Prenatal characteristics and prognosis of congenital pulmonary airway malformation: a retrospective multicenter cohort study.

This study evaluated the impact of prenatal characteristics of congenital pulmonary airway malformation (CPAM) on postnatal outcomes, and investigated management strategies and outcomes in surgical versus non-surgical cases. This retrospective multicenter cohort study included 145 cases of CPAM diagnosed by prenatal ultrasound between January 2019 and May 2024. The cohort was stratified into two groups based on prenatal cystic volume ratio (CVR): CVR < 1.6 (n = 131) and CVR ≥ 1.6 (n = 14) for comparative analysis. Live-born infants were further categorized into surgical (n = 40) and non-surgic

Bronchopulmonary sequestration in an adult patient in his late 60s.

Bronchopulmonary sequestration (BPS) is a congenital anomaly of the lung characterised by non-functioning pulmonary tissue that receives its blood supply from an anomalous systemic artery, most commonly from the aorta. Although typically diagnosed in childhood or adolescence, BPS can present later in life and may mimic more common pulmonary conditions. We present the case of a male patient in his late 60s with a persistent non-productive cough, in whom imaging revealed a sequestered lung segment with a feeding vessel arising directly from the descending aorta. This case emphasises the importan

Activating KRAS mutations mark premalignant cystic structures in congenital pulmonary airway malformations.

BACKGROUND: Congenital Pulmonary Airway malformations (CPAM) may lead to malignant degeneration, and therefore many surgeons opt to resect CPAM even in asymptomatic patients. Previously, we identified Kirsten rat sarcoma virus (KRAS) mutations in a subset of CPAM patients, possibly indicating a pre-malignant state. In order to unify treatment strategy in (asymptomatic) patients we focused on KRAS mutations as a potential risk factor for developing malignancy in CPAM. METHODS: Resected lung tissue of CPAM patients was separated in affected region ("cyst") and non-affected region ("control") to

Speech and language therapy management of laryngotracheal stenosis - what has changed.

PURPOSE OF REVIEW: To examine how the clinical management of adults with laryngotracheal stenosis (LTS) by speech and language therapists (SLTs) has evolved over the past decade. The review highlights changes in assessment, counselling, and rehabilitation practices in response to advances in surgical interventions and explores implications for multidisciplinary care. RECENT FINDINGS: Recent literature and clinical experience demonstrate that SLTs play a critical role in managing voice and swallowing outcomes for patients with LTS. While established care pathways exist for reconstructive surge

Sputum from Individuals with Primary Ciliary Dyskinesia Drives M2-like Macrophage Polarization.

INTRODUCTION: Primary ciliary dyskinesia (PCD) is a rare, congenital condition in which impaired ciliary function leads to bronchiectasis and progressive lung function decline. Bronchiectasis development is believed to involve infection and inflammation but is incompletely understood. Macrophages play a central role in cellular immune response, contributing to both pathogen clearance and immunoregulation. Depending on local stimuli, macrophages are polarized towards pro-inflammatory (M1) or pro-resolution/phagocytic (M2) phenotypes. This study aims to investigate the effects of PCD sputum on m

Impulse oscillometry and traditional pulmonary function testing: correlation, advances and clinical implications.

Impulse oscillometry system (IOS) is an effort-independent pulmonary function testing technique that assesses respiratory system mechanics during tidal breathing and has gained increasing interest as a complementary modality to traditional pulmonary function tests (PFTs), while its physiological relevance and clinical role alongside conventional tests remain incompletely defined. This narrative literature review synthesises evidence on the correlations between IOS parameters and traditional PFT indices, summarises clinical applications across major chronic respiratory diseases, and discusses e

The Protective Effect and Molecular Mechanism of Tetrandrine on Male Reproductive Damage Caused by Silicon Dioxide.

The long-term inhalation of free silica dust causes silicosis-a prevalent occupational hazard-yet its systemic effect on male reproductive toxicity remains underexplored. Tetrandrine (Tet) is the only plant-derived anti-silicosis drug approved in China. This study investigates silica (SiO2) -induced male reproductive damage and evaluates Tet's protective effects. Sixty male C57BL/6 mice (6-8 weeks) were divided into control, SiO2 exposure, and SiO2 + Tet groups. SiO2 was administered via intranasal infusion and Tet via gavage. Mice were sacrificed at day 7 (male reproductive injury model corre

Tidal breathing nasal nitric oxide measurement as a test for primary ciliary dyskinesia in young children.

BACKGROUND: Nasal nitric oxide (nNO) measurement during velum closure is a useful test for primary ciliary dyskinesia (PCD) but is not feasible in young children. Measurement during tidal breathing is an alternative approach. Our objectives were to define a protocol with age-specific cut-off nNO values during tidal breathing and evaluate this method as an adjunctive test for PCD. METHODS: A standardised method for nNO measurement during tidal breathing was developed and tested at the University of North Carolina in children aged <6 years with PCD, asthma, cystic fibrosis and healthy controls.

Airway microbiome dysbiosis in severe pneumonia: metagenomic evidence of pathogen expansion and commensal depletion.

BACKGROUND: The pulmonary microbiome is increasingly recognized as a key determinant of pneumonia severity, yet its clinical implications remain incompletely understood. Disruption of microbial ecology, or dysbiosis, may impair host immune responses and exacerbate disease progression. This study aimed to characterize microbiome alterations associated with severe pneumonia and their correlation with host inflammatory and coagulative parameters. METHODS: In this multicenter, prospective observational cohort study conducted across nine hospitals in Shanghai (2021-2025), bronchoalveolar lavage fl

The safety of conservative management of asymptomatic congenital pulmonary airway malformations (CPAMs) in children: A systematic review.

PURPOSE: The optimal management of infants born with asymptomatic congenital pulmonary airway malformations (CPAMs) remains controversial. However, emerging evidence increasingly supports a conservative non-operative approach for these patients. We aimed to evaluate this evidence with the hope of clarifying optimal management for these patients. METHODS: A comprehensive search strategy was developed. Electronic databases MEDLINE and Embase Classic + Embase were searched from inception to December 7th, 2023. EBM Reviews and APA PsycInfo were searched from inception to November 30th, 2023. The

Declining frequency of surgical interventions over time in subglottic stenosis.

BACKGROUND: Soft tissue subglottic stenosis (SGS) progressively reduces the airway dimension below the vocal folds. The cause is unknown. As no cure is available patients face a recurrent need for surgical treatment. Previous research has not established whether the need for repeated interventions diminishes or intensifies as the disease evolves. Our hypothesis is that the number of surgical interventions decrease with disease duration. If true, these findings may have an impact on the surgical setting. AIMS/OBJECTIVES: To investigate if the number of surgical interventions decreases with dis

Carboxymethyl Cellulose/Sodium Lignosulfonate Composite Hydrogel with Toughness, Adhesiveness, and Antibacterial Properties for Monitoring Respiration in Heart Failure Patients.

The advancement of personalized medicine is increasingly reliant on wearable health monitoring technologies. While hydrogels offer great promise for skin-integrated sensors due to their biocompatibility, flexibility, and conductivity, developing materials that simultaneously possess robust mechanical properties, reliable adhesion, and antimicrobial efficacy remains a challenge. To address the critical need for monitoring nocturnal respiratory abnormalities in heart failure patients, we have successfully developed a hydrogel composed of acrylic acid (AA), carboxymethyl cellulose (CMC), double-b

Lipidomic profiling reveals medium-chain acylcarnitines alterations as metabolic signatures in IgA nephropathy.

Immunoglobulin A nephropathy (IgAN), the most common primary glomerulonephritis worldwide, exhibits highly variable clinical progression, ranging from asymptomatic urinary abnormalities with potential for spontaneous resolution to progressive kidney failure over 10–20 years. The limited understanding of IgAN pathogenesis continues to constrain the development of effective therapeutic strategies. To elucidate key pathogenic mechanisms, we performed both untargeted and quantitative lipidomic profiling in patients with IgAN. Our results revealed significant disruptions in carnitine metabolism, pa
