Aortic remodeling and pulmonary autograft performance over 2 decades after the Ross procedure in the pediatric and young adult population.

OBJECTIVES: The Ross procedure is a well-established treatment for nonrepairable aortic valve disease in children and young adults. This study evaluates the mid- and long-term clinical outcomes of more than 20 years of autograft hemodynamics/performance with the Ross procedure in infants, children, and young adults. METHODS: We retrospectively analyzed 149 consecutive patients who underwent the Ross procedure at our center between March 1996 and August 2023. All surgeries were performed as root replacements by a single experienced surgeon. Sinus of Valsalva and annulus diameters were measured

Left ventricular outflow tract obstruction repair after atrioventricular septal defect correction.

This video tutorial presents the case of surgical correction of left outflow tract obstruction after atrioventricular septal defect repair. The left outflow tract obstruction was presented as a fixed fibromuscular membrane combined with accessory tissues from the left atrioventricular valve. The surgical correction included fibromuscular membrane resection combined with myotomy and resection of additional tissues arising from the atrioventricular valve. The repair was conducted via a transaortic approach. Post-operative echocardiography demonstrated a good result of the surgery without flow ac

Fufang Huangqi Jianpi Oral Liquid alleviates spleen deficiency diarrhea via modulation of the NF-κB/Nrf2 signaling pathway and apoptosis.

INTRODUCTION: Human childbirth remains a complex and risky process for both mothers and infants, even with modern advancements in medical care. This study investigated the prevalence of obstetric interventions, namely caesarean sections, episiotomies, and forceps deliveries, along with the role of maternal-foetal body size mismatch in influencing delivery outcomes. METHODS: Utilising two datasets from similar archival sources in two Swiss cities (Basel and Lausanne) from the 1920s, we explored the relevance of a mismatch between the body size of the mother and the foetus as a risk factor for

Surgical Ligation of Isolated Type I Aortopulmonary Window Without Cardiopulmonary Bypass is a Safe and Cost-Effective Surgical Technique.

IntroductionAortopulmonary window (APW) is an extremely rare congenital anomaly of the heart where there is a direct communication between the ascending aorta and main pulmonary artery. Surgical repair of APW on cardiopulmonary bypass (CPB) is considered the standard of surgical care. In this study, we selected patients with a type I APW to undergo direct surgical ligation to determine if this technique is safe and can produce equivalent results to standard open-heart repair.MethodTwelve infants with simple or isolated type I APW underwent direct surgical ligation by a single surgeon at Childr

Self-Expanding Aortic Endoprosthesis for Conduit Preparation Prior to Transcatheter Pulmonary Valve Replacement.

BACKGROUND: Preparation of the right ventricular outflow tract (RVOT) using covered stent placement is a common preparatory step for transcatheter pulmonary valve replacement (TPVR) to reduce the risk of disruption associated with dilating calcified RVOT conduits. AIM: We present a novel method employing a self-expanding covered aortic endoprosthesis and discuss its advantages as an alternative strategy. METHODS: Patients with congenital heart disease undergoing RVOT conduit preparation with the GORE TAG Thoracic Branch Endoprosthesis Extender (W.L. Gore & Associates, Newark, DE) followed by

Histological examination of the pulmonary artery and aorta in an adolescent undergoing the Ross procedure.

BACKGROUND: The Ross procedure offers several advantages for adolescents requiring aortic valve replacement, but progressive pulmonary autograft dilation is a well-described risk. To provide novel insight into the pre-Ross histology in an adolescent with bicuspid aortic valve (BAV)-associated aortopathy, we describe the extracellular architecture of the pulmonary artery (PA) compared to the native ascending aorta. METHODS: A 15-year-old with BAV, symptomatic moderate aortic insufficiency and aortic stenosis, and ascending aortic dilation underwent Ross. Intraoperative specimens included the m

Noonan Syndrome With Complex Pulmonary Stenosis at High Altitude: Impact of Delayed Diagnosis.

BACKGROUND: Noonan syndrome is a genetic disorder affecting 1 in 1,000 to 2,500 live births, with cardiac involvement in up to 80% of cases, predominantly pulmonary valve stenosis. Socioeconomic barriers often delay diagnosis in resource-limited settings. CASE SUMMARY: A 22-year-old male presented with severe oxygen desaturation at high altitude, initially attributed to COVID-19. Clinical evaluation revealed characteristic Noonan syndrome features and a grade IV/VI systolic murmur. Cardiac imaging demonstrated severe dysplastic pulmonary stenosis with supravalvular membrane, right-to-left shu

Mid- and Late-term Left Ventricular Diastolic Function After the Ross Procedure.

The Ross procedure is a common surgical option for congenital aortic valve disease, but little is understood about the long-term ventricular mechanics of patients who undergo the Ross procedure. This study evaluated the late-term diastolic function of Ross patients compared to patients who underwent other types of index surgical repairs for congenital heart lesions. Patients who underwent catheterization for transcatheter pulmonary valve implantation (TPVi) at a single center between 2010 and 2022 were retrospectively reviewed. Patients who had undergone a Ross procedure were compared to those

Half-turned truncal switch for transposition of the great arteries with left ventricular outflow tract obstruction.

OBJECTIVE: The Rastelli operation is accepted as 1 of the standard techniques for complete transposition of the great arteries (TGA) and TGA-type double-outlet right ventricle with left ventricular outflow tract (LVOT) obstruction. The half-turned truncal switch (HTTS) operation has been reported as an alternative to the Rastelli approach. The aim of this study was to compare these 2 operations for children with TGA and LVOT obstruction (TGA/LVOTO). METHODS: Between 2012 and 2024, 11 patients underwent TGA/LVOTO repair. The median age at repair was 12 months and median weight was 8.4 kg. Preo

Early Stent Frame Fracture of SAPIEN 3 Valve in the Pulmonary Position.

A 15-year-old post-REV patient underwent transcatheter pulmonary valve replacement with a 23 mm SAPIEN 3 for right ventricular outflow tract (RVOT) stenosis and severe regurgitation. At 9 months, recurrent stenosis revealed multiple frame fractures. Balloon redilation up to 24 mm was limited by aortic root compression and recoil, prompting surgery. This first reported case of early SAPIEN 3 fracture in the pulmonary position highlights structural risks in complex RVOT anatomies.

Surgical Aortic Valve Replacement For Bicuspid And Tricuspid Valve Disease: 7-Year Outcomes In >1100 Patients.

INTRODUCTION: Bicuspid aortic valve affects 0.5-2% of the population in developed countries. Given uncertainties about the best aortic valve replacement (AVR) option in this often younger, low-risk, population, it is important to understand how newer bioprostheses perform in these patients. The primary objective of this analysis was to compare 7-year outcomes of surgical AVR (SAVR) with the Avalus bioprosthesis between patients with a congenital bicuspid or tricuspid valve. METHODS: This prospective, non-randomized study included 1132 patients with aortic valve stenosis or chronic severe aort

Aortic Valve Function and Aortic Dilatation Associated with Bicuspid Aortic Valve in Early Childhood.

INTRODUCTION: The natural history of bicuspid aortic valve (BAV) remains inadequately characterized, leaving uncertainties regarding whether associated aortic dilatation arises from an inherent susceptibility or primarily results from altered flow dynamics across the aortic valve. We aimed to describe the evolution of valve function and aortic dilatation at preschool age in children diagnosed with BAV neonatally. METHODS: The population study, the Copenhagen Baby Heart Study (n > 25,000), which was performed in 2016-18, diagnosed 196 newborns (0.8%) with BAV. In the present follow-up study, c

Impact of hypoplastic left heart syndrome anatomic subtype on mortality, ventricular function, and atrioventricular valve function in the current era.

OBJECTIVE: The aortic atresia/mitral stenosis (AA/MS) subtype in hypoplastic left heart syndrome (HLHS) has been associated with poor single-ventricle palliation outcomes. We evaluated whether HLHS anatomic subtype influences transplant and Fontan-takedown-free survival and right ventricular (RV) and tricuspid valve (TV) function in the current era. METHODS: Retrospective chart review was done for 219 consecutive neonates with HLHS who received a Norwood at our center (2006-2023). Primary outcomes were freedom from mortality/transplant/Fontan takedown post-Norwood (n = 219) and Fontan (n = 13

Aortic regurgitation as a cause of sudden cardiac death with aortic and left ventricular remodelling - the role of the bicuspid valve.

BACKGROUND AND OBJECTIVE: Aortic regurgitation (AR) results in blood flow from the aorta back into the left ventricle, which leads to left ventricular hypertrophy and dilation which, in a clinical setting, leads to heart failure and death. However, it is not a well-recognised cause of sudden cardiac death (SCD). METHODS: We identified 10 cases of AR with no other cause of death from our database of 8,551 cases of SCD. All these cases had a full autopsy with negative toxicology. Diagnosis of AR was based upon the presence of prominent ridges on the edge of the aortic valve (AV) cusps with aort

Cardiogeriatrics: the current state of the art.

It is estimated that by 2050, 17% of the world's population will be greater than 85 years old, which, combined with cardiovascular disease (CVD) being the leading cause of death and disability, sets an unprecedented burden on our health and care systems. This perfect storm will be accompanied by a rise in the prevalence of CVD due to increased survival of patients with pre-existing CVD and the incidence of CVD that is associated with the process of ageing. In this review, we will focus on the diagnosis and management of common CVD conditions in old age, namely: heart failure (HF), coronary art

Congenital continuous retrograde basilar flow suggests type B interrupted aortic arch in a neonate: A case report.

Reversed flow in the basilar artery can be acquired or congenital. Acquired reversed flow in the basilar artery can result from acute thrombosis of the basilar artery or retrograde vertebral artery flow. Congenital continuous retrograde basilar artery flow has not been described. We report a 2-day-old male presenting with hypocalcemic seizures which led us to obtain a Duplex echoencephalogram. An echocardiogram was subsequently ordered. In the coronal plane through the anterior fontanelle, retrograde flow was seen in the basilar artery and the right vertebral artery. In the axial plane through
