Incidental finding of supra cardiac total anomalous pulmonary venous connection with Tetralogy of Fallot: A case report.

Tetralogy of Fallot (TOF) is the most common cyanotic congenital heart disorder (CHD) worldwide and is known to be associated with a variety of other CHDs. This case report explores a rare incidental combination of TOF and supracardiac total anomalous pulmonary venous connection (TAPVC). Such a coexistence is exceedingly uncommon with only a handful of reported cases which prompts the need for documentation, especially its management in a developing country. Prompt diagnosis is challenging due to atypical symptoms which emphasises the importance of comprehensive imaging. Surgical intervention,

Persistent Left Superior Vena Cava Beyond Anatomy.

Persistent left superior vena cava (PLSVC) is the most common thoracic venous anomaly, occurring in 0.3% to 0.5% of the general population and in 10% of patients with congenital heart disease. Historically considered benign, PLSVC is increasingly recognized for its clinical significance in the fields of interventional cardiology, electrophysiology, and surgery. The present narrative review combines the available evidence regarding PLSVC, focusing on its embryology, anatomical variants, diagnostic challenges, and tailored management strategies, to address clinical uncertainties and improve proc

A novel CTNND1 genotype-phenotype association: total anomalous pulmonary venous connection in a fetus.

We present the first case of CTNND1 variant with a prenatal clinical phenotype of total anomalous pulmonary venous connection. The genetic analysis identified a heterozygous pathogenic variant NM_001085458.2: c.2098C>T (p.R700*) in the CTNND1 gene of the fetus. This case expands the phenotypic spectrum of CTNND1 gene, provides a novel clinical reference for prenatal diagnosis, and suggests that the CTNND1 gene plays a crucial role in early cardiac development.

Efficacy of Right Ventricle-to-Pulmonary Artery Conduit as a Pulmonary Blood Source at Unifocalization of Major Aortopulmonary Collateral Arteries.

Multimodality treatment with catheter-based intervention is still commonly used for pulmonary atresia with ventricular septal defect (PA/VSD) or hemi-truncus with major aortopulmonary artery collateral arteries (MAPCAs) requiring unifocalization. A total of 12 patients with PA/VSD with MAPCAs (n = 10) or hemi-truncus (n = 2) who had undergone unifocalization since 1994 were enrolled. The development of the central pulmonary artery (cPA) was absent in 2 patients, diminutive (< 2 mm) in 3, sizable (> 2 mm) in 5, and unilateral (hemi-truncus) in 2. Treatment strategy was determined according to t

Residual AVNRT Despite Antegrade Slow-Pathway Block Unmasked by Ventricular Extrastimuli.

BACKGROUND: Antegrade slow-pathway (SP) block is widely accepted as an endpoint for ablation of atrioventricular nodal reentrant tachycardia (AVNRT). However, whether it reliably excludes residual reentrant circuitry remains uncertain. CASE SUMMARY: A woman in her 40s with recurrent AVNRT and persistent left superior vena cava (PLSVC) underwent a third ablation procedure after 2 prior recurrences following catheter ablation. Sinus-rhythm activation mapping identified a pivot site targeted for SP modification. Although atrial extrastimulus testing demonstrated elimination of antegrade SP condu

Lobectomy for right lower lobe lung cancer with anomalous systemic arterial supply and partial anomalous pulmonary venous connection: a case report.

BACKGROUND: Anomalous systemic arterial supply to the basal lung (ASASB) and partial anomalous pulmonary venous connection (PAPVC) are rare congenital anomalies; their coexistence in a patient with lung cancer has not been previously reported. CASE PRESENTATION: A 68-year-old woman was referred for a right lower lobe mass detected on screening. Contrast-enhanced computed tomography showed an aberrant artery from the abdominal aorta supplying segments 9-10 and drainage of all right upper pulmonary veins into the superior vena cava. Qp/Qs on echocardiography was 1.3. Three-dimensional reconstru

Successful Extensive Pulsed Field Ablation for Atrial Fibrillation Without Leadless Pacemaker Interference.

Although Pulsed field ablation (PFA) has demonstrated efficacy for atrial fibrillation, its safety in patients with leadless pacemaker remains uncertain. This case suggests that extensive PFA involving persistent left superior vena cava, superior vena cava, and left atrial posterior wall can be safely performed without device interference.

Antithrombotic Therapy on Antithrombin Resistance in a Mouse Model.

BACKGROUND: Antithrombin resistance (ATR) is a congenital thrombotic predisposition caused by abnormal prothrombin, FII (Factor II). Abnormal FII with ATR characteristics, when converted to thrombin, FIIa (Factor IIa), shows a reduced affinity of binding to AT (antithrombin), resulting in reduced anti-FIIa and a thrombotic tendency. Data on the efficacy of anticoagulants in ATR are currently insufficient. METHODS: Mouse models of ATR were generated to investigate the efficacy of anticoagulants. Because FII R596L, which causes ATR in humans, was considered to correspond to R593L in mice based

Chromosomal abnormalities in isolated persistent left superior vena cava: A systematic review and meta-analysis.

Persistent left superior vena cava (PLSVC) is the most common systemic venous anomaly detected prenatally, yet the risk of chromosomal abnormalities in truly isolated cases remains uncertain. This systematic review and meta-analysis aimed to determine the pooled rate of chromosomal anomalies in fetuses with isolated PLSVC and no associated structural anomalies, with particular attention to extractable denominators for genetic testing. A systematic search of PubMed/MEDLINE, EMBASE, and the Cochrane Library was performed from January 2000 through March 2026. Studies reporting data on chromosomal

Adult cor-triatriatum sinistrum-a sinistral lesion with varied presentations: a case series.

BACKGROUND: Cor-triatriatum sinistrum (CTS) is a rare congenital heart disease, characterized by a membrane dividing the left atrium into two chambers. Echocardiography is the primary imaging modality. However, computed tomography and magnetic resonance imaging both play important roles in delineating the anatomy of the condition and also determining the associated anomalies. Although thought to be a disease of childhood, CTS can also present in adulthood with varied clinical manifestations like dyspnea, palpitations, and syncopal attacks. In CTS, the left atrial membrane causes pulmonary veno

Single-stage repair of Scimitar variant with Swiss cheese ventricular septal defects without deep hypothermic circulatory arrest.

Scimitar variant is a rare partial anomalous pulmonary venous connection that lacks right lung hypoplasia and cardiac dextroposition. Its coexistence with Swiss cheese-type ventricular septal defects (VSDs) is extremely rare and can result in complex hemodynamic changes. A 23-month-old boy with Scimitar variant and multiple Swiss cheese-type VSDs underwent single-stage repair. Preoperative imaging revealed anomalous drainage of the entire right pulmonary venous return into the inferior vena cava and a persistent left superior vena cava. The dominant perimembranous defect was patched, while sma

Contact Strategies and Isolation Endpoints in Pulsed Field Ablation of Persistent Left Superior Vena Cava: A Case Series.

Persistent left superior vena cava (PLSVC) can trigger atrial fibrillation. Thermal ablation of this vessel carries risks of phrenic nerve palsy and coronary artery spasm, whereas pulsed field ablation still lacks procedural guidance for the PLSVC diameter taper, which narrows from the distal segment to a dilated coronary sinus ostium. We report two contact strategies applied in three patients: a guidewire-anchored rail with steerable sheath tension for a fixed-loop catheter, and loop-diameter adjustment for a variable-loop catheter. Acute bidirectional block was confirmed by pacing in all thr

Fully Automated Biometric Parameter Measurement in Prenatal Ultrasound Screening for Total Anomalous Pulmonary Venous Connection.

Total anomalous pulmonary venous connection (TAPVC) is a severe congenital heart disease, yet its prenatal detection rate remains suboptimal. To support prenatal ultrasound screening of TAPVC, the post-left atrium space (PLAS) index and the left-atrial posterior-space-to-diagonal (LAPSD) ratio measured in the four-chamber view (4CV) have been proposed as useful biometric parameters. In this study, we developed a novel approach that integrates automated 4CV extraction (AE) from fetal cardiac ultrasound videos with automated measurement of these indices. The heart, crux, and descending aorta wer

Treatment Outcomes and Temporal Changes in Pleural Fluid Characteristics in Persistent Postoperative Chylothorax Following Congenital Heart Surgery.

Postoperative chylothorax remains a significant complication after congenital heart surgery; however, little is known about temporal changes in pleural fluid characteristics during prolonged disease. We retrospectively reviewed 82 children with postoperative chylothorax treated between 2007 and 2023 to evaluate the outcomes of a stepwise treatment strategy, identify factors associated with refractory disease, and investigate serial pleural fluid findings. Patients were managed with dietary modification, total parenteral nutrition, octreotide, and additional intervention when necessary. Conserv
