Automated, Standardized, Quantitative Analysis of Cardiovascular Borders on Chest X-Rays Using Deep Learning.

BACKGROUND: The analysis of cardiovascular borders (CVBs) in chest x-rays (CXRs) traditionally relied on subjective assessment and does not have established normal ranges. OBJECTIVES: The authors aimed to develop a deep learning-based method for quantifying CVBs on CXRs and to explore its clinical utility. METHODS: This study used a prevalidated deep learning to analyze CVBs. A total of 96,129 normal CXRs from 4 sites were used to establish age- and sex-specific normal ranges of CVBs. The quantified CVBs were standardized into z-scores for newly inputted CXRs. The clinical utility of the z-s

PP2A Attenuates Thoracic Aneurysm and Dissection in Mouse Models of Marfan Syndrome.

BACKGROUND: Recent studies show that hyperactivation of mTOR (mammalian target of rapamycin) signaling plays a causal role in the development of thoracic aortic aneurysm and dissection. Modulation of PP2A (protein phosphatase 2A) activity has been shown to be of significant therapeutic value. In light of the effects that PP2A can exert on the mTOR pathway, we hypothesized that PP2A activation by small-molecule activators of PP2A could mitigate AA progression in Marfan syndrome (MFS). METHODS: Two distinct mouse models of MFS underwent daily oral administration of small-molecule activators of

Reconstruction of the isolated left vertebral artery in total aortic arch replacement for type A aortic dissection via a single upper right hemisternotomy approach.

OBJECTIVES: The isolated left vertebral artery (ILVA) is a rare congenital abnormality of the branches of the aortic arch. Its presence can influence both the surgical procedure and the prognosis of total aortic arch replacement. The goal of this study was to assess the early postoperative outcomes of ILVA reconstruction performed during total aortic arch replacement via a single upper right hemisternotomy approach. METHODS: From October 2018 to March 2024, patients diagnosed with type A aortic dissection who underwent total aortic arch replacement via a single upper right hemisternotomy at t

Genotype-Phenotype Correlation Insights Through Molecular Modeling Analysis in a Patient with Loeys-Dietz Syndrome.

BACKGROUND: Pathogenic variants within the gene encoding transforming growth factor β (TGF-β) are responsible for Loeys-Dietz syndrome (LDS), a heritable thoracic aortic disease sharing clinical features with Marfan syndrome, including craniofacial and skeletal abnormalities as well as aortic root aneurysms and dissections. In contrast to Marfan syndrome patients, who rarely develop aneurysms or dissections beyond the aortic root, LDS patients frequently exhibit vessel aneurysms in locations other than the aortic root. Here, we report the case of a 61-year-old patient who initially presented w

Aortic and arterial manifestations and clinical features in TGFB3-related heritable thoracic aortic disease: results from the Montalcino Aortic Consortium.

BACKGROUND: Pathogenic variants in TGFB3 may lead to a syndromic genetic aortopathy. Heritable thoracic aortic disease (HTAD) and arterial events may occur in TGFB3-related disease but there are limited outcomes data on vascular events in this condition. METHODS: Clinical data, phenotypical features and aortic outcomes in individuals with pathogenic/likely pathogenic (P/LP) TGFB3 variants enrolled in the Montalcino Aortic Consortium registry were reviewed. RESULTS: 34 individuals (56% male, median age 42 years, IQR 17-49, range 3-74 years) with P/LP TGFB3 variants were studied. Craniofacial,

Improved quantification of aortic regurgitation with direct regurgitant jet measurement by four-dimensional flow cardiovascular magnetic resonance in complex congenital heart disease.

BACKGROUND: Due to the presence of complex flow states and significant jet eccentricity in patients with congenital heart disease (CHD), accurate quantification of aortic regurgitation (AR) using standard echocardiographic or conventional cardiovascular magnetic resonance (CMR) imaging measures remains challenging. Four-dimensional flow (4DF) CMR permits transvalvular flow quantification under non-laminar flow states, although it has not been well validated for AR quantification in CHD. METHODS: In 186 patients with moderate or complex CHD, we evaluated the agreement between different methods

Management of Cardiovascular Health Issues in Turner Syndrome: Expert Insights and Expanded Recommendations From the 2024 Guideline Development Team.

Turner syndrome (TS) is frequently complicated by congenital heart disease (CHD). While left-sided lesions such as bicuspid aortic valve (BAV) and coarctation of the aorta are the most common structural heart lesions in TS, other anomalies, such as aortic arch malformations, hypoplastic left heart syndrome (HLHS), persistent left superior vena cava (LSVC), and partial anomalous pulmonary venous return (PAPVR), are also relatively frequent. Standardized mortality is increased threefold in individuals with TS compared to the general population, with cardiovascular complications, including aortic

Diagnostic value of cardiac magnetic resonance imaging during transition care in adolescents with Turner syndrome.

BACKGROUND: Turner Syndrome (TS) is a chromosomal disorder frequently associated with congenital cardiovascular abnormalities, particularly bicuspid aortic valve (BAV), coarctation of the aorta (CoA), and aortic dilatation. These conditions substantially increase the risk of aortic dissection. Although echocardiography (ECHO) is commonly used for cardiac monitoring, its limitations in evaluating aortic morphology require cardiac magnetic resonance imaging (CMR), as recommended by recent guidelines. CMR offers a comprehensive alternative, especially during the transition from pediatric to adult

Distinctive Amelogenesis Imperfecta in Loeys-Dietz Syndrome Type II.

Loeys-Dietz syndrome (LDS1-6) is caused by mutations along the transforming growth factor-β (TGF-β) signaling pathway and features aortic aneurysms and craniofacial dysmorphology. Mutations that cause LDS can be found in the genes encoding transforming growth factor-β (TGF-β) ligands (TGFB2 and TGFB3), receptors (TGFBR1 and TGFBR2), and signal transducers (SMAD2 and SMAD3). Variable enamel anomalies are seen in patients with LDS, but the most severe enamel defects with advanced attrition have been observed specifically in some patients with mutation in the TGFBR2 gene (LDS2). We used human spe

Aortic regurgitation as a cause of sudden cardiac death with aortic and left ventricular remodelling - the role of the bicuspid valve.

BACKGROUND AND OBJECTIVE: Aortic regurgitation (AR) results in blood flow from the aorta back into the left ventricle, which leads to left ventricular hypertrophy and dilation which, in a clinical setting, leads to heart failure and death. However, it is not a well-recognised cause of sudden cardiac death (SCD). METHODS: We identified 10 cases of AR with no other cause of death from our database of 8,551 cases of SCD. All these cases had a full autopsy with negative toxicology. Diagnosis of AR was based upon the presence of prominent ridges on the edge of the aortic valve (AV) cusps with aort
