap left by ultrasound. METHODS: Thirty-seven MRI scans of fetuses previously diagnosed with myelomeningocele were reviewed by 2 neurora

A 6-minute adaptation of the wheelchair propulsion test to assess and estimate aerobic fitness in young wheelchair users with spina bifida: criterion validity and test-retest reliability.

PURPOSE: To determine the validity and reliability of an adapted 6-minute wheelchair propulsion test, referred here as the 6 m-WPT, to assess aerobic fitness in SBw. A secondary aim was to develop a prediction model for oxygen uptake (VO2peak) derived from this adapted test. MATERIALS AND METHODS: This cross-sectional observational study included a test-retest design. Twenty-four SBw (median age 10.0 [10.0-12.4] years) completed the arm-crank cardiopulmonary exercise test (CPET), the 10-metre Shuttle Ride Test (10 m-SRT), and the 6 m-WPT to examine the 6 m-WPT validity. Fortify Alabama: Corn Masa Fortification for Spina Bifida Prevention.

Neural tube defects (NTDs), such as myelomeningocele, are serious birth defects preventable with adequate maternal folate intake. In the United States, Hispanic populations have disproportionately high NTD rates. Mandatory fortification of enriched cereal grains in the 1990s significantly reduced NTDs but excluded corn masa flour (CMF), a staple in many Hispanic diets. This shortcoming may contribute to persistent disparities in NTD rates. We pursued a state legislative mandate for folic acid fortification of CMF to reduce NTDs. We also developed a legislative model that can be replicated nati

Assessing the utility of bulbocavernosus reflex for predicting urological outcome in children with spinal dysraphism surgery.

PURPOSE: Surgical untethering for spinal dysraphism carries the risk of neural damage affecting sphincter control. Whilst anal sphincter motor-evoked potentials (MEP) are commonly used, they assess efferent pathways and do not directly reflect bladder innervation. The bulbocavernosus reflex (BCR) evaluates both afferent and efferent components of the sacral reflex arc, potentially serving as a better intraoperative predictor of urinary function. This study aimed to (1) assess the correlation between baseline BCR and preoperative urinary continence and (2) determine whether intraoperative BCR l

Cerebrospinal fluid matrix-remodeling biomarkers in neonates with myelomeningocele: MMP-9, TIMP-1, and TGF-β1 expression.

PURPOSE: Matrix metalloproteinases (MMPs) and their endogenous inhibitors, tissue inhibitors of metalloproteinases, regulate extracellular matrix remodeling and neural tissue homeostasis. Transforming growth factor β1 modulates inflammation, ECM deposition, and tissue repair. Dysregulation of these pathways may contribute to congenital spinal dysraphism. This study quantified cerebrospinal fluid MMP-9, TIMP-1, and TGF-β1 in neonates with myelomeningocele and evaluated their potential as pathogenic mediators or biomarkers. METHODS: Preoperative CSF samples were collected from 48 neonates with

Multi-tissue DNA methylation analysis to identify an appropriate surrogate tissue for a unique neurological tissue specific to spina bifida.

BACKGROUND: Animal models suggest a role of epigenetic mechanisms, including DNA methylation, in neural tube closure; however, studies characterizing DNA methylation profiles in nervous system tissue from humans with spina bifida are limited. In this study, we assessed DNA methylation profiles in dural tissue of infants with spina bifida, collected at the time of surgical closure of the defect, and examined whether whole blood or buccal swab are appropriate surrogate tissues, as they are more practical to collect in large-scale epidemiological studies. DNA methylation was measured in dural tis

Life expectancy in open spina bifida.

AIM: To estimate life expectancies for individuals with open spina bifida, stratified by age, sex, functional ability, and specific impairments. METHOD: In the present study, extensive data from 1659 persons in California (1986-2019) were analysed using standard methods. Empirical mortality rates were calculated, and rates were also derived for various cohorts using logistic regression. Life tables were then constructed, from which life expectancies were obtained. RESULTS: Survival varied significantly by ambulatory and feeding ability, and by bowel/bladder continence. For example, at age 5
